EPIDEMIOLOGY OF LUNG CANCER Adenocarcinoma is the most common right now and is the most common. It is the third most common cancer and is the most common cause of cancer death. RISK FACTORS OF LUNG CANCER Smoking, Asbestos, Radiation, Arsenic Chromium, Nickel. Genetics, Scar Formation (Wegeners, Fibrosis, Scleroderma) Air pollution, HIV. CLINICAL SIGNS AND SYMPTOMS OF LUNG CANCER Pulmonary Symptoms, most common is cough, Hemoptysis (which ca lead to asphyxia) Chest Pain (dull achy), Dyspnea (secondary to pneumonia, atelectasis, lymphatic tumor spread, pleural effusion and pneumothorax), hoarsenes (when laryngeal nerve involved), pleural effusion (serous, serosangiounous, bloody) which may be caused by metastasis into pleural fluid. Super Vena Cava Syndrome occurs when central tumor invades SVC which causes red, edematous face, prominent veins throughout the body and this is more common in small cell lung cancer. Pancoast tumor is a lung cancer located in the apex of the lungs. Pancoast tumor may lead to thoracic outlet syndrome (pain weakness in arm and hand). Horner Syndrome causes miosis, anhydrosis and ptosis (MAP) and this is more common in Non Small Cell Lung Cancer. Paraneoplastic syndromes are due to hypercalcemia (PTHrP) bone mets, calcitriol release, cytokine. SIADH causes increase water absorption leading to dilution hyponatremia, cerebral edema, anorexia, nausea vomiting. Resolves with chemo and treatment. Lambert Eaton Myasthenic Syndrome (LEMS) is a condition with auto-antibodies of the calcium channels in the Neuromuscular Junction. Proximal Muscle weakness and rising and standing. Hematologic derangements such as anemia, leukocytosis, thrombocytosis and hypercoagulability. Hypertrophic osteoarthropathy may lead to digital clubbing, periositis, Arthralgia, Cushings due to release of Ectopic ACTH. Bronchoalverolar carcionma is in situ carcinoma and has better prognosis. Most common in Non-Smokers (BAC) Squamous Cell Lung Cancer Irritation of bronchus causing metaplasia, then dysplasia, then carcinoma in situ and finally lung cancer. Also associated with Keratin pearls and cavitation from central necrosis. Goes into hilar nodes and gets late metastasis. Asymptomatic and can be deteetd with sputum cytology. Large Cell Lung Cancer Lots of cytoplasm, unknown origin and is a waste basket column. Peripheral and has late metastasis and can use cancer. TREATMENT OF LUNG CANCER Small Cell Lung Cancer metastasizes early on and therefore surgery is not an option. Cisplatin, Carboplatin and vincristin is also used. Responds very well to chemotherapy. Prophylactic cranial irradiation (PCI) to prevent brain metastasis. Surgery is possible if the tumor is very small and no metastsis Non Small Cell Lung Cancer is not responsive to chemotherapy or radiotherapy so surgery is the only option.

