First you must understand how inflammation of the tendon could occur. Many times when tissues in the body become overused they have a tendency of developing small tears from micro-trauma. The tendon of the hip flexor is no exception. These tears within tendons and other soft tissues fill up with scar tissue overtime. This scar tissue is not as flexible and resilient as normal tendinous tissue and as a result can tear again easily and the cycle will continue, thus leading to the development of a repetitive stress injury. This is how hip pain when running can become chronic. At P2 Sports Care we believe much of the RICE method can be done at home when the injury is acute (fresh). However, rarely do we consider Hip Flexor Tendonitis as an acute or traumatic injury. When most people feel pain in the hip region it has actually had a “silent” problem for months, so it is perfect for our type of treatment. This condition normally responds quickly to types of treatment which focus on removing scar tissue, such as Active Release® and Graston. Careful palpation of the area may in fact reveal the problem is not of the primary hip flexor tendon (psoas tendon) at all, but rather from tendonitis of other structures. The rectus femoris, obturator externus, pectineus, sartorius and the anterior portion of the hip capsule can be the primary structures causing hip pain when running of similar in presentation. If you have a provider with skilled palpation skills, the exact structure is not hard to locate. We focus on a systematic approach to improving the health of the problematic tendons/ muscles and give instructions for prevention of re-injury.

These are great for all the muscles around the hip including, gluteus medius and gluteus maximus, IT band, hamstrings, hip flexors, and others. Now for sitting you will do a seated hip flexion and a squat at your chair. These are great because you can do them at work, or if you have been sitting for a long time. The final hip exercise is a lunge. It sounds pretty simple, but if done right, it can be pretty challenging. You don’t have to bring your knee all the way to the floor, and you can use a target if you need one. In this video, I’m going to talk a little bit about why this happens and show you 3 hip flexor stretches to release tight hips. One common cause of tight hip flexors is habitual sitting. When we sit for long periods of time, we end up with long-term restriction in the front of the hip, which results in hip and low back discomfort. So what can you do about it? The first thing is to be aware of how you’re sitting: https://www.youtube.com/watch?v=34ma6_WA8M8 The second is to stretch the front of the hips. Here’s 3 great hip flexor stretches you can do: 1. Psoas stretch over bolster - Place a bolster or rolled up yoga mat underneath your hips. - Bend both knees into your chest, so your back is rounded towards the ground. - Hug the right knee in and slowly reach the left leg out. Pause when you feel a tug down the front of the left hip. - For an extra stretch, reach the left arm overhead. Continue to sink your ribs to the ground and hug the right leg tight. Hold for 30 seconds and repeat on second leg. 2. Prone (face down) hip flexor stretch - Lie on your stomach with legs a few inches apart and your forehead resting on your hands. - Gently press your pubic bone towards the ground to lengthen out the low back. Bend your right knee and catch your right ankle with your right hand. If you have to arch your back to do this, use a strap instead. - Keeping the knees close to one another (or if possible touching), continue to imprint your pubic bone towards the floor, so you feel a stretch down the front of your right leg. Hold for 30 seconds and repeat second side. 3. Kneeling hip flexor stretch - Come to kneeing with your right leg forward. - Gently tuck your pelvis, so your pubic bone moves towards your rib cage and your low back flattens. - Slowly shift your weight towards your right leg and pause when you feel a gentle stretch in the front of your left hip. - For a greater stretch, reach your left arm overhead and lean to the right. Hold for 30 seconds and repeat second side. When you sit, you can do external rotation of the hip to help to release this muscle. Simply take one ankle and place it on the opposite knee. Then you can bend forward at your hips. You can even bring the knee you aren't using up towards you to feel it more. By doing this Piriformis stretch, it is less likely to become very stiff and impinge upon your sciatic nerve. Kneeling Lunge- Front and back knee shoulder width apart, roll your pelvis forward, squeeze your back glute and rock deeper and deeper into the stretch. Once opened up, try lifting your leg up behind you for a good quad stretch. 