When I consider purchasing an individual health insurance plan for myself or my family, do I have any financial obligations beyond the monthly premium and annual deductible? Answers: It depends on the plan, but some plans have the following cost-sharing elements that you should be aware of. Co-Payments: Some plans include a co-payment, which is typically a specific flat fee you pay for each medical service, such as $30 for an office visit. After the co-payment is made, the insurance company typically pays the remainder of the covered medical charges. Deductibles: Some plans include a deductible, which typically refers to the amount of money you must pay each year before your health insurance plan starts to pay for covered medical expenses. Coinsurance: Some plans include coinsurance. Coinsurance is a cost sharing requirement that makes you responsible for paying a certain percentage of any costs. The insurance company pays the remaining percentage of the covered medical expenses after your insurance deductible is met. Out-of-pocket limit: Some plans include an out-of-pocket limit. Typically, the out-of-pocket limit is the maximum amount you will pay out of your own pocket for covered medical expenses in a given year. The out-of-pocket limit typically includes deductibles and coinsurance. But, out-of-pocket limits don't typically apply to co-payments. Lifetime maximum: Most plans include a lifetime maximum. Typically the lifetime maximum is the amount your insurance plan will pay for covered medical expenses in the course of your lifetime. Exclusions & Limitations: Most health insurance carriers disclose exclusions & limitations of their plans. It is always a good idea to know what benefits are limited and which services are excluded on your plan. You will be obligated to pay for 100% of services that are excluded on your policy. Beginning September 23, 2010, the Patient Protection and Affordable Care Act (health care reform) begins to phase out annual dollar limits. Starting on September 23, 2012, annual limits on health insurance plans must be at least $2 million. By 2014 no new health insurance plan will be permitted to have an annual dollar limit on most covered benefits. Some health insurance plans purchased before March 23, 2010 have what is called "grandfathered status." Health Insurance Plans with Grandfathered status are exempt from several changes required by health care reform including this phase out of annual limits on health coverage. If you purchased your health insurance policy after March 23, 2010 and you're due for a routine preventive care screening like a mammogram or colonoscopy, you may be able to receive that preventive care screening without making a co-payment. You can talk to your insurer or your licensed eHealthInsurance agent if you need help determining whether or not you qualify for a screening without a co-payment. There are five important changes that occurred with individual and family health insurance policies on September 23, 2010. Those changes are: 1. Added protection from rate increases: Insurance companies will need to publically disclose any rate increases and provide justification before raising your monthly premiums. 2. Added protection from having insurance canceled: An insurance company cannot cancel your policy except in cases of intentional misrepresentations or fraud. 3. Coverage for preventive care: Certain recommended preventive services, immunizations, and screenings will be covered with no cost sharing requirement. 4. No lifetime maximums on health coverage: No lifetime limits on the dollar value of those health benefits deemed to be essential by the Department of Health and Human Services. 5. No pre-existing condition exclusions for children: If you have children under the age of 19 with pre-existing medical conditions, their application for health insurance cannot be declined due to a pre-existing medical condition. In some states a child may need to wait for the state's open-enrollment period before their application will be approved.

