Relactation in case if the mother has not fed the infant due to any conditions like the flat nipple or if there was any medical condition in the mother and the baby was not fed and the mother wishes to start relactating ,that is the ,other wants to feed the baby. First and the foremost thing is the mother has to be calm and the relaxed and stress free and the relactation can happen much faster. Along with that they can approach a lactation consultant. They will advise you on methods of stimulation of milk production. Some of the ways are coming to the mother, the mother has to take a good diet, increased liquids, the fluids should be 3 to4 liters in a day, in any form, that can be milk or water or juice or anything of that sort or food contents like fenugreek, garlic, coconut, all these help to increase the milk production. Starting the process there should be a nipple stimulation. That is there should be a routine breast pump should be used for every 10 to 15 minutes as this stimuates the nipple and increase the prolactin content and helps in the milk production. And also the bay should be kept near the mother, the baby should be bedded near the other. The skin to skin contact itself helps in the relatching and the increased milk production in the mother. Also some medications like galactogogues medications can be started in the mother, in case by all the other methods, the mother is not able to produce good amount of milk.

It sounds like you had an issue with low milk supply last time, and you're wanting to know what you can do in the future to successfully breastfeed your babies. And it's definitely worth trying again. I say give it your best shot. And oftentimes, it's circumstantial. Sometimes mothers don't make a lot of milk with their 1st, and they successfully breastfeed their 2nd, 3rd, 4th - however many babies they have in the future. There's nothing you can really do prior to delivery, but once you go into the hospital and your admit nurse is asking you questions, she'll probably talk to you about your desires for labor and delivery, and for your postpartum experience, including "Are you planning on breastfeeding or not?". And at that point, ask her if you can see a lactation specialist in the hospital. They'll work with you before you go home to help you gain confidence and to make adjustments, if necessary, so that you can indeed breastfeed. There's also outpatient lactation support, so if you leave the hospital and you're struggling, or you feel like your baby is not latching properly (which can be a factor in a low milk supply), then talk with your baby's doctor and they can refer you to a lactation clinic near you. You can see them again, and again, receive the same support that you got in the hospital. Another important thing is skin-to-skin after delivery. This is where we just put a diaper and a hat on the baby, and then we put their bare chest on the mother's bare chest, and cover you both up. And this actually helps with breastfeeding. It helps to prime a baby for breastfeeding. There are studies that have shown (and I've actually seen it in real life with my own kids, and with babies in the hospital) if you leave them on a mother's chest for long enough, they'll do this little shimmy, and fall over, and find it themselves. And it's amazing that they come out with that reflex. It doesn't end there. Skin-to-skin is encouraged for your entire hospital stay and in the weeks following, after you've gone home with your baby. Research has also shown that the more time a mother spends with her baby after delivery, the faster her milk will come in and the more of it she'll make, so that's key and important for you. Now, of course there are times when mothers and babies have to be separated after delivery, because one or both needs special attention, and so staff will do what they can to get you both back together as soon as possible if that ends up happening with you. And not all is lost - the benefits of skin-to-skin and breastfeeding still exist if you have to be separated for a time after delivery. I am no longer taking fenugreek or blessed thistle to help with milk production. A friend of mine brought it to my attention that fenugreek can effect iron absorption so I don't want to mess with my iron since I am havig problems with it already. I am still taking domperidone and I am up to 9 pills a day. I hope to make it to 30oz in a day and have Dagney on all breast milk! Wouldn't that be exciting.

A new mom just became the first transgender woman to breastfeed her own baby. According to a recently published case report from Tamar Reisman and Zil Goldstein, two doctors at New York City's Center for Transgender Medicine and Surgery, a 30-year-old transgender woman came to their clinic to inquire about breastfeeding her infant. The woman's partner was pregnant but did not want to breastfeed, and the woman hoped her doctors could find a way to help her feed the baby herself. At the time, she had been on a feminizing hormone regimen since 2011 and was taking testosterone-suppressing spironolactone, along with estradiol and progesterone. While she had not undergone gender confirmation surgery, her breasts were fully developed, so Reisman and Goldstein set about inducing lactation. The woman took a drug that fosters milk production (called domperidone) on top of increased doses of her hormones. As New Scientist reports, the Food and Drug Administration cautions against using domperidone to increase lactation out of concerns for cardiac health and general safety, so the patient had to procure the drug from Canada instead. On top of all this, she used a breast pump on each breast for five minutes every day. After a month of treatment, she was producing milk drops, so doctors upped her drug dosage and accelerated her pumping schedule: After three months—two weeks before the baby was due—the woman was producing eight ounces of breast milk per day. Once the baby was born, she functioned as the sole food source for her baby for six weeks. According to the case report, "the child's pediatrician reported that the child's growth, feeding, and bowel habits were developmentally appropriate" throughout breastfeeding. Reisman and Goldstein know of no other case like this documented in medical literature, and it opens up big possibilities both for transgender mothers who don't want to rely on formula, and for women who have trouble with lactation. "We want to present our patients with the full range of reproductive choices, and this is one step closer to that," Reisman told The Washington Post. The above procedure mimics the common protocol for inducing lactation—minus the feminizing hormone spironolactone. But spironolactone, which can also be used to treat hormonal acne, high blood pressure, and heart conditions, appears safe for use by breastfeeding people. What may be less safe, according to Madeline Deutsch, M.D., director of clinical services at the Center of Excellence for Transgender Health at the University of California San Francisco, is the makeup of this woman's milk. "There are unknowns about the nutritional picture of the milk," Deutsch (who is herself a trans woman and a new mom) told the Washington Post. She would like to see more data on what medications mothers might pass to their infants during breastfeeding in general, and in this particular case, a milk analysis to gauge whether or not this baby was getting the nutrients it needed. Still, breastfeeding helps build newborns' immune systems, and is thought to lower their risk of obesity, asthma, type 2 diabetes, sudden death, childhood leukemia, and a laundry list of other maladies.
