Metered-dose inhaler demonstration for nursing students and nurses using the open and closed mouth technique. Learn the proper technique on how to use a metered-dose inhaler. What is a metered-dose inhaler? It’s an inhaler used to administer a set amount of medication into the lungs. Medication used as a MDI include bronchodilators, corticosteroids etc. As a nurse is very important to teach the patient proper technique on how to use the MDI so the correct amount of medication enters into the lungs rather than collecting in the mouth. It is best to use a spacer with a metered-dose inhaler (this will be demonstrated in the next video). A spacer will ensure the correct amount of medication enters the lungs and doesn’t stay in the mouth (plus it makes it easier to use the inhaler). Some of these inhaled medications (like corticosteroids) can be irritating to the mouth and can cause thrush. If a spacer is not used, the open-mouth technique (which takes much dexterity) or the closed-mouth technique can be used (both will be demonstrated in this video). Be sure to teach the patient to use the method prescribed by the physician. First, you must prime the inhaler. This needs to be done when: using it for the first time, it has been 1 or more weeks since last used, been dropped or clean. Every inhaler is different in how many sprays are needed to prime the inhaler, so always read the instructions (watch the video for demonstration). Then use either the open or closed mouth technique to administer the medication. SMITH COUNTY (KYTX) November 1, 2012 By Dana Hughey Every year, the families of almost 420- thousand women lose a wife, mother, sister or daughter to the nation's number one killer. It's heart disease that's killing the women you love and most women don't even know it. Meet a Lindale woman whose life was touched by heart disease... Not once, but twice in "A Woman's Heart," a KYTX CBS 19 Special Report. Holly rand lost her mother and grandmother just 3 years apart. Both died of heart attacks. In both cases, it was sudden. And both times, their cardiovascular disease went undiagnosed until their deaths. It's a loss she doesn't want her family, or yours, to have to go through. Holly Rand's mother, Sharon, didn't live long enough to see her daughter succeed as a business owner. She died just after celebrating her 35th wedding anniversary. "I lost my mother at her young age of 56 and my young age of 32, at the time my daughter was only 2. She barely knew her Nana," says Holly Rand. Sharon had heart disease, and didn't know it, but, there were signs. "A cough that was really deep and bad," says Holly Rand. Holly says her mom felt bad enough to go to an emergency room in another part of Texas just 10 days before she died. "(The doctor) put her on a prescription of albuterol. The autopsy said it probably led to the final effect of her having a heart attack," says Holly Rand. Sharon's death lead to so many "why's" for her daughter. Why didn't the doctors diagnose the heart disease? Why didn't her mom take better care of herself? "It was kind of like, why don't women know this?" asked Holly. And, why didn't Holly's mom get checked, knowing that her own mom died of heart disease? "Women get busy in our homes and we miss the symptoms and we don't go to the doctor," says Holly. That's the very premise behind a video, "Just a Little Heart Attack" from the American Heart Association's Go Red for Women campaign. It's meant to be a powerful wake-up call to women to understand their risk for heart disease and empower them to make their health a priority. "Here is what is a little bit scary to me 43% of women with heart attack reported having no chest pain whatsoever," says Deb Taylor, Chief Nursing Officer for the Louis and Peaches Owen Heart Hospital, Trinity Mother Frances Hospitals and Clinics. You should call 911 and have an ambulance take you to the emergency room, because if your situation worsens as you're driving down that road, we want you in the care of someone who can help you right away," says Deb Taylor. Taylor says while some of the risk factors-- like genetics are out of your control, there are things you can control-- like blood pressure, weight, diet, your activity level and stress. All risks holly took a close look at herself. "I didn't have the energy or the focus. It was literally in December that I started working out and in January of 2012 I started eating a heart healthy diet, taking vitamins. She also now reminds the women in her life-- sisters, friends and co-workers to care of their hearts and listen to their bodies, so they won't miss those special moments her mom missed. Holly doesn't have to see a cardiologist yet, but it's a conversation she's had with all her doctors. Cardiovascular experts say you should too and they suggest you stay educated on the risk factors and symptoms, then do your best to control them. Researchers talked with women who didn't feel chest pain during their heart attack... 95% of them said in hindsight, they did have some signs and symptoms, but didn't put 2 and 2 together. Deb Taylor says that's why getting to the ER is so important, so doctors can perform an EKG and check your blood work to see if you're having a heart attack.

