Roche have withdrawn Roaccutane/Accutane from sale and they deny this withdrawal is linked to costly litigation in the US, due to Inflammatory Bowel Disease believed to be caused by isotretinoin. This video and other personal accounts together with talks by Professor David Healy, Professor Heather Ashton, Dr Andrew Herxheimer at our conference can be viewed at http://vimeo.com/user1816628/videos In this video, Jon Medland, who bravely agreed to be interviewed at the APRIL charity conference in 2008, gives his moving account of the sequence of events, following his son's decision to take Roaccutane to deal with moderate acne on his back, is just one example of a suicide linked to adverse reaction (ADR) probably triggered by medication. In a recent review of Yellow Card reports of ADRs by patients overseen by Professor Anthony Avery of Nottingham University and 5 other universities, for which I was Chair of the Advisory Group: Reports of suicidal thoughts was the 11th most commonly reported adverse reaction, almost as common as reports of rashes reported as linked to several different medications. I recently attended the inquest of a young woman who has been prescribed Roaccuate plus Dianette (Diane-35) and Prozac (fluoxetine) and Zopiclone a sleeping pill. All drugs linked to side effects of suicidal thoughts, actions and completed suicide. The Coroner only focussed on one drug and chose to ignore the others, probably because he unaware to the risks of each one of the four drugs, let alone the lethal combination of taking them all together. There are shortly to be inquests for young women who took their own lives, one in Australia who's suicide was linked to Dianette also known as Diane-35 or cyproterone-acetate and ethinylestradiol. The other in the UK linked to psychiatric drugs given following a trip abroad when the young woman used anti-malaria drug Malarone. All antimalarials are linked to psychiatric ADRs. My experience with Accutane. I only covered the big stuff: Cognitive impairment, anhedonia, IBS symptoms, etc. My skin's ability to heal has definitely been affected as well, and I also seem more prone to aches and pains.

I felt compelled to share my opinion on depression as a potential side effect of accutane. I still encourage looking into accutane toanyone who has suffered serious acne and tried everything. However, accutane is a serious drug and there are some serious side effects to keep in mind. This isn't meant as a personal attack on anyone. I just wanted to share some thoughts : )

I wanted to have a chat about Accutane side effects, and answer a question I keep getting about taking Accutane if you have mental health issues such as depression or anxiety. Should you take it? Let's chat about it. Hey guys! So we're at month 5, and that's pretty awesome! I will keep doing monthly updates on my progress and changes until I finish this medication. I'm not sure when I'll finish Accutane, it depends on the state of my skin and what my dermatologist thinks is best. I am on 20mg of Accutane (Isotretinoin), but I take a generic version of Oratane. I also take 25mg of Spironolactone every day, and have a vegan diet. Accutane is a potent oral drug from Hoffman-LaRoche that has been proven highly effective for reducing acne symptoms dramatically after only one course of 15-20 weeks. http://hotprdcts.com/acnenomore With its undoubting success at battling even the toughest cases of acne and ever-growing popularity among teens as a miracle drug, why is Accutane still considered the most controversial pharmaceutical on the market? The answer: a documented list of severe and sometimes permanent side effects. Although some of the side effects associated with the use of Isotretinoin are mild and tolerable (dry lips, dry eyes, nosebleeds, extremely dry skin, increased sensitivity to the sun and thinning hair) there is in fact a long list of serious side effects that have been reported due to the use of Accuttane which persisted even after the drug therapy was stopped. Among those are: Severe aches and pains in the lower back and large joints, severe allergy reactions that include moderate rashes and difficulty to breath, vision problems, hearing loss, permanent liver damage, persistent migraines and headaches and skeletal hyperostosis which is an excessive bone growth that occurs along the sides of the spine. But perhaps, the two most bothersome of all Accutane side effects are its potential to cause severe birth defects if taken by pregnant women or women that became pregnant while taking the drug and the suggested link between the intake of Accutane and the changes in mood, severe depression and suicidal thoughts. Since, Isotretinoin can be harmful to the fetus, it should not be used during pregnancy and women should avoid pregnancy for at least a month after the use of Accutane. Additionally, because of its severe side effects, nursing mothers must avoid the drug completely. Although researchers still argue whether Isotretinoin causes depression or not, Roche the