Why do I ovulate and not get pregnant? I’m assuming you are not using birth control methods that would interfere with that. No, I’m trying to get pregnant. If you’ve had social diseases, the inflammation they cause can block the Fallopian tubes, preventing pregnancy. I was treated for that ages ago. The inflammation and blockage of the tubes to the ovaries will prevent conception, and that damage could have been done in the months or years before you realized you had a relatively silent disease. I get regular doctor checkups, and no one said that was an issue. You could have reproductive machinery working at full tilt but it fails because other hormones are in the way. For example, a thyroid problem could cause your body to ovulate too late in the cycle for an embryo to implant before your next period. The only hormones raging are the ones driving my biological clock. You might ovulate but not get pregnant because the endometrium is too thin, but that’s something a doctor would have to check out. You could try getting more iron and micro-nutrients in general, since that might impede conception. I’m taking prenatal vitamins now. There’s the possibility that you’re reading signs like mucus changes that suggest you are ovulating but are not really. I’m using ovulation tests as religiously now as I used to take birth control. Speaking of which, around one percent of pregnancies are reported as virgin births. This would not be a virgin birth. True, but the egg is only good for a day or two, though sperm theoretically lasts five days in her body. If you have sex a day or two after the egg’s ideal time, that would explain why you are not getting pregnant. One solution your husband would not object to, is having sex every other night to ensure that there is sperm in there whenever the egg makes its arrival So sex on the weekends means I might not get pregnant. Be patient, do it right, try to stay away from stimulants that may increase your energy but mask the underlying problem. Focus on the long-term reproductive goals and timing. And then when the right time comes your health professional should be able to advise you. And if the timing is not right, forcing it does not really help. It is a very challenging situation but it’s not something that cannot be overcome. First of all, congratulations on your pregnancy! That is so exciting but the fact that you're so sick that you can't hold anything down obviously is not. Nausea and vomiting is something that affects 90% of pregnant women. It's most severe, generally speaking, in the first trimester and most women by 16-17 weeks start to feel better. But for some women, it does continue on throughout the pregnancy. Hopefully that's not you and you're just paying your dues now. If you're unable to keep food or fluids down, this is a reason to get checked out immediately. You mentioned that you've been to the emergency room a couple of times. Stay in close contact with your OB provider because they'll want to keep a close watch on things, make sure that you're not losing too much weight and that you're not becoming dehydrated or mineral deficient. They'll talk to you in more detail about that if it's a problem for you. But eat when you can. Drink when you can. If all you can do is suck on a popsicle or ice, do that. Some women feel that if they keep some crackers or something at their bedside in the morning and eat a couple before they even get up out of bed to go to the bathroom, that that helps to keep the nausea at bay a little bit. Obviously avoid things that make you want to throw up. If you have cravings, something that sounds good to you, it's okay to indulge moderately but don't eat so much that you make yourself sick. Maybe small, more frequent meals throughout the day can help. If these basics don't help you feel better, then sometimes medications are necessary, especially if it's causing you to have to go to the ER occasionally. So the doctor can talk to you about medication options that will probably and hopefully help you feel better. For many years I’ve worked with herbs. I love the purity of them and their effectiveness when used to treat everything from infections and fatigue to depression and clearing stress. High on the list of my favorite herbs is Ashwagandha. It is also one of the most powerful herbs in Ayurvedic healing. It's been used since ancient times to impart the vigor and strength of a stallion to the body. In fact, in Sanskrit, the name itself means "the smell of the horse." Ashwagandha has long been known for its rejuvenating properties. Recently an excellent study reported in Biomed Research International discovered that Ashwagandha could significantly improve female sexual functions when women are given it in a concentrated form as a root extract. Fifty women diagnosed with female sexual dysfunction, including lack of sexual desire, poor sexual arousal, little or no female orgasmic experience, and an inability to become aroused through genital stimulation, were given this remarkable herb in an attempt to find out what, if