I think it's really important to know how therapy really works and why therapists ask you the questions they do. Most therapist will ask you at the beginning "what brought you here today" and try to work out what your main goal for therapy is. This helps us better create a plan. From there we begin the assessment process. This could be diagnosis, figuring out what you have worked on before, your school and family history, etc. While doing this, we will also be giving you homework and trying out new tools to see what works for you and what doesn't. The main goal for any therapist is that you don't need to see us anymore. That being said, know that coming back to therapy is normal and most therapists have an open door policy :) So let your process be just that and go back if you need to. The Centre for Addiction and Mental Health (CAMH) is looking for ways to make treatment more accessible. Researchers from the CAMH BPD clinic are examining whether patients could benefit from a shorter length of treatment which could reduce wait times in existing programs.

First I want to make sure you all understand that this is a personality disorder that we are very careful about diagnosing. There are many times in our lives when we may meet this criteria. It could be because we young and therefore unable to care for ourselves, or we may have a disability which makes it difficult for us to care for ourselves. All of these situations and circumstances need to be considered before making this diagnosis. That being said, many of us may struggle with Dependent Personality Disorder, so I will go over what it is and how it is diagnosed. Dependent personality disorder is a mental health condition in which people struggle with the fear that they cannot care for themselves. They honestly believe that if they had to, they couldn't. So they look to other people in their life to take care of them, tell them what to do, when to start a project, etc. DSM-5 (1) has difficulty making everyday decisions without an excessive amount of advice and reassurance from others. They struggle to make any decision because they honestly fear that they won't make the right one. So they look to others for support with it. (2) needs others to assume responsibility for most major areas of his or her life. They feel like they cannot care for themselves, so they give the responsibility over to someone else that they think could do a better job. (3) has difficulty expressing disagreement with others because of fear of loss of support or approval. Support or approval is so important to us, that we don't want to fight in case that scares them away. (4) has difficulty initiating projects or doing things on his or her own (because of a lack of self-confidence in judgment or abilities rather than a lack of motivation or energy). This can be hard in school or work because they are unable to do anything on their own or take the lead on a project. (5) goes to excessive lengths to obtain nurturance and support from others, to the point of volunteering to do things that are unpleasant. (6) feels uncomfortable or helpless when alone because of exaggerated fears of being unable to care for himself or herself. (7) urgently seeks another relationship as a source of care and support when a close relationship ends. Like I have stated above, they worry so much about being able to care for themselves, that if one caretaker is gone, they will frantically find someone to fill that spot so that they can continue their life. (8) is unrealistically preoccupied with fears of being left to take care of himself or herself. This is a core belief that anyone with DPD has, and this is why they try so hard to keep a caretaker in their life and give them the responsibility of making their life decisions. If you are struggling with this, know that you are not alone! Safe and Effective Management of Pain and Addiction was presented on January 16, 2018; by Dr. Carl Christensen, MD, Ph.D., D-FASAM; and Dr. Mark A. Weiner, MD, D-FASAM. The Institute of Medicine estimates around a 100 million Americans suffer with chronic pain, and it’s estimated that about 10% of our population has or has had a substance use disorder. Both chronic pain and substance use disorders are major public health challenges, and treating concurrent pain and substance addiction is especially challenging. Common prescribing practices intended to provide relief of acute and chronic pain can trigger relapse in people with substance use disorders and have also fueled an epidemic of opiod misuse, addiction and overdose death. People with pain deserve relief, and the good news is there are many strategies for both acute and chronic pain management that are safe and effective for people at risk of or in recovery from substance use disorders. This presentation discusses various methods of pharmacologic and non-pharmacologic pain management and their relative risks and benefits, and describes creative approaches to effective pain relief for people in recovery from substance use disorders. The presentation includes a short demonstration of naloxone resuscitation. This presentation is part of the Dawn Farm Education Series, a free, annual education series developed to provide accurate, helpful, hopeful, practical, current information about chemical dependency, recovery, family and related issues. The Education Series is organized by Dawn Farm, a non-profit community of programs providing a continuum of substance use disorder treatment and recovery support services. Carl Christensen, MD, PhD, FACOG, D-FASAM, ABAM: Dr. Christensen is a Clinical Associate Professor at the Wayne State University School of Medicine and has a private medical practice in Ann Arbor, Michigan. Dr. Christensen specializes in the treatment of addiction and of chronic pain, especially pelvic pain. He obtained his MD and PhD in Biochemistry at Wayne State University School of Medicine and did his residency in Obstetrics and Gynecology at Hutzel Hospital. He then completed a Fellowship in Gynecologic Oncology at Duke University Medical Center. He is certified in Addiction Medicine and is a Fellow of the American Society of Addiction Medicine. Dr. Christensen is the current Medical Director of the James Wardell Women’s Recovery Center, the Medical Director at the Tolan Medical Research Clinic in the Department of Psychiatry at WSU, and the Medical Director for Dawn Farm. He is a member of the Advisory Board of Families Against Narcotics in Wayne County, Michigan. He is the past president of the Michigan Society of Addiction Medicine and the current Medical Director of the Michigan Health Professional Recovery Program. Dr. Christensen has received numerous teaching awards. He has been named one of the “Top Docs” in Addiction Medicine in Hour Magazine since 2006.

