Antibiotics are medicines used to treat bacterial infections. There are different types of antibiotics that work against different strains of bacteria and some types of parasites. When used properly, they either kill or inhibit the growth of bacteria that cause a particular health problem. This in turn helps your natural immune system fight the infection. But antibiotics often have side effects that cause other temporary health issues. Some common side effects include diarrhea, nausea, vomiting, vaginal yeast infections, allergic reactions and inner ear problems. 1. One way to deal with side effects of antibiotics is to stay healthy so you don’t have to take them for long. Take steps to support your health in the following way. 10. Take Antibiotics as Directed. Last but not least, you must always take your antibiotic medicine as directed by your physician. For instance, some antibiotics should be taken only with water. Others need to be taken with food in order to increase their absorption in the body and ward off an upset stomach, a common side effect of antibiotics. If you’ve been treated with a course of antibiotics for a long or a short time, or if you’ve been using food that has antibiotics in it, without you knowing, or if you’ve been misusing antibiotics in any way, you might start to have health problems. You might suffer from certain complaints that will make your life a little bit more difficult. Side Effects of Antibiotics Let me discuss the complaints that you might have from use of antibiotics. The most affected organs would probably be your intestines. That’s because the antibiotics are going to kill the bacterial flora, which is the friendly bacteria in your intestines that is responsible for digestion and making vitamin K, which is important for you. So, you will notice that you’re having trouble in your gut, so, you will start to have bloating, gas, irregular bowel movement, so, you will have constipation, then diarrhea and then bloating again and so on. You might also suffer from heartburn and feeling generally unwell. You might also start to notice bleeding from the gums or from the nose. That’s also related to the lack of vitamin K. You might also suffer from yeast infections, which is a fungal infection that can be anywhere on your body, either on your skin or in your mouth or in the genital area. All that is because your bacteria flora – the friendly bacteria in the gut – has been affected by the antibiotics. So, what we need to do now is correct the level of bacterial flora in your gut. How do we do that? First of, you will need to lead a healthy lifestyle. So, you drink a lot of water. Avoid bad foods. Bad foods are rich in sugar, carbohydrates that are unhealthy, like pasta, packed foods, processed foods – these are all unhealthy foods. Instead of that you should eat vegetables, fruits, grains, healthy fats (that’s vegetable oils mainly). Probiotics and Prebiotics You can also use probiotics. These are very healthy foods that are fermented, so that helps the growth of the good bacteria in the gut. Probiotics are mainly: yogurt, kefir and yeast. If you are suffering from any problems with kefir or yeast, I would suggest that you take it slowly. So you start with very small amounts, like one spoon a day, and then increase it gradually. If you go to the shop and want to buy probiotics, look for the name lactobacillus on it – that’s the good bacteria for you. You can also use foods that are called prebiotics. Prebiotics are foods that help the growth of bacteria, because they don’t get absorbed in the body, they stay for a long time in the intestines. Like, leeks (garlic, onions), honey, bananas, these are all very good foods for your bacteria flora. You can also try fermented fresh vegetables. That means, pickles that are pickled in a healthy way. So, you can try to pickle at home. So, try for example the ginger, to pickle it at home, or maybe cucumbers, also are healthy. But don’t add too much salt. So, pickled vegetables are very good for your gut flora. If you’re a fan of soups you can try bone broth or gelatin soups. You can buy bones from the butcher, and what’s inside – the broth – you can make soups with it, and that’s very good for your gut flora. You can also add onions to it, or any vegetables of your choice. It’s very important to know that your gut flora affects the function of your intestines, and healthy intestines affect the whole body. Stool color and significance The color of stool can vary dramatically and can also be a clue as to whether various disease states are present. Normal stool is brown due to its composition: bacteria, water, bile, bilirubin, broken-down red blood celIs and indigestible plant matter like cellulose, along with small amounts of protein and fat. Red stool is most worrisome as it indicates bleeding in the lower GI tract from conditions like hemorrhoids or diverticulosis, or more serious conditions like rectal cancer. Red stool can also be caused by ingesting red food coloring or beets. While it should always be reported, it's not always an ominous sign. Green stool can occur with rapid transit through the intestines where bile doesn't have a chance to be broken down to its final brown color. Green can also be a sign of Crohn's disease, antibiotic use, ingestion of leafy greens or iron therapy. Yellow stool can be the result of gallbladder dysfunction which causes improper handling of bile. Infection with giardia lamblia produces a characteristic yellow diarrhea. In addition to causing diarrhea, different types of infection in the GI tract, whether viral, bacterial or parasitic, may cause changes in stool color. White stool can be a sign of fat malabsorption, as with pancreatitis and pancreatic cancer, but barium used for X-rays can also give the same appearance. Mucus in the stool can give it a whitish appearance and may be due to inflammation or benign conditions like IBS. Black stool should trigger a search for bleeding from the upper part of the GI tract (esophagus, stomach or small intestine), but can also be seen with iron therapy, heavy meat consumption, and bismuth-containing compounds. Light-appearing clay-colored stools are characteristic of liver disease and decreased bile output, but can also be caused by antacids containing aluminum hydroxide. Vitamins and supplements commonly cause changes in urine color but may also change stool color.