In testimony that Maryland Senator Barbara Mikulski called bone-chilling, Colorado mom Peggy Robertson recently shared with a Senate committee the unbelievable story of how a major health insurance carrier refused to insure her and what they suggested she do to make herself eligible for coverage. In 2007, Robertson was denied coverage by Golden Rule Insurance, a subsidiary of UnitedHealth Group. In a letter explaining the denial, the company claimed that Robertsons previous C-section made her a financial risk for the company. If she had another C-section within the first three years of coverage, the company would not profit from her policy. It was a risk they werent willing to take and because the Colorado Insurance Division would not allow them to exclude C-section benefits for the first three years, they denied her coverage altogether. Robertsons story may sound all too familiar, but unfortunately it doesnt end there. In that same letter, Golden Rule went on to suggest what Robertson might do to make herself eligible for coverage: Get sterlized. That an insurance company would dare to even hint that a healthy woman should be medically sterilized so they can make a profit is, in the words of Senator Mikulski, morally repugnant. But lets face it, women already get the short end of the stick when it comes to health care coverage. Possessing a womb makes us a liability in the eyes of insurers and more than half of them dont even offer maternity coverage. Those who do will often raise the premiums of women who have had C-sections or, as in Robertsons case, refuse to cover them at all. Of course, women arent the only ones getting a raw deal from profit-driven health insurance companies. Americans are suffering all over under our current system of health care. But when the largest insurance company in the United States feels like they have a right to plan your family to benefit their bottom line, we have a serious problem. What will it cost to buy health insurance on the ACA's Insurance Marketplace? Perhaps the biggest question patients might have, that question is answered in this, the ninth installment of the Texas Medical Association's video series explaining the new Health Insurance Marketplace (sometimes called "exchange") under the federal health law, the Affordable Care Act. The physician, Carlos "Hey Doc" Cardenas, MD, Chair of TMA's Board of Trustees, answers, "Here's a surprise, friend. The cost of your insurance depends on what you want to buy, but it also depends on you. The insurance companies can charge you more or less depending on where you live, how old you are, how many people you want to cover in your family, and whether you smoke. What you pay also depends on what insurance you choose. Buy a bronze or silver plan, and you'll pay less each month in premiums. But you will have to pay more of the costs when you actually use the insurance. The opposite happens with gold and platinum plans. They have higher premiums and lower out-of-pocket costs. Remember too, you can buy plans that cover more kinds of health care services -- but they will cost more. And your income determines whether you can get government help to pay for your insurance. Memorial Healthcare System is now providing residents in South Broward with integrated primary and behavioral healthcare under the same roof as part of a federally and county funded grant aimed at delivering personalized treatment plans for patients. The Mind/Body Wellness Project – administered by Memorial Outpatient Behavioral Health Center and South Broward Community Health Services– will form a Core Team that provides the services at its newly expanded outpatient clinic located in Hollywood, Florida. The federal grant, awarded by the Substance Abuse and Mental Health Services Administration with additional funding through the Broward County Department of Human Services, will provide for a dedicated nurse practitioner, an RN Care Coordinator, a Medical Assistant, an Integrated Care Manager/Social Worker, a Peer Wellness Coach and a Psychiatric Pharmacy Resident. The resources and personnel provided by the grant is now in addition to the center’s established multidisciplinary and multicultural team of licensed psychiatrists, advanced registered nurse practitioners, licensed psychotherapists and certified peer support specialists who use effective evidence-based approaches to help patients achieve their goals. Additional services will include nutritional services, exercise classes, smoking cessation and chronic disease self-management support. Each ancillary program is evidence-based and targets behavioral changes toward promoting health outcomes of the patient, said the center’s medical director, Clara Alvarez Villalba, MD. “Logistically, The Mind/Body Wellness Project and the Telepsychiatry will increase the access to a full range of medical care for patients. It will help improve coordination of care among providers.” The services are being provided at the Center for Behavioral Health, 3400 N. 29th Avenue, Hollywood, Florida. Additional clinical rooms have already been constructed and are now fully operational. As part of this initiative, telepsychiatry units will be installed at two South Broward Community Health Centers and one at Memorial Outpatient Behavioral Health Center so patients will be able to access behavioral health providers via video as part of their primary care visit. Project director David Freedman said having a clinic that provides both mental and physical healthcare will benefit patients who often seek help in one area, but not both. “While mental disorders including substance use issues are common in the United States, the burden of chronic illness is particularly concentrated among these patients,” Freedman said. Adults with both physical and behavioral health issues experience greater morbidity and mortality than those in the general population that do not have these co-occuring medical issues, Freedman said. Under the program, the Core Team will meet weekly to develop, oversee and manage a personalized treatment plan for each patient. Patients’ disease specific improvements will be evaluated initially to establish a baseline followed by six-month measurements of blood pressure, body/mass index, waist circumference, breath CO, plasma glucose, HgA1c, HDL and LDL cholesterol and triglycerides. Mental health symptom reduction and sobriety over time will be monitored as well. For more information about the program, call (954) 276-3455.