There are many alcohol related disorders in the DSM: alcohol use disorder, alcohol intoxication, alcohol withdrawal, other alcohol induced disorders & unspecified alcohol related disorder. The last two disorders are sort of catch all types of disorders if you don't quite meet the criteria for one of the others listed. Now that you know the landscape of alcohol disorders, let's get into alcohol use disorder: There are 11 criteria (or symptoms) that the DSM discusses, and in order to be diagnosed with this, you must have at least 2 of them. These symptoms or criteria must have occurred within the last 12 months. 1. Alcohol is often taken in larger amount or over a longer period than was intended. 2. There is a persistent desire to or unsuccessful effort to cut down on alcohol consumption or control it's use. 3. We spend a lot of time in activities necessary to obtain alcohol. 4. Craving or intense urge to use alcohol. 5. Recurrent alcohol use resulting in a failure to fulfill major role obligations at work, school or home. 6. Continued alcohol use despite having persistent or recurrent social or interpersonal problems, caused or exacerbated by the effects of alcohol. 7. Important social, occupational, or recreational activities are given up or reduced due to alcohol use. 8. Recurrent alcohol use in situations in which it is physically hazardous. 9. Alcohol use is continued despite knowledge of having a persistent or recurrent physical or psychological problem that is likely to have been caused or exacerbated by alcohol use. 10. Tolerance: when it takes more alcohol than it used to in order to get the same effect. 11. Withdrawal: please be careful with this! It can be life threatening to quit cold turkey without the supervision of a doctor. So please reach out for help prior to just stopping on your own. Treatment options: We could start by going to a detox house in order safely stop drinking. Then we can step down into a hospital program or inpatient treatment center. I say those separately because inpatient treatment centers can be in homes and much more warm and therapeutic than hospital programs. After that we can move out into a recovery home or halfway house and do day treatment (this allows us to slowly transition back to into life and they believe it lowers relapse rates). Then we can step down into outpatient therapy and group therapy. This could be seeing someone like me and going to AA or another group therapy available. If you are not the one suffering from this, but are close to someone who is, Al-Anon is a great resource as well. We may not realize it, but by living with someone who is an alcoholic, we have our own struggles and issues we need to work on. Al-Anon is a great place to do that. They even have Alateen for those who are younger. Those who view this lecture will learn: - The diagnostic framework for identifying when attachment-based parental alienation is, and when it is not, responsible for producing the parent-child conflict involved in high-conflict divorce - The four key treatment phases necessary for the effective treatment and resolution of an attachment-based model for parental alienation - The essential features of the child’s psychological experience surrounding parental alienation that are key to the child’s therapy and a restoration of the child’s affectional bond with the currently targeted-rejected parent - The role of the alienating parent in the child’s treatment and recovery, and approaches to managing and working with the alienating parent during and following treatment Bio: Dr. Craig Childress is a licensed clinical psychologist specializing in child and family therapy, parent-child conflict, marital conflict, and child development. Dr. Childress has an additional background specialty in early childhood mental health involving the attachment system and the neuro-development of the brain during childhood. Prior to entering private practice, Dr. Childress served as the clinical director for a children’s assessment and treatment center operated under the auspices of California State University, San Bernardino. He also was on medical staff at Children’s Hospital of Orange County as a pediatric psychologist, where he served on a collaborative project with the UCI Child Development Center regarding the early identification of ADHD in preschool-age children. In addition to his private practice, Dr. Childress currently teaches graduate-level courses in child development, diagnosis and psychopathology, psychotherapy and treatment planning, and research