A basic definition of Clostridium Difficile for Nurses C –diff is a bacteria that causes inflammation of the colon which is called colitis. Toxins produced by c-diff attack the intestinal lining. Those at high risk for C-diff are: • Elderly patients usually over the age of 65. • Those patients with recent hospitalizations • Patients with recent antibiotic C-diff usually occurs as a 3-step process: 1. First step is destruction of the normal flora (the current antibiotic may have killed good bacteria along with bad bacteria) 2. Ingests C-Diff bacteria (primarily acquired nosocomially as the spores can be abundant in the hospital environment) 3. Developments of C-diff disease – Usually around 7 day incubation. Signs and symptoms: • Abdominal cramping and pain • Watery stool ( 10-15 per day) • Dehydration • Fever • Renal failure • Pus or blood in stool Tests: • Stool cultures (can take several days to grow) • Colonoscopy/sigmoidoscopy Treatments: • Try to stop current antibiotic that triggered the illness if possible • Start new antibiotic to combat C-Diff bacteria (usually flagyl or vanco) • Surgery to remove part of colon affected • Probiotics – helps restore healthy organisms such as bacteria and yeast to help restore the healthy balance of the intestine. May also deter further C.diff infections. This bacteria can live on surface for a long period of time. It is found in feces. This handwritten video lecture will review causes, clinical findings, investigations work and management of diarrhea for medical students taking the USMLE ETIOLOGY or CAUSES Infectious is number one cause of diarrhea (90%). Non infectious include medications, toxin ingestion, ischemia. CLINICAL FINDINGS Vomiting and diarrhea is associated with Staph aureus, Bacillus Cereus, Norwalk Virus. Watery Diarrhea is assoicated with Clostridium perfringens, ETEC, and viral diarrhea. Inflammatory diarrhea is caused by campylobacter, salmonella, EHEC, shigella. Inflammatory diarrhea is also associated with pain and fever. Systemic symptoms are reactive arthritis which is associated with slamonella, campylobacter, shigella, yersinia. Reactive arthritis is associated with arthritis, urethritis, conjunctivits. Hemolytic Uremic Syndrome (HUS) is caused by E Coli (0157:H7) and Shigella. Syptoms are related to hemolysis and uremia. EVALUATION of Diarrhea. If mild than no work up is necessary. However, alarm signs require work up. These alarm signs are severe dehydration, bloody stool, Fever greater than 38.5. History of antibiotic use, abdominal pain and immunocompromised patients should get a workup done. First step in evaluation is to take a thorough history to limit and narrow down the pathogen causing diarrhea. If travel history focus on Giardia (Russia, Swimming, Mountanous), ETEC, Norovirus (cruise ship). What type of food have they ate. Meat food poisoning is caused by staph aureus (egg), clostridium perfringens (canned meat), EHEC (hamburgers), Bacillus (Chicken fried rice), Salmonella (eggs and veg), Yersinia (Milk and cheese), Listeria (more comon in pregnant women). Poultry is primarily associated campylobacter. Diarrhea form seafood is associated with vibrio cholera and vibro parahemolyticus. Patients associated with daycare and get diarrhea there are certain pathogens. Shigella, Rotavirus, Norovirus, Giardia, Cryptosporidium. Hospital Acquired Diarrhea is primarily associated with clostridium dificile (anti-biotics). Immunocompromised patients have increased risk of cryptosporidium and isospora. Pathogens associated with fever and diarrhea suggests invasive pathogens such as salmonella, shigella, campylobacter as well as virus and entamoeba histolytica and clostridium dificile. Time can also help determine the cause of diarrhea. If vomiting occurs 1-8 hours after heating then a preformed toxin was ingested. This is most commonly associated with bacillus cereus, S. aureus and Clostridium perfringens. If vomtiing occures 8-72 hours then diarrhea is caused by enterotoxin, primarily Vibro cholera, ETEC. 1-8 days are caused by enteroadherent which prevents absorption of food into gut. This is caused by EPEC and EHEC, Giardia and cryptosporidium. Lab investigation for diarrhea should be focused on details from history. CBC, Hematocrit, and WBC is important. Stool Studies - fecal leukoctyesfor WBC in stool are positive in salmonella shigella but this is also found in Ulcerative colitis and crohn's disease as they all cause some sort of inflammation. If blood is present then dysentary pathogens such as campylobacter, salmonella, enterhemorrhagic E. Coli, and entamoeba histolytica. Stool culture is not done routinely, unless it is very severe infection. Ova and parasites if there is persistent diarrhea that is not going away, recent travel, daycare, homosexual or AIDS patients. If there is bloody diarrhea but leukocyte negative. Fecal antigen test for Giardia, and Rotovirus. Testing for toxin such as clostridium dificile if there is a history of anti-biotics. Fecal osmolar gap and fecal fat is used in chornic diarrhea, not acute. TREATMENT Initial treatment for diarrhea focuses on relieving dehydration with oral rehydration therapy. Use IV if very severe. Gatorade is not optimal, but it is okay. Empirical anti-Biotics can be given for diarrhea, but it is not always required because most are self-limiting. Sometimes anti-biotics can actually worsen symtoms of diarrhea. Most commonly prescribed is flouroquinolones such as ciprofloxacin, norfloxacin. Clostridium dificile of metronidazole, vancomycin. EHEC should not be given anti-biotic due to release of shiga toxin. Culture and sensitivity can guide the rest of treatment.

