What is a Treatment Plan & how do we make one? Today I talk about treatment plans and what therapists really do behind the scenes. I think it's really important to know how therapy really works and why therapists ask you the questions they do. Most therapist will ask you at the beginning "what brought you here today" and try to work out what your main goal for therapy is. This helps us better create a plan. From there we begin the assessment process. This could be diagnosis, figuring out what you have worked on before, your school and family history, etc. While doing this, we will also be giving you homework and trying out new tools to see what works for you and what doesn't. The main goal for any therapist is that you don't need to see us anymore. That being said, know that coming back to therapy is normal and most therapists have an open door policy :) So let your process be just that and go back if you need to. The Preventure program is a substance abuse prevention program that targets four groups of adolescents at risk: impulsive, sensation seeking, hopeless, and anxiety sensitive youth. Using psychoeducational, cognitive, and behavioral strategies delivered to personality-specific groups of youth in the schools, the Preventure program has been shown in a set of randomized controlled trials (RCTs) to have lasting effects on adolescents’ mental health and addictive behaviors. For example, the anxiety sensitivity intervention reduces panic attacks, drinking behavior, and alcohol-related problems, and the sensation seeking intervention is particularly effective for reducing and preventing growth in youth cannabis use. In this webinar, Dr. Stewart will begin by introducing the background theory and research that stimulated the development of the Preventure program. She will then provide an overview of the content and structure of the intervention. Finally, she will review the established evidence base for the program and plans for next steps in the Preventure research program. Preventure has been reviewed and recommended by SAMSHA’s National Registry for Evidence-based Programs and Practices (NREPP) and the U.S. Surgeon General’s Report on Addiction. She is well known for her research on psychological factors (i.e., personality, motives) contributing to alcohol abuse, pathological gambling, and the comorbidity of emotional and addictive disorders. Dr. Stewart is interested in applying the findings of her research to provide evidence to guide clinical interventions for individuals living with different emotional and addictive disorders. Dr. Stewart, along with Dr. Patricia Conrod co-developed the school-based substance abuse prevention program, Preventure, which is now used in several countries around the world (e.g. the UK, Australia, the Netherlands). Dr. Stewart is a member of the Scientific Advisory to the Canadian Centre on Substance Abuse, and Editor-in-Chief for the Journal of Gambling Issues. Dr. Stewart founded the Centre for Addiction Research at Dalhousie (CARD), a virtual centre at Dalhousie fostering collaborations among faculty members conducting research on addiction, and is on the steering committee of the Quebec-Maritimes node of the CIHR-funded Canadian Research Initiative in Substance Misuse (CRISM). She is Co-Director of Research in the Department of Psychiatry, and co-directs the new MSc program in psychiatry research at Dalhousie. Dr. Stewart receives funding from several research agencies including the Social Sciences and Humanities Research Council (SSHRC), the Canadian Institutes of Health Research (CIHR), the Nova Scotia Health Research Foundation (NSHRF), the National Center for Responsible Gambling (NCRG), and the Manitoba Gambling Research Program (MGRP).

What is Dependent Personality Disorder? First I want to make sure you all understand that this is a personality disorder that we are very careful about diagnosing. There are many times in our lives when we may meet this criteria. It could be because we young and therefore unable to care for ourselves, or we may have a disability which makes it difficult for us to care for ourselves. All of these situations and circumstances need to be considered before making this diagnosis. That being said, many of us may struggle with Dependent Personality Disorder, so I will go over what it is and how it is diagnosed. Dependent personality disorder is a mental health condition in which people struggle with the fear that they cannot care for themselves. They honestly believe that if they had to, they couldn't. So they look to other people in their life to take care of them, tell them what to do, when to start a project, etc. DSM-5 (1) has difficulty making everyday decisions without an excessive amount of advice and reassurance from others. They struggle to make any decision because they honestly fear that they won't make the right one. So they look to others for support with it. (2) needs others to assume responsibility for most major areas of his or her life. They feel like they cannot care for themselves, so they give the responsibility over to someone else that they think could do a better job. (3) has difficulty expressing disagreement with others because of fear of loss of support or approval. Support or approval is so important to us, that we don't want to fight in case that scares them away. (4) has difficulty initiating projects or doing things on his or her own (because of a lack of self-confidence in judgment or abilities rather than a lack of motivation or energy). This can be hard in school or work because they are unable to do anything on their own or take the lead on a project. (5) goes to excessive lengths to obtain nurturance and support from others, to the point of volunteering to do things that are unpleasant. (6) feels uncomfortable or helpless when alone because of exaggerated fears of being unable to care for himself or herself. (7) urgently seeks another relationship as a source of care and support when a close relationship ends. Like I have stated above, they worry so much about being able to care for themselves, that if one caretaker is gone, they will frantically find someone to fill that spot so that they can continue their life. (8) is unrealistically preoccupied with fears of being left to take care of himself or herself. This