Dr. Ronald Krauss, M.D. is the director of atherosclerosis research at Children’s Hospital Oakland Research Institute, Adjunct Professor at UCSF and UC Berkeley. Dr. Krauss is really one of the pioneering scientists that changed the way we all think about cholesterol. He developed an assay that allows the quantification of low-density lipoprotein particle size and concentration (known to the wider world mostly as just "LDL cholesterol") based on a technique which determines the size of the particle based on physics...meaning the speed at which it flies through the air. This test which has not yet been fully adopted into all clinical practice in the manner that the total LDL cholesterol test has is known as the "ion mobility" test. In this episode, Rhonda and Ron discuss what HDL and LDL cholesterol are, what they do in the body, critical distinctions between the particle sizes of LDL and HDL, and how they play a role in heart disease. We talk about what small, dense LDL particles are, how they form, what effect eating saturated fat versus refined carbohydrates have on LDL particle size and heart disease risk and more generally what the main risk factors for heart disease are. Statins and Memory Loss? Forget About It

A new study found that, contrary to previous reports, statins were not likely a cause of memory loss. “People who have high cholesterol should be on statins,” said lead study author Brian L. Strom, MD, chancellor of Rutgers Biomedical and Health Sciences, in a press release. “People shouldn't steer away from the drug because of false fear of memory problems." About 1 in 4 deaths in the US are caused by heart disease, according to Dr. Strom and team. Statins, such as atorvastatin (brand name Lipitor) or rosuvastatin (Crestor), are commonly prescribed to reduce this risk. Statins are a class of drugs that lower cholesterol. Dr. Strom and team looked at the link between statins and memory loss. These researchers compared 482,542 patients taking statins to 482,543 patients who weren’t on statins. About 5 percent of the second group were taking nonstatin cholesterol-lowering drugs, such as cholestyramine (brand name Prevalite). Dr. Strom and team found that most of the patients in the first group reported some memory loss during the first 30 days of taking statins. However, most of the patients taking nonstatins also reported memory loss. These findings suggest that these drugs may not actually cause memory loss — but rather may lead to a “detection bias." In other words, patients may be more likely to notice existing memory issues when they start taking a new drug — because they are more focused on their health, Dr. Strom and team said. “When patients are put on statins or any new drug, they’re seen more often by their doctor, or they themselves are paying attention to whether anything is wrong,” Dr. Strom said. “So if they have a memory problem, they’re going to notice it. Even if it has nothing to do with the drug, they’re going to blame it on the drug."

Mayo Clinic Experts Advise Caution with New Cholesterol Drugs- PCSK-9 Inhibitors

In a viewpoint released today in The Journal of the American Medical Association Cardiovascular Special Issue, Mayo Clinic endocrinologists and researchers are warning against the “premature and widespread adoption” of a new type of drug that reduces cholesterol, PCSK-9 inhibitors. They argue that little is known about their long-term safety and efficacy on outcomes that matter to patients. Given the long track record of efficacy and safety and the low cost of statins, they think that it is premature to adopt PCSK-9 inhibitors as alternatives to statins. The Food and Drug Administration has recently approved the new class of drugs for patients with high LDL-cholesterol who also have familial hypercholesterolemia or with clinical cardiovascular disease who are seeking secondary prevention or cannot tolerate statins. The Mayo experts are predicting four situations that will fuel abandoning statins in favor of the PCSK-9 inhibitors: Patients who have muscle problems or other symptoms suggestive of statin intolerance. Patients who have a heart attack or a stroke while using a statin. Patients who may not to be taking statins every day. Patients who have cholesterol levels that remain high despite statins.
