Hormone Therapy and Long Term Mortality
Read more at www.biobalancehealth.com JAMA, the Journal of the American Medical Association is one of the major pieces of literature that doctors read to obtain new information about the knowledge and practice of medicine. For years JAMA has been publishing articles challenging the use of Hormone Replacement Treatment, particularly for women. In the 1980s HRT took off as a miracle treatment for women who were aging and post -menopausal. Many doctors and their female patients were getting into this treatment it was an exciting time for women who wanted to live long and healthy lives. Then, in order to validate this movement in modern medicine, the Women’s Health Initiative put together a research program to check out the legitimacy of hormone replacement treatment. This research program was checking the replacement of estrogen and provera. In just a little time they obtained data that caused them to believe that women receiving this treatment were at an increased risk for breast cancer and death. The news media got on board and rushed to publish warnings to women and their doctors to say stop killing women! They did not really verify the research they just accepted the warning and rushed out to say the sky is falling. Dr. Maupin had been watching this issue unfold. After twenty five years as a gynecologist treating women who became postmenopausal she did not see an increase in breast cancer or death in women who received HRT. So she continued to practice medicine the way she knew and used the data and the research she had obtained from Europe to bolster her argument that the treatment was safe and efficacious. Many doctors in our community ran scared and scared their patients and said “ I won’t practice this way and I don’t think you should go to her to obtain HRT.” This week, eighteen years after the controversy over the WHI study which was fatally flawed a study has been released that tracked over 27000 women who had received HRT for between five and seven years or more. This is considered long term treatment. The result is exciting: there is no supporting data that shows any increase in risk of death from all cause mortality among these women based on whether or not they have received hormone replacement therapy. There is no data that says HRT will increase your risk of death from any cause of death, particularly heart disease and cancers of all types. They are not saying it will keep you alive, but they are saying it won’t kill you. Now that we do not have to look at the risk of death with anxiety and fear, we can look at research that will talk about the quality of life and the health and improved quality of life that can happen when people obtain hormone replacement therapies when they need them. Not only can you safely consider the replacement of estrogen, you should also look at other hormones that our bodies make and use which decline as we age. Talk to your doctor about whether or not they are aware of this exciting new article in the most recent issue of JAMA and whether or not they are willing to recommend and provide HRT treatments for aging people.

Health Alert: Hormone study and breast cancer risk
CINCINNATI (Liz Bonis) -- More questions being were raised Thursday, April, 16, about two hormone replacement therapies for women. Hormone replacement therapy, or HRT as it's known, is usually a combination of both estrogen and a synthetic progesterone called progestin. It is usually used to reduce hot flashes, sleep problems, and other symptoms of menopause. In women who have had a hysterectomy, often only estrogen alone is given. A study released Thursday was a follow-up to a previous women's health initiative study that was released in 2002. It found the combination therapy increased a woman's chances for breast cancer, stroke, heart disease, and blood clots. The new study was an analysis of the same women, but now 13 years later, and looks only at the way HRT may influence breast cancer risk.Experts at the Cleveland Clinic said results showed using estrogen alone may not influence breast cancer risk in the same way as the combined therapy of estrogen and synthetic progesterone. The study followed more than 16,000 women on the combined therapy. It found women who got off this therapy had the risk drop in about three years. Women did still have a higher breast cancer risk as time went on; the study did not raise death rates overall from it. In the women who were only on estrogen about 10,000 were followed. They found the estrogen alone actually led to a reduced risk. But if they stopped that the reduced risk went away over time.Cleveland Clinic doctors said hormones influence risk but don't cause breast cancer. They suggest people talk with their own doctor about their personal health history before making a decision about taking or not taking hormones. Rhonda Tipton-Swisher, Breast Cancer Patient Treated with Bioidentical Hormones

