I was diagnosed with Stage I rectal cancer at age 35. One week later, I had a radical surgery - laparoscopic assisted low anterior resection pf rectal cancer with wide/clear margin and total mesorectum excision. 3 days after surgery, I learned that no cancer found in 17 lymph nodes and I am now cancer free! I have no family history of colorectal cancer. I feel lucky that I found this early and got an easy cure. However, I (as well as my kids are) am on close GI surveillance from now on. 10cm of my colon/rectum are gone. It's a new body that I have to learn to live with. While it may not be the subject of casual conversation, rectal bleeding is something you should discuss with your doctor. But very often people are embarrassed and brush it off. "A lot of people think 'oh I have bleeding from my bottom, it's just hemorrhoids' and that's not always the case," says Dr. Jeffrey Neale, a colorectal surgeon on medical staff with Lee Memorial Health System. When someone presents with bleeding, the first thing doctors want to do is rule out cancer. "We have to work them up with our gastrointestinal colleagues to make sure there's no bleeding from the rest of the colon or in the rectum and make sure there's no polyps or cancers present," says Dr. Neale. Most times, the bleeding results from either a hemorrhoid or fissure. Both are visible on a rectal exam. While they may have the same end point- they are very different conditions and have symptoms that set them apart. "If you're having pain it could be an anal fissure. An anal fissure is a tear in and around the anus that can cause quite a significant amount of pain," Dr. Neale says. Generally caused by trauma, strain or bowel disease, fissures are associated with pain and blood during a bowel movement. A hemorrhoid is an engorged or inflamed vein in the rectal region. Clotted or external hemorrhoids may involve a lump. And constant discomfort, not limited to using the bathroom. "There's inside hemorrhoids and outside hemorrhoids. They're all treated similarly even though they're somewhat different," says Dr. Neale. "Outside hemorrhoids may present with pain and a large purple bump and if that's slowly going away they usually would resolve on it's own." Here are the common signs and symptoms of CRC: NO SYMPTOM AT ALL Diarrhea (that will not stop) that lasts for more than a few days Constipation (you can’t “go” to save your life) that lasts for more than a few days Unintended or unexplained weight loss (or weight gain) Vomiting Narrow stools – poo looks pencil-thin Change in stools – poo just looks different, smells different Gas, bloating and cramps Weakness and fatigue – no energy at ALL Anemia Feeling like you need to go poo – and still feeling like you need to poo once you’ve gone Killer abdominal pain and discomfort Blood in stool (although not always visible.) The majority of those with FAP inherited the disorder from mom or dad. Lynch Syndrome (HNPCC) Patients with Lynch Syndrome carry a genetic mutation in the “spell check” genes that create a high risk for developing colorectal cancer (80%) within their lifetime, among other cancers. Know your Family History If you have a family history of colon or rectal cancer, you might be at an increased risk. Screening needs to begin five years earlier from the age of your family member’s diagnosis. Family history typically involves first-degree relatives like moms, dads, siblings, grandparents, aunts, uncles and cousins. However, do not dismiss second-degree relatives’ history when examining cancer occurrence in your family. Colon cancer and cancer of the rectum usually begin as a small polyp. While most colon polyps are benign, some do become cancerous. Colon cancer symptoms may include a change in bowel habits or bleeding, but usually colon cancer strikes without symptoms. That's why it's important to get a colon cancer screening test, such as a colonoscopy. If the cancer is found early, the doctor can use surgery, radiation, and/or chemotherapy for effective treatment. •Many people survive colon and rectal cancer. Histologically, adenomas are classified in three groups: tubular, tubulovillous, and villous adenomas. K-ras mutations and microsatellite instability have been identified in hyperplastic polyps. Therefore, hyperplastic polyps may also have malignant potential in varying degrees. Bleeding is the most common symptom of rectal cancer, occurring in 60% of patients. Bleeding often is attributed to other causes (eg, hemorrhoids), especially if the patient has a history of other rectal problems. Profuse bleeding and anemia are rare. Bleeding may be accompanied by the passage of mucus, which warrants further investigation. Change in bowel habits is present in 43% of patients; change is not evident in some cases because the capacity of a rectal reservoir can mask the presence of small lesions. When change does occur it is often in the form of diarrhea, particularly if the tumor has a large villous component. These patients may have hypokalemia, as shown in laboratory studies. Some patients experience a change in the caliber of the stool. Large tumors can cause obstructive symptoms. Tumors located low in the rectum can cause a feeling of incomplete evacuation and tenesmus. Occult bleeding is detected via a fecal occult blood test (FOBT) in 26% of all cases. Abdominal pain is present in 20% of the cases. Partial large-bowel obstruction may cause colicky abdominal pain and bloating. Back pain is usually a late sign caused by a tumor invading or compressing nerve trunks. Urinary symptoms may also occur if the tumor is invading or compressing the bladder or prostate. Malaise is a nonspecific symptom and present in 9% of rectal cancer cases. Bowel obstruction due to a high-grade rectal lesion is rare, occurring in 9% of all cases. Pelvic pain is a late symptom, usually indicating nerve trunk involvement, and is present in 5%
