A clicking or snapping feeling or sound around your hip joint (snapping hip) may bother you or cause you to worry. But if your hip is not painful, in many cases the click or snap is nothing to worry about. Home treatment may be all that is needed for minor hip symptoms. The precise location of your hip pain can provide valuable clues about the underlying cause. Problems within the hip joint itself tend to result in pain on the inside of your hip or your groin. Hip pain on the outside of your hip, upper thigh or outer buttock is usually caused by problems with muscles, ligaments, tendons and other soft tissues that surround your hip joint. Hip pain can sometimes be caused by diseases and conditions in other areas of your body, such as your lower back or your knees. This type of pain is called referred pain. Pain can arise from structures that are within the hip joint or from structures surrounding the hip. The hip joint is a weight-bearing joint. The joint consists of two main parts: 1.Ball (femoral head) 2.Socket (acetabulum of the pelvis) Anterior hip pain (deep groin pain) 1-Arthritis of the hip Treatment •Conservative treatment: physical therapy, anti-inflammatory medication, injections •Surgical treatment: total hip replacement. 2-Labral tear •Diagnosis by MRI. •Conservative treatment: physical therapy, anti-inflammatory medication, injections •Surgical treatment: debridement or repair. 3-Stress fracture •Diagnosis usually by MRI. •Surgical treatment: usually fixation of the fracture is performed. Early diagnosis is important. 4-Avascular necrosis Treatment •Conservative cases: diagnosis by MRI. Early stages treatment includes decompression and stem cell injection. •Severe cases: usually diagnosed by X-ray. Severe cases treatment includes total hip replacement. •Intermediate cases: intermediate treatment includes vascularized fibular graft. Lateral hip pain Trochanteric bursitis Treatment •Conservative treatment:physical therapy, anti-inflammatory medication, injections. •Surgical treatment: excision of the bursa rarely utilized. Posterior hip pain Piriformis syndrome Conservative treatment •Physical therapy & stretching •Anti-inflammatory medication •Injections Surgical treatment in rare cases with release of the piriformis tendon. Far posterior pain (SI joint and lower spine conditions) •Injection of the SI joint is the only method to diagnose pain of the SI joint.

First you must understand how inflammation of the tendon could occur. Many times when tissues in the body become overused they have a tendency of developing small tears from micro-trauma. The tendon of the hip flexor is no exception. These tears within tendons and other soft tissues fill up with scar tissue overtime. This scar tissue is not as flexible and resilient as normal tendinous tissue and as a result can tear again easily and the cycle will continue, thus leading to the development of a repetitive stress injury. This is how hip pain when running can become chronic. At P2 Sports Care we believe much of the RICE method can be done at home when the injury is acute (fresh). However, rarely do we consider Hip Flexor Tendonitis as an acute or traumatic injury. When most people feel pain in the hip region it has actually had a “silent” problem for months, so it is perfect for our type of treatment. This condition normally responds quickly to types of treatment which focus on removing scar tissue, such as Active Release® and Graston. Careful palpation of the area may in fact reveal the problem is not of the primary hip flexor tendon (psoas tendon) at all, but rather from tendonitis of other structures. The rectus femoris, obturator externus, pectineus, sartorius and the anterior portion of the hip capsule can be the primary structures causing hip pain when running of similar in presentation. If you have a provider with skilled palpation skills, the exact structure is not hard to locate. We focus on a systematic approach to improving the health of the problematic tendons/ muscles and give instructions for prevention of re-injury.