Rhonda was diagnosed with receptor-negative breast cancer. She inquired about bioidentical hormone therapy after she was told she should expect menopause during chemotherapy. The testosterone pellets, Dr. Maupin points out, help you become leaner, decrease belly fat, give you muscle back, and it allows you to burn calories while you sleep. Rhonda also had symptoms of fibromyalgia, causing her intense bone and muscle. Her doctor had her on narcotic pain relievers for more than a year before she first came to BioBalance. This stuff that vitally important for you to know. But first, let me ask this: Do you feel or have you fever been afraid, confused or frustrated about your own menstrual, peri-menopausal or menopausal issues? I know I once did. Intense hormonal changes can make you feel like you’re going crazy, so you’re not even sure who you are any more. And these symptoms can start as early as 10 years before menopause arrives. Natural hormonal shifts are natural transformations in every woman’s life. And, believe it or not, they can even be profoundly beneficial. But you need to learn how best o handle them. We women are constantly told that we have to take drug-based artificial hormone drugs known as HRT to do this. I’m here to tell you this is most certainly not the way to go. Why? Because medically based HRT— using synthetic drugs can produce massive side effects. It is also incapable of delivering genuine healing. It cannot treat fundamental causes. It can only mask symptoms. In the process, synthetic HRT hormones can poison your body long-term. Drug-based synthetic estrogens, progestins and progestogens commonly prescribed as HRT are fundamentally dangerous. Used long-term, they may even be life-threatening. We women need to band together and challenge the “wisdom” of conventional medical practices. Indiscriminate doling out of potent drug-based hormones can undermine a woman’s fertility as well as trigger the development of her menstrual agonies—from PMS and endometriosis to cancer of the breast and womb. The current attempt to make every woman a “patient” for the rest of her life by subjecting her body to long-term HRT is also a way of diminishing her personal power and undermining her control over her own body. Conventional HRT is most certainly not the answer. I believe passionately that it’s time for a radical overhaul in how we handle women’s issues. There are safer, better, more effective ways. As celebrated expert in the natural treatment of peri-menopause and menopause, Dr Jonathan Wright, Medical Director of Tahoma Clinic in Washington—a long-time advocate of bio-identical, that is natural hormones—says, “Replacing estrogen that your body is no longer producing with the synthetic versions found in conventional HRT is like replacing parts designed for a Chevy with those made for a Mercedes.” More about thes alternatives in a moment. Researchers found that long-term use of synthetic estrogen and progestin significantly increased women’s danger of stroke, blood clots, heart attack, invasive breast cancer and stroke. Then the Journal of the American Medical Association (JAMA) published another study, showing that women who take a combination of synthetic estrogen and progestin are also at high risk of getting a highly aggressive form of breast cancer. Other studies have revealed that HRT can increase your risk of Alzheimer’s, dementia and asthma. More new research indicates that combined synthetic estrogen-progestin HRT increases your risk of severe forms of breast cancer as well as your chances of dying, from the disease as well as other causes. Let’s look at the alternatives. There are many. Bio-identical hormones—commonly known as “natural” hormones—do a far safer, more effective job of helping most women handle the challenges they face. In the largest clinical trial to date, the combination estrogen-progestin (Prempro) increased the risk of certain serious conditions. According to the study, over one year, 10,000 women taking estrogen plus progestin might experience: Seven more cases of heart disease than women taking a placebo Eight more cases of breast cancer than women taking a placebo Eight more cases of stroke than women taking a placebo Eighteen more cases of blood clots than women taking a placebo An increase in abnormal mammograms, particularly false positives The study found no increased risk of breast cancer or heart disease among women taking estrogen without progestin. Over one year, however, 10,000 women taking estrogen alone might experience: Twelve more cases of stroke than women taking a placebo Six more cases of blood clots in the legs than women taking a placebo An increase in mammography abnormalities The effect of hormone therapy on mammograms is important, because it suggests that women on hormone therapy may need more frequent mammograms and additional testing. Who should consider hormone therapy? Despite the health risks, estrogen is still the gold standard for treating menopausal symptoms. The absolute risk to an individual woman taking hormone therapy is quite low — possibly low enough to be acceptable to you, depending on your symptoms. The benefits of short-term hormone therapy may outweigh the risks if you: Experience moderate to severe hot flashes or other menopausal symptoms Have lost bone mass and either aren't able to tolerate other treatments or aren't benefitting from other treatments Stopped having periods before age 40 (premature menopause) or lost normal function of your ovaries before age 40 (premature ovarian failure) Women who experience premature menopause or premature ovarian failure have a different set of health risks compared with women who reach menopause near the average age of about 50, including: A lower risk of breast cancer A higher risk of osteoporosis A higher risk of coronary heart disease (CHD) In addition, hormone therapy appears to reduce the risk of osteoporosis and CHD when started soon after menopause in young women. For women who reach menopause prematurely, the protective benefits of hormone therapy may outweigh the risks.