Alcohol - How Alcohol Affects The Body - What Causes A Hangover

In this video I discuss how alcohol affects the body, and some of the side effects of alcohol in the human body. I go through the path of alcohol in the body, the damage from alcohol, and what causes a hangover. I also discuss how drinking alcohol over time can harm your body, such as by causing a fatty liver. Transcript (partial) We are going to take a little trip of what happens to alcohol in the body. Once it is consumed alcohol goes down the normal food path of digestion. From the mouth through the esophagus and into the stomach. Here, about 20% of alcohol is absorbed through the stomach lining into the bloodstream, which means it is getting into the bloodstream very quickly. From the stomach, the alcohol that was not absorbed in the stomach next travels to the small intestine. One note here, if there is no food in the stomach, so an empty stomach, or if the alcohol is not consumed with any food, it gets to the small intestine very quickly. In the small intestine, the rest of the alcohol is absorbed into the bloodstream and travels to the liver. So, in the liver, an enzyme called alcohol dehydrogenase, which is also present in the lining of the stomach, which we will call ADH, oxidizes the alcohol, or ethanol molecule. In basic terms this means that the enzyme comes in and changes the chemical structure of ethanol, so, ethanol becomes acetaldehyde. This substance is known to be toxic and carcinogenic, or poisonous and cancer causing. This acetaldehyde is then metabolized down to a substance called acetic acid, which is less harmful to the body. Acetic acid can then be broken down into carbon dioxide and water. When alcohol is present, the liver will work on metabolizing it first. So, fatty acids can accumulate, which is why so many heavy drinkers develop fatty livers. It is estimated that the liver can eliminate about 0.5oz of alcohol per hour, which is about 1 beer, or 1 glass of wine, or 1 shot. The heart then pumps the alcohol rich blood to the lungs. Some of the alcohol in the lungs is breathed out every time you exhale causing your breath to smell of liquor. The lungs send the alcohol containing blood back to the heart where it is pumped to all parts of the body, including the brain. Once alcohol enters the brain, it slows down nerve cells that control your ability to move and think. So, judgment becomes impaired and movement becomes disrupted. Some people will begin to sweat and most will smell like alcohol. Alcohol also decreases the body’s production of anti-diuretic hormone. Antidiuretic hormone helps your kidneys manage the amount of water in your body. The decrease of this hormone causes the kidneys to not reabsorb water; instead it is excreted as urine, causing the body to become dehydrated. If alcohol consumption continues, it could lead to loss of consciousness. And massive alcohol consumption or binge drinking could lead to alcohol poisoning. This happens when there is a high concentration of alcohol in the bloodstream and this could result in coma, respiratory depression or possibly death. Now let’s look at the aftereffects of alcohol over consumption…the dreaded hangover. The exact causes of a hangover are not completely understood, but there are several factors that may contribute to it. The chemical acetaldehyde is formed from ethanol, it is believed that this chemical is what causes the headaches associated with hangovers. The increase in urination leading to dehydration, which could cause the thirst, dry mouth and dizziness. Some immune cells produce substances called cytokines, which can contribute to nausea and. Some alcoholic beverages increase the release of gastric acid in the stomach, and delay the emptying of the contents in the stomach, which could be the reason for stomach pain associated with hangovers. Alcohol can also interfere with the livers production of glucose, the main form of energy for cells, which could contribute to dizziness, disorientation and lack of energy. The long term effects of alcohol over consumption include anemia, which is a low amount of oxygen carrying red blood cells. It can lead to cell death in the liver cells and brain cells, leading to these organs not functioning properly. The risk of heart failure increases; as does the risk of stomach and intestinal problems, and many heavy drinkers have high blood pressure. Over consumption of alcohol can also lead to relationship problems, depression, and employment problems. And these are just a few of the long term problems associated with constant over consumption of alcohol. It is always about moderation. Limiting yourself to 1 or 2 drinks from time to time is probably a good strategy. As you can see, over consumption of alcohol has a lot of negative effects on your body, and consistent over consumption of alcohol has catastrophic effects on your body.
