The ratio of INTRAcellular to EXTRAcellular potassium is important for generation of action potentials and is essential for normal functions of neurons, skeletal muscles and cardiac muscles. This is why potassium levels in the blood are strictly regulated within a narrow range between 3.5 and 5mmol/L. As the normal daily dietary intake of potassium varies widely and can be as much as 100mmol a day, the body must keep blood potassium levels within the normal limits. This is achieved by 2 mechanisms: - Excretion of potassium through the kidneys and intestines - Shifting of potassium from the extracellular fluid into the cells by the sodium/potassium pump. The pump is mainly regulated by hormones such as insulin and catecholamines. Hyperkalemia is defined as a serum potassium concentration HIGHER than 5mmol/L. Hyperkalemia may result from decreased excretion, excessive intake, or shift of potassium from INSIDE the cells to EXTRA-cellular space. The most common scenario is a RENAL INsufficiency combined with excessive potassium supplements OR administration of certain drugs. Impaired kidney function is most prominent Mild hyperkalemia is often without symptoms, some patients may develop muscle weakness. Slow or chronic increase in potassium levels is less dangerous, as the kidneys eventually adapt by excreting more potassium. Sudden onset and rapid progression of hyperkalemia can be fatal. As potassium levels INcrease in the EXTRAcellular space, the MAGNITUDE of potassium gradient across the cell membrane is REDUCED, and so is the ABSOLUTE value of the resting membrane potential. Membrane voltage becomes less negative, moving closer to the threshold potential, making it EASIER to initiate an action potential. The effect this has on excitability of myocytes, however, is complex. While initial changes seem to increase myocyte excitability; further rise of potassium has the OPPOSITE effect. This is because the value of membrane potential at the onset of an action potential DETERMINES the number of voltage-gated sodium channels activated during depolarization. As this value becomes less negative in hyperkalemia, the number of available sodium channels DEcreases, resulting in a SLOWER influx of sodium and subsequently SLOWER impulse conduction. ECG changes produced by hyperkalemia follow a typical pattern that correlates with serum potassium levels: peaked T-wave, P wave widens and flattens, PR interval lengthens, QRS complex widens and eventually blends with T-wave. Diagnosis on the basis of ECG alone very difficult. Acute hyperkalemia must be suspected in any patient having new bradycardia or conduction block, especially in those with renal problems. Severe hyperkalemia is treated in 3 steps: - Calcium infusion is given to rapidly REVERSE conduction abnormalities. - Insulin to stimulate the sodium/potassium pump, promoting INTRA-cellular shift - Hemodialysis to remove potassium

In some cases, babies may need to receive treatment at a hospital. RSV is a virus, so unfortunately there are no medications that can cure it or shorten its course. Symptoms of RSV in babies In older children, RSV can cause symptoms similar to that of a cold. But in babies, the virus causes more severe symptoms. RSV is most commonly transmitted from November to April, when cooler temperatures bring people indoors and when they’re more likely to interact with each other. RSV tends to follow a timeline of symptoms. Anywhere from two to eight days after a baby is exposed to the condition, they may start to experience symptoms. Initial symptoms may not be all that noticeable, such as a decreased appetite or a runny nose. More severe symptoms will appear anywhere from one to three days later. Symptoms a baby may have with RSV include: • breathing that is faster than normal • difficulty breathing • cough • fever • irritability • lethargic or behaving sluggishly • runny nose • sneezing • using their chest muscles to breathe in a way that appears labored • wheezing When to see a pediatrician for RSV Some babies are more vulnerable to the symptoms of RSV. This includes children who were born prematurely, and/or babies with lung or heart problems. RSV cases can range from mild cold symptoms to severe bronchiolitis. But if you suspect your baby has RSV, it’s important to call your pediatrician and/or seek emergency medical care. These symptoms include: • baby appears dehydrated, such as sunken fontanels (soft spots) and no tear production when they cry • coughing up thick mucus that is gray, green, or yellow in color • fever greater than 100.4°F (38°C) in babies younger than 2 months • fever greater than 101°F (38.3°C) for babies ages 3 to 6 months • fever greater than 103°F (39.4°C) in babies older than 6 months • thick nasal discharge that makes it tough for baby to breathe Seek immediate medical care if your baby’s fingernails or mouth are blue in color. This indicates your baby isn’t getting enough oxygen and is in severe distress. Treatment for RSV in babies In the most severe cases, RSV may require the help of a breathing machine known as a ventilator. This machine can help to inflate your baby’s lungs until the virus has had time to go away. Doctors used to (and some still do) treat most cases of RSV with bronchodilators. But this is no longer recommended. Examples of bronchodilator medications include albuterol, which is under the brand names: • ProAir HFA • Proventil-HFA • Ventolin HFA These are medications used for people with asthma to help open up the airways and treat wheezing, but they do not help the wheezing that comes with RSV bronchiolitis. If your little one is dehydrated, their doctor may also provide intravenous fluids (IV). Antibiotics won’t help your baby’s RSV because they treat bacterial infections. RSV is a viral infection. Preventing dehydration in babies with RSV Providing fluids, such as breast milk or formula, can be important to preventing dehydration in your baby. You can ask your doctor if you should give your baby an electrolyte-replacing solution. Keep your baby in an upright position, which makes it easier for them to breathe. You can keep your baby in their car seat during the day. At night, you can raise your child’s mattress by about 3 inches. You can place an object underneath your baby’s mattress to keep it higher up. Always place your baby on their back to sleep. Limiting your baby’s exposure to cigarette smoke is also vital to keeping them healthy. Cigarette smoke can make your baby’s symptoms worse.

