Stage 4 is beyond pelvic cavity. Investigations are Transvaginal Ultrasound, Biopsy with pipelle, Dilation and curretage, and hysteroscopy. Treatment depends on staging, surgery of hysterectomy with bilateral salpingo-oopherectomy (BSO), pelvic wash and lymph nodes. Omentectomy and this can be done laparoscopy. If there is metastasis then also do chemotherapy and if it recurs than perform radiation therapy

The more periods you've had, the more exposure your endometrium has had to estrogen. 3. Never having been pregnant Women who have never been pregnant have a higher risk of endometrial cancer than do women who have had at least one pregnancy. 4. Older age As you get older, your risk of endometrial cancer increases. Endometrial cancer occurs most often in women who have undergone menopause. 5. Obesity Being obese increases your risk of endometrial cancer. This may occur because excess body fat alters your body's balance of hormones. 6. Hormone therapy for breast cancer Women with breast cancer who take the hormone therapy drug tamoxifen have an increased risk of developing endometrial cancer. If you're taking tamoxifen, discuss this risk with your doctor. For most women, the benefits of tamoxifen outweigh the small risk of endometrial cancer. 7. An inherited colon cancer syndrome Hereditary nonpolyposis colorectal cancer (HNPCC) is a syndrome that increases the risk of colon cancer and other cancers, including endometrial cancer. Today we're going to talk about the association of estrogen dominance as a phenomenon, and it's relationship to breast cancer. Now before we can understand that relationship we have to back up and understand what estrogen dominance really is. In the female, the balance between two critical female hormones, called estrogen and progesterone, is very very important. The estrogen in the body is normally being balanced by progesterone. So the estrogen progesterone ratio has to be in perfect shape for us to feel good. Now, if we are exposed to an environment, either by external means or internal means where the estrogen level goes up, or where the progesterone go down much more than the estrogen, then the ratio between estrogen and progesterone will therefore increase significantly. In other words, if you have a certain level of estrogen and a certain level of progesterone they usually balances each other. So if the estrogen goes up, then you have a phenomenon called estrogen dominance. This can happen if you take in excessive estrogen from external sources, such as hormone from animals that is fed growth hormone as well as other chemicals that boost the estrogen level. It can also caused by estrogen level that's increased in the body because you're taking in more estrogen from petroleum sources such as plastic, shampoos and nail polishes, etc. They all fall into a class of petroleum byproducts called xenoestrogen because they behave estrogenically. So, in other words if the estrogen level goes up, then you have a situation called estrogen dominance. At the same time, if you have progesterone goes down significantly, more than what it should, and it's not able to balance, therefore, if you have low progesterone, then even though your estrogen level can stay the same or even reduce, the fact that progesterone levels dropped so much more create an estrogen dominance situation. So in either way, the body will recognizes as increases estrogen. Well, what does it do? Estrogen dominance is a condition where itself will help many of the estrogen sensitive cells to grow. Well, what are the estrogen sensitive cells? They include the ovaries, the endometrium, the breasts, etc. So, classically, you can have fibrocystic breast disease, polycystic ovaries, endometriosis, uterine fibroids, and ultimately breast cancer. So that is one strong association, and indeed we've known now that excessive estrogen can cause cancer in the breasts. So that connection is very strong. Now don't forget that estrogen is also made in the ovaries, as well as the adrenal glands. So if you are under a lot of stress, your adrenal is gonna work hard and then your estrogen level is gonna go up. Similarly, if you have a diet that is very high in sugar, and you may have cysts in your ovaries, and your ovarian system may be putting out more estrogen as well. So it is very important to understand that estrogen dominance can lead to breast cancer as part of what we call the estrogen dominance continuum. So resolving the root problem, which is excessive estrogen, is a key to prevent breast cancer. Or if you already have breast cancer, it's to be able to help to support the body so it stops the cancer cells from going further by depriving the cancer cells of estrogen which it likes to grow upon and thrive upon. Dr. McDonald explains the correlation between obesity and uterine cancer. Matt McDonald, MD: A very frequent question I am asked from my patients when diagnosed with uterine cancer is how did this happen or why did this happen to me. There is really two answers to that question. One is there is a genetic syndrome called Lynch syndrome or HNPCC that puts certain families at risk for uterine cancer and so if you have a patient or you come from a family where you see many cancers on one side of the family and those cancers that we worry about being linked to Lynch syndrome are colon cancer, uterus cancer, ovary cancer, kidney cancer, and some other more rare instances of certain brain cancers, you may actually be in a family that has a genetic mutation that puts a woman at risk for uterus and ovary cancer. The most important risk factor for a woman developing uterine cancer is really one thing. 