anything, it might do to enhance their sexuality. Twenty-five of them took 300 mg of Ashwagandha root twice a day. The other twenty-five received a placebo during the eight-week period of the study. Researchers evaluated their sexual functions, including lubrication, arousal, desire, satisfaction, orgasm, pain, and overall sexual activity response to therapy, at four weeks and then again at eight weeks during the study. Those who received Ashwagandha reported significant improved sexual function scores when it came to orgasm, satisfaction, arousal, and lubrication. They experienced heightened sexual desire and even a growing number of successful sexual encounters by the end of the eight weeks compared to the women who'd been given a placebo. Researchers also discovered that Ashwagandha given in this way lowers the experience of chronic stress, which interferes with sexual response by lowering serum cortisol. They also reported another possible mechanism by which Ashwagandha enhances female sexuality: it was by "offsetting androgen deficiency syndrome, which is seen as contributing to a lack of sexual desire in some women." What is also interesting is that this wonderful herb even appears to increase serum testosterone, which plays an important part in sexual functioning in both men and women. Sometimes it does have a more appealing taste in some areas. If you haven't already tried it, try putting a filter in your house, and see if that makes a difference, and makes the taste more palatable to you. You mentioned that you have tried sugar packets before to see if adding flavor would help. It's best if you use sugar-free, because then you're not getting extra calories that might make you consume more than you should each day. Keep trying a few different varieties until you find one that works for you. It is very important to stay hydrated, because if you're not, then your brain will actually start to release a hormone that tells your body to retain fluid. And this same hormone mimics the hormone that causes your uterus to contract. Or in other words, if you're dehydrated, you will start to have contractions, and of course, when you're preterm, that is something you want to stay far away from doing. Generally speaking, it's best if you can drink 8 tall glasses of water a day. And the best way to tell if you're getting enough is to turn around and look at the toilet before you flush it. I know that's everyone's favorite thing to do, but if your urine is light yellow to clear, then you're getting enough to drink, whatever you're doing. But if it's apple juice color or looks a little bit concentrated, then you're not drinking enough, and should do everything you can to drink more water. Today, I will like to talk to you all about a question, which I’m sure all the fertility doctors have to discuss about on a daily basis. I, of course, have to have this kind of conversation every day. The question is: Dr. Zhang, I have in-vitro fertilization, so called IVF, procedure done. I have this amount of embryos transferred but I’m not getting pregnant. Why am I not getting pregnant? They told me that my embryos looks great. I am so careful and I am doing everything that you said. But why am I not getting pregnant? So I want to discuss this question with you. While we want everybody to get pregnant, we do everything possible trying to improve everybody’s chance to get pregnant. But nevertheless, we just couldn’t get everybody pregnant at any time. And this is the question that we all have to discuss about. So, let’s talk about this subject. The answer is one of the two. If you are not getting pregnant after embryo transfer, either the quality of the embryos are not good enough or your environment, your body, the uterus, or the uterine cavity, are not optimal to receive the embryos. So this will be one of the two reasons. 70% of the time is due to the embryo quality and 30% of the time is due to endometrial environment, your body. While there is no crystal ball, there’s no technology that can confirm in any failed embryo transfer, whether it belongs to the embryo quality or due to the endometrial environment. But as I said, 70% of the time is due to embryo quality. Then to further figure out whether in each individual case, whether it is due to endometrial environment or embryos, we can look for a few hints. If the lady, aged 39 years or older, if the embryo was not blastocyst stage embryo, or morphology is perfect, then most likely it is embryo quality. If the lady is 35 years or younger, and never got pregnant before, and the embryo look super, a very good quality embryo, but they are not getting pregnant, that could be the endometrial environment. So if it due to the endometrial environment, then we will do further investigation by chemically. We can check the blood for specific antibodies or factors that are against embryos implanting. We can look at the physical environment of the uterus. We can do ultrasounds. We can do special ultrasounds. We can do hysteroscopy to look at the inside of the cavity and look for