(22 Mar 2017) LEADIN: A cancer hospital in Yemen says it is running out of medicine to treat its patients. Doctors at the Al-Amal centre believe people will die unless it receives more supplies soon. STORYLINE: Another busy day at the Al-Amal cancer centre. This is the only place in Hodeida where patients can receive cancer treatment for free. And in a country with widespread poverty, that means people are prepared to queue outside for their turn to see a doctor. Inside the building, hospital's beds are filled with desperately ill people. The centre's manager, Dr. Yasser Nour, says they have travelled from other provinces to receive treatment. With few other functioning medical facilities, Al-Amal has become very crowded. It treats around 2,500 patients every year but there are fears it will run out of medicine by the end of the month. Patient Hamama Yusuf Al-Nahari is on one of the wards, and she is seriously ill. "I suffering, I'm dying of this," she says. Desperate fathers hold their sick children tightly, hoping the centre might be able to save their young lives. But the doctors warn they don't have the resources they need. Dr. Abdullah Ahmed Umair, a oncology and radiation therapy consultant, says they have "severe shortage of medicines, especially chemotherapy". He believes patients will relapse or even die from their cancers. "Please, please, we need urgent support," he says, urging the World Health Organisation and other aid agencies to bring them more supplies. But as the shelves in the hospital's pharmacy get emptier, time is running out. Edhi Foundation The Edhi Foundation is a non-profit social welfare program in Pakistan, founded by Abdul Sattar Edhi[3] in 1951. Edhi is the head of the organization and his wife Bilquis, a nurse, oversees the maternity and adoption services of the foundation. Its headquarters are in Karachi, Pakistan. The Edhi Foundation provides 24-hour emergency assistance across the nation of Pakistan and abroad. The Foundation provides, among many other services, shelter for the destitute, free hospitals and medical care, drug rehabilitation services, and national and international relief efforts. Its main focuses are Emergency Services, Orphans, Handicapped Persons, Shelters, Education, Healthcare, International Community Centers, Blood & Drug Bank, air ambulance services, Marine And Coastal Services. Edhi established his first welfare center and then the Edhi Trust with a mere Rs. 5000 {Approx. $55.56}. What started as one man operating from a single room in Karachi is now the Edhi Foundation, the largest welfare organization in Pakistan. The foundation has over 300 centers across the country, in big cities, small towns and remote rural areas, providing medical aid, family planning and emergency assistance. They own air ambulances, providing quick access to far-flung areas. In Karachi alone, the Edhi Foundation runs 8 hospitals providing free medical care, eye hospitals, diabetic centers, surgical units, a 4- bed cancer hospital and mobile dispensaries. In addition to these the Foundation also manages two blood banks in Karachi. There are 15 " Apna Ghar" ["Our Home"] homes for the destitute children, runaways, and psychotics and the Edhi Foundation states that over the years 3 million children have been rehabilitated and reunited with their families thorough the Edhi network. The foundation also has an education scheme, which apart from teaching reading and writing covers various vocational activities such as driving, pharmacy and para-medical training. The emphasis is on self-sufficiency. Edhi's vision is to create an institution that will carry on his life's work and survive for a long time to come. His dream is that of a Pakistan as a modern welfare state, which provides a safety net for the poor and needy while providing basic health and education with vocational skills. A welfare state Edhi feels is the only way to tackle Pakistan's myriad social problems. He hopes that one day, Pakistan will be a model for other developing countries. Programs Welfare activities Baby cradles (jhoolas) are installed at most Edhi centers, where unwanted or abandoned babies may be left. Adoptions are