manufacturer of Accutane, openly shares information claiming that Accutane and its generic, isotretinoin, can alter mood: "Synthetic derivatives of vitamin A, can negatively impact brain chemicals such as dopamine and serotonin." The FDA website warns: "All patients treated with isotretinoin should be observed closely for symptoms of depression or suicidal thoughts, such as sad mood, irritability, acting on dangerous impulses, anger, loss of pleasure ... mood disturbance, psychosis, or aggression." Moreover, statistics show that between 1989 and 2003, there were 216 reported drug-linked suicides among U.S. teens younger than 18. Of these, 72 suicides were linked to Accutane (isotretinoin). The next highest number of suicides in that time period -- 55 -- involved Prozac (fluoxetine). Accutane works by reducing the size and output of the sebaceous glands and thus stops excess oil production. The problem is that Accutane damages your skins sebum glands and liver function in the process. By the time you stop taking the drug, your skin oil glands will return to function but your liver will not be able to properly balance your hormones and prevent the over production of sebum. Isotrteinoin also normalizes your keratinization (the shedding of dead skin cells), improves fat metabolism and reduces inflammation. But all of these benefits can be easily achieved using all natural dietary, supplementation and skin care alternatives and without the side effects. For example: Foods that are rich in Vitamin A and Beta Carotene can help reduce the secretion of oil by the sebaceous glands. Simple home made peels made of Papaya can easily replace the peeling action of Isotrerinoin and sleep optimization, and regular exercising including vitamin, C, E, Zinc and Colostrum can boost the immune abilities of the body and thus reduce inflammation. Accutane is not the solution to acne. Not only does it treat the symptoms of the disease and not the internal cause, stopping your skin's natural oil production, can in most cases, result in extra dryness combined with the acceleration of the skin's aging process (yes! your skin will look older). Moreover, Accutane severely damages your liver function among other side effects, which in turn destroys your delicate inner balance and your natural healing abilities. A Canadian study confirms that Accutane increases depression risk. The study found that Accutane more than doubles the risk of depression. The study is the first controlled investigation to find a statistically substantiated link between isotretinoin (the active ingredient in Accutane) and depression, Dr. Anick Berard, from CHU Sainte-Justine Research Centre in Montreal, and colleagues stated in a report in the Journal of Clinical Psychiatry. The researchers studied 30,496 people from Quebec who had at least one prescription of Accutane between 1984 and 2003. Among these people, 126 had a reported depression case. The researchers looked for Accutane use five months before the reported depression case (risk period) and compared it to a five-month control period. After adjusting for potential risk factors for depression, the study found that exposure to Accutane increases the risk of depression by 2.6 times. The research report finishes by recommending that "current guidelines should possibly be modified to include psychiatric assessments of patients prior to and during isotretinoin therapy." There are two known pathways Accutane can lead to depression: lower availability of serotonin and decreased brain activity in the areas that mediate depression. Earlier research has shown that Accutane reduces the availability of the neurotransmitter serotonin. Serotonin is also known as the "feel good" hormone. Low levels of serotonin have been linked consistently to many psychiatric symptoms, such as aggression, anxiety disorders, and suicidal ideation. Notes of Precaution * Because side effects are common and can be serious, this drug should only be used by providers experienced with its issues. * Most patients are advised to have blood tests monthly during Isotretinoin treatment. * Notify your doctor if you are allergic to Isotretinoin, any other medications, parabens, or any of the ingredients in Isotretinoin capsule. * Notify your doctor if you or anyone in your family has a history of suicidal thoughts or depression, mental illness, or inflammatory bowel disease. * Avoid unnecessary or prolonged exposure to sunlight and to wear protective clothing, sunglasses, and sunscreen as Isotretinoin may make your skin sensitive to sunlight. * Be aware that Isotretinoin can cause changes in your thoughts, behavior, or mental health. * Isotretinoin may cause eye dryness, making wearing contact lenses uncomfortable both during and after treatment. * Isotretinoin may also impair night vision. * Avoid hair removal by waxing, laser skin treatments, and dermabrasion during treatment and up to six months afterward. * Isotretinoin should be stored at room temperature and be kept out of the reach of children.

