Diffuse patterned hair loss (DPA) is a variant of typical, common male pattern hair loss. With DPA, we see even thinning across the top of the scalp with no hair loss from the back and sides of the head. Because the hair on the sides and back is permanent, those with a DPA hair loss pattern are good candidates for a hair transplant, and they are good candidates for medical therapy as well. Those with a DUPA hair loss pattern, on the other hand, lose hair not only from the top of the head but also from the back and sides. Because those with a DUPA hair loss pattern lose hair all over, there's no permanent hair to use for donor hair, and this makes them poor candidates for a hair transplant. In men, diffuse unpatterned hair loss (DUPA) is less common than DPA; perhaps only 5% of men with male pattern balding have DUPA. On the other hand, DUPA is the most common type of hair loss in women, and this is why women are not always good candidates for surgical hair restoration. We diagnose DUPA in men by looking for miniaturization or changes in hair diameter. In DUPA, we find changes in diameter not only on the top of the scalp but also on the sides. Diffuse unpatterned hair loss can be difficult to detect if someone is young because often this pattern of thinning does not become apparent until the late twenties. This is one of the reasons we don't do a hair transplant on someone young, because if you have DUPA, the back and sides become thin, the scars may become visible -- and that's whether you do FUT or FUE. Cindi asked what my backstory was, and I realize that I never discussed it in its entirety! I present to you my hair thinning & alopecia story. :) Learn more on the blog: https://cornerofhopeandmane.com/ Generally, my hair thinning is pretty diffuse throughout as I have androgenetic alopecia (also called androgenic alopecia). This type of alopecia differs from others as it typically presents as overall thinning, with a concentration an inch or two back from the forehead. Usually the immediately hairline stays intact--but still thin. So, my main bald spot is right in that area...right where my sunglasses might hit if they were propped up on my head. What's next for me and my hair thinning and hair loss? Who knows. For right now, I wear hair toppers are my solution of choice, but I imagine as my balding progresses (such a tough word as a woman) I'll eventually shave my hair off fully and wear full wigs all the time.

Female pattern hair loss (FPHL) has also been called androgenetic alopecia, male pattern baldness in women, and diffuse alopecia. FPHL is a common condition, affecting up to 50% of women. It presents as hair thinning, mainly on the crown, but sometimes also affecting the parietal, posterior vertex and bitemporal areas. There is relative preservation of hair density on the occipital scalp. The frontal hairline is usually preserved. There is often accentuation of the thinning in the midline, just posterior to the frontal hairline (Christmas tree pattern). Rarely, there is deep bitemporal notching and frontal recession similar to male balding. This pattern of thinning in a woman suggests possible hyperandrogenism. FPHL can occur anytime after puberty, however there are two peaks of onset, in a woman’s 20′s and her mid to late 40′s. FPHL can also occur in association with hormonal changes such as post-partum and perimenopausally. In my experience, the most common time of onset is mid to late 40′s, often in association with other signs of impending menopause. A patient with FPHL often complains of being able to see her scalp, experiencing sunburn on the scalp for the first time or a decreased hair density, particularly affecting the crown. Most patients perceive that their hair loss is occurring in a pattern with sparing of the occipital region although some patients feel that the hair loss is diffuse.

A 26-year old gentleman has diffuse hair loss. He wants to know if there is hope to restore his hair. Dr. Amiya Prasad, a hair transplant specialist, explains that when someone with hair loss comes to his practice, he first asks the age of onset or how old was the patient when he started losing hair. He also asks how quickly this is progressing and if there are other male family members who have similar patterns or hair loss. In male pattern hair loss, there is progressive miniaturizing of hair. Hair normally grows in a cycle and at the end of every cycle, it sheds. When the new hairs come in, they come back thinner. Dr. Prasad would typically assess the top of the scalp with a microscope to identify the amount of hairs that are thinning relative to the amount of hairs that are still thick. There are two medical options to treat hair loss. Finasteride is a drug that inhibits an enzyme called 5-alpha reductase that is responsible for the conversion of testosterone to dihydrotestosterone (DHT). Hairs that are sensitive to dihydrotestosterone will progressively thin and go away. When someone has that type of hair loss takes finasteride, it slows that progression and can allow them to have thicker hair for a long period of time. Unfortunately, medical studies have shown possible long-term sexual side effects of taking finasteride. In addition, literature does not provide enough information about all the factors that cause hair thinning because dihydrotestosterone is not the whole story. Many men will take finasteride and still progressively lose hair. The other drug is minoxidil. Minoxidil is applied topically and appears to keep hair around longer so that it appears that the scalp has more coverage. However, it does not reverse hair thinning. For a patient like this gentleman, Dr. Prasad doesn’t go for hair transplant first. This is a significant change in the paradigm of how to treat hair loss. The standard approach before is to do a hair transplant, but