There are new medications all the time, some with less side effects than the older medications, but with just as much efficacy, which means ability to control the seizures. What's happened in the last 10-15 years, is that a whole bunch of new medications have been developed that are in many ways better than the ones we had 20 years ago, and 30 years ago. So what we're going to talk about is, how to decide on medications. What medications work for you, and what to watch for you in terms of side effects. What's important is discuss with your Neurologist what type of Epilepsy you have, because that helps to decide what type of medications might be best for you. And when you're discussing with your Neurologist, he or she will go over the side effects of the medication, and help to pick what is the best medication for you. So when you're choosing a medication, you need to choose the medication that works for the type of seizures, the one with the lowest risk of side effects, and the one that's most efficacious. And not to be scared to realize that sometimes the first medication you pick, might not be the right medication and you might have to try different medications until we get the seizures under control. But this you'll discuss with your Neurologist and come up with a game plan of what's best for you.

The medications I use which I horribly pronounced are Depakote 500mg Three daily Depakote 250mg Three daily Dilantin 100mg Three daily Trileptal 600mg Two daily Phenobarbital 60mg Two daily the other medications are to help my liver, kidney and stomach which are bad thnx to the pills. It's indicated for both seizure types. The major problem with it is sedation, sleepiness. That's why the newer medications have a lot less sleepiness and side effects and waking issues. Patty McGoldrick: Now it still is used a lot of times in infants, children who present with neonatal seizures, and that's one of the first medications that's given to them. It's also used for people who present in status, which is prolonged seizures, and who may be in the emergency room. Not necessarily as the first medication that's used for that, but somewhere down the line. Dr. Wolf: The problem is in the emergency room, when you use it, it can really knock you out, and put you to sleep. And of course, if you give too much of it, it actually can affect your respiration. Patty: Now, the other big issue with phenobarbital is that we've been using it for years to control seizures in newborns and in infants. And actually, the latest research has shown it can cause much more cognitive damage than we initially thought. So we're trying not to use that as rapidly as we did in the past. We use some of the newer medications. We try to get children off phenobarbital pretty quickly if they were using it. Dr. Wolf: There were studies in the 80's showing that it can affect hyperactivity in young children, as well as reading scores and attention scores. So again, this drug can have a lot of potential cerebral side effects. That's why it's nice about some of the newer medications that are out there that might have less cognitive effects. So it's important to discuss with the people taking care of you whether this is the right medication for you, and what are the other choices.

So those are those generalized ones where I stare and zone and don't respond. And it's one of the older medications that have been used to treat this very effectively. So there was a study done a few years ago that compared a bunch of different medications for the treatment of abson seizures and actually this is the one that most people used and was the most efficacious. The biggest side effect with Zarontin is that it can upset your stomach. So some kids we give Tums when they are taking the medication, we make sure that they take the dose after they eat. It's available liquid and it's available in pills for those kids who can swallow pills. So it's a great medication, good for controlling abson seizures, those staring and zoning out episodes. Again the major side effect issue is the stomach problems. If you take it with food it's pretty well tolerated. There is always a small risk of a weight gain. But if they get too much of the medication they can get tired and fatigued. But again a good choice, first line choice for patients with abson seizures.

It is FDA approved in adults over the age of eighteen. Its main side effects are sedation and some dizziness. But it works well. We've had very good efficacy with it. Comes in an iv formulation and also in oral, both liquid and pills. Its an interesting medication because its a slow sodium channel blocker. So works differently than some of the other sodium channel drugs like trilaptel and dylantin and tegretol; and can be used in combination with those medications to help again stabilize the neuron and create better seizure control. So its another medication that should be discussed with the person who's providing your care, watch for side effets and satutory start out slowing on the dosing ussually at fifty milligrams twice; and day and gradually increase it. Its shown to work best at doses of about two hundred milligrams twice a day. You can go up a little higher than that but there's not much efficay beyond that. So its another good medication to discuss with your doctor.

For the record, I have been diagnosed with temporal lobe epilepsy and it is much more controlled now with meds! I rarely get any form of seizure anymore, it's great having a break!!! I can finally breath I describe these as tiny because compared to the worst ones I can get these ARE TINY!! Come to think of it, if anyone were to witness me having one of these attacks they would never even consider I was having a seizure.. These ones are very slight, coming and going very quickly. What happens is I'm thrown back into a 'dream' immediately also being hit with intense deja vu. With these smaller ones I am able to sit still, and I tend to just freeze up without moving much. A bigger seizure involves a lot of eye darting, back and forth, and complete loss of reality and a harder time coming out of it with bouts of crying and anger with the confusion... An even bigger seizure would be me not remember a darned thing that I do... It hits and escilates sooooooo quickly and intensely and I get to the point where I just black out. It would be hard getting a seizure like that on video especially being on medication that isn't allowing those seizures to even happen anymore!! YAYYYYYYYY!!! But, I would still like to know what happens and exactly what I am doing during one of those bigger ones....... The University of Minnesota is part of a national research project examining epilepsy and pregnant women. Researchers are trying to better calculate medication doses. The challenge is that pregnant women process medication differently, so finding the right balance is tough. "The body changes the way it handles medication. Concentrations can drop so much it leaves them at risk of seizure," said Angela Birnbaum from the U's College of Pharmacy. The study is designed to last five years. During that time, they hope to collect data from 500 different women in 19 states.