methodology through the University of Phoenix. Substance abuse in persons with severe mental illness (dual disorders) is a common problem for many consumers that contributes to a worse course of illness. Recent advances have shown that treatment is most effective when it integrates both mental health and substance abuse services into a cohesive package that is delivered in a manner informed by the consumer's motivation to change and his or her level of involvement in the treatment system. This presentation will briefly highlight the problem of dual disorders, and then describe the fundamental ingredients of integrated treatment. Special emphasis will be placed on the stages of treatment. This webcast provides a learning aid for clinicians to match their interventions to consumers' individual motivations to work on their substance abuse or mental health problems. References Mueser, K. Counseling Theories assert that problems stem from ineffective relationships or thoughts in adulthood. Personality Theories speculate that distress stems from more innate, long standing problems often starting in childhood Learning Theories emphasize the fact that distress and behavior is learned from exposure to rewards and punishments Theories: Person Centered Humans are good and forward moving unless they are blocked Blockages often occur from a lack of unconditional positive regard which leads to low self esteem and low self efficacy By creating a nurturing, positive environment, people will naturally move in the right direction. 6 necessary conditions required for change: Therapist-Client Psychological Contact: a relationship between client and therapist Theories: REBT/CBT Focuses on changing the current evaluations and/or reactions Distress is caused by a combination of the event and the person's perception of the event By using the A-B-Cs, people can evaluate their beliefs and reactions (consequences) to events. A-B-C CBT/REBT: Irrational Thoughts Focus on the present Avoid discussing symptoms and complaints. Focus on what counselees can do directly-act and think. Spend less time on what they cannot do directly: changing their feelings and physiology. Avoid criticizing, blaming and/or complaining Remain non-judgmental, but encourage people to ask: Is what I am doing getting me closer to the people I need? Focus on specifics. Who are counselees are disconnected from Help them make specific, workable plans to reconnect with the people they need. Follow through on what was planned by helping them evaluate their progress. Be patient and supportive but keep focusing on the source of the problem, disconnectedness. Personality Theories: Psychoanalysis The conscious mind is what you are aware of at any particular moment, your present perceptions, memories, thoughts, fantasies, feelings Working closely with the conscious mind is what Freud called the preconscious, what we might today call "available memory." The largest part by far is the unconscious. It includes all the things that are not easily available to awareness The id, the ego, and the superego Personality Theories: Erickson We develop through a predetermined eight stages. Progress through each stage is determined by our success in all the previous stages. Each stage involves certain developmental tasks If a stage is managed well, we develop a certain virtue or strength Learning Theories: Classical Conditioning Learning Theories: Operant Conditioning Operant conditioning is the use of consequences to modify the occurrence and form of behavior (Treatment planning) Reinforcement is a consequence that increases a behavior. +/- Punishment is a consequence that decreases a behavior. +/- Extinction is the elimination of a behavior by removing the reward. Learning Theories: Social Learning People learn from one another, through observational learning, imitation, and modeling. People can learn by observing behavior and the outcomes of those behaviors. Learning can occur without a change in behavior. Cognition, awareness and expectations of future consequences can have a major effect on the behaviors that people exhibit. Reciprocal causation: The person, the behavior, and the environment can have an influence on each other. 4 conditions that are necessary before an individual can learn Attention: the person must first pay attention to the model/situation Retention: the observer must remember the behavior that has been observed. Rehearsal: the third condition is the ability to replicate the behavior. Motivation: Learners must want to demonstrate what they have learned.

