Nonetheless, we know that if the cause is pregnancy, the cure is delivery. This can be more complicated if you develop it preterm and you mentioned--- it sounds like with some of your pregnancies, you did develop it preterm. But you're able to carry your babies to full term, so that's good. Whether or not you're going to get it again, that's a great question. It depends on a few different factors. Mostly, it depends on the severity of the case and how early you started to develop it. Overall, a woman who's had preeclampsia in the past has a seven-fold chance of having it on her next pregnancy. But, again, it depends on how severe it was and how early it hit. So studies have shown that women who develop a severe case in their second trimester have a 20-65% chance of getting it the second time or with subsequent pregnancies. But women who just had a mild case of it only have a 5-7% chance of developing it again. Now, there are certain things that can increase your chances. These things are your weight before you get pregnant-- for example, any woman who has a BMI greater than 26 going into pregnancy has a higher risk of developing preeclampsia. A woman who has chronic high blood pressure has a higher chance. These are actually things that you can modify and control. You can go into your next pregnancy making sure that you're at a healthy weight. Eat good food, exercise, and do what you can to control your blood pressure. Those are the only things within your power. Other than that, there's not really anything you can do to prevent it. My recommendation to you going into your next pregnancy would be to talk to your doctor. They know the specifics of your past history and they may have more recommendations based on that. My guess is they're going to be more vigilant in watching you this time for symptoms of it and if they start to develop, they'll do all the same testing that they've done in the past and then decide when it's time for you to deliver. You did mention that you want to have a natural birth this time. That's totally, 100% fine, except for some times, there are indications for having an epidural. If it's best for you or the baby, then the nurses and doctors that are taking care of you can give you guidance on that based on the circumstances of your individual situation. For example, if you have severe preeclampsia, then everyone might feel a little more comfortable with you having an epidural because it can affect things actually in a good way. It can reduce your blood pressure temporarily. It's not a cure for the problem, it's more just like a band-aid. It gets your blood pressure to run a little bit lower during labor and can reduce your chances of having complications like seizure and those sorts of things just because it helps control your blood pressure a little bit better. The treatment so f hypertension in pregnancy depends on what kind of hypertension we are dealing with. If it is a chronic hypertension means it is a hypertension that is present prior to pregnancy. So continue the medicines if she was taking any. If you feel the medicines are not safe, we change the medication. Some medicines can give rise to certain abnormalities in the baby. Because the blood pressure has been long lasting, that is present present before the pregnancy, we check our various organs which could have been affected by this increased blood pressure like our eyes, heart, as well as functioning of the kidneys. We monitor the blood pressure during the pregnancy and woman with hypertension, they have a 20 to 25% chances of developing a superimposed preeclampsia. The pregnancy induced change can also happen. We have to monitor the baby as well. So we tell her to keep a count of how many movements she feels per day, which is called as the kick count and we ask her to come for regular prenatal checks and clinically assess how is the growth of the baby. She might require regular ultrasound monitoring to assess the growth of the baby as well as the Doppler to study the blood supply to the baby. If the blood pressure is due to gestational hypertension, where only the baby is risked she might not require medicines. So if the diastolic reading is less than 100,then we may not even start her on medicines or we may start with a low dose medicines to control the blood pressure and I all the organs are functioning normally, if the blood pressure is normal, then we may not take a decision to deliver her early. But if the BP is rising, if it is more than 110, then she will be having some warning symptoms. Her differ organs if the body will start getting affected. The baby will also get affected. The baby is going to be growth restricted, then we have to take a decision to prematurely deliver this woman.

