Women's Health: Menopause A Smooth Transition Part One featuring Dr. Nancy Lonsdorf Menopause is meant to be natural, and not only meant to be natural but, as ayurveda describes, it's meant to be smooth; it's meant to be something that we go through as comfortably as the transition from childhood to adulthood. That puberty and that whole hormonal changes don't have to be stressful; they don't have to be a disease or a condition. If we treat our bodies right, if we stay in balance, that transition should be able to be quite smooth. In fact, epidemiological data shows that only about one-quarter of women actually have such severe symptoms that they seek out treatment or hormone therapy, and one-quarter of women don't have any symptoms virtually at all as they go through this transition. Just the period stops and life goes on as it did before. And about half of women have something kind of in between. So, the point here is that it is a transition. The body is going from a certain state of monthly ups and downs and rhythmic manner of all of our hormones, and we have this sloughing off of the uterine lining on a monthly basis. The body goes from that to not having that whole thing. It goes to more of a steady state of hormones at a lower level and an absence of that flow, that elimination every month. So, what we know now from ayurveda is confirmed by modern research, now, that reducing stress can help this transition be smooth; because cortisol, the stress hormone — it turns out, a study published in Menopause found that women who are highly stressed were five times more likely to have the hot flash problem as women who did not describe themselves as highly stressed. So, handling stress, taking enough time for yourself, meditation, even deep breathing, has been shown to dramatically lower hot flashes. And that would be just breathing in slowly, breathing out slowly, breathing in slowly, breathing out slowly for a few minutes at a time. That can also be done through the traditional yoga pranayama of alternate nostril breathing: out, in, out, in, out, and you just do it in the natural rhythm. That balances the sympathetic and the parasympathetic nervous systems and reduces stress. Secondly, diet can be important. If we're drinking alcohol in excess, if we're eating too much sugar, we’re taking too much caffeine, eating too many hot spices — all of these factors create more metabolic heat or can trigger hot flashes. So eliminating those stressful foods, caffeine and too much alcohol, and having a more balanced diet — lots of fresh fruit, lots of fresh vegetables, especially those juicy vegetables such as zucchini and cucumber — wonderfully cooling — as well as other greens. And secondly, drinking lots of water. At this time of life, not cold water; we don't want to create a cramping inside, a closing off of the channels; we want to create a nice elimination of impurities, because the body is no longer doing that through the menstrual cycle. So room-temperature water, or warm water if you feel better with that, and you can add some spices to that water that will also help the natural change in hormones and detoxification your body wants to do. Women's Health: Menopause A Smooth Transition Part 2 featuring Dr. Nancy Lonsdorf We actually have two built-in methods that our body has of coping with the loss of estrogen. One of them is the fact that we have adrenal production of estrogen and progesterone as much as we have ovarian. In fact, after menopause our adrenals kind of kick in as — like a backup system. But, if our adrenals are so busy making cortisol from all the raw materials it needs to make hormones, it will not be able to make as much estrogen and progesterone. So therefore, again, handling stress directly affects our balance of hormones. Ashwagandha is an excellent herb for helping to balance that hormonal function. Secondly, phytoestrogens, meaning plant-based substances found naturally in foods, can help provide our body with enough natural estrogen without overstimulating it like artificial estrogen or bio-identical hormones in excess can do. For example, flaxseed — excellent source of phytoestrogens — ideally ground fresh each day. Two tablespoons taken in the morning has been shown to help reduce hot flashes. If you'd like to take advantage of the wisdom of ayurveda, it is known for thousands of years that shatavari, or Indian asparagus — asparagus racemosus is its Latin name — is a very potent phytoestrogen which also has cooling effects. So, for women going through menopause or perimenopause — who have heavy bleeding, hot flashes, irritability, or skin problems — shatavari is a wonderful balancer for those kinds of conditions. A combination of herbs can often be more potent than any herb alone, and has balancing effects to avoid an overstimulation of any one herb. So what I would recommend in that case would be a formula called Hot Flash Relief, which has the shatavari, plus it has other cooling herbs such as Indian sarsaparilla, which cools the liver and helps it cope with impurities. And thirdly, it has sandalwood, one of the most cooling substances on the planet, and it has a variety of other herbs to support the uterus, to purify the blood, and to help balance the woman’s hormones and her mood and emotions at this time. One of the things that my patients have often told me they found helpful is a formula called Midlife for Women I. The recommendation ayurvedically is that one