then arranged for the infants. Destitute homes (Apna Ghar) service orphans, runaways, and the mentally ill. Warehouses hold supplies for emergency situations. Missing Persons Service posts bulletins for missing persons in the press, on radio and TV. Prisoner's Aid provides provisions and legal aid to imprisoned persons. Graveyard Services provide washing and burials for unclaimed bodies. Religious services are provided according to the deceased's religious affiliation. Public Kitchens feed the hungry in Karachi, Lahore, and Rawalpindi Emergency Services Note The Edhi Foundation is listed in the Guinness Book of World Records as having the largest volunteer ambulance organization in the world. Field Ambulance Service provides 1800 ambulances (the largest ambulance fleet in Pakistan and the largest private ambulance service network in the world). Air Ambulance Service provides two airplanes and one helicopter. Marine and Coastal Service provides weather warnings, air-drops food supplies, and recovers dead bodies. Emergency Checkposts provides immediate, emergency care. Projects Highway Projects are developing new Edhi centers across the nation of Pakistan. Blood and Drug Banks provide medicine to needy persons. They also collect and store blood for transfusions. Cancer Research Hostel houses 50 cancer patients in Karachi. International Activities Refugee Assistance provides help to refugees in Pakistan. International Community Centers are established in several nations to assist Pakistanis and other Asians overseas. Cirrhosis of the liver nursing NCLEX review over the symptoms, nursing care/management, pathophysiology, complications, and treatments. Cirrhosis is a liver disease where liver cells become extremely damaged due to long-term/severe damage. This damage can be from a viral infection (Hepatitis B or C), heavy alcohol consumption, obesity, hyperlipidemia, bile duct problem, autoimmune etc. These damaged liver cells are replaced with fibrous tissue, and this leads to scarring of the liver. The role of the liver is to filter the blood and perform the following functions to it: metabolizing, detoxifying, storing, producing etc. When liver cells are damaged these processes do not happen. Complications of cirrhosis include: portal hypertension, splenomegaly, ascites, jaundice, hepatic encephalopathy, esophageal varices etc. All of these conditions are discussed in depth in the video. During the early stages of cirrhosis the patient may be asymptomatic, however, in the late stages the patient will have signs and symptoms that include: asterixis, fetor hepaticus, jaundice, reduced platelets and white blood cells, spider angiomas, ascites, gynecomastia, increase ammonia level etc. Nursing interventions for cirrhosis include monitoring for bleeding, abnormal glucose levels, implementing a proper diet: if patient has confusion (hepatic encephalopathy) they need a low protein diet, however, if patient isn't confused they need a high lean protein diet, low sodium, fluid restriction, assessing for rupture of varices, administering lactulose etc. Treatment for cirrhosis includes a liver transplant and medications such as diuretics, blood products, vitamin K, beta blocker or nitrates. In addition to a paracentesis. which helps remove fluid from the abdomen, if ascites is present. Medicines I take and don't take and medical supplies

I take so many less medications now than I did when I first found out I was sick because we were trying to figure out a nice pain and nausea and whatever else regimen and find the sweet spot. However over the course of some weeks I decided to just take less and less until I didn't take anything at all. Then I would add back in some nausea medication and some pain medication when needed which was usually just around when I got chemotherapy which is every two weeks. It takes about one week to feel up for having a normal day where I don't feel so sick anymore. And then I have a whole week ahead of me of just being like a normal person who just happens to be attached to a TPN line for 16 hours a day. But fortunately most of those hours are spent in the late evening, through the night and early morning.