is a core belief that anyone with DPD has, and this is why they try so hard to keep a caretaker in their life and give them the responsibility of making their life decisions. If you are struggling with this, know that you are not alone! Also, please reach out for help, because the sooner we start talking about and working on our struggles, the sooner we can start to feel better. They Idolize A Cheating Lifestyle There are shows out there that are not really good for couples that are trying to have a stable relationship. Some of us are smart enough to pick that up from a mile away, but a cheater…nah. For them, this is like music to their ears. With all the reality shows on TV, you will notice how they seem to support the cheating partner. They will take it a notch higher and instead of working things out in a relationship when things get tough, they will go hang out with a person they feel comfortable with, who they should not be hanging out with. My advice to you is…Run! They won’t change any time sooner. 2. They’re Excessively Needy I’m sure this is not news for most of you…but I will say it any way. Cheaters want your time, money, attention, energy, affection or a combination of these in excess. You don’t give it to them, they act out to get it. By act out I mean they use manipulation tactics to get what they want. Sorry to break it to you, but however much you do for this person, they will never be satisfied and you will find out they cheated on you sooner or later. When you ask them the reason for their behavior, they’ll blame you for not doing certain things. Let me offer you some comfort though. This type seeks validation through the eyes and praise of others and if you are not offering them that, then you are in a lot of trouble my friend... So always be the first to complement them on everything…literally. 3. They’re Highly Attractive It is no news that physical attraction is the top way cheaters maintain an endless stream of victims. Studies show that women are more susceptible to this than men as they are more likely to cheat based on physical attraction. For a cheater, their attraction is their greatest asset. If you’ve heard of the saying, ‘Forbidden fruits taste sweetest’, you will throw that out of the window once you see them and actually believe they are worth taking the risk for. So…beware. To be forewarned is to be foretold. 4. They’re Overly Flirtatious Flirting precedes dating and we all except that once we agree to take things seriously, flirting kind of stops. But not for the cheater. No matter how much they hide it, they will display overly flirtatious behaviors at some point in the relationship. The root source of this is jealousy, insecurity and the need for attention. Worse still, they’ll flirt in front of their partners and take it as harmless fun. And when certain people tend to see that in a person, they know that sociopathic tendencies are not too far away. If you think their behavior will improve when you are away, then you my friend have been deceived. 5. They’re Insecure I know it sounds a little bit contradictory to their nature but cheaters are really insecure. It goes a deeper to their past probably because of the pain they have carried for some time, internalize it and finally spread it to their partners. They may be highly attractive with an array of other traits that are quite appealing but because of this insecurity they may act out because they feel they do not deserve you and are sure down the line that you will soon leave them. So what does this lead to? Self-destruction. Cheating to them is a way to feel avoid and ensure that they are never alone. So be careful peeps! A-B-C CBT/REBT: Irrational Thoughts Focus on the present Avoid discussing symptoms and complaints. Focus on what counselees can do directly-act and think. Spend less time on what they cannot do directly: changing their feelings and physiology. Avoid criticizing, blaming and/or complaining Remain non-judgmental, but encourage people to ask: Is what I am doing getting me closer to the people I need? Focus on specifics. Who are counselees are disconnected from Help them make specific, workable plans to reconnect with the people they need. Follow through on what was planned by helping them evaluate their progress. Be patient and supportive but keep focusing on the source of the problem, disconnectedness. Personality Theories: Psychoanalysis The conscious mind is what you are aware of at any particular moment, your present perceptions, memories, thoughts, fantasies, feelings Working closely with the conscious mind is what Freud called the preconscious, what we might today call "available memory." The largest part by far is the unconscious. It includes all the things that are not easily available to awareness The id, the ego, and the superego Personality Theories: Erickson We develop through a predetermined eight stages. Progress through each stage is determined by our success in all the previous stages. Each stage involves certain developmental tasks If a stage is managed well, we develop a certain virtue or strength Learning Theories: Classical Conditioning Learning Theories: Operant Conditioning Operant conditioning is the use of consequences to modify the occurrence and form of behavior (Treatment planning) Reinforcement is a consequence that increases a behavior. +/- Punishment is a consequence that decreases a behavior. +/- Extinction is the elimination of a behavior by removing the reward. Learning Theories: Social Learning People learn from one another, through observational learning, imitation, and modeling. People can learn by observing behavior and the outcomes of those behaviors. Learning can occur without a change in behavior. Cognition, awareness and expectations of future consequences can have a major effect on the behaviors that people exhibit. Reciprocal causation: The person, the behavior, and the environment can have an influence on each other. 4 conditions that are necessary before an individual can learn Attention: the person must first pay attention to the model/situation Retention: the observer must remember the behavior that has been observed. Rehearsal: the third condition is the ability to replicate the behavior. Motivation: Learners must want to demonstrate what they have learned.