90% of all the uterine cancers or endometrial cancers that we take care of are associated with obesity. Women that are overweight and this is actually goes up literarily depending upon how heavy a woman is meaning a 200 pound patient versus a 300 pound patient actually will have different degrees of estrogen in her body. So the heavier woman is will have a higher concentration of estrogen in her bloodstream and what estrogen does is actually continually drives the lining of the uterus to grow and so the estrogen hormone on the lining of the uterus otherwise known as the endometrium is commonly similar to like fertilizer for your lawn. It stimulates the endometrium to continually grow but to keep the endometrium safe, a woman should have regular monthly cycles for their periods and that has caused by a change in the estrogen and progesterone concentration in her bloodstream and that triggers a monthly sloughing of the endometrium which is protective of the endometrium meaning protective at preventing that woman from developing uterine cancer. Commonly what we see in our overweight patients is that they stop having regular monthly cycles and the reason for that is that interplay between estrogen and progesterone gets distorted and there is too much estrogen in that patient's bloodstream leading to a constant driving of the growth of the endometrium and that month after month after month stimulatory growth pattern of the endometrium leads to abnormalities, leads to cancer. We will do a much better job at preventing uterine cancer if we can do something about the epidemic obesity in this country. So the most important thing a woman can do in this country to help prevent uterine cancer is maintain a good body image and body size. It is a lot more difficult to do and easier said than done, but it is incredibly important for patients that are overweight to understand that if you are 50 pounds overweight, and that depends on your height, but if you are 50 pounds overweight your risk is 10 times the regular normal population of developing uterine cancer and that number actually goes up, the heavier you are. If you are 100 pounds overweight, it is a higher risk. So it is very important for our patients to know the risk of obesity for other medical symptoms and medical problems. It is alarming to me how many women that develop endometrial or uterine cancer do not know the association of obesity to that cancer. Dr. Fadi Abu Shahin, a gynecologic oncologist on the medical staff of Lee Health, says endometrial cancer can be caused by age, genetics, or medication. “Endometrial cancer is cancer of the endometrium, which is the lining of the uterus on the inside. The most common is what we call unopposed estrogen where the woman’s body is producing estrogen only and not producing progesterone to counter act the effects of estrogen in the uterus.” That can lead to overgrowth of the lining of the uterus and can become cancerous over time. Women who are at risk for endometrial cancer may have a genetic predisposition or polycystic ovarian syndrome, where they don’t ovulate regularly. In both cases doctors may prescribe birth control or hormone therapy as a preventative. “The best treatment for those women is to put them on birth control pills or some form of hormonal therapy to prevent and protect the uterus from exposure to estrogen,” said Dr. Abu Shahin. There’s no regular screening for endometrial cancer, but doctors say if women experience symptoms they need to call their physician. “In women after menopause any bleeding is abnormal. In this video as well as in my article on my website on this very topic, called 'Estrogen Dominance', explains the phenomenon of fibrocystic breast disease and its significance. Let's take a step back. When the body is growing, estrogen is necessary. It is oftentimes balanced with progesterone. When estrogen is in excess, then it flows into various tissues including the breasts and fibrocystic breast disease becomes a symptom. The breast feels enlarged, it can often become rocky, and on ultrasound you can have multiple cysts and this can be very uncomfortable. These cysts can often times be made worse during the menstrual cycle. In severe cases the estrogen causes not only the cysts to grow but also goes to other places in the body including the uterus leading to endometriosis, as well as ultimately cancer. So, fibrocystic breast disease is one of the earliest signs of a body in hormonal imbalance. Most people, conventional doctors especially, will tend to resolve this with the use of birth control pills. There are good things to this but at the same time it really suppresses the symptoms and does not address the real, underlying cause of the fibrocystic breast disease. Now I urge you, if you have these type of symptoms, of course, check with your doctor to make sure that everything is fine. But, if you have other symptoms, including things like wandering tension, irritability, brain fog, as well as period pain and heavy bleeding, you have to think, what is really going on that causes the body to behave like this. These are not normal behaviors, these are behaviors that your body is trying to send you a message and the message is do not put more estrogen in the system. Luteal Phase: The Uterine Lining Phase