Today's lesson is a bit more practical, especially if you're going to be going to travel in an English-speaking country, you might want to know some of these words. We're going to look at some medical vocabulary, and to talk about the issues that you're facing. Okay? First of all, let's break down the medical vocabulary to three sections. We have injuries, we have illnesses, we have diseases. Okay? First thing to understand, an injury is always about the physical body. Okay? It's bones, it's tissues, it's skin, it's all these things. Illness is inside the body, but it's usually shorter. Okay? It doesn't last a long time, and you can usually get rid of it; you can fix it somehow. Diseases, on the other hand, they're also inside, but they take a long time and quite often they can kill you. Okay? Some diseases, nothing you can do about them; there's no cure for them. But some diseases, you can treat. Okay? We'll talk about that as well. You can treat them, but there's no cure necessarily. So, let's start with injury. An injury to your physical body can come in different forms. All of these are called "wound", "wound", it sounds like an "oo" sound. "Wound". Okay? A wound is when you do something to physically harm your body. A "burn". If you touch the stove... You're pulling out bread from the oven, you touch it. Or the stove, you're cooking something and you touch the hot plate, you will burn your hand. Okay? So, if you burn your hand, it'll stink a little bit, it will hurt a lot, but you can put some lotion on it, take care of it. A "break". Now, if you go skiing and by accident you fall down, you can break your leg. Or if you go bicycling, like off-road, like trail biking, you fall down, you break your leg, break your arm, break something. Like: "crack", a bone inside somewhere broke. But you could also fall and "sprain" something. A sprain means like almost a break. Like, for example, you fall down on your ankle... You can't see my ankle. It's down there, but I'm like twisting it. If I fall down on it, I won't break my bone, but it will get all swollen. Okay? It'll puff up. It'll be blue and black, and very, very painful. And maybe I won't be able to walk on it, but I didn't break anything. Okay? So, these are examples of injuries. Next, we have illness. Now, everybody gets ill at some time. If you say "sickness", it means the same thing. Sickness/illness, exactly the same thing. You can get a "cold". [Coughs] And sneezing, and coughing, and whatever. You can get a "flu" will usually be with a... Usually comes with a fever. Many of you know "influenza", so we just say "flu" for short. Okay? This is not very fun. You sit in bed for a few days, but eventually it goes away, hopefully. And we always... We often talk about a "bug". Now, when we talk about a bug, we're talking usually about a virus. So we say: "There's a bug going around." So during certain times of the year, you'll get on the bus and somebody on the bus is sick or somebody coughed and then grabbed onto the handle. Then you come on the bus, you grab the handle. The bug comes inside you, and then the next day you're ill. You have a flu. You have a cold. You have something. So we say there's a bug going around. Next, we have a disease. Now, a disease is a very harsh thing. Okay? There is mental diseases, there is physical diseases. Something that is "chronic" means that it continues for a long time; it doesn't go away. Like even if I have-[coughs]-a chronic cough, it means I'm always coughing; it doesn't go away. Then, if you get tested and you find out that, for example, you had a tumour let's say. You had something growing inside you, you think maybe it's cancer, you go get it tested and then you find out it's "benign". It means it's not dangerous. It's not going to do anything to you. It won't develop into the disease. But then there are some diseases that are "terminal". "Terminal" means end. So, basically, if you have a terminal disease, you're going to die. Okay? Sad, but true. That's how it works. Cancer is a terminal disease in most cases. In some cases, it goes away for a little while, but it can come back. But if you have a terminal disease, you're probably going to pass away. Now, we "heal" injuries. Okay? You go to a doctor, you go to the hospital, they do something, they fix your arm. Then eventually, after a little bit of time, your broken arm, your broken bone heals.

Cancer treatment shortfall creates further human tragedy