Usually when patients have problems with asthma/allergies/sinusitis (especially asthma) the typical conventional response is to give them medications to inhibit their immune system, mainly steroids and prednisone. As a matter of fact, it's the first line treatment that's recommended to treat people with those types of problems. They created Advair (it's a medicine called serevant and prednisone) so you don't necessarily know that there are steroids in that medication. It's a spray that works every twelve hours. What it does is that it dilates your bronchioles, gives you a bunch of steroids that would get systemically absorbed, and over time, you form less of your own steroids (or cortisol) that you would need to fight off allergies. The best way is to get at the cause of the problem, use certain vitamins and nutrients, and you can shut off some of the allergic response, and get people off some of these medications. I was on prednisone - twenty milligrams a day for two and a half years - so I know a lot about prednisone (that was for a type of joint pain and inflammation). The bottom line is: get at the source of the problem, make your body more alkaline/less acidic, and eliminate the foods and chemicals that act as triggers, and you can get people off these medications that cause all sorts of problems down the road. What kinds of problems? Well there are sleep problems. You put someone on steroids, they wind up having insomnia; inability to get REM sleep. They wind up with fatigue; it drops their blood sugar. They wind up with chronic yeast problems. It's immunosuppression. these people wind up getting colds more frequently, needing more and more antibiotics, all of which never gets at the cause of the problem, and then they get stuck on the medicines like I did. There are ways to get at the source of the problem, and pick lower-risk high-gain approaches then pick these steroids that have lots of systemic problems with them; thinning your bone, osteoporosis, and a whole range of other problems. Metered-dose inhaler demonstration for nursing students and nurses using the open and closed mouth technique. Learn the proper technique on how to use a metered-dose inhaler. What is a metered-dose inhaler? It’s an inhaler used to administer a set amount of medication into the lungs. Medication used as a MDI include bronchodilators, corticosteroids etc. As a nurse is very important to teach the patient proper technique on how to use the MDI so the correct amount of medication enters into the lungs rather than collecting in the mouth. It is best to use a spacer with a metered-dose inhaler (this will be demonstrated in the next video). A spacer will ensure the correct amount of medication enters the lungs and doesn’t stay in the mouth (plus it makes it easier to use the inhaler). Some of these inhaled medications (like corticosteroids) can be irritating to the mouth and can cause thrush. If a spacer is not used, the open-mouth technique (which takes much dexterity) or the closed-mouth technique can be used (both will be demonstrated in this video). Be sure to teach the patient to use the method prescribed by the physician. First, you must prime the inhaler. This needs to be done when: using it for the first time, it has been 1 or more weeks since last used, been dropped or clean. Every inhaler is different in how many sprays are needed to prime the inhaler, so always read the instructions (watch the video for demonstration). (Uploaded for my Doctor) "You dodged a bullet today Heather," the emergency room doctor told me today that I almost died. I had a severe asthma attack. For no particular reason, I was only painting a small canvas. You can tell from my graffiti hands! It is the third attack I've had in a short time, so I started to videotape it for my doctor. I discovered the hard way that Isaac has been playing with all my inhalers, and he literally ran them all dry. So, I did not have an inhaler, but I did have a nebulizer. Little did I know how severe it actually was, and still is. (Been in the hospital 5 days now) I couldn't walk two steps, so Chris called for help. My hero hubby. I keep asking for my purse for more Albuterol, which wasn't reaching my lungs. Please be careful asthma sufferers. #AsthmaExacerbation 🙇 The fire department came, literally to my rescue! They saved my life. I had begun to turn grey and pass out. They started an IV, gave me injections, and carried me to a waiting ambulance. These men saved my life. All of the things the firefighters did to help me, I cannot even remember, as I had no oxygen to the brain. How do you begin to thank someone for saving your life? I owe everything to those firefighters. 🙇 Once I got to the hospital, the doctor told me that the nebulizer and inhaler would NOT have helped me today, and if I didn't come into the hospital I probably wouldn't have survived! 🙇 FYI! I didn't realize that when you're in the midst of an attack, if your inhaler does not help and over 4 breathing treatments do not help, you must get steroid injections, there is no two ways about it. And the fact that Isaac playing with my inhaler saved my life because it forced me to go to the emergency room! A blessing! Friends, not being able to breathe is definitely one of the most frightening experiences in my life. 