Irwig reveals persistent and possibly permanent side effects associated with a common hair loss drug. Men taking medication for hair loss may suffer from prolonged and possibly irreversible sexual dysfunction, according to a recent study by Assistant Professor of Medicine Michael S. Irwig. The study, recently published in the Journal of Sexual Medicine, reveals that finasteride--an antiandrogen that treats hair loss and is found in popular male pattern baldness drug Propecia--can cause persistent sexual dysfunction, including low sexual desire, erectile dysfunction and problems with orgasms. While labeling on the medication in the U.S. currently warns about possible reversible sexual side effects, there is no reference to the effects being persistent, says Dr. Irwig, who conducted the study along with Swapna Kolukula of the Greater Baltimore Medical Center. Other countries, including the United Kingdom and Sweden, have documented persistent risks and required medical companies to include them in labeling. Dr. Irwig first became aware of the problems caused by finasteride several years ago when he encountered several men who reported they had developed sexual dysfunction while taking the medication. "It's been very frustrating for a lot of these men because they've sought care from medical professionals who have looked at the literature and have not seen a risk of persistent sexual dysfunction," says Dr. Irwig. "So a lot of these patients have been told to see psychiatrists and psychologists and that it's all in their head." Dr. Irwig then noticed that men had reported sexual dysfunction for months after they stopped taking the medication. "I came across a website called propeciahelp.com with more than 1,400 registered users—many young healthy men who developed the same sexual side effects from finasteride—and I discovered that nobody had published a series looking at these men—who they are, how long the sexual side effects lasted, what types." Dr. Irwig's study included results from interviews with 71 men aged 21 to 46 years old to assess how long they took finasteride, the type and duration of sexual side effects and their sexual frequency before and after the medication. Dr. Irwig discovered that 94 percent developed low libido, 92 percent developed erectile dysfunction and decreased arousal, and 69 percent developed problems with orgasm. On average, the men used finasteride for approximately 28 months but experienced persistent sexual side effects for an average of 40 months, from the time they ceased taking the medication to the time of the interview. Dr. Irwig also found that the average number of sexual episodes per month dropped after finasteride use. "It turns out that almost all the men had multiple sexual function problems," says Dr. Irwig. "Before finasteride use, the men experienced average sexual activity of approximately 26 episodes per month, but after use, it came down to approximately eight per month —an almost two-thirds reduction." "Twenty percent of patients I interviewed experienced persistent sexual dysfunction for more than five years, which makes me wonder if their persistent sexual dysfunction is permanent," he adds. Dr. Irwig says roughly 5 percent of men who take medicine will experience sexual dysfunction, but that "out of that 5 percent, it's hard to tell how many will experience persistent symptoms." "We know that this is a potential problem, but we can't quantify what the exact risk is. I can't tell a man if he has a 1-in-100 chance, or a 1-in-1000 chance of developing persistent sexual dysfunction, but it's pretty clear there's a relationship here," he says. Topical Rogaine can be used as an alternative to Propecia —applied directly on the skin, it does not get absorbed by body and thus does not cause sexual side effects—but Dr. Irwig says Propecia is very effective in treating hair loss and thus is commonly prescribed. So Dr. Irwig says it is crucial physicians treating male pattern hair loss discuss the possibility of persistent sexual side effects associated with finasteride with their patients. One of the best FDA-approved hair loss medications on the market to slow down male pattern baldness is finasteride, also known as Propecia. Finasteride works by blocking testosterone from converting into DHT. An medical study by the National Center for Biotechnology Information (NCBI) revealed that taking finasteride at dosages of 0.2mg/day or higher was still effective in treating male pattern baldness, although it wasn’t as effective as taking the 1mg pill. this will lower the efficacy of the drug, but many will agree that something is always better than nothing. I’ll leave the link to the scientific study so that you can read about it if interested. 2. For those individuals who have been looking for a safer way to take finasteride, a topical form is now available. Many claim that it halts the progression of male pattern baldness effectively, with little chance of any unwanted side effects due to the minimal absorption into the rest of the body. The only bad thing is that finasteride is not as easily available to find in topical solution. Finasteride works by eliminating testosterone from converting to DHT. As CBS 2′s Pam Zekman reports, some men say they have been devastated by a side effect of the baldness drug Propecia. "I feel like my twenties have been taken away from me and it's not fair," said one young man who asked for anonymity. "I wouldn't wish this on anybody." He was prescribed Propecia to battle his thinning hair and says sexual side effects started the first day he used the drug. "I wasn't able to get an erection or it took me a very long time," he said. "I can't maintain an erection now." At the time he started taking Propecia the insert said less than 2-percent of the men who took it had "sexual side-effects", and that those went away when the patients stopped taking the drug. But, it's been two years since Tom last took Propecia and his problem continues. "I basically feel like my penis and brain are not connected anymore," he said. That's not surprising, according to a recent study conducted by Dr. Michael Irwig of George Washington University. "The novel part about this study was that it is the first study to describe persistent sexual dysfunction after stopping the medicine where as previous studies have all reported that there have