subtle pathological changes that may prevent embryo from implantation. If we think it is embryo quality, then of course we can try to change the protocols. Try to do the retrieval at a different time, to see if it can improve the egg quality by means of seeing if the embryo can make it to the so called blastocysts stage embryo. By the way, I just want to cut in and explain a little about how we can have a better knowledge about the quality of the embryos. Three things we can do: a) look at the morphology, basically look at the shape/image of the embryo. If the embryos can make it all the way to Day 5, Day 6 embryo, so called blastocysts stage embryo then the embryo have a good chance to be a good quality embryo. We also can estimate the quality of the embryo by looking at the age of the patient. If the embryo is produced from a patient who is 38 years or younger, then there’s a good chance to have a good quality embryo. If they are 40, 41, 42, there is an increased chance of poor quality embryo. This is how we can hint at the quality of the embryos. Now I am sure you all know about a new technology called Pre-implantation Genetic Screening (PGS) test, which you can take a few cells from an embryo, the so called blastocysts stage embryo. Then we can do the advanced technology using the molecular technology called Next Generation Sequencing, or Comparative Genomic Hybridization, the abbreviation is CGH, or NGS. Then we can further check the number of chromosomes of these embryos. Then you can get further information about the quality of the embryos. And indeed, if the embryo can make it to blastocysts stage embryo and the NGS test prove the chromosome number is normal, and this embryo will have at least 50% to get patient pregnant. So this is the way to figure out the embryo quality. If patient can move on to the next step, then we can work on both side, and try to improve the quality of the embryo and also thoroughly evaluate the uterine cavity. Thank you.

Up to 90% of women report feeling some kind of nausea or vomiting during the first trimester of their pregnancy. So it's pretty normal if you're pregnant and you feel nauseous. Maybe you want to eat a ton all the time because that kind of keeps the nausea at bay. There are a couple of over-the-counter medications that you can try - vitamin B6, vitamin B12, and ginger can help with nausea, and so can changing what you eat, when you eat it, and the type of food you eat. Many women find that if they eat small frequent meals throughout the day, it helps with the nausea, like if you keep a full stomach. And many women have found relief if they just put a few crackers at their bedside, and eat a few even before you get out of bed to go to the bathroom, and that just kind of sets a better tone for the entire day. Morning sickness is not named well because you can feel sick at any time during the day - morning, mid-day, night, even in the middle of the night. Also drink a lot of water. Some women find that if they just constantly sip on water, or suck on ice, or have hard candy in their mouth, it helps to keep the nausea at bay. But some women also find that even the taste of water turns their stomach. So if that's the case, just try adding some sort of sugar-free flavoring to your water and see if that helps. Now if all else fails, there are other medications that can help, and talk with your OB provider. Sometimes stronger medications are necessary, especially if you're not able to keep food or fluids down, because this can lead to malnutrition and other complications of pregnancy. 6 Unusual Signs of Iron Deficiency Iron is a mineral that plays a vital role in health and well-being. Without it, many bodily functions would malfunction. “The primary role of iron is to carry oxygen in the blood to every cell in the body,” says Beth Thayer, RDN, MS, director of the Center for Health Promotion and Disease Prevention at Henry Ford Health System in Detroit. Iron is an important component of hemoglobin, the protein in red blood cells that carries oxygen from the lungs and transports it throughout the body. If your body doesn't have enough iron, it cannot produce enough healthy oxygen-carrying red blood cells. Iron deficiency can cause anemia, which means you have too little hemoglobin. Women in their childbearing years are at higher risk for iron deficiency because of the loss of blood during menstruation. According to the Centers for Disease Control and Prevention, the recommended daily dietary allowance for iron for 19-to-50-year-old women is 18 milligrams (mg). Pregnant women should be consuming 27 mg per day. Adult men, on the other hand, need about 8 mg of iron per day. “Women who are pregnant need the most amount of iron,” says Marilyn Murr, MD, clinical assistant professor of family and community medicine at the School of Medicine at the University of Texas Health Science Center at Houston. “The baby acts as a parasite taking iron, and the woman increases her blood volume to circulate through the baby and create the baby’s blood volume.” There are two types of iron: heme iron from animal sources and non-heme iron from plant sources. Heme iron is absorbed more readily by the blood. Foods rich in iron include beef, liver, oysters, beans, fortified cereals, and dark leafy greens like spinach. Insufficient iron levels can wreak havoc all over the body. Here are seven surprising signs of iron deficiency. 