Controlling Post Birth Control Pill Acne [DermTV.com Epi #521]
For many different reasons you may have decided to pursue a course of birth control pills, and for a variety of other reasons, you may have decided to stop taking them. However, when you stop taking "The Pill" you risk a major acne breakout. The good news is that it's treatable. In this episode of DermTV, Dr. Schultz discusses the causes of post birth control pill acne as well as how to treat post birth control pill acne. Amongst the pills, some birth control pills can control acne completely, and a few, like Ortho Tri-Cyclen, are even FDA approved for treating acne. But if you're taking an acne friendly birth control pill, and you decide to stop it, you risk a major acne breakout. Today I'll tell you how to treat it if it happens to you. Stopping birth control pills can cause major acne breakouts because the extra estrogens in the pill partially suppress your androgens, which are the hormones that cause excess oil production... and excess oil production is the root cause of acne. So stopping the Pill causes oil production to rapidly ramp up. Then add previously idle pore bacteria, which now have this new surge of oil to feast on, and the pus pimples and cysts are off and running, causing the exasperating outbreaks. Controlling this unwelcomed regression to aggressive adolescent acne often requires a three-pronged approach. First, you need a regular, topical routine consisting of an acne cleanser and toner, along with spot treatments for individual blemishes. These treatments can contain topical antibiotics, salicylic acid, resorcinol, benzoyl peroxide, or other pimple fighting ingredients. Also be sure to apply a light-weight glycolic serum or lotion to your entire face at bedtime to help prevent and treat clogs. Second, you'll probably need oral antibiotics to neutralize the pus and cyst-forming potential of the flourishing pore bacteria. And third, regular visits to a dermatologist to clean out clogs, pimples and cysts will result in faster control. Yes, you've substituted one pill for another, but the antibiotic is only temporary and can be discontinued after your post-Pill acne is finally under control. Which can take just a few months! Contraception and Birth Control for USMLE Step 2
Simple handwritten Contraceptive (Birth Control) lecture for medical students taking the USMLE. Will be looking at how contraceptives work and the pharmacology. Also the different contraception methods, such as Oral contraceptive, Intrauterine Devices, Implants. Contraception is a method of females to prevent pregnancies. There are several contraceptive methods, however, there is no 100% full proof method. Tier 1 failure rate is less than 1%, Tier 2 have a contraceptive failure rate 2-3% and finally tier 3 is the barrier method and they are about 80% effective. Tier 1 is made up of Intrauterine devices (mirena, Copper), DMPA, Implanon (subcutaenous) and sterilization (tubal ligation or vesectomy). Tier 2 are the Oral Contraceptive Pills, Patches and Rings, and they are made up of comboniation or just progesterone. Tier 3 is the barrier methods include condoms, diaphragm and the spermicidies which prevents implantation. Can also prevent STDs and HPV. Intrauterine devices are implantedin the uterus. They are made up of mirena and copper. Mirena secretes levonogestrel which is a type of progesterone that has three effects. Thickens the cervical mucosa which prevents sperm form ascending into the uterus. The endometrial wall is also thickened that decreases the chance of implantation. Increase tubal motility so it is not in blastocyst and it can't implant into endometrium. Copper Intrauterine device leads to inflammation response which causes the same effects. Mirena can work up to five years, whereas copper can work for 10 years. In Mirena there will be decrease bleeding and cramping. However, Copper can increase bleeding and cramping. Contra-indications to Intrauterine Devices is pregnancy, history of breast cancer, leiomyomas, any uterine infection such as PID and endometritis, especially chlamydia. Any bleeding where the cause is unknown. Relative complications are anatomical uterine anomalies (size and shape), Heart Disease because sometimes perforates and then bleeding can affect the heart. Any history of ectopic pregnancy. Finally there may be complications such as expulsion of the device within 1 month. Perforation, ectopic pregnancy, infection (PID) which usually happens within first 2 months. Sterilization and generally irreversible. Tubal ligation and vesectomy are the two methods. Vesectomies are more effective. TIER 2 are the hormonal methods there are two types which are known as combined and progesterone only. In combined there is pill patch and ring. In Progesterone only there is pill injection and implant. No LH surge means there is no ovulation and there is atrophy of the endometrium and therefore there is no implantation possible. Cervix becomes thick and edematous and prevents sperm from coming in. Combined method is when there is estrogen and progesterone. The pill can begin any day however effects won't begin for 7 days. 21 days and 7 days off. Off days are for the withdrawal bleed. Also helps regulate and normalize the menstrual cycle. The nuvaring enters the vagina and slowly releases estrogen and progesterone. Every three weeks it needs to be removed for 1 week. Patch also slowly release estrogen and progesterone under the skin which gives 60% greater than pills. Progestin only method in pill form is a daily pill that is continuous which causes irregular breakthrough bleeding. Injection form Depo-Medroxyprogesterone Acetate (DMPA) is injected into the skin and slowly released and it lasts for three months. Implanon is implanted subcutaneously and slowly releases etanogestrol and lasts for 3 years.