been reversible sexual side effects." Lawsuits have been filed against Merck — the maker of propecia — in both Canada and the U.S. Attorney Alan Milstein filed one of the cases. "The warnings that came with this drug did not adequately tell the potential users of the drug what to be aware of," Milstein said. Rosemary McGeady joined the lawsuit after her son Randy killed himself. "He hated all the things that the drug had done to him," said McGeady. "He hoped his family and friends would understand why he didn't want to live anymore. " Dr. Robert Brannigan, of Northwestern Memorial Hospital, says he sees half a dozen patients a year who have told him, "I went on this medication and after that I noticed these changes in my sexual health." He says there needs to be a long-term study of Propecia "It's always been a question in my mind, was this a lingering drug effect or are their other factors coming into play?" Dr. Brannigan said. As for the 22 year old young man who asked for anonymity, "I wouldn't trade my hair for not having or not being able to have sex you know, that's just kind of a no brainer." Merck recently changed the wording on their label to acknowledge reports of sexual dysfunction even after people stop taking the drug. But, a spokesman for Merck disputes the findings in Dr. Irwig's study. the spokesman says Propecia passed three clinical trials before the drug was approved and that once men stop taking Propecia the sexual side effects go away. Drugs to treat hair loss and an enlarged prostate may lead to another problem, erectile dysfunction. In a few men, the problem may be permanent. Researchers at North Western Medicine looked at data on nearly 12,000 men. Those who took Finasteride or Dutasteride for a prolonged period, had an increased risk for erectile dysfunction. In some cases, the problem lasted for many months or even years after stopping the drugs. Everyone's risks are different, so it's a good idea to consult your physician before discontinuing any medications.

Finasteride works by lowering the body’s levels of DHT, the main cause of hair loss in both men and women. Clinical trials have shown that finasteride can reduce and even stop the progression of hair loss in 86% of men taking the drug. While side effects from finasteride are rare, less than 5% of men treated with finasteride reported sexual side effects such as erectile dysfunction, decreased libido, and ejaculation disorder. 2. Avodart (Dutasteride)- like finasteride, dustasteride works by blocking the body’s levels of DHT. However, clinical studies show that dutasteride reduces serum DHT significantly more than does finasteride. A study was done on male subjects who received dosages of finsateride and dutasteride. The study showed that finasteride decreased serum DHT by about 70% while dutasteride decreased serum DHT by more than 90%. Side effects are more common than finasteride and can include similar sexual side effects, but the majority do not suffer from major side effects. 3. Hair-Growth Drug Linked to Erectile Dysfunction That Lasts for Years

Young men who take drugs to treat hair loss or an enlarged prostate may experience an unwanted side effect — erectile dysfunction — and the problem can persist long after these men stop taking the drugs, a new study finds. Men under age 42 in the study who took one of these drugs for more than 205 days were nearly five times more likely to have long-term erectile dysfunction than men who took the drug for less than 205 days, according to the study. One of the drugs in the study was finasteride, which is sold under the brand names Propecia and Proscar. Finasteride affects how cells make testosterone; it may be taken for hair growth or to shrink the prostate.The other drug, dutasteride, is taken to shrink an enlarged prostate. It is sold under the brand name Avodart and is one of the drugs in the medication Jalyn. The new findings linking these drugs to debilitating sexual dysfunction should be of particular interest to prescribers and patients, senior study author Dr. Steven Belknap, a research assistant professor of dermatology at Northwestern University Feinberg School of Medicine in Chicago, said in a statement. "Our study shows [that] men who take finasteride or dutasteride can get persistent erectile dysfunction, in which they will not be able to have normal erections for months or years after stopping finasteride or dutasteride," Belknap said. In the new study, which was published today (March 9) in the journal PeerJ, the researchers looked at data on more than 12,000 men in the Northwestern Medicine Enterprise Data Warehouse, a database of medical records for patients treated at Northwestern Medicine. They focused on men ages 16 to 89 who had been prescribed one of the drugs at least once from 1992 to 2013. The researchers looked at the both the dosage of the drugs that the men were prescribed and the length of time they took the medications. In addition, they looked for any diagnoses of sexual dysfunction, such as low libido, erectile dysfunction and persistent erectile dysfunction. Results showed that 167 of the men (1.4 percent) who took either finasteride or dutasteride developed erectile dysfunction that lasted for a median of about three and a half years after they stopped taking the drug. In men ages 16 to 42, the length of time that the men took either of the drugs was a better predictor of their risk of persistent erectile dysfunction than other factors, such as the dosage of the drug, the researchers found. That length of time was also a better predictor than other medical conditions that have been linked with erectile dysfunction, such diabetes or hypertension according to the study. Among the men in this age group who developed erectile dysfunction after taking the drugs, the median duration of their ED was about four years, the researchers found. Both finasteride and dutasteride work by blocking the conversion of the hormone testosterone into its more active form, 5-alpha-dihydrotestosterone (DHT). DHT can contribute to male pattern baldness because it dampens another chemical signal in the body that fuels hair-cell growth. In a meta-analysis published in 2015 in the journal JAMA Dermatology, the same group of researchers found there was not enough evidence to establish that finasteride was safe to take for hair-loss treatment in men.