2. Internal/External Rotations- Put your feet out infront of you with your knees bent at 90 degrees. Rotate both knees down to the same size while keeping your entire foot on the ground the whole time. Do this 10 times and then try popping your butt off the ground by thrusting through your hips. 3. Frog- Start on your hands and knees then spread your knees as wide as is currently comfortable. Put your feet directly behind your knees and keep your entire foot on the ground the whole time. Rock back and forth 10 times going deeper and deeper into the stretch and hold as deep as you can for 10 seconds. 4. Prayer Squat- Squat down with your feet a little wider than shoulder width and place your elbows inside of your knees with your hands together. Gently press your knees outward with your elbows and if this is too tough, let your knees return back to normal positioning and push them out gently over and over 10 times until you can hold with your knees pushed out. Dr. Ebraheim’s educational animated video describing knee arthritis and knee pain . Causes of knee pain may be related to: •Sports •Trauma •Inflammation or arthritis •Osteoarthritis Cartilage is a tough and flexible connective tissue that gives shape, support and provides a cushion between bones in the joints. Unlike other connective tissue, cartilages do not have its own blood supply, instead, it is composed primarily of water, chondrocytes, collagen, and proteoglycans, which produce and maintain a structural matrix giving cartilage tissue its form and function. Proteoglycans are composed of numerous glycosaminoglycans attached to a core protein. The predominant glycosaminoglycans in cartilage are chondroitin sulfate and keratin sulfate. Within the cartilage matrix, there are numerous prototype and monomers attached by link protein to a molecule of hyaluronic acid. This is all woven with collagen to form an elastic and compressible structure. Normal articular cartilage also known as hyaline cartilage provides a smooth crystal clear gliding surface to aid in the motion of the joint. The structure of hyaline cartilage is divided into four layers; superficial, middle, deep and calcified layer with the calcified layer representing a transition from articular cartilage to bone. This cartilage is designed to achieve and maintain proper function over the majority of a person’s life. If the cartilage is subjected to excessive weight, overuse, improper alignment or injury, it begins to wear away leaving bone to rub on bone. Articular cartilage does not have the ability to heal itself, however, there are several treatment options available to aid in the repair of injured cartilage. The knee is the most common joint in the body with cartilage injury issues. The knee joint is composed of femur, tibia, and patella. There are two types of cartilage in the knee joint, the articular cartilage and the meniscus, which is a shock absorbing cartilage. There are also important ligaments within the knee joint, the anterior and the posterior cruciate ligaments. The goal of treating cartilage injury is to reduce pain and improve the function of the affected area. Anti-inflammatory medications are commonly used to combat-injured cartilage but they can potentially cause cardiovascular or gastrointestinal toxicity. Damaged cartilage can also be treated with a series of injections. Steroid injections such as cortisone are powerful anti-inflammatory drugs used in the treatment of osteoarthritis. Hyaluronic acid is a substance produced naturally in the body and is present within cartilage when isolated. Hyaluronic acid is a thick viscous solution that can be added to the natural hyaluronic acid of the knee joint. This solution uses purified Hyaluronic acid derived from either rooster combs or genetically engineered cells. Glucosamine is another option for treatment that can be injected directly into the joint. Glucosamine is a modified sugar that is formed by the human body and is used to form larger molecules involved in the formation and repair of cartilage. Other methods include gene therapy, platelet-rich plasma, growth factors, stem cells, drilling and debridement of cartilage may help with injured cartilage. Occasionally the cartilage is replaced, regrowth and implanted. When the cartilage is damaged, the choice is often simple as the patient will need a total knee replacement. Many different types of designs and materials are currently used in total knee replacement surgery. Nearly all consists of three components, a patellar component, which consists of durable plastic, a tibial component, made of a durable plastic cushion often held within a metal platform and the femoral component, which is usually made of highly polished metal. Total knee replacement is the most predictable option to treat severe arthritis of the knee joint. When the arthritis is minimal, almost all treatment methods will work, however, when the extent of arthritis is moderate, there are many different treatment options presented, but none of them are predictable. It is logical to pursue and recognize the efficacy of platelets and stem cells in cartilage regeneration, simply because it is better for the patient to be able to regenerate their own cartilages and for it to be replaced with an artificial joint.