Breast Cancer Risk Is Higher in Women with Low Muscle Mass and High Percent Body Fat
Read more at www.biobalancehealth.com Several articles have come to the fore in the last few months that focus on the risk of High body fat and Low Muscle Mass in the occurrence of Breast Cancer. Since the threat of estrogen replacement has been put to rest, women have been asking for the real risks of breast cancer. These new research studies have starred in News week and Healthcare magazines. It is important because many other studies before these two studies, have found that breast cancer is less severe in women who have taken ERT (estrogen), and not as frequent in women who are normal weight and who have a high muscle mass, verses women who are obese without much muscle. These facts also tie in with my over 30 years of experience in OBGYN. Sometimes it is easier to understand and judge the results of research when we understand the mechanisms behind that research. It is well proven that women who are obese often find their breast masses late in the progress of a cancer when they are larger because fat hides the mass from both the mammogram and from the patient feeling the mass themselves. This risk of being overweight is simply anatomic, with fat obscuring both visualization and tactile evaluation for the cancer. The next factor is that obesity accelerates after menopause. The only change at menopause is the loss of Testosterone and Estradiol, and the increased production of Estrone. Breast Cancer and Obesity is more common after menopause. As women gain fat after menopause they generally develop it in their abdomen which makes even more estrone and consequently more fat! As fat mass increases the metabolic rate drops. This is because muscle mass is replaced by fat. Muscle is the organ that burns the most calories in the body. As you lose estrogen, then testosterone, the metabolism decreases. The opposite is true that if testosterone and estradiol are replaced then the occurrence of obesity decreases and so does the occurrence of breast cancer. A study reported in JAMA Oncology studied the muscle mass and fat mass in women with stage 2 or 3 breast cancer. The women who had a higher muscle mass had a much better survival of their cancer! This is due to the fact that muscle is stimulated by exercise and testosterone, but after age 50, no amount of exercise can build significant muscle mass without Testosterone. We work with aging patients all the time and know that replacing testosterone with bio-identical testosterone improves the immune system and in that way decreases breast cancer occurrence and recurrence. Not only does testosterone replacement decrease the cause of death from every cause, but it stimulates bone growth, muscle growth, and decreases heart disease and diabetes. After all the years we have mistakenly aimed our guns at the wrong cause of breast cancer, estrogen replacement, now we can aim our efforts in the proper direction, toward the benefits of hormone replacement, weight loss, and weight bearing exercise to gain muscle mass.

In the beginning she was accused of quackery by other physicians. Fourteen years ago many doctors were afraid of the hormone replacement movement, in part because of the warnings that had arisen out of the Women’s Health Initiative study in 2002. At that time the study was in progress many women and their doctors were turning to the treatment of replacing hormones as a miracle treatment for aging women. The WHI study was cut short because they encountered frightening data regarding the development of breast cancer in women who were in the study. Rather than complete the study, evaluate the results, and then present them, the authors of the study panicked and announced that hormone replacement, especially of estrogen, could cause breast cancer and increase the risk of death. Dr. Maupin did not believe the science that was available to see at that time from the study. She had her own experience and that of her patients to use to evaluate the degree of risk, especially for the development of Breast Cancer. She continued to practice hormone replacement therapy with estrogen and progesterone and testosterone and thyroid and other hormones. Her belief then and now was that our bodies make these hormones when we are young and begin to stop making them as we age. Her thought and that of similar doctors engaged in hormone replacement treatments was that if one could restore the young healthy normal balances of hormones then one could avoid all together or could heal many of the illnesses of aging that led to a loss of independence and to fragility and limitation that caused their final years to be considerably more difficult and limited than necessary. In the early years, Dr. Maupin was criticized by many doctors as being unrealistic and improperly focused in her treatment planning. However as time passed the medical community and the research doctors and scientists came out with a stinging critique of the WHI study and completely rejected their conclusions. These doctors began to say that hormone replacement therapy is not going to lead to increased risks of Cancers or heart disease or death. Now many of the doctors who used to criticize Dr. Maupin send their wives and their patients to her for her treatment of hormone replacement.