Preeclampsia is a multi system dysfunction characterised by hypertension, proteinuria and oedema. Exact cause is not known, but the genetics plays an important role. Other causes are faulty placentation, release of abnormal cytokines causing the trigger of the blood pressure as well as endothelial dysfunction. A woman is becoming pregnant before the age of 20 years or after the age of 40 they are at increased risk. If it is a new paternity then also chances of preeclampsia is high. If it is an IVF pregnancy then also the risk is high. If another is having chronic hypertension or a previous history of preeclampsia then also incidence is high. The other causes being polyhydraminos, chronic hypertension, uncontrolled type1 or type 2 diabetes. In cases of multiple pregnancy also the chances of developing preeclampsia is more. If a mother or sister of the pregnant lady had a history of preeclampsia or if she is suffering from a connective tissue disorder like systemic lupus erythematosus then also there is increased risk of preeclampsia. We can do some modification in their diet and medicinal part so that we can modify their disease and reduce the complication in the mother and the child.

If her condition is normal then she is discharged but we advise her to continue monitoring of the BP at home and maintain a chart of it and keep a daily chart and she has to come back to the doctor and the number of visits after delivery is going to be maintain, weekly visits are done. Usually the BP comes back to normal in few days or few weeks after delivery or within 3 months. So if the BP comes back to normal in 3 months then we know it is a case of gestational hypertension or it is a case of preeclampsia and it is normalized but we warn the woman that she is at risk of developing preeclampsia in the nest pregnancy or developing BP at sometime later in life and even the cardiovascular risk is gong to increase. So some lifestyle changes are advised, Tell her to regular exercise. If the BP in pregnancy is due to chronic hypertension, then it is not going to normalize. So she has to continue taking the medicines even 3 months after delivery.

Preeclampsia is a serious condition associated with pregnancy, and in the last 2 decades, we have seen a 25% increase in the number of cases. Genetics may play a factor. Research has shown that if you have a 1st degree relative who's had it, then your chances are 2 to 4 fold times higher than someone who doesn't have a 1st degree relative with a history of preeclampsia. Weight before getting pregnant also plays a factor. Research has shown that women who have a body mass index of 26 or higher before getting pregnant have a higher chance of developing preeclampsia, and so do women with chronic hypertension. So genetics can't be modified, but your weight and your blood pressure can, and so if you are overweight or if your blood pressure is high, talk with doctor before conceiving and do what you can to get those things under control so that you and your baby can be optimally healthy during the pregnancy. Of course there are women who just randomly get preeclampsia and they don't have any risk factors. In fact, this happens frequently. There isn't really anything that we can peg it down to. And if a woman has had preeclampsia in the past, then the natural question is, "Is it going to happen again?" And really, it depends on how severe your case was and how early on it happened. Overall, a woman who's had preeclampsia in the past has a 7 fold higher chance of it. But if you had an early case of it, like if it started to develop in the 2nd trimester and it was severe, then you have a 20% to 65% chance of developing in again. But if it started to develop later on in the pregnancy and it was more mild, then your chances (according to research) are only about 5% to 7% of developing it again. About 18% to 20% of women in the United States suffer from migraines, and thankfully, most of them report that during pregnancy, they have less of them than they did before. But about 5% of women who had migraines previous to getting pregnant report that they get worse. Only 2% of women who have longterm migraine problems developed their first migraine during pregnancy, so it's unusual to get your first one during the 1st trimester or so, but it does happen. If you develop headaches during the 1st trimester, know that they can be normal because it's due to hormone changes in your body, but they should just be mild and they shouldn't be super persistent or get worse. So if you feel like you're having migraines because you've had them before and know what's going on, call your doctor and they'll decide what treatment would be best for you. Generally speaking, doctors prescribe tylenol first for it and then go on from there if tylenol isn't helping the issue. If you've never had a migraine before, call your doctor if you're having persistent and intense pain, and they'll talk to you more about the quality of the pain, what brought it on, if there were other symptoms associated with it, what makes it worse, what makes it better, and then decide what your headache is due to. And again, if it is indeed a migraine, they'll tell you what you can take to help it feel butter. But in a small number of cases, there are more serious underlying problems associated with headaches, and so the doctor will also want to rule those out too. After 20 weeks of pregnancy, the first thing a doctor will consider if a woman has a headache is preeclampsia. And this is associated with high blood pressure, so the first thing they'll do is test your blood pressure. And if that's normal, then they'll consider other underlying causes, but that has to be ruled out first.