1. Extreme Fatigue and Exhaustion The principle sign that your body is not getting enough iron may be the most difficult to link to iron deficiency. “This is one of the most common signs of iron deficiency because it means your body is having trouble carrying the oxygen to your cells so it’s affecting your energy levels,” Thayer says. People lacking enough iron in their blood often feel sluggish, weak, and unable to focus. Though fatigue can be the sign of numerous conditions, if it does not go away with adequate rest, consider having your iron levels checked. 2. Frequent Infections Iron plays a key role in a healthy immune system, so lower levels of the mineral can make someone more susceptible to infections. “Red blood cells help to transport oxygen to the spleen, which is one place where infections can be fought off,” Dr. Murr says. Red blood cells also carry oxygen to the lymph nodes, which house infection-fighting white blood cells. “When someone has an iron deficiency, the white blood cells aren’t being produced as well, and they’re not as strong because they’re not getting enough oxygen, making that person more susceptible to infections,” she says. 3. Pale Skin Though it’s not always the case, pale skin is often associated with being sick, and there’s good reason for that. Hemoglobin gives skin its rosy color, so low levels cause the skin to become lighter. “When red blood cells become low with iron, they become smaller and paler in the center so skin also becomes paler,” Murr says. This may be easier to detect in people with lighter complexions, but no matter what your skin tone, if the area inside your bottom eyelid is lighter than normal, this may be a sign of iron deficiency. 4. Swollen Tongue Lack of oxygen can also cause muscles to enlarge and become painful. “It happens to all the muscles really, but the tongue is the only one you can see,” Murr says. Cracks on the side of the mouth are also common among people with iron deficiency. 5. Restless Legs Syndrome Some people who have iron deficiency develop restless legs syndrome, a disorder that causes you to have a strong urge to move your legs. The urge often comes with an unpleasant, crawling sensation in the legs that can make it hard to sleep. 6. Hair Loss Iron deficiency, especially when it develops into anemia, can cause hair loss. “When hair follicles don’t get enough oxygen, they go into a resting stage, and hair falls out and doesn’t grow back until anemia is improved,” Murr says. It is normal to lose about 100 strands of hair per day. However, if you notice your hair loss is excessive and it is not growing back, this may be a sign of iron deficiency. Research conducted in Japan revealed that asthma attacks in school children were significantly lowered in those subjects taking a daily vitamin D supplement of 1200 IU a day. 7.) Periodontal disease - Those suffering from this chronic gum disease that causes swelling and bleeding gums should consider raising their vitamin D levels to produce defensins and cathelicidin, compounds that contain microbial properties and lower the number of bacteria in the mouth. 8.) Cardiovascular disease - Congestive heart failure is associated with vitamin D deficiency. Research conducted at Harvard University among nurses found that women with low vitamin D levels (17 ng/m [42 nmol/L]) had a 67 percent increased risk of developing hypertension. 9.) Schizophrenia and Depression - These disorders have been linked to vitamin D deficiency. In a study, it was discovered that maintaining sufficient vitamin D among pregnant women and during childhood was necessary to satisfy the vitamin D receptor in the brain integral for brain development and mental function maintenance in later life. 10.) Cancer - Researchers at Georgetown University Medical Center in Washington DC discovered a connection between high vitamin D intake and reduced risk of breast cancer. For a woman, being too thin, or anorexic, will definitely hinder pregnancy, since the body does not have the proper nutritional requirements, or the required strength. Alternatively, obesity can also be a sign. Obesity is accompanied by hormonal imbalance, which affects the reproductive system and pregnancy. Signs of Infertility in Men: Like females, in males too, either obesity, or anorexia, is an indication that he is infertile. Apart from these, anatomical defects may also be signs of infertility. Undescended testicles, or damage to scrotum and the gonads, are possible indicators too. Wearing tight undergarments, or exposing the testicles to heat, may render the person unable to produce the required number of sperms, or unable to produce sperms altogether, resulting in infertility. Determining