takes this product for a couple of years before menopause is expected, because it helps to clear the channels; it helps digestion; it helps us digest our food so we don't make waste which is called ama, meaning impurities that can accumulate in our tissues, block our channels and interfere with our hormonal connection between the hormones and the cells. Ayurveda says that if we accumulate waste from not digesting our food well, then we will have a variety of problems including menopausal symptoms — more serious kinds of menopausal symptoms that don't go away so easily. So taking this product for a couple of years before menopause helps to support good digestion, good metabolism, good elimination. It's also very good for mood, which can start to become a little unstable, or we might become more sensitive or more irritable as the hormones start to change, approaching menopause. So Midlife for Women I, and then Hot Flash Relief and shatavari as a single herb; aloe vera juice also for cooling and cleansing; taking extra care of yourself, reducing stress, getting more rest; all of these things help to create a smooth menopause that you were meant to have. Hormones and Ayurveda While differences between men and women in the west are often explained through the lens of the hormones, Ayurveda has its own understanding of the endocrine system. According to Ayurveda, hormones are the fire element of the blood, and as such are qualified by the pitta (fire) dosha. For women, and for all human beings, the state of balance of the individual depends on whether the doshas of vata, pitta and kapha are in balance. Disharmony in the doshas can lead to very specific symptoms in women that are also then reflected in their menstrual cycle and, if persistent, in peri-menopause and menopause. These are exactly like the hormones made in the ovaries. The human body knows no difference from these and what is produced genetically. The bio-identicals are derived initially from plant sources and crafted to be the same composition that is naturally produced by your body. Thyroid, adrenal, and sex hormones are available also bio-identically. Unfortunately, too many women in our country are not taking hormones because of the false concern that they may cause cancer. Medical doctors were hoodwinked by the media in 2006 to believe that taking hormones is risky. Three reports that show a minimal increase of breast and uterine cancer contradict the previous forty years of benignity in over 1,000 studies. Today, a woman who is age fifty and in good health could very well spend half her lifetime in a postmenopausal hormone imbalance and hence, the subject to increase of morbid problems. Female hormones are the only hormone abnormality in all of medicine that are not routinely replaced. If the patient has a low thyroid level, doctors prescribe thyroid. If she has low parathyroid output, doctors give her parathormone (Forteo). No physician would dare withhold insulin from a diabetic, but most physicians are not giving female hormones to those who need it most. Recent statistics show that less than 20 percent of postmenopausal women receive hormonal replacement and that most of them begged their physicians to prescribe them. This is a great question!! The first sign is that you have abnormal menstrual cycles; and the most obvious sign is when you don't have your menstrual cycle at all. There is a difference between perimenopause and actual menopause. Perimenopause the process in which your body is starting to decline in estrogen and progesterone production. But menopause is complete - your body is not producing estrogen and progesterone. There are a lot of symptoms of menopause and perimenopause, including hot flashes, fatigue, foggy brain, and moodiness similar to when you are having premenstrual syndrome (but just a little bit different). So what are some natural remedies for perimenopause symptoms such as hot flashes. There are a couple ways to answer that. First, if you are going to do herbal remedies, you want to focus on phytoestrogen or plant based estrogen, such as black cohosh or soybean-type. I recommend using your food as medicine first. So look for things that have phytoestrogens naturally. My favorite - edamame. You can go to any Japanese restaurant, farmer's market or grocery store and buy it frozen. All you need to do is steam it, add a little sea salt and you can eat them right out of the pods. Now if you want a supplement, black cohosh, red clover and evening primrose are some great natural supplements that you can use to combat hot flashes. I personally prefer evening primrose oil or you can do red clover tea. You want to be careful about supplementing too many estrogens because they can actually have a negative effect. If you happen to have a history of breast cancer, or a family history of breast cancer, I'd highly recommend against too much phytoestrogen. The other thing you might want to take a look at is going to your local integrative medicine, or holistic doctor, and getting a full panel of your hormones done - because it's not just estrogen and progesterone that contribute to hot flashes, but also testosterone. So go and get a full panel of blood tests including your thyroid, estrogen, progesterone, testosterone, and have them check on your adrenal glands. That way, they can look at a 3D view of you and not just assume that it's your estrogen and progesterone that's low and causing your hot flashes. Namaste!