Descriptions Signs of Bipolar Disorder: When it comes to mental illness, there are plenty of stereotypes. But in reality, mood disorders can be hard to pinpoint—particularly in people with bipolar disorder symptoms. Here are 10 signs that mood problems may be due to more than a quirky or difficult personality. 1. always happy Bipolar disorder is characterized by up-and-down episodes of mania and depression. During a manic phase, some patients can have a total break from reality. But hypomania, which is also a symptom of the disorder, is a high-energy state in which a person feels exuberant but hasn’t lost his or her grip on reality. A person’s mood can be elevated, they may have a lot of energy and creativity, and they may experience euphoria. This is the "up" side of bipolar disorder that some people with the condition actually enjoy—while it lasts. 2. inability Having a house full of half-completed projects is a hallmark of bipolar disorder. People who can harness their energy when they are in a hypomanic phase can be really productive. Those who can’t often go from task to task, planning grand, unrealistic projects that are never finished before moving on to something else. 3. depression A person who is in a bipolar depressive state is going to look just like someone who has regular depression. Unfortunately, typical antidepressants alone don't work well in patients who are bipolar. They can even make people cycle more frequently, worsening their condition, or send someone into a break-with-reality episode. "Antidepressants can be downright dangerous in people with bipolar because they can send them into mania," he says. 4. irritability Some people with this condition suffer from "mixed mania," where they experience symptoms of mania and depression at the same time. During this state, they are often extremely irritable. Everyone has badd days, which is one reason this kind of bipolarity is much harder to recognize. 5. rapid speech Some people are naturally talkative; we all know a motormouth or Chatty Cathy. But "pressured speech" is one of the most common symptoms of bipolar disorder. This kind of speech occurs when someone is really not in a two-way conversation. The person will talk rapidly and if you try to speak, they will likely just talk over you. 6. work problems People with this disorder often have difficulty in the workplace because so many of their symptoms can interfere with their ability to show up for work, do their job, and interact productively with others. In addition to having problems completing tasks, they may have difficulty sleeping, irritability, and an inflated ego during a manic phase, and depression at other times, which causes excessive sleeping and additional mood problems. A lot of the workplace problems can be interpersonal ones, Dr. Malone says. 7. alcohol man About 50% of people with bipolar disorder also have a substance abuse problem, particularly alcohol use, Dr. Bearden says. Many people will drink when they are in a manic phase to slow themselves down, and use alcohol to improve their mood when they are depressed. 8. affair When they are in a manic phase, people with bipolar disorder can have an inflated self-esteem. "They feel grandiose and don't consider consequences; everything sounds good to them. Two of the most common types of behavior that can result from this are spending sprees and unusual sexuall behavior. 9. lack sleep People with this condition often have sleep problems. During a depression phase, they may sleep too much, and feel tired all the time. During a manic phase, they may not sleep enough—but still never feel tired. Even with just a few hours of sleep each night, they may feel great and have lots of energy. 10. flight of ideas This symptom may be something that is hard to recognize, but it occurs frequently when someone is in a manic phase. People feel like their mind is racing and that they can't control or slow down their thoughts. This flight of ideas sometimes occurs with pressured speech. People with bipolar may not recognize or admit that their mind is racing out of control, says Dr. Bearden. It depends upon really a number of factors. It also depends upon the person who is being intervened upon and their decision to embrace recovery in their life. It takes an enormous amount of strength and courage to embrace recovery and so the person who, it's really up to the person who is trying to get clean and sober to get and to stay sober. We also know that there are two different kinds of interventions. The one kind is very unconfrontational and it acts as a surprise to the person. So in other words the person doesn't know that the intervention is happening. The interventionist is hired and contacts the persons family members and the other significant people in the person's life and then they surprise them and they intervene on them. The other kind of intervention is more what's called invitational so that the person who's suffering from the disease of addiction or substance abuse knows that this is happening and it's a much slower, more involved process. So the two are very different and it all depends on the person who's doing the intervention. And it also depends upon the person who is being intervened upon - what their personality is like, how they would receive all of this. So there are lots of variables that go into this. But again we know, I know from the work that I do that interventions can be highly, highly effective.

The good news is that women in that age group respond quickly. In 3 to 6 months things start to turn around, assuming that they get their food under control, because usually at that age there’s something happening with food. But at that stage in our lives, our mid 40s to 50s, diet changes can do probably three-quarters of the job. It’s just getting a person to the point where they’re able to implement the diet changes. We also give a short introduction on the core elements of evidence based addiction treatment (CBT) and suitable Schema Therapy techniques. We will argue that the Self-Soother can be a hidden mode, illustrated by case examples of Cluster C-patients in different treatment settings, such as General Mental Health, Addiction and Forensic settings. We will present different Mode Conceptualizations. The practical part of the workshop consists of: A demonstration of a mode dialogue with the Self-Soother; here we will focus on the specific pittfalls with cluster C patients that make them tend to avoid behavior change.