Both were stopped. Next month he begins challenge trials for Sinemet to safely determine if that was the cause. Sinemet can be amazing for some people with Dystonia and success would be.......excellent. UPDATE: Sinemet has been reintroduced, THIRTEEN years later, in a challenge trial. So far we have total success, meaning no rash, and we have DOPA RESPONSIVENESS. (another anecdotal note for any medical people: the episode above began with a severe dyskinetic reaction to Lamictal, with diarrhea, about 3/4 of the way through a conservative titration schedule - the titration was planned to be conservative because the child was already known to have drug sensitivity problems. When the diarrhea began, an immediate fast wean was started of lamictal - one week. At the end of the week, he was still moving too much and was prescribed Sinemet. Four days later four small lesions appeared - it took about 3-5 days after that for the lesions to cover the sun exposed parts of his body. So tehnically Lamictal was no longer on board and Sinemet was just initiated. FINAL NOTE: this child has had another suspected DRESS syndrome reaction years later, to trileptal, with fever, rash, and elevated liver enzymes.)

Antipsychotics are drugs used to treat symptoms of agitation, delusions, schizophrenia, and aggressive behavior. Doctors looked at nearly 300,000 people. They found people on the highest doses of the newer types had the greatest risk of fatal heart attack compared to people on the older medications. She is turning 2 this July. Since February of this year, she is in and out of the hospital and had several tests which none of her four pedia-neurologist who attended her seemed to identify. The symptom started when she was 1 year and seven months old. After that, random tests have been done trying to determine the cause of it (EEG, MRI, Blood Test, AFP, Beta-HCG, Peripheral Smear, and Plasma Lactate). Doctors said that she has a rare seizure disorder, yet they are not certain what it is. Apart from her seizures, she has muscle weakness that she now can't walk, her eyes got affected that every time she's stressed, her eye balls turns apart from each other...she's taking trileptal as her maintenance for seizure, and some vitamins like nueromin (DHA), vitamin C, taurine, follic acid, b-complex polynerv, and niacin. Comprehensive metabolic screening, expanded newborn screening, 24-hour urine VMA test, and therapy are our next move this mid of June 2012. Right now, she's doing ok while we're waiting for the result of her laboratory test, but still she has myoclonic jerks every time she sleeps. She is still experiencing myoclonic jerks 5 to 10 minutes before she falls asleep and some series of "inconvenience (being irritable)" that lasts up to 20 to 30 minutes. Despite all of these, my daughter still remains strong and still turns on the charm. We are hoping that through this email someone could be consulted on what our little Andee's condition is. Bipolar disorder 1 vs 2, which is worse? There is a common misconception that bipolar 2 is a milder form of bipolar 1 and therefore probably less of a problem. This can lead to people feeling as though they don't need to get treatment if they have bipolar disorder. In this video I give you the definitions of bipolar 1 and bipolar 2 disorders. Bipolar disorder is a mood disorder that has two states. Depressive episodes and manic or hypomanic episodes. Manic episodes are defined as the following according to the diagnostic and statistical manual fifth edition A period of abnormally and persistently elevated, expansive, or irritable mood and abnormally and persistently increased activity or energy, lasting at least 1 week and present most of the day, nearly every day. And then you have 3 of the following: or 4 if your mood is irritable instead of elevated 1. Inflated self-esteem or grandiosity. 2. Decreased need for sleep 3. More talkative than usual or pressure to keep talking. 4. Flight of ideas or racing thoughts 5. Distractibility 6. Increase in goal-directed activity or psychomotor agitation 7. Excessive involvement in activities that have a high potential for painful consequences (e.g., engaging in unrestrained buying sprees, sexual indiscretions, or foolish business investments). Hypomania is the same thing has the same symptoms except that they only need to last for four days. Also a second main difference with hypomania is that the symptoms don't cause real impairment in your functioning. The reason bipolar 2 is not really a milder form of bipolar 1 is because people with bipolar 2 disorder tend to have a more chronic course to their illness. They tend to spend more time depressed than people with bipolar 1. So bipolar 1 may be more intense, but bipolar 2 can be more chronic and persistent. Also the number of lifetime episodes of hypomania and depression tends to be more for bipolar 2 disorder than for bipolar 1 disorder or major depression.