Poor Casper. After rescuing this big boy from the outdoors (his previous owner's abandoned him), we found him doing this strange cough. It didn't look like a hairball cough and happened often enough that it concerned us. So we took this video to show our vet. And sure enough, this is the telltale sign of kitty asthma. They also took X-rays to verify. This is what a feline asthma attack looks and sounds like. During an asthma attack, a cat will put his head down near the ground and stretch his neck out (like a seal). This is not a hairball cough. Feline asthma affects approximately 1% of the american domestic feline population. Casper is now taking a steroid, Prednisolone, daily which has helped to decrease the frequency of his attacks. If you see your cat doing this, take him to the vet for a check-up.

On event day I climbed up 3,190 stairs and down 3,190 stairs in 60-min 37-seconds (5 times up and down the 29-story building which exceeded my reported goal of 4 laps... too bad I was not able to shave off that last 37-seconds to complete all 5 laps within the OFFICIAL hour). 5 laps is 145 stories up and 145 stories down I prepared and participated in this event by: 1. Repeatedly walking up and down stairs and hills for many weeks leading up to the event, sometimes into darkness with a headlamp. By preparing early, my body got used to the motions and so event day was more enjoyable than it was miserable. I would experience some soreness after practice sessions. 2. Flat footed, lifting my body with my hamstrings so my calves would not burn out 3. Steady continuous hiking pace on event day that I felt I could sustain for a long time (slightly faster than my practice pace), but not running or skipping stairs on the way up. Starting out too fast on an endurance Hour event like this would not work well. 4. Well hydrated before and during event 5. Took bronchodilator and nasal spray 30-min before warming up 6. Warmed up slowly over a 20-30min period before my official start time, where I very slowly increased my breathing and heart rate up to about 150-160bpm and then held it at that level until start time. 7. Heart rate was 190-205bpm while climbing up stairs (used heart rate monitor) 8. It helped to have light gloves on when going down the stairs so I did not blister my hands on the rails. 9. Event day was fun. Legs felt ok after it was over. However, the next two days after that my legs were definitely sore due to pushing myself a little harder during the event than I did during practice sessions. Cleveland Clinic News Service - April 21, 2017 CLEVELAND - They’re often posted on highway signs or mentioned during the weather forecast, but what exactly is an ozone alert day? According to Sumita Khatri, M.D., of Cleveland Clinic, ozone alert days usually happen during the months of May through September, when there’s more heat, sunlight and pollution from motor vehicles. “Oxygen gets converted to ozone, which can be an irritant to the airways and for people with lung and heart problems, this can be a problem,” said Dr. Khatri. Dr. Khatri said air quality alerts are intended to help you change your behavior so that you are not around areas of air pollution, or adding to it. She said there is a difference between ozone and particulate matter, both which can cause the need for an air quality alert. Ozone is typically a summertime pollutant, whereas particulate matter are small pieces of particles suspended in the air, which can be present throughout the year. Both can cause folks with heart and lung problems to suffer irritation of the lungs resulting in coughing and the need to use a rescue inhaler. Dr. Khatri said symptoms can sometimes take a day or two to show up. In the days following poor air quality alert, doctors typically see more people visit the emergency room for care. She said the best thing to do during an alert is to stay indoors during the afternoon hours, when levels are at their highest, and to especially avoid exercising outdoors. Even those who don’t experience breathing issues can take steps to help protect those around them who might. “During hot, humid days, not pumping gas into your vehicle, because the escaped fuel goes into the atmosphere and that can be translated to ozone,” said Dr. Khatri. “Also if you can ride-share and not be stuck in traffic, with all that traffic pollution, that also adds particles to the air.” Dr. Khatri reminds us that although people with heart and lung problems see the most effects, everyone can be affected by air quality. For those who have an established action plan with their doctor, it’s important to always be prepared and have a rescue inhaler ready at all times and to seek help as soon as symptoms appear.
