According to new research, long-term antibiotic use may cause bowel polyps, which sometimes leads to colorectal cancers. The study focused their efforts on a group of 16, 642 nurses who used antibiotics for two months or more. The study’s authors stated, “Antibiotics fundamentally alter the gut microbiome… and [reduce] the resistance to hostile bugs. Diarrhea is defined as the passing of three or more unformed loose stools. There are three types of diarrhea, one is acute where it can last anywhere between one to seven days. It can be persistent if it is lasting up to one month and it can be known as chronic if it is lasting for more than four weeks. Now the causes of an acute diarrhea could be a virus like your rotavirus, it could be a bacteria like salmonella or shigella, it could be a parasite or protozoa like giardiasis and it could also be induced by an antibiotic. Now the causes for a chronic diarrhea can include inflammatory bowel disease, irritable bowel syndrome, food intolerance like a cow’s milk protein allergy. The symptoms of gastroenteritis include loose stools which can just be watery or it can be mucousy with blood in it and it could be foul smelling. Now some or all of the following symptoms may be present along with this diarrhea. There is vomiting, fever which can be low or high grade, and cramping abdominal pain. Now dehydration is the biggest complication that can be associated with diarrhea and vomiting. What are these signs of dehydration? The child can have sunken eyes, little or no tears upon crying, dry lips and mouth. The soft spot on the scalp can be sunken in infants. There can be increased heart rate, there can be tenting of the skin, there can be decreased urine output, the child may be excessively sleepy or irritable. Severe dehydration can be fatal and should be prevented by early administration of oral rehydration therapy. Do not use homemade ORS solution as the proportion of the electrolytes may be wrong and can cause more harm. Prepare the ORS exactly as per the instructions on the sachets. Now after consulting with your physician, you may want to replace with the recommended quantities of ORS at home for mild dehydration. The fluid replacement accounting for approximately 50ml per kilogram should be accomplished in the first three to four hours in small quantities given every 15 minutes. For example, in infants you could give one to two teaspoons every 15 minutes. In toddlers, you could do three teaspoons every 15 minutes and older children could use about half an ounce every 15 minutes. Now after this three to four hour replacement, you need to then replace with 10ml per kilogram for every loose stool thereafter. Moderate dehydration can be corrected with ORS in an emergency room under medical supervision. If there is severe dehydration, it definitely warrants immediate hospitalization for correction of the electrolyte imbalance by intravenous fluids. In terms of management and supportive treatment, we need to know that viral diarrhea gets better on its own with supportive treatment such as rest and incorporating certain dietary changes such as; do not give your child any fluids or foods containing sugar, avoid dairy products other than yogurt because curd contains lactobacillus which is very good for the gut. You can eat rice and rice based products such as rice, rice congee, khichdi, idlis, etc. Apples, apple sauce, and bananas can be given. If your baby is breastfeeding, continue to do so. If your baby is on a formula, a change to a lactose free formula may be warranted if there is no improvement with all this dietary changes. Feed your baby small amounts frequently to support their caloric requirement through this infection and hydration with fluids, especially your ORS is very important and you can give other fluids like water, tender coconut water, diluted curd which is known as butter milk. Bacterial diarrhea can make your child very sick and will require some investigation of the stool and the blood and will be followed by treatment with appropriate antibiotics by your doctor. Diarrhea is defined as abnormally liquid or unformed stool that is produced at high frequency (greater than 200g/day) lasting less than 14 days. This handwritten video lecture will review causes, clinical findings, investigations work and management of diarrhea for medical students taking the USMLE ETIOLOGY or CAUSES Infectious is number one cause of diarrhea (90%). Non infectious include medications, toxin ingestion, ischemia. CLINICAL FINDINGS Vomiting and diarrhea is associated with Staph aureus, Bacillus Cereus, Norwalk Virus. Watery Diarrhea is assoicated with Clostridium perfringens, ETEC, and viral diarrhea. Inflammatory diarrhea is caused by campylobacter, salmonella, EHEC, shigella. Inflammatory diarrhea is also associated with pain and fever. Systemic symptoms are reactive arthritis which is associated with slamonella, campylobacter, shigella, yersinia. Reactive arthritis is associated with arthritis, urethritis, conjunctivits. Hemolytic Uremic Syndrome (HUS) is caused by E Coli (0157:H7) and Shigella. Syptoms are related to hemolysis and uremia. EVALUATION of Diarrhea. If mild than no work up is necessary. However, alarm signs require work up. These alarm signs are severe dehydration, bloody stool, Fever greater than 38.5. History of antibiotic use, abdominal pain and immunocompromised patients should get a workup done. First step in evaluation is to take a thorough history to limit and narrow down the pathogen causing diarrhea. If travel history focus on Giardia (Russia, Swimming, Mountanous), ETEC, Norovirus (cruise ship). What type of food have they ate. Meat food poisoning is caused by staph aureus (egg), clostridium perfringens (canned meat), EHEC (hamburgers), Bacillus (Chicken fried rice), Salmonella (eggs and veg), Yersinia (Milk and cheese), Listeria (more comon in pregnant women). Poultry is primarily associated campylobacter. Diarrhea form seafood is associated with vibrio cholera and vibro