Over the last few years, a number of clinical pilot studies have examined the therapeutic potential of psilocybin for a variety of mental health conditions; including anxiety, OCD, smoking and alcohol dependence. David Erritzoe et al have recently completed a similar feasibility study in treatment-resistant major depression. Based on previous observations in healthy psychedelic naïve volunteers of long-lasting increases in the personality trait “openness” after a single high-dose of psilocybin, Dr. Erritzoe et al speculated that similar changes in personality may occur in patients with major depression after their treatment with psilocybin. This talk will report on a clinical trial involving 19 pa- tients with treatment-resistant depression who received two doses of psilocybin in an open label pilot study. Significant changes in personality traits, as assessed with pre- and post-treatment personality measurements using the full 240-item NEO-PIR, were observed. These data and their relationships with treatment response will be presented in this talk. BIOGRAPHY David Erritzoe is a medical doctor and clinical psychiatrist. After finishing medical school at Copenhagen University, he obtained his PhD on serotonergic neurotransmission in the Center for Integrated Molecular Brain Imaging (CIMBI) in Copenhagen. He is post-doctoral researcher at Imperial College London under the mentorship of Professors Anne Lingford-Hughes and David Nutt. There, he investigates dopaminergic and opioid neurotransmission in addiction. Additionally, he is part of a research programme together with Professor Nutt and Dr Carhart-Harris conducting research in the neurobiology and the treatment potential of MDMA and classic psychedelics.

Today we talk about alcoholism or what the DSM 5 calls it now "alcohol use disorder." There are many alcohol related disorders in the DSM: alcohol use disorder, alcohol intoxication, alcohol withdrawal, other alcohol induced disorders & unspecified alcohol related disorder. The last two disorders are sort of catch all types of disorders if you don't quite meet the criteria for one of the others listed. Now that you know the landscape of alcohol disorders, let's get into alcohol use disorder: There are 11 criteria (or symptoms) that the DSM discusses, and in order to be diagnosed with this, you must have at least 2 of them. These symptoms or criteria must have occurred within the last 12 months. 1. Alcohol is often taken in larger amount or over a longer period than was intended. 2. There is a persistent desire to or unsuccessful effort to cut down on alcohol consumption or control it's use. 3. We spend a lot of time in activities necessary to obtain alcohol. 4. Craving or intense urge to use alcohol. 5. Recurrent alcohol use resulting in a failure to fulfill major role obligations at work, school or home. 6. Continued alcohol use despite having persistent or recurrent social or interpersonal problems, caused or exacerbated by the effects of alcohol. 7. Important social, occupational, or recreational activities are given up or reduced due to alcohol use. 8. Recurrent alcohol use in situations in which it is physically hazardous. 9. Alcohol use is continued despite knowledge of having a persistent or recurrent physical or psychological problem that is likely to have been caused or exacerbated by alcohol use. 10. Tolerance: when it takes more alcohol than it used to in order to get the same effect. 11. Withdrawal: please be careful with this! It can be life threatening to quit cold turkey without the supervision of a doctor. So please reach out for help prior to just stopping on your own. Treatment options: We could start by going to a detox house in order safely stop drinking. Then we can step down into a hospital program or inpatient treatment center. I say those separately because inpatient treatment centers can be in homes and much more warm and therapeutic than hospital programs. After that we can move out into a recovery home or halfway house and do day treatment (this allows us to slowly transition back to into life and they believe it lowers relapse rates). Then we can step down into outpatient therapy and group therapy. This could be seeing someone like me and going to AA or another group therapy available. If you are not the one suffering from this, but are close to someone who is, Al-Anon is a great resource as well. We may not realize it, but by living with someone who is an alcoholic, we have our own struggles and issues we need to work on. Al-Anon is a great place to do that. They even have Alateen for those who are younger.