the patient still continuously loses hair. Many patients will opt to do another hair transplant and another one every few years until they reach the end of the donor area. Unfortunately, there’s a mismatch between the amount of hair that they have in donor and the amount of scalp they need covered. Eventually, people will have plugs on their scalp which shows an obvious hair transplant. In Dr. Prasad’s practice, he has developed a non-surgical treatment called Hair Regeneration. A material for wound healing called extracellular matrix that is used to help regenerate tissue and skin by recruiting adult stem cells can be used for hair restoration. When this material is injected in a formulation combined with platelet-rich plasma, it is remarkable in how effectively it can reverse hair thinning. Thinning in the frontal hair line is commonly hormonal, that is what is called as androgenetic alopecia. So mainly here we need to reduce the hormone effect on the hair and we also need not stimulate the hair growth and many at times in addition to this, there is a component of traction here. For example in many guys and women who have very long hair and who put a pony, whenever there is a lot of reaction on these hairs then also it can give rose to hair loss. In these situations, they go for a receding frontal hair line which may be prominent and however it is because of the androgenetic alopecia, not only there is frontal recession, but also there is also vertical thinning that is thinning on the top of the scalp. A 35-year-old female was diagnosed with thyroid disease 3 years ago post partum. Her disease is now regulated but her hair has not recovered, even though it has been over a year. She wants to know if she is a candidate for a hair transplant. Dr. Amiya Prasad, a specialist in hair restoration, says that the photos this woman submitted are what many women notice if they have hair loss - the widening of the part. Unfortunately, there are limited options when it comes to female hair loss and these options have to be properly evaluated before a person makes a fully informed decision. He advises not do the transplant at this time. In male pattern hair loss, there is the recession of the temples, expanding crown, a bald spot area and eventually the areas meeting in the middle and progressing. With women, that hair loss pattern tends to be more diffuse. Hormonal issues also play a big role in hair loss for women. Lastly, genetics can also be major factor for female pattern hair loss. In Dr. Prasad's practice, one of the things that he found particularly frustrating in transplanting in women is there's very small space between the hairs. In contrast to male pattern hair loss, an area can be smooth and bald and when he places the grafts, he's going through skin that doesn't have existing hair. Even if he places grafts between existing hairs, there is often very large gaps so he's not shocking the existing hair. He routinely uses a microscope in his examinations with hair. When he looks at women's hair that is thinning, the space between the hairs is often very limited. So even if he places a hair in between, she may lose one or two existing hairs from stress, also known as shock loss (telogen effluvium). Sometimes, even if the stress is over, they may not get back all the hair that you lost. Dr. Prasad has been developing a method of treatment for female pattern hair loss called Hair Regeneration. This is a treatment using platelet-rich plasma combined with a wound healing material called extracellular matrix (ECM) by ACell. When ECM was used for hair transplant procedures to heal donor scars, it was noted that thin hairs became thicker. This is based on a wound healing mechanism that restores native stem cell activity that restores hair growth so it becomes thicker. In addition, it turns out that the thinned hairs that regenerate into thickened hair also comes back darker. Dr. Prasad treats male and female pattern hair loss, as well as conditions such as alopecia areata with Hair Regeneration treatment. He does the Hair Regeneration treatment to 99% of his female patients rather than a hair transplantation, with the exception being a loss in the frontal hairline. If there is a loss of frontal hairline, then a hair transplantation makes a lot of sense. But if there is diffuse thinning, he would first do an evaluation, examination, review of hormonal status and a proper medical work-up to eliminate all other factors. Acute telogen effluvium has an abrupt onset. The active shedding lasts 1 to 4 months and can lead to thinning of the hair over the entire scalp. The bitemporal areas may be most obviously thinned. Once the daily shedding returns to normal, it may take several months for the scalp hair to regain its normal density. The patient’s hair goes back to normal within 1 year of onset of TE in 95% of people. A specific trigger can be found in approximately 75% of patients with acute TE. Chronic TE is defined as increased shedding lasting at least 6 months, but usually greater than one year. Chronic TE can last for many years with fluctuating severity. Its onset may be abrupt or gradual. A definite trigger is often not identified in patients with chronic TE. Chronic TE usually affects middle-aged women, many of whom give a history of extremely thick hair prior to the onset of their problem. Women with chronic TE may have normal-appearing hair (albeit thinner than the patient’s normal hair) or may show shorter frontal hairs or bitemporal thinning. Women with chronic TE often state that they have lost half or one third of their hair. A woman with chronic TE may complain that she is unable to grow her hair as long as she could in the past. Unlike in FPHL, the occipital and crown part widths are approximately the same in patients with both acute and chronic TE.