Now we'll be talking about the Luteal Phase of the whole Ovulation/Menstrual Cycle. The Luteal Phase happens after the Ovulation Phase, which really is part of the Follicular Phase. So when you have the Luteal Phase, it's the same for all women - it lasts approximately 14 days, and doesn't really change. Now in the other phase where estrogen was really increasing, now we have a different hormone in this cycle that's increasing. So you have in this part, estrogen is actually decreasing, and you have progesterone hormone which is increasing. Now the progesterone hormone has a different role than what estrogen did. So progesterone here is really to grow and thicken that uterine lining. And the reason being is that is what you want to be the wonderful nutrient home so that if the egg actually does get fertilized and become an embryo, it can actually find a nice lovely home and implant inside the uterine lining. Now with that being said, progesterone is also the hormone that can cause women to have those PMS symptoms, or pre-menstrual symptoms. So women may notice they have more mood swings, or more acne, or more bloating, things like that. So if you have those symptoms, you will be aware that you are in this part of your cycle. Now with that being said, there's 2 ways this can go. If the egg does get fertilized and become an embryo and implants in the uterine lining, now what happens is there is a component of the embryo that will actually start releasing another hormone, and it is called HCG, which is the short term. The very long term of that is Human Chorionic Gonadotropin. This hormone is actually the hormone that gets tested in pregnancy kits, whether or not you take a pregnancy kit test in the office or at home, that is actually what is being tested. And so you can actually test for this hormone about 2 weeks after your ovulation window, or probably a better way to look at it would be 5 weeks from the first day of your period. And that way you can know the right time to take a pregnancy test. Now if the egg doesn't get fertilized and doesn't become an embryo, then another change is going to happen. Your uterus is going to get a signal that there is no embryo, and then progesterone levels which were on the rise are now going to start to decrease. And what that is gong to do, is that lining that the progesterone was making really thick and growing that lining, is now going to signal to the uterus that it's going to get time to shed it. And that's when women notice that they have their menses or menstrual part of their cycle. When women have menstrual cycles, the normal range for a menstrual cycle is anywhere from 24-35 days. Most women tend to have 28 day cycles or 30 day cycles, but it can be normal in the 24-35 day range, and this can help you understand that you're probably having a cycle where ovulation is occurring. If you're having more cycles that fall out of that range, more like 21 days or 36-38 days, it could be an indication that you're not actually ovulating. So if this is occurring, you might want to contact your provider, and then get further evaluation done to see if you are actually ovulating or not.

There is really two answers to that question. One is there is a genetic syndrome called Lynch syndrome or HNPCC that puts certain families at risk for uterine cancer and so if you have a patient or you come from a family where you see many cancers on one side of the family and those cancers that we worry about being linked to Lynch syndrome are colon cancer, uterus cancer, ovary cancer, kidney cancer, and some other more rare instances of certain brain cancers, you may actually be in a family that has a genetic mutation that puts a woman at risk for uterus and ovary cancer. The most important risk factor for a woman developing uterine cancer is really one thing. 90% of all the uterine cancers or endometrial cancers that we take care of are associated with obesity. Women that are overweight and this is actually goes up literarily depending upon how heavy a woman is meaning a 200 pound patient versus a 300 pound patient actually will have different degrees of estrogen in her body. So the heavier woman is will have a higher concentration of estrogen in her bloodstream and what estrogen does is actually continually drives the lining of the uterus to grow and so the estrogen hormone on the lining of the uterus otherwise known as the endometrium is commonly similar to like fertilizer for your lawn. It is a lot more difficult to do and easier said than done, but it is incredibly important for patients that are overweight to understand that if you are 50 pounds overweight, and that depends on your height, but if you are 50 pounds overweight your risk is 10 times the regular normal population of developing uterine cancer and that number actually goes up, the heavier you are. If you are 100 pounds overweight, it is a higher risk. So it is very important for our patients to know the risk of obesity for other medical symptoms and medical problems. It is alarming to me how many women that develop endometrial or uterine cancer do not know the association of obesity to that cancer.