parahemolyticus. Patients associated with daycare and get diarrhea there are certain pathogens. Shigella, Rotavirus, Norovirus, Giardia, Cryptosporidium. Hospital Acquired Diarrhea is primarily associated with clostridium dificile (anti-biotics). Immunocompromised patients have increased risk of cryptosporidium and isospora. Pathogens associated with fever and diarrhea suggests invasive pathogens such as salmonella, shigella, campylobacter as well as virus and entamoeba histolytica and clostridium dificile. Time can also help determine the cause of diarrhea. If vomiting occurs 1-8 hours after heating then a preformed toxin was ingested. This is most commonly associated with bacillus cereus, S. aureus and Clostridium perfringens. If vomtiing occures 8-72 hours then diarrhea is caused by enterotoxin, primarily Vibro cholera, ETEC. 1-8 days are caused by enteroadherent which prevents absorption of food into gut. This is caused by EPEC and EHEC, Giardia and cryptosporidium. Lab investigation for diarrhea should be focused on details from history. CBC, Hematocrit, and WBC is important. Stool Studies - fecal leukoctyesfor WBC in stool are positive in salmonella shigella but this is also found in Ulcerative colitis and crohn's disease as they all cause some sort of inflammation. If blood is present then dysentary pathogens such as campylobacter, salmonella, enterhemorrhagic E. Coli, and entamoeba histolytica. Stool culture is not done routinely, unless it is very severe infection. Ova and parasites if there is persistent diarrhea that is not going away, recent travel, daycare, homosexual or AIDS patients. If there is bloody diarrhea but leukocyte negative. Fecal antigen test for Giardia, and Rotovirus. Testing for toxin such as clostridium dificile if there is a history of anti-biotics. Fecal osmolar gap and fecal fat is used in chornic diarrhea, not acute. TREATMENT Initial treatment for diarrhea focuses on relieving dehydration with oral rehydration therapy. Use IV if very severe. Gatorade is not optimal, but it is okay. Empirical anti-Biotics can be given for diarrhea, but it is not always required because most are self-limiting. Sometimes anti-biotics can actually worsen symtoms of diarrhea. Most commonly prescribed is flouroquinolones such as ciprofloxacin, norfloxacin. Clostridium dificile of metronidazole, vancomycin. EHEC should not be given anti-biotic due to release of shiga toxin. Culture and sensitivity can guide the rest of treatment.

Infection of cholera could also be a reason for a watery diarrhea. People suffering diabetics and hypertension also becomes victim of it. Let us discuss some of the causes of diarrhea in details. Bacteria In a routine on daily basis we are consuming lot of all type of bacteria, yet there are some bacteria which are not suitable for the health and disrupt the balance of our body. These unsuitable bacteria can cause the symptoms of diarrhea. If you are suffering from a chronic diarrhea it is advisable that you may consult your doctor immediately. Viruses Viruses are another cause of bacteria. Some viruses such as Norwalk virus and rotavirus are considered as the common cause of diarrhea in the young children. Their body recovers in few days without the use of antibiotic and medication but they should go for a complete bed rest and use proper hydration. Do you see blood in your urine when you go pee? Why you could be urinating blood and what you should do about it. I need your advice. Why is there blood in my urine? I remember my mother's advice: if nothing is bleeding or broken, you don't need me or a doctor. But I am bleeding somewhere. There's blood. That means that you'll need a doctor. Do I have to? Doctors are expensive. One possible cause is a tear in the bladder. That could be a source of blood. How would that be fixed? Surgery? Maybe surgery, maybe letting it heal on its own. That depends on the severity. What else could cause this? For women, the blood from a period could be mistaken for blood in the urine. For guys, an STD could cause it. How would an STD cause blood in the urine? When someone says STD, we think of sores and lesions on the outside. But the infection can go up the urethra, and every time you pee, the sores are ripped open. Oh, God, that's disgusting. They even shot guys up with mercury as a cure, as in a night with Venus the love goddess, a lifetime of Mercury. It gets worse. It could get life threatening in the days before antibiotics, because the pustules could get worse until it blocked the flow. I am no where near that bad. It just burns. You could have a bladder infection. That infection could cause bleeding, which them comes out the only flow. Wouldn't I have to have a fever? In most cases, yes. But you could have the infection with a low grade or recurring fever. I wish there was a more benign case. I don't want to go in imagining horrific growths and tumors and surgery. You might just have kidney stones. Those are incredibly common, but often painful, causes of blood in the urine. I heard about how they use sonic waves to vibrate it until it breaks. Then you have to pass it. That hurts like heck, but it's better than surgery. If you were older, another cause could be an enlarged prostate. All of these possibilities are frustrating. I've got to get to a doctor. That's what I said the first time!

Pancreatitis is inflammation of the pancreas. It happens when digestive enzymes start digesting the pancreas itself. Pancreatitis can be acute or chronic. Either form is serious and can lead to complications. Acute pancreatitis occurs suddenly and usually goes away in a few days with treatment. It is often caused by gallstones. Common symptoms are severe pain in the upper abdomen, nausea, and vomiting. Treatment is usually a few days in the hospital for intravenous (IV) fluids, antibiotics, and medicines to relieve pain. Chronic pancreatitis does not heal or improve. It gets worse over time and leads to permanent damage. The most common cause is heavy alcohol use.