the signs of infertility: There are many medical procedures for detecting the signs that help to determine whether an individual is infertile or not. Doctors usually prescribe one or more of the following medical tests: . Hysterosalpingography. A dye injected into the vagina is monitored to check for blockage in the fallopian tubes or uterus. . Laparoscopy. If disease and other physical problems are present in the ovaries, fallopian tubes, or in the uterus, infertility is suggested. This may be detected through laparoscopy. Infertility is a problem which can be solved if it is addressed in the holistic way, which is, using a multifaceted method of healing. These functions are particularly important in tissues that undergo frequent cellular division (like hematopoietic cells) and during periods of rapid cellular division (such as infancy and pregnancy). Folate is found naturally in leafy vegetables and Folium is the Latin word for leaf. Today many grains are also fortified with Folate. With so many items having Folate added a deficiency from decreased intake of Folate is not very common. Usually a deficiency is caused by an increased demand for folate through pregnancy or hemolytic anemias such as Sickle Cell Disease. Folate deficiencies in pregnant women are linked with the occurrence of Neural Tube Defects in the fetus. A deficiency can also be caused by drugs that inhibit the pathways that Folate is involved in such as Trimethoprim (antibiotic) and Methotrexate (Chemotherapy). A Folate deficiency primarily causes anemia (decreased red blood cells). Anemias can present with a wide range of symptoms including weakness, fatigue, pale skin and shortness of breath. Folate Deficiency causes a specific type of Anemia called Macrocytic/Megaloblstic Anemia where the size (Mean Corpuscular Volume) of the red blood cells increased. This is because as the creation of DNA is inhibited the cell cycle in the hematopoietic cells is stalled. Cellular division is stopped but the cell continues to grow resulting in a smaller number of cells which are larger than normal cells. Patients with this type of anemia also often present with Hypersegmented Neutrophils on a blood smear, which is a neutrophil with 5 or more "segments" in the nucleus. You are also going to see an elevation of Homocysteine levels in Folate Deficiency. This is because B12 needs to receive a methyl group from Folate so it can pass it on to Homocysteine to create Methionine. Less Folate means B12 doesn't have a methyl group to pass on and Homocysteine builds up. Unlike B12 deficiencies, Folate deficiency does not result in a buildup of Methylmalonic Acid or neurological symptoms. Vitamin B12 (Cobalamin) is a vitamin, like Folate, that is important for 1 carbon chemical reactions. B12 is found in many animal products and Vegans are at the highest risk of deficiency. B12 taken in through the diet is bound to proteins. Stomach acid and digestive enzymes must first separate B12 from the proteins. If there is a deficiency of stomach acid due to disease or medication (like proton pump inhibitors or antacids) it can cause decreased absorption and B12 deficiency. Once it is no longer protein bound, B12 binds Intrinsic Factor which is released by the Parietal Cells in the stomach. Intrinsic Factor then chaperones the B12 to the terminal ileum where it is absorbed. Pernicious Anemia is the autoimmune destruction of parietal cells which leads to less Intrinsic Factor production. Less intrinsic factor means less B12 absorption and B12 deficiency. Damage to the terminal ileum, such as in Crohn's Disease, can also inhibit B12 absorption. Folate and B12 work closely together handing off methyl groups to each other. It is sort of like a game of hot potato. Folate doesn't "want" the methyl group so it passes it on to B12. B12 doesn't "want" it either so it passes it off to methionine. In effect, B12 helps recycle methyltetrahydrofolate back into tetrohydrofolate which can be used to create DNA. This means that a deficiency of B12 can cause a deficiency of Folate as less Folate is being recycled into its "active" form. Therefore, B12 deficiency present very similarly to Folate deficiency. You get Macrocytic/Megaloblastic Anemia with Hypersegmented Neutrophils and increased Homocysteine levels. If you mistakenly diagnose Folate Deficiency when it is really B12 Deficiency the patient will get better with Folate supplementation, but permanent neurological damage will be done overtime. Odd chain fatty acids are broken down to eventually give Methylmalonyl CoA. B12 is a cofactor in the process that converts methylmalonyl CoA into Succinyl CoA which can then be used in the TCA cycle to generate energy. If there is not enough B12 this reaction is slowed and Methylmalonic Acid builds up. This Methylmalonic Acid build up is toxic to neurons and leads to demyelination in the posterior and lateral columns of the spinal cord. This is called Subacute Combined Degeneration and presents with peripheral numbness/tingling, spasticity, and loss of vibration and proprioception.