(9 Dec 2015) LEAD-IN: Access to lifesaving cancer treatment is under threat in war-torn Yemen. The country's National Oncology Center has warned it will have to stop providing care if medical shortages continue. STORY-LINE: As Yemen's war rages on, daily life is getting harder. Access to food and water has left millions of people vulnerable - and now medical services are also at risk. The country has been hit with a shortage of medical supplies. It's meant treating the seriously ill has become increasingly difficult. Now The National Oncology Center has stepped in. They say they will have to stop providing treatment if medical supplies run out. Doctor Afif Al Nabahy, Director of the National Oncology Center appeals for help. "We are calling urgently for all the human rights bodies, the United Nations, charitable institutions and civil society organisations and pharmaceutical companies in the whole world for an immediate intervention to stop the deterioration in the treatment of cancer patients in the Republic of Yemen, and very soon the medication treatment for cancer patients in Yemen will not be provided to them." The National Oncology Centre provides free cancer treatment, but families must find the medication first which poses big problems. This young boy is suffering from cancer, and is so weak he can't walk without being partially carried. His mother, Ayman, says she and others like her have to search high and low for the correct drugs. Metastatic breast cancer patients share medical records, tumors with researchers

Latonya Wilson is trying to fight breast cancer in a novel way, with a saliva sample. Wilson has stage IV metastatic breast cancer, which means her cancer has spread and cannot be cured. But she’s hoping to help other women through an unusual, crowd-sourcing project, driven by people like her. Wilson spoke to FOX 13 Tampa Bay's Dr. Joette Giovinco about why she got involved. Everybody hollers about 'We need a cure, we need a cure,” Wilson says. “So if you want a cure, you want better treatments, then participate in this." This is the Metastatic Breast Cancer Project, a non-profit research project with more than 2,600 participants, promoted by social media and patient advocacy groups. Volunteers fill out questionnaires, submit saliva samples, and they give researchers access to their medical records and tumor samples. In return, they get updates on what researchers are finding. They can even tour the lab, where scientists are looking for genetic clues that could lead to early, more targeted interventions. Dr. Nikhil Wagle, who heads up the MBC Project says this is different from how most research studies are run. “This is a way for patients to take participation and research into their own hands,” Dr. Wagle says. Since the project began a year ago, researchers have identified small groups of patients who have responded well to standard chemotherapy, or to new immunotherapy treatments. They’re hoping that DNA analysis will explain why. Early finding are encouraging women like Jessica Leip, who was diagnosed with metastatic breast cancer in 2013. Her reason for joining the study is simple. “Because, if you go online and you Google this disease, you know, they have you dead in 5 years,” Leip told FOX. “I hope, I hope there's a cure. We need a cure." The data collected will be shared with other research teams, and the participants will be updated on what researchers are discovering. For now, Latonya Wilson says just participating gives her comfort. "I helped by providing my medical information,” Wilson says. “I helped by trying to get other people to participate in this. Gastroesophageal reflux disease (GERD) nursing NCLEX lecture on the symptoms, GERD diet, nursing interventions, and treatment. GERD is a chronic condition where stomach contents is flowing back up into the esophagus. What causes gastroesophageal reflux disease? GERD is mainly due to a weak or damaged lower esophageal sphincter. The LES is a collection of circular muscles at the end of the esophagus that closes and prevent toxic acids and GI contents from flowing back into the esophagus once it enters the stomach. What causes the LES to become weak? Many things such as pressure on the LES from pregnancy, obesity, hiatal hernia, or overeating along with medications such as antihistamines, calcium channel blockers, sedatives, and even smoking. In addition, GERD can be caused by delayed gastric emptying or impaired motility of the esophagus. Signs and symptoms of GERD include: heartburn, regurgitation (bitter taste in the mouth), dry chronic cough especially at night, upper GI pain, voice changes, nausea, problems swallowing etc. Complications of gastroesophageal reflux disease include: narrowing of the esophagus (strictures), Barret's esophagus, bleeding, increased risk of esophageal cancer, aspiration, lungs infections, and ear infections. What is the treatment for GERD? It is treated with lifestyle changes (smoking cessation, weight loss, diet changes etc.), medications, and in the worst cases surgery (fundoplication). How is gastroesophageal reflux disease diagnosed? An endoscopy is performed to assess the esophagus for changes such as erosion or strictures etc. Esophageal manometry assesses the function of the esophagus' ability to squeeze food down into the stomach and how the lower esophageal sphincter closes. PH monitoring measures the acid amounts in the esophagus for