Hip stretches can help relieve pain in your hips and your low back as well. For most stretches, you want to try to hold the stretch for 30 seconds, and do three of them on each side. This is an exercise and a stretch since it loosens up the pelvis and core muscles as well as strengthens them. It’s a great one to start with. The next stretches are the butterfly stretch, the cat/dog stretch, and a glute figure four stretch. These do a great job of stretching muscles around the hip including the adductors or groin area, low back muscles, hip flexor muscles, and your glutes and piriformis. The next stretches are for the hamstrings and the hip flexors. When these are tight, they can tilt the pelvis one way or the other causing pain in the hips and low back. The IT band and the quads are often not stretched well, and can cause pain and imbalances in the hips. The prayer stretch is a great whole body stretch as well as a great stretch for your low back, hips, and glutes. The last stretch is for the quadratus lumborum (QL). People often forget about this muscles or don’t know it’s roll in the hips and pelvis. Don’t forget about this one. If you, just like 15 to 25 percent of people in the US, suffer from chronic knee pain, or just get tired sore feet from constantly being on the go, pay attention to these 6 exercises to cure chronic knee, foot or hip pain. You don’t need an expensive treatment if you regularly perform the simple workout at home. Knees are the second largest source of recurrent pain, and once you add in the feet and hips, it turns out that the legs can cause big problems for a lot of people. Simple tip-toeing, besides strengthening your calves, helps improve your balance by reducing the area in contact with the ground, which makes keeping your balance quite a challenge. It strengthens your glutes and core muscles, improves your posture while simultaneously relieving back pain, increases the flexibility of your ankles, stretches the muscles around your shins, which are an otherwise difficult muscle group to target. Heel raises, ankle circles, resistance training and toe games are all effective exercises for relieving pain in different parts of your legs. Try to do rolling foot massage on a daily basis and do as many reps as you want or have time for. It gives the bottom of your foot a nice stretch and prevents foot-related injuries in the long run. It’ll improve your flexibility, range of motion, and overall comfort while walking or standing. -Resistance training targets the muscles in your calves and inner and outer thighs, not only toning but also strengthening them. -Toe games is an important exercise because it strengthens the tiny muscles in your toes and feet. They tend to be underdeveloped, which can be a big problem since they support your arches, keep your toes properly aligned, and even contribute to balance. -Rolling foot massage works the muscles in the heels and balls of your feet. It’s also a nice relaxing massage for your arches. -If your feet are killing you after a long day, use one thumb to put pressure on the spot between your big toe and pointer toe and use the other thumb to press the point between your ring toe and pinky toe. Gently massage these spots for up to 15 minutes for easy and quick relaxation. Often, people are told that something in an X-ray or MRI is the reason they have pain. This could be a labral tear, arthritis, or cam or pincer bone shapes called femoroacetabular impingement (FAI/hip impingement). It's important to remember that none of these things have been shown to be definitively linked to hip pain. In fact, in study after study, there is no relationship between the severity of labral tears, arthritis, or FAI bone shapes and pain. The second mistake is stopping all movement. Often doctors will tell you stop moving because it will accelerate damage to your hips. If you're familiar with the history of back pain and spinal fusion surgery, this may sound familiar. For years, if you had chronic back pain, doctors would look at X-rays and MRIs for signs of arthritis, degenerative discs, and other pathologies. Surgery was often considered the best option when a few months of rest and/or physical therapy failed. Over the decades, high quality research has shown that long periods of bed rest actually make things worse, that spinal fusion is not better than long term exercise based, non-invasive treatment, and that spinal fusion itself doesn't reliably relieve back pain! This is a familiar pattern that happened with shoulder impingement, knee meniscus tears, and seems to now be happening with hip surgeries. The third most common mistake with hip pain is giving up too soon on non-invasive approaches. A spine surgeon, Dr. David Hanscom, wrote a book about how to get out of back pain without surgery. After years of performing spinal fusions and seeing their disappointing results, he strongly advises that people dedicate themselves to lengthy, detailed exercise regimens before ever considering surgery. And if first attempts at therapy or training fail, he strongly suggests that people be patient and diligently seek out other help and other ways to help their muscles. You need to learn about your unique body and what it uniquely needs. Hip Arthritis Stretches & Exercises - Ask Doctor Jo

These hip arthritis stretches and exercises are easy to do in the comfort of your home and should provide some pain relief. See Doctor Jo's blog post about this at http://www.askdoctorjo.com/content/hip-arthritis-stretches-exercises The first exercise will be sitting down. Make sure your back is against the chair, and your knees and hips are at about a 90 degree angle. Then you will bring your knee straight up towards the ceiling, and this is called a hip flexion exercise. Make sure you do slow and controlled movements. Start off with 10 and work your way up to 20-25. If those become easy, then you can add ankle weights. The next exercise will be a sit to stand. Again, you want this to be controlled movements. Try not to just pop up and plop down. Put your feet about shoulder width apart and lean forward as you stand up, then stick your bottom back and have your chest forward while you sit back down. Try to also keep your feet flat and your knees behind your toes when doing this. If that is easy, you can just tap your bottom on the chair and not sit all the way back down. Start off with 10 of these, and then work your way up to 20-25. So now get down on the ground, and you will stretch out those hip flexor muscles. Keep the knee of the leg you want to stretch on the ground, and then bring the other leg up into a lunge like position. Try to keep your back upright and straight. If you lean forward with your upper body, you won’t get a good stretch. Shift your hips forward until you feel a stretch in front of the thigh. Hold this for 30 seconds and do three of them. Lying down on the ground in supine or on your back, you will do a hip abduction exercise. With your legs straight out, try to keep your toes pointing up toward the ceiling through the whole movement. Then slowly slide your leg out to the side, and then slide it back in. Start off with 10 of these. If you get to 20-25 and they are easy, then you don’t need to do them in this position anymore. Now roll onto your side for the clamshell exercise. This one is harder than it looks. You want to keep your hips straight up and down, perpendicular to the floor. Your hips will want to roll back, but keep them up as much as you can. Then keeping your knees together with one on top of the other, bend your knees slightly towards your body. Then lift the top leg up slightly, like a clamshell opening up. It doesn’t have to be a big movement because you want to keep your hips from moving. Start off with 10 of these, and then work your way up to 20-25. If they become easy, you can add a weight or use a resistive band. The last exercise is a bridge. Even though this is a simple movement, it really works the hamstrings and glut muscles. Lie on your back with your knees propped up. Slowly lift your butt off the ground until your hips are in a straight line with your chest and knees. You can go one segment of the spine at a time to really loosen up your back. Start off with 10 of these, and then work your way up to 20-25. How to Fix “Low Back” Pain (INSTANTLY!)