🙇 I have to stay in the hospital until tomorrow, at least, so please, please say a prayer for me tonight! Thank you so much and love you guys! If your asthma symptoms keep getting worse even after you take medication as your doctor directed, you may need emergency room care. Your doctor can help you learn to recognize an asthma emergency so that you'll know when to get help. When to see a doctor: If your asthma flares up, immediately follow the treatment steps you and your doctor worked out ahead of time in your written asthma plan. If your symptoms and peak expiratory flow (PEF) readings improve, home treatment may be all that's needed. If your symptoms don't improve with home treatment, you may need to seek emergency care. When your asthma symptoms flare up, follow your written asthma plan's instructions for using your quick-acting (rescue) inhaler. PEF readings ranging from 50 to 79 percent of your personal best are a sign you need to use the quick-acting (rescue) medications prescribed by your doctor. When to seek emergency medical treatment Seek medical attention right away if you have signs or symptoms of a serious asthma attack, which include: Severe breathlessness or wheezing, especially at night or in the early morning The inability to speak more than short phrases due to shortness of breath Having to strain your chest muscles to breathe Low peak flow readings when you use a peak flow meter No improvement after using a quick-acting (rescue) inhaler #Station17 #station17Firefighters #ColoradoSpringsFireFighters The Fire House Projects Co. The Story of Mad Eye, My one-eyed kitty..

This is a story about my one-eyed kitty. He was found in a ditch on the side of the road by some teenagers. They brought him into my place of work (a veterinary hospital) not knowing what to do with him. Upon further inspection, it was noted that he had a SEVERE upper respiratory infection and the feline herpes virus. He also had an eye that was popped out of his head. He was emaciated (about 1 lb) and was barely able to breathe. It seemed that the only solution was euthanasia. When he was just about to be euthanized, i put my hand in the bucket to grab him out and he started purring and licking me and nuzzling my arm. After that, i knew that it was fate and that I had to help this poor little baby. I had to force feed him and give him antibiotics for about 1 month. After that, he started eating on his own. Then i took him home and let him get stronger with the antibiotics and food until his upper respiratory infection was at bay, then he went in for surgery to remove his popped out eyeball. We continued him on antibiotics and he had to wear an e-collar for about 1.5 months. After that, he was completely healed. He now rules the roost and is the best most amazing little fighter that i could ever imagine. He had a really rough start to life, but he is now doing amazingly well. In November, he was diagnosed with Feline Asthma. He was having about 20 asthma attacks per day, unnoticed by me (the moron) who just thought he was coughing up a hairball. he is now treated (with prednisolone) and his Aero-Kat inhaler with Albuterol to make sure he is not having issues breathing anymore. He weighs about 10 lbs now, and everything else is going well. SMITH COUNTY (KYTX) November 1, 2012 By Dana Hughey Every year, the families of almost 420- thousand women lose a wife, mother, sister or daughter to the nation's number one killer. It's heart disease that's killing the women you love and most women don't even know it. Meet a Lindale woman whose life was touched by heart disease... Not once, but twice in "A Woman's Heart," a KYTX CBS 19 Special Report. Holly rand lost her mother and grandmother just 3 years apart. Both died of heart attacks. In both cases, it was sudden. And both times, their cardiovascular disease went undiagnosed until their deaths. It's a loss she doesn't want her family, or yours, to have to go through. Holly Rand's mother, Sharon, didn't live long enough to see her daughter succeed as a business owner. She died just after celebrating her 35th wedding anniversary. "I lost my mother at her young age of 56 and my young age of 32, at the time my daughter was only 2. She barely knew her Nana," says Holly Rand. Sharon had heart disease, and didn't know it, but, there were signs. "A cough that was really deep and bad," says Holly Rand. Holly says her mom felt bad enough to go to an emergency room in another part of Texas just 10 days before she died. "(The doctor) put her on a prescription of albuterol. The autopsy said it probably led to the final effect of her having a heart attack," says Holly Rand. Sharon's death lead to so many "why's" for her daughter. Why didn't the doctors diagnose the heart disease? Why didn't her mom take better care of herself? "It was kind of like, why don't women know this?" asked Holly. And, why didn't Holly's mom get checked, knowing that her own mom died of heart disease? "Women get busy in our homes and we miss the symptoms and we don't go to the doctor," says Holly. That's the very premise behind a video, "Just a Little Heart Attack" from the American Heart Association's Go Red for Women campaign. It's meant to be a powerful wake-up call to women to understand their risk for heart disease and empower them to make their health a priority. "Here is what is a little bit scary to me 43% of women with heart attack reported having no chest pain whatsoever," says Deb Taylor, Chief Nursing Officer for the Louis and Peaches Owen Heart Hospital, Trinity Mother Frances Hospitals and Clinics. Deb Taylor is the Chief Nursing Officer for the Louis and Peaches Owen Heart Hospital at Trinity Mother Frances Hospitals and Clinics. She says women don't always experience