a 24 hour period as the patient perform activities of daily living (a small tube stays in the esophagus to help measure the acid amounts) Medications used to treat GERD include: Antacids, histamine receptor blockers, proton-pump inhibitors, and prokinetics. Nursing interventions for GERD include: diet teaching (avoids foods that relax the lower esophageal spincter: peppermint/spearmint, alcohol, coffee, fatty/greasy foods), smoking cessation, limiting acids foods such as citrus and tomatoes, avoid eating right before bed, sitting up an hour after a meal, and eating small meals rather than large ones. Monitor the patient for complications, assessing quality/frequency of pain, medication history, and administering medications per MD order. The thyroid may be affected by several diseases. Hyperthyroidism occurs when the gland produces excessive amounts of thyroid hormones, the most common cause being Graves' disease, an autoimmune disorder. In contrast, hypothyroidism is a state of insufficient thyroid hormone production. Worldwide, the most common cause is iodine deficiency. Thyroid hormones are important for development, and hypothyroidism secondary to iodine deficiency remains the leading cause of preventable intellectual disability. In iodine-sufficient regions, the most common cause of hypothyroidism is Hashimoto's thyroiditis, also an autoimmune disorder. In the 1930s Dunham developed "Dance Anthropology" based on her experience with the dance of other cultures. The Dunham Technique is a blend of African, Caribbean, ballet, and modern dance that incorporates the chakras, core strength, and breathing techniques. The instructor was Theodore H. Jamison: Dancer/Performer/Teacher/Choreographer. Theo is the Program Director for SIUE East St. Louis Center for the Performing Arts, and was certified by Katherine Dunham herself to teach the world renowned Dunham Technique. Colostomy irrigation procedure nursing teaching video for ostomy care. The purpose of performing colostomy irrigation is to remove stool from the colon by instilling an enema of warm water through the stoma. This stimulates the colon to empty it contents. A physician may order ostomy irrigation when stoma output is low or the patient wants to train the colon for bowel continence. Ostomy irrigation is used for patients who have a descending or sigmoid colostomy. Why? Because these patients still have majority of the large intestine in place and is able to produce formed stool when compared with other types of ostomies (hence these patients will have the most success with ostomy irrigation for bowel training). Patients with an ileostomy are not candidates for this procedure because the stool is always a liquid consistency and it is rich in fluid and electrolytes. It takes about 6-8 weeks for ostomy irrigation to be successful when training the colon. The result will be that the patient will not have bowel movements between irrigations and will not have to wear a continuous pouching system. Instead, the patient will wear a stoma cap which is a very small closed pouch. It is best to perform colostomy irrigation at the same time every day and one hour after meals. Ostomy irrigation can take (from start to finish) 1 hour. Supplies you will need: Irrigation bag with tubing and cone, warm water of 500 cc to 1000 cc (depending on physician's order), water-soluble lubricant, irrigation sleeve with clip (if patient has a two-piece ostomy system: irrigation sleeve will attach to the flange/base plate or if patient has a one-piece pouching system: irrigation sleeve will attach to the face-plate via ostomy belt), towels/wash cloths, gloves, PPE (gown). Abnormal lung sounds that include crackles (formerly called rales), stridor, wheezes (formerly called rhonchi), pleural friction rub, and stridor. There are two types of abnormal breath sounds such as discontinuous and continuous. Discontinuous sounds include: coarse crackles, fine crackles, and pleural friction rub. Continuous sounds include: low-pitched, monophonic wheeze & high-pitched, polyphonic wheeze, and stridor. Fine crackles sound like high-pitch, crackling noises mainly on inspiration and can't be cleared when coughing. Coarse crackles are low-pitch, wet bubbling sounds that can occur mainly during inspiration but can extend into expiration. Pleural friction rub is a harsh, grating sound heard during inspiration and expiration. Low-pitched, monophonic wheezes are heard mainly in expiration and have a low-pitched whistling tune or whine with ONE sound quality. High-pitch, polyphonic wheezes are heard mainly in expiration and have a high-pitched musical instrument tune with MORE THAN ONE sound quality. Stridor is heard during inspiration and is a high-pitched whistling or gasping sound with a harsh sound quality. Hyperparathyroidism nursing review lecture of hyperparathyroidism symptoms, pathophysiology, causes, and nursing interventions. Hyperparathyroid is the excessive