This movie shows my progression over a 7 month period using the prescribed drug Accutane (50mg/day). Luckily, it did not cause any mental side-effects (thoughts of suicide etc.) but it did yield some physical side-effects including dry skin/lips, some aches, dry eyes, bloody nose etc. This is my personal experience and it might be different for others. Highly recommended to anyone suffering from acne if you've tried over the counter products already.. I tried *everything* the pharmacist recommended without luck before accutane. Be patient, stick with it, and before you know it, you'll have clear skin! Good luck :)

Accutane is a potent oral drug from Hoffman-LaRoche that has been proven highly effective for reducing acne symptoms dramatically after only one course of 15-20 weeks. With its undoubting success at battling even the toughest cases of acne and ever-growing popularity among teens as a miracle drug, why is Accutane still considered the most controversial pharmaceutical on the market? The answer: a documented list of severe and sometimes permanent side effects. Although some of the side effects associated with the use of Isotretinoin are mild and tolerable (dry lips, dry eyes, nosebleeds, extremely dry skin, increased sensitivity to the sun and thinning hair) there is in fact a long list of serious side effects that have been reported due to the use of Accuttane which persisted even after the drug therapy was stopped. But perhaps, the two most bothersome of all Accutane side effects are its potential to cause severe birth defects if taken by pregnant women or women that became pregnant while taking the drug and the suggested link between the intake of Accutane and the changes in mood, severe depression and suicidal thoughts. Since, Isotretinoin can be harmful to the fetus, it should not be used during pregnancy and women should avoid pregnancy for at least a month after the use of Accutane. Additionally, because of its severe side effects, nursing mothers must avoid the drug completely. Although researchers still argue whether Isotretinoin causes depression or not, Roche the manufacturer of Accutane, openly shares information claiming that Accutane and its generic, isotretinoin, can alter mood: "Synthetic derivatives of vitamin A, can negatively impact brain chemicals such as dopamine and serotonin." The FDA website warns: "All patients treated with isotretinoin should be observed closely for symptoms of depression or suicidal thoughts, such as sad mood, irritability, acting on dangerous impulses, anger, loss of pleasure ... mood disturbance, psychosis, or aggression." Moreover, statistics show that between 1989 and 2003, there were 216 reported drug-linked suicides among U.S. teens younger than 18. Of these, 72 suicides were linked to Accutane (isotretinoin). The next highest number of suicides in that time period -- 55 -- involved Prozac (fluoxetine). Accutane works by reducing the size and output of the sebaceous glands and thus stops excess oil production. The problem is that Accutane damages your skins sebum glands and liver function in the process. By the time you stop taking the drug, your skin oil glands will return to function but your liver will not be able to properly balance your hormones and prevent the over production of sebum. Isotrteinoin also normalizes your keratinization (the shedding of dead skin cells), improves fat metabolism and reduces inflammation. But all of these benefits can be easily achieved using all natural dietary, supplementation and skin care alternatives and without the side effects. Hey guys, For the last few weeks I've been heavily debating going on this medication. If you've been following my journey so far, you'd know this wasn't a decision I took lightly. I have a bad history with this medication, it doesn't fit with my personal values, and I'm scared. But a lot of you showed me support, love and encouragement when I talked about it, so I feel more confident in progressing with my treatment. I know a few of you also said "Please don't!!" and honestly - I'd rather not take it. I have put this off a long time because I'd rather not take such a heavy, controversial medication. But the way I see it is this: My acne may subside and become less over time, but it may never fully go away. If this drug makes it go away for good, then I want to at least give it a go. I'm going to be taking notes of my progress, and talking openly about my emotions and side effects on my channel while taking it. If I have a bad episode, I'll know, and I will act. Isotretinoin treatment Part 2: Relapse, side effects, monitoring, depression Risk factors for relapse are: Young teenagers Back and chest acne Sinus tract lesions Excess granulation tissue Androgen excess There are several risk factors for recurrence. Patients with a high probability of relapse or failure with isotretinoin have been identified. Young teenagers with nodulocystic acne will clear with isotretinoin therapy but the chance of recurrence within 2 years is high. They may require 2 to 4 courses of therapy to maintain a durable remission. Patients with acne of the chest and back relapse more frequently. Patients who form epithelial sinus tracts have a poor response to therapy. These linear cystic structures extend with time as they propagate through the dermis. Isotretinoin and intralesional steroids may clear the lesions but surgical excision is often required. Female patients with androgen excess states