heart attacks with crushing chest pain. Possible signs that women experience and overlook prior to a heart attack are-- upset stomach, fatigue, sleep disturbance, shortness of breath, and anxiety. During a attack-- many women feel a shortness of breath, unusual fatigue, weakness, cold sweats and dizziness. "If you're feeling those symptoms at home. You should call 911 and have an ambulance take you to the emergency room, because if your situation worsens as you're driving down that road, we want you in the care of someone who can help you right away," says Deb Taylor. Taylor says while some of the risk factors-- like genetics are out of your control, there are things you can control-- like blood pressure, weight, diet, your activity level and stress. All risks holly took a close look at herself. "I didn't have the energy or the focus. It was literally in December that I started working out and in January of 2012 I started eating a heart healthy diet, taking vitamins. She also now reminds the women in her life-- sisters, friends and co-workers to care of their hearts and listen to their bodies, so they won't miss those special moments her mom missed. Holly doesn't have to see a cardiologist yet, but it's a conversation she's had with all her doctors. Cardiovascular experts say you should too and they suggest you stay educated on the risk factors and symptoms, then do your best to control them. Researchers talked with women who didn't feel chest pain during their heart attack... 95% of them said in hindsight, they did have some signs and symptoms, but didn't put 2 and 2 together. Deb Taylor says that's why getting to the ER is so important, so doctors can perform an EKG and check your blood work to see if you're having a heart attack.

Transcript: Slide 1: Hi, I’m Anthony Simons and I will be presenting on the respiratory disease Emphysema. Slide 2: Emphysema is one half of a condition called COPD. COPD stands for Chronic Obstructive Pulmonary Disease. It is a disease that makes it hard for people to breath. It is usually a mix of emphysema and chronic bronchitis. Bronchitis inflames the passageways that carry air to the lungs, while emphysema is when the air sacs are damaged. More specifically, the inner walls of the alveoli air sacs are damaged and rupture over time. This creates a larger air space but less surface area, which in turn reduces the oxygen that can enter the bloodstream. As I stated earlier, shortness of breath is the staple indicator of emphysema. It is most prevalent in older individuals and those that smoke. Slide 3: The main symptom is shortness of breath. People with emphysema will have trouble especially with climbing stairs. Coughing and wheezing are also present. If lips or fingernails turn a blue or gray color, it means the disease has taken a bad turn and requires immediate attention. Many of those with emphysema will not be as mentally alert. Pursed-lipbreathing is a unique symptom to emphysema. The individual won’t exhale completely to try and empty trapped air in the lungs. They purse their lips to only leave a small opening. This causes air to be blocked when exhaled, which increases the pressure in collapsed air ways. They are then opened and trapped air escapes. Some people also develop a “barrel chest.” This is caused by the air being trapped in obstructed airways. Slide 4: The leading cause of emphysema is smoking. It causes damage to lung tissue and decreases blood flow. The smoky air causes cilia function to decrease, which usually help clear mucus. Mucus production is also increased, resulting in bad air flow. General air pollution can contribute to emphysema in similar ways as smoking tobacco does. One specific cause is Alpha-1-antitrypsin deficiency. When there isn’t enough of this substance produced, it can’t fight off trypsin. Trypsin is a digestive enzyme that, when not removed quickly enough, will start to destroy lung tissue. In general, being male or of an older age increases the chance of getting emphysema. Slide 5: There are some complications that can develop from emphysema. In those that have the disease to a more serious degree, pneumothorax is a possibility. Although uncommon, sometimes extremely damaged lungs can collapse. Heart problems can occur if the pressure is increased in the arteries due to the damage in the lungs. Some people may develop bullae. This is a large hole in the lung, sometimes up to half the size of the lung. This also increases the chance of a collapsed lung. Slide 6: There are a multitude of treatments for emphysema. Two options are bronchodilators and steroids, which can both be inhaled. Bronchodilators such as albuterol will help by relaxing and opening air passageways. Steroids decrease inflammation, but also come with side effects over time. Vaccines and antibiotics should be taken as needed to help fight off infections that occur more easily because of emphysema. If patients find it harder to breath, they may be given oxygen which can be delivered in different ways. Those that have emphysema caused by alpha-1-antitrypsin deficiency can be given AAT to help decrease damage done to the lungs. This is called protein therapy. Lastly, surgery can be used for some patients. A whole transplant can be done if a good match is found and the disease is serious enough. Lung volume reduction surgery is a more common procedure. Surgeons will go in and remove small pieces of lung from the patient. This allows the remaining lung to expand better and help improve their breathing.