production of parathyroid hormone by the parathyroid gland. This parathyroid disorder leads to hypercalcemia and hypophosphatemia. Signs and symptoms of hyperparathyroidism include bone fractures, renal calcui, constipation, excessive urination, nausea, vomiting, epi-gastric pain, EKG changes (shorten QT interval) and lab values that reflect hypercalcemia and hypophosphatemia. Causes of this parathyroidism include primary hyperparathyroidism and secondary hyperparathyroidism. Primary hyperparathyroidism is caused by a malfunctioning parathyroid due to an adenoma, hyperplasia, or cancer. Secondary hyperparathyroidism is caused by a disease that affects the function of the parathyroid such as chronic kidney disease, hypocalcemia, and vitamin-d deficiency. Nursing interventions include monitoring vitals, straining urine for possible renal calcui, monitoring EKG, and administering medications as prescribed by the doctor. The goals of medication treatment in hyperparathyroidism are to decrease the calcium and PTH levels, and maintain hydration. Good and Bad Foods For Diabetes | Diabetes Free | PlayEven English Diabetes mellitus (DM), commonly referred to as diabetes, is a group of metabolic disorders in which there are high blood sugar levels over a prolonged period. Symptoms of high blood sugar include frequent urination, increased thirst, and increased hunger. If left untreated, diabetes can cause many complications. Acute complications can include diabetic ketoacidosis, hyperosmolar hyperglycemic state, or death. Serious long-term complications include cardiovascular disease, stroke, chronic kidney disease, foot ulcers, and damage to the eyes. Diabetes is due to either the pancreas not producing enough insulin or the cells of the body not responding properly to the insulin produced. There are three main types of diabetes mellitus: • Type 1 DM results from the pancreas's failure to produce enough insulin. This form was previously referred to as "insulin-dependent diabetes mellitus" (IDDM) or "juvenile diabetes". The cause is unknown. • Type 2 DM begins with insulin resistance, a condition in which cells fail to respond to insulin properly. As the disease progresses a lack of insulin may also develop. This form was previously referred to as "non insulin-dependent diabetes mellitus" (NIDDM) or "adult-onset diabetes". The most common cause is excessive body weight and insufficient exercise. • Gestational diabetes is the third main form, and occurs when pregnant women without a previous history of diabetes develop high blood sugar levels. Prevention and treatment involve maintaining a healthy diet, regular physical exercise, a normal body weight, and avoiding use of tobacco. Control of blood pressure and maintaining proper foot care are important for people with the disease. Type 1 DM must be managed with insulin injections. Type 2 DM may be treated with medications with or without insulin. Insulin and some oral medications can cause low blood sugar. Weight loss surgery in those with obesity is sometimes an effective measure in those with type 2 DM. Acute kidney injury (also called acute renal failure) nursing NCLEX review lecture on the nursing management, stages, pathophysiology, and causes (prerenal, intrarenal, postrenal). What is Acute Kidney Injury? It is the SUDDEN decrease in renal function that leads to the build up of waste in the blood, fluid overload, and electrolyte imbalances. What are the causes of Acute Kidney Injury? There are three causes, which are based on location. The first is known as prerenal injury and this is an issue with the perfusion to the kidneys that leads to decreased renal function. A second cause is known as intrarenal injury, and this is due to damage to the nephrons of the kidney. Lastly, postrenal injury is due to a blockage located in the urinary tract after the kidney that can extend to the urethra. This is causing the back flow of urine, which increases the pressure and waste in the kidneys. Stages of Acute Kidney Injury: There are four stage of acute kidney injury, which include initiation, oliguric, diuresis, and recovery stage. The initiation stage starts when a cause creates an injury to the kidney and then signs and symptoms start to appear. This leads to the oliguric stages. The patient will void less than 400 mL/day of urine during this stage and will experience increased BUN/creatinine levels, azotemia, hyperkalemia, hypervolemia, increase phosphate and decreased calcium levels along with metabolic acidosis. After this stage the patient can progress to the diuresis stage and this is where the patient will void 3-6 Liters of urine per day due to osmotic diuresis. The patient is at risk for hypokalemia, dehydration, and hypovolemia. The last stage is recovery and the patient's glomerular filtration rate has returned to normal. Therefore, the kidneys are able to maintain normal BUN and creatinine levels, electrolyte, and water levels.