To pill or not to pill? That is the question. Hormonal Birth Control Pills Hormonal or chemical birth control pills will be explained in this video, with the pros and cons of each type. But before I demonstrate the different types, I would like to explain to you how hormonal birth control pills work. When a lady is pregnant, ovulation (or egg release) does not occur. That’s how our body’s system normally works. The hormones in a woman’s body are balanced in a certain way that makes the hormones that stimulate the release of the eggs – called the ovulation hormones – to become inactive during pregnancy. So we thought: If we give the woman pregnancy hormones in pills, we will be able to inhibit ovulation, in other words: to stop it from happening, which, in turn, would prevent the pregnancy to occur. So the scientists decided to put the pregnancy hormone and the female hormone in a pill. This gives us an idea about the first type of birth control pills. This type consists of synthet-ic forms of hormones – the pregnancy hormone progesterone (called progestin) and the female hormone oestrogen. On the other hand there’s another type of birth control pills that consists of the pregnancy hormone only, and in a very small amount. It is called the mini Pill or the Progesterone-Only Pill (P.O.P.). The pills I will talk about are not the only hormonal birth control methods. We can take other forms of chemical or birth control at the doctor’s office, which would have the same mechanism of action. So, back to the pills. Let me explain them now. Let’s start with a combined pill. “Combined” because it has a combination of hormones. Let’s know more about it. The combined pill consists of progestin – a synthetic progester-one (the pregnancy hormone) – and oestrogen (the female hormone). You will need to take a pill daily for 21 days, starting from the 7th day of your menstruation. Even if you’re still bleeding, you must start using the pill. Take a pill daily for 21 days and stop the pill for 7 days, taking into consideration that your period cycle is 28 days. The combined pill is a good birth control choice in cases of: • You don’t mind to take hormones for birth control • You usually suffer from pains [I’m not sure about what comes here → 2:40] during menstruating, since it will ease the dysmenorrhea. • If your period is not regular, this will regulate it. • You bleed heavily during the period: it will decrease the amount of bleeding, which in turn might prevent you from becoming anaemic. • If you have acne: this might improve your skin condition. • You’re not a smoker The combined pills are not good if you … don’t wish to take any hormones … are Breastfeeding … are a heart patient … have hypertension or high blood pressure … have risk of thrombosis or atherosclerosis … have had diabetes for more than 20 years … have a liver or gall bladder disease … have a history of breast cancer in the past 5 years … are suffering from recurrent migraines or have the symptoms that proceed mi-graine, called the aura … are overweight … have been taking medications such as antibiotics, epilepsy medications or anti-depressants All these may weaken the effect of the combined pill. Hi everyone - So I finally got my hands on Yasmin birth control pills for good or lets hope for good. I do have a hormonal dis-balance which I need to take care of and my gynecologists prescribed these to me. I want to mention that my problem is a light form of hersoitism and ovarian cysts that are considered hormonal dis balance. Now I am very sensitive to the male hormone in my body and I need to create more female hormones so that I can stop gaining weight or getting unwanted hair on places women shouldn't get. I wanted to start the course earlier but I was prescribed Diane 35 a different birth control pill that was not good for me. It made my lymph nodes swallen and very painfull an dmy menstrual cycle went really bad. So I wanted something lighter especially when I have lost my mom of breast cancer. I am still consernd about that though but I have no other choice than taking hormone pills. So If you have any diea what are the side effects, if you took them on a long run please share with me here your impression. 05:38 - I am a 24 year old female with acne on my face, back and shoulders. My skin was clear before I went on the birth control pill at age 17, which I was only on for a few months. I also have very thin hair (both my parents have thick hair, so it's not genetic) My menstrual cycle is short and very painful. I was put on psych meds against my will from age 7 to 15 including topamax, lithium, abilify, seroquil and too many others to even remember. I was obese during this time and consumed a diet of mostly fried and processed meat. After getting myself off the drugs, I lost 100lbs, became anorexic and lost my menstrual cycle for three years. Today, I am a healthy weight (130lbs @ 5'3")but still have the issues I mentioned. Compared to the other two options, antibiotics have many fewer side effects, work faster - often in just a few weeks - and are easier to use. But when antibiotics actually do bring your acne under control, do you need to continue to take them forever? And if you do discontinue them, will your acne just rebound and breakout all over again? Fortunately, the answer to both is no. I like to explain this by saying that your skin has a memory. When it's "used" to breaking out, it continues to, and when it's used to not breaking out, it tends to stay clear for quite a while. For the scientists out there, you could call this the inertial theory of acne! So when your acne is controlled with antibiotics, and stays clear for a few weeks, you can gradually decrease the dose until you are no longer taking it and still maintain acne free skin. The theory is that when your skin has stopped breaking out and gotten used to not breaking out, by gradually decreasing the dose, you sort of "fool" the body into thinking you're still taking the full dose. So instead of taking a pill twice a day, you go down to once a day for two weeks, then perhaps once every other day for two weeks. By this time, your skin has gotten so used to not breaking out, that your acne stays quiet for at least many months and sometimes even longer. A new review of 30 years of research has concluded that progestin contraceptives do not cause depression. The new study, published in the journal Contraception, concludes that progestin pills aren't linked to women developing depression at any age. A vasectomy procedure is a permanent method of birth control for men also known as male permanent sterlisation, in which the surgeon cuts and seals off a section of the male reproductive tract in order to prevent sperm from exiting the man’s body during ejaculation. Vasectomy is one form of sterilisation. In a conventional vasectomy, a scalpel will be used to make a small incision on one side of your scrotum, the thin skin pouch that houses the testicles. Your physician will isolate the vas deferens and lift it through the incision. At this point you may feel a pulling sensation, the vas deferens will be cut and a small piece may be removed. Next, the newly cut end of the vas deferens will be tied, sealed with surgical clips or cauterised. The vas deferens will be returned to the scrotum and the incision is closed with the stitches. The same steps will be repeated on the other side of the scrotum. In the non-scalpel method, your doctor will use a special ringed instrument to hold and expose the vas deferens.