present with acne. They relapse within 6 months of stopping isotretinoin. Oral contraceptivesand spironolactone are appropriate treatments. Women with high abdominal fat content called android obesity have waist circumferences greater than 80 cm and waist-to-hip ratios greater than 0.8. They look like Humpty-Dumpty with large upper bodies, flat buttocks and thin legs. They have a high incidence of diabetes, increased triglycerides and uterine cancer. Side Effects Teratogenicity Increased cholesterol and triglyceride Dry lips and nose bleeds Dry skin and eczema Contact lens users Headaches Bone pain Granulation tissue formation The potential to cause major fetal abnormalities is the major side effect of isotretinoin therapy. Woman who are pregnant or breast feeding do not take the drug. Isotretinoin is prescribed only by physicians who have registered with the drug manufacturer. A detailed manufacture created system for pregnancy prevention is an important part of the treatment experience. Increased cholesterol and triglycerides are a common problem. People who develop hypertriglyceridemia during isotretinoin therapy are at increased risk for future hyperlipidemia. Triglycerides and cholesterol levels are measured each month. All patients experience dry lips. Nose bleeds are more common in patients with a past history of nasal bleeding. Lotions and petrolatum based lubricants applied to the lips inserted into the nose helps. Dry skin and eczema commonly occurs in the winter especially in atopics. Contact lens users may experience dry eyes and need to use artificial tears or wear glasses. Headaches are uncommon but can be severe. Pseudotumor cerebri has been reported. Musculoskeletal symptoms are common in athletes. Bone pain, muscle and tendon injuries can occur. Some lesions are unstable, break down. bleed and crust. Early treatment with high doses of isotretinoin can stimulate granulation tissue that leads to the development of pyogenic granuloma-like lesions. Initial treatment consists of cool wet compresses, topical and or oral steroids. Start with very low doses of isotretinoin 0.1 mg/kg/day and increase slowly to 0.5 mg/kg. Explosive, highly inflammatory cystic acne is treated with oral steroids. Isotretinoin is started only after inflammation has been controlled. Abnormal liver function test are rare. Skeletal hyperostosis of apparently no consequence are sometimes seen on radiographs. Monitoring A pre-treatment CBC, fasting triglyceride, LFTs and pregnancy tests are obtained and repeated 4 weeks later. Triglyceride tests are repeated until the response to isotretinoin is established. Monthly pregnancy testing should continue until 1 month after cessation. The Depression Controversy To date, no causal relationship between isotretinoin and psychiatric adverse events has been established. A review of the existing literature concluded that there was no evidence to support a causal connection of the drug to depression or suicide. Adverse Drug Reaction reports suggest that depression and suicide or suicide attempt rates are well below those of the general population. As their skin improves, isotretinoin patients” moods also tend to improve, rather than the opposite.

Today I wanted to give you an update on what I'm taking, how my skin is, if I've had any Accutane side effects, and chat about mental health. Hey guys, So it's been a bit over a week since I started Accutane, and since a lot of you mentioned that you were also taking this medication, I figured that having chats about it is probably a good way forward while I'm taking it. So a few things that I outlined in this video, plus a bit more info: - I am in my second week of taking Accutane. I'm still only on 10mg, but will begin to introduce 20mg next week. - I am also taking 25mg of Spironolactone, a medication that is meant for high blood pressure (I think...) but has been known to improve acne due to regulating hormonal imbalances. I think I've been on it about a month, and remain neutral on its effectiveness since it can take a few months to work. But I'm definitely noticing the diuretic effect! - In addition, I'm still taking Prednisone. This was prescribed to me by my dermatologist, and is just to help calm my severe acne down. I started on a full tablet, and now I'm on half a tablet. I will stop taking Prednisone in a few days. - I have found a few scientific journals about Accutane and the effects of this medication on people in different situations, but I haven't found any specific stories relating to deaths. There are mentions of depression and suicide in some papers I've read, but nothing that formally links any of it together. I cannot find this apparent 10 year study that proves they are not linked, and that's the one I want to see! - TRUST YOURSELF. Write in a journal, understand your feelings and emotions when you're in a calm state and recognise when you're feeling off. I think that if you're young and taking Accutane, then it can be incredibly hard to manage your emotions. Don't dismiss your mental health - tell someone if you're not okay. I went on Accutane 6 years ago... and am finally sharing my experience! It was a very difficult drug to be on... my lips would bleed and my skin looked worse than it ever has during treatment. While difficult, I am glad I followed through because I haven't had any issues with acne since completing my second course of Accutane. I wanna hear about your crazy Accutane stories in the comments!