HELLO and welcome back to my channel today we are talking about my time with the Influenza A h3n2 virus + Double Pneumonia. I have now spent $300+ on medicines, $300+ on doctors copays and who knows what for an Emergency room visit....all from the flu and its complications in the month of jan/feb 2018 I actually wrote his for my health care policy homework but I find it important to share my story and urge even HEALTHY young adults to get help ASAP..... I found an interesting article in regards to the H3N2 Influenza virus however prior to discussing the article I want to give you a background summary of why this touches so close to home for me. SHARE THIS with your friends:... January 19-26 2018 I had the H3N2 Influenza virus. During this time I felt as though my chest was caving in, I was throwing up and had uncontrollable gastrointestinal issues. I first saw my doctor for the FLU and was diagnosed January 21st just two short days after it started, as I felt like I was going to pass out at this same time my 3 year old got the H3N2 Influenza as well and we were a pair of very very sick people. January 27-30 flu symptoms subsided but i had no voice and was getting short of breath so i went back to the doctor again on the 31, where i was seen for chronic Laryngitis and I was diagnosed with lower right lobe pneumonia. I started antibiotic and felt okay but I couldn’t stop coughing and couldn’t catch my breath this went on for a few days February 3rd i went back to the URGENT care to get checked for severe neck pain and ear infection as i couldn’t hear from my left ear and at this point I felt so sick that I was scared. I got x-rays again and the pneumonia spread to both of my lungs, and i was put on an even stronger antibiotic (Levaquin), i also have a pinched nerve in my neck and severe ear infection AND laryngitis at this point. February 5 i admitted myself to the ER as i couldn’t breath when dropping my son off at daycare, my Oxygen level was alarmingly low and heart rate was 170 this was scary for me, but i got fast treatment, new xrays for pneumonia, a EKG to see if my heart was okay, 3 breathing treatments, steroids for my lungs and 2 IV fluid bags, my heart was racing because i was again dehydrated. I was released from the hospital at 2pm after a 6 hour stay and went to see my Primary care doctor who prescribed me nebulizer albuterol to help me breath. February 7, I am on the mend however my three year old son starts showing signs of coughing. I take him in on February 9th and I was scared that he might of contracted Pneumonia from me. Sure enough he had single lung pneumonia and was put on antibiotics, the next day February 8 my son couldn’t stop throwing up and coughing, I called nurse on call and they sent us straight to Urgent care where his pneumonia spread to both lungs, he was them placed on steroids and double antibiotics. He is finally on the mend as of February 13 2018. Now that I have “aired my garbage” in a sense, you might be asking yourself why I am sharing personal information with you all. I am sharing this because I want people to know the importance of catching Flu symptoms as soon as possible. In the article linked below you will read about the severity of the Flu outbreak in Wisconsin. I am unsure if Minnesota’s flu season has been severe, however I live in Madison Wisconsin and people are dropping like flies. Now I must add that I got my flu shot, my son did not get a flu shot and I regret that decision. My son and I have been sick for a MONTH, and we are just now on the mend. One thing I disagree with in this article is the author states “if you’re generally healthy and you get the flu, you may only need to see a doctor if your fever goes higher, your cough gets worse or you have trouble breathing” The reason I disagree with this statement is because I am a “generally” healthy individual who doesn’t get sick often, my immune system is strong and I tend to bounce back from general colds and coughs quite quickly. I am a 26 year old young and healthy adult and I was taken out for a MONTH with the flu, and if I hadn’t been on top of my health I could have if people are just staying home and “Waiting it out” they can cause themselves more harm than good. Children and elderly are more susceptible to secondary illnesses due to weak immune systems however healthy adults are coming down fast with the illnesses as well because their immune system hasn’t been subject to this strand of the flu yet.