The irony is, a lot of times what we feel is rooted in the lower back is actually caused by muscles not in the back at all. In this video, I’m going to show you how the glute medius could be the real cause of your back pain and a quick exercise you can do to relieve your discomfort instantly. Once gone, I’m also going to show you a few additional exercises you can do to make sure your low back pain never returns. The gluteus medius is sandwiched between the glute maximus and minimus and lies in and around the hip area. The role of the muscle is to abduct your hip or lift your leg out to the side in either standing or side lying and to keep your pelvis level whenever you take a step. Prolonged sitting during the day as well as an unequal weight distribution when standing are two of the most common reasons for this muscle to get weak and imbalanced. You can quickly test to see if you have the makings of a weak glute medius by standing with your feet shoulder width apart and lifting one foot off the ground. First take note whether or not your opposite hip drops significantly. If it does, this would indicate a glute weakness on that side. You would repeat with the opposite foot as well. When you do this you would also want to see if you had to dramatically shift your weight to one side just to lift that foot off the ground. If you do, this would indicate that you have an unequal weight distribution when standing and it would be especially troublesome when squatting. To fix this quickly, you will want to lay down on the ground with your affected side on top. If your right lower back was bothering you then you would want to lie on your left hip. From here, take your thumb and place it on the area most sore. You should feel that this is going to happen just to the outside of the bony prominence of your pelvis. From here, push to hold back the trigger point and start flossing your leg down and in front of you and then back and up. Your hip should be extended and then lifted into abduction towards the ceiling (being sure to point the toes down to keep the glute medius in focus). Do this about 10 times until you feel the tension in the muscle release. Now, you can burn out the spasm in the trigger point by getting into the fully contracted position of the glute medius muscle and holding as long as you can. Generally, because this muscle is often very weak, this may not be any longer than 30 seconds to a minute. Since the underlying cause is weakness in the gluteus medius muscle you will want to back this up with some exercises for your low back that you can do a few times a week. I show you three options for this. The first is the hip bump against the wall. The second is called the sack swinger, and can be done with a dog leash if you don’t have a formal dip belt. The last is actually doing the same movement that you did for the treatment, but this time as a strengthening exercise for your low back. Whatever you do, just be sure you are consistent. You will also see that as you relieve the tension in this muscle that your squat performance improves as well. Your depth should be increased and any low back pain that you felt by doing the exercise should be gone since you will now be able to do them with equal force through each leg. So, it's going to be on this outer lateral aspect of the foot. You can see there's kind of a protrusion here. This is the base of the fifth metatarsal. You can easily find that. That's one of your markers. Your other marker is this heel bone. This is a calcaneus. You're going to want to find the top. That's your other marker. So, between here and here is this kind of little triangle area. Around here is housed the reflexes to the hips and a little bit to the knees. If you start to move above you'll start to go into the arm reflexes and other reflexes. We're going to be really concentrated in here. We're also going to do some work from out here. There are some sciatic and hip reflexes that also course through here. For this place between here and here I'm going to use thumb walking. With thumb walking, again, you're just doing this walking and bending this joint here. You're going to come down here, you're going to get up a little bit, and then come back down. You're not going to pop out. You're going to go in with continued pressure. You're just going to go in again. And in again. There we go. We could also do a little bit of that press and slide which Dr. Manzanares does. You press in, then you kind of slide into that area. Press, and then slide into the area. When you work this outer area you'll also use thumb walking, but you'll come across out through here. So, you're supporting the foot with this hand. I'll lift it up so you can see it a little bit better. It's going to be all through here. So, you start here, and you just come down, and it can be really intense as well especially for women. So interact with your person and if they let you know that it's too deep just ease up a little bit. Come down through here, and come back, and then meet that point through here. You could even just do a point pressure. You’d just press in. And you'll feel it. You might feel a little bit of a change in the tissue texture in that area which I feel here. You'll want to give that a little bit more pressure. When you let go just ease up. Of course, you'll want to do both sides. If for some reason someone is experiencing hip pain you don't want to necessarily just do the side that they're experiencing it on. You want to do both. Just because the pain might be on the right side, for instance, doesn't necessarily mean that it's not coming from the other side. In addition, you could add a little bit of lower back reflexes and lymphatic groin around here with your finger walking technique. Also the heel of the foot you can thumb walk. I like to thumb walk side, and it'll also give attention to that area. It's kind of doing the hips, the knees, the whole pelvic region, and then you can also do a little bit of the lower back through here with your thumb walking just so that you're giving attention to the whole area. That's thumb walking, finger walking, and reflexology for the hip and knee area.