Once a woman starts treatment for planning pregnancy after she has undergone treatment for ectopic pregnancy she is under immense mental pressure and trauma. She has lots of questions in her mind because most of the patients might have lost their one fallopian tube. Some of the patients still will be regaining their one fallopian tube if they have undergone medical management of ectopic pregnancy with injection methotrexate. But we have a common query when they should start planning, whether their fertility potential will be normal, how much time will they take to become pregnant or whether everything will be normal or whether there will be chances of recurrence or not. So they are under too much of emotional stress, so they need more of understanding and more of emotional support. As per data it has been found that there is no time limit to wait for planning pregnancy after you have undergone treatment for ectopic pregnancy. But, on an average we gynaecologist would say that you should wait for two to three menstrual cycles after the treatment of ectopic because the woman is out of the mental trauma from the past experience, as well as the hormonal balance comes back to normal and the ectopic pregnancy site which has been removed and stitched reverts back to normal regain its previous strength as well as comes back to normal. In some cases where the mother has undergone treatment of ectopic with injection methotrexate we tell the couple to wait atleast till the beta HCG comes back to normal and then we advice a folic acid supplement for atleast three months and then they should start planning for pregnancy. Because folic acid levels are lowered in the body by the inj.methotrexate and if there is a deficiency of folic acid it might lead to development of neural tube defect. Another common anxiety in women who have undergone ectopic pregnancy treatment is that how much time will I take to conceive as I have already lost one of my fallopian tube. But they should not be disheartened because one fallopian tube is lost, but another one is there and sometimes both the ovaries will ovulate simultaneously and sometimes only one ovary will ovulate and next time of the cycle the next ovary will ovulate. It has been found that about 65% of the women conceive with first 18 months of try and more than 85% they conceive after 2 years of try. So ideally these couple should adopt a healthy life style, they should have adequate folic acid supplementation, increased intake of green vegetables, fruits. They should try for the pregnancy by using the ovulation tests or they can go for the basal body temperature. They should try for pregnancy around the time of the ovulation. They could also take the help of the vaginal discharge or breast tenderness or the changes that happen in the middle of the month. If they don’t conceive with all these measures then they should seek the gynaecologist’s opinion. If the woman is under the age of 35, they can try for one year, no problem. But if the age is more than 35 and if within 6 months they are not getting pregnant then they should seek the gynaecologist’s advice. Here we advice them to complete thyroid hormonal assay and most important thing is a tubal patency test in which we do the histrosalpingogram and we check the patency of the fallopian tubes, whether it is open or because of previous impact it is blocked. If everything is fine and if a woman is having irregular cycle we help them in conceiving by ovulation induction, sometimes by intra uterine insemination. In persistent cases they can go for the IVF.

These include lying still for a while after having sexual intercourse, avoiding the bathroom for about thirty minutes after having sexual intercourse, and most importantly, figuring out how many times one should have sexual intercourse. Men often neglect their own part to play in this whole scenario. Thank you for watching my 25 week pregnancy update! My legs have been getting sore and swollen - my midwife recommended I buy some magnesium spray to help out with this so I'll be doing that soon. I've also been very tired and find myself laying or sitting down more throughout the days. Though I'm tired and can't do much, there is still so much I want to prepare! The baby's room is not even started yet, and there are a few other projects around the house that I would like to have done to make things easier for when baby is here.