This video will be on the pathology of lung cancers. This is the most common cause of cancer related deaths. Some risk factors for lung cancer, asbestos, arsenic, and radon among others. Lastly, you can compress on the first cervical rib and cause thoracic outlet syndrome. This compresses the brachial plexus as well as the blood vessels and leads to atrophy, weakness and pain in the shoulder and arms. Lastly a complication of any tumor is paraneoplastic syndromes. Now what are the specific types of lung cancers? Neoplasms are broken into two categories: small cell and non-small cell tumors. Let's discuss non-small cell first. 1) Squamous cell carcinoma: loves to cavitate. Microscopically shows keratin pearls on histology and intercellular bridges. Paraneoplastic syndromes include hypercalcemia due to it's release of PTHrP. 2) Large cell: shows large pleomorphic cancer cells. Releases b-Hcg and can cause gynecomastia and galactorrhea. 3) Adenocarcinoma: seen in non-smokers and in women. Instead it's due to activating mutations of proto-onco genes like KRAS, EGFR, and ALK. Histologically you'll see the glandular cells and they are mucin positive. There is a special subtype called bronchoalveolar. From your Clara cells, it grows along your alveolar wall, thickens it and CXR looks like interstitial/atypical pneumonia. 4) Bronchial Carcinoid tumor: not only seen in the gut, it can also be seen in your lungs. It is from the neuroendocrine cells of your lungs and will be small blue cells that are chromogranin A+. Grossly they look like a polyp and rarely mets. They release serotonin, causing vasoconstriction and wheezing. Now let's talk about small cell lung cancers. These cancers are associated with smoking as well as a mutation in L-MYC. This is a neoplasm of your neuroendocrine cells, in particular your Kulchitsky cells. Again neuroendocrine cells are chromogranin A+, as well as neuron specific enolase. Small cell lung cancer is related to many paraneoplastic syndromes. They can release ACTH (Cushings), ADH (SIADH), and antibodies against your brain (encephalitis) or presynaptic calcium channels of your neuromuscular junction (Lambert Eaton syndrome). Abnormal cells cluster to form a tumour and as they multiply, the growing tumour interferes with the lung’s ability to provide oxygen to the body. Lung cancer is the most common cancer in the world: 1.8 million cases were diagnosed in 2012 alone worldwide. And in 2015, there were 1.69 million deaths worldwide related to the disease. In 2016 alone, it is estimated that 28,400 Canadians were diagnosed with lung cancer, making up 14% of all cancer cases in the country. And that, in the same year, 20, 800 died from the disease. This is more than a quarter of all cancer deaths, and more than breast cancer, prostate cancer and colorectal cancer combined. This represents an average of 57 Canadians who succumb to this disease every day. Signs and symptoms of lung cancer include: • a persistent or worsening cough • constant chest pain made worse by deep breathing or coughing • blood-stained sputum • shortness of breath • wheezing • fatigue • loss of appetite, and • weight loss. While smoking is by far the most common cause of lung cancer, the reality is that smokers, ex-smokers and never-smokers are at risk. In other words, if you have lungs, you can get lung cancer. In addition to smoking, other risk factors exist, including: - Exposure to radon - Second-hand smoke - Exposure to other substances - A family history of lung cancer - Air pollution - And radiation therapy to the chest. The discovery of new diagnostic tools and treatment options has changed the way the disease is treated. For example, biomarkers are substances found in the body that serve as a guide in the personalized treatment of lung cancer, while new discoveries are emerging, such as targeted therapies and immuno-oncology treatments which use the body’s own immune system to fight lung cancer. With such progress being made, researchers, medical professionals, patients and their families are hopeful that by working together they will continue to succeed in reducing stigma and improving support, research and innovative therapies to help reduce lung cancer rates in Canada, and around the world.

Lung cancer, also known as carcinoma of the lung or pulmonary carcinoma, is a malignant lung tumor characterized by uncontrolled cell growth in tissues of the lung. The vast majority (80–90%) of cases of lung cancer are due to long-term exposure to tobacco smoke. About 10–15% of cases occur in people who have never smoked. These cases are often caused by a combination of genetic factors and exposure to radon gas, asbestos, or other forms of air pollution, including second-hand smoke. Worldwide, lung cancer is the most common cancer among men in terms of both incidence and mortality, and among women has the third highest incidence, and is second after breast cancer in mortality. Learn the signs of lung cancer to catch the disease early and ensure a positive outcome. Below are some early signs of lung cancer according to healthline.com. No Warning Lung cancer may not produce any noticeable symptoms in the early stages. Breathing Changes Shortness of breath or becoming easily winded are also possible symptoms of lung cancer. This symptom can occur if lung cancer blocks or narrows an airway, or if fluid from a lung tumor builds up in the chest. Make a point of noticing when you feel winded or short of breath. If this symptom occurs after climbing the stairs to your house, bringing in groceries, or performing another task you could previously do without finding it hard to breathe, don’t ignore it. Pain in the Chest Area Lung cancer may produce pain in the chest, shoulder, or back area. This aching feeling may not be associated with coughing.
