Schizophrenia in Women - The Role of Hormones and Estrogen Modulating Agents

Estrogen is a neuroprotective agent and the estrogen protection hypothesis is implicated in the development of schizophrenia in women. In women estrogen delays the onset of schizophrenia. In the peri-menopausal period, fading estrogen secretion in vulnerable women can increase the risk of schizophrenia. The ADEPT trial showed that addition of transdermal estrogen (100 or 200 mcg) to antipsychotic therapy improved symptoms of schizophrenia. Selective estrogen receptor modulators (SERM's) such as Raloxifene exerts similar neurotransmitter effects to estradiol within the brain, while acting primarily as an estrogen antagonist in breast tissue and the endometrium. In women with refractory schizophrenia, raloxifene is a promising, well-tolerated agent that has potential application in clinical practice. Thus, schizophrenia in women has a strong hormonal basis and should be taken into account in clincial practice.

Tamoxifen does not prevent Breast Cancer Tamoxifen is an antagonist of the estrogen receptor in breast tissue. Some breast cancer cells require estrogen to grow. Estrogen binds to and activates the estrogen receptor in these cells. Tamoxifen is metabolized into compounds that bind to the estrogen receptor but do not activate it. They only prevent estrogen from binding to its receptor Tumor evolves along the following stages: Stem cell transformation Estrogen dependence - Dysplasia - Carcinoma in situ - Carcinoma with receptors Estrogen independence - Carcinoma without receptors - Anaplastic carcinoma Estrogen is not a carcinogen. It does not transform a normal stem cell into a malignant. Estrogen promotes growth only of cells with estrogen receptors. Tamoxifen is metabolized into compounds that bind to the estrogen receptor but do not activate it. They only prevent estrogen from binding to its receptor. Tamoxifen is an anti-promoter, and not an anti-carcinogen! When they say "Prevention of Breast Cancer" they actually observe the slowing down of tumor growth. Modeling the estrogen receptor pathway and how it is perturbed by BPA for UNC ENVR630: Systems Biology We examined the estrogen receptor pathway and modeled it by creating and performing a puppet show, which was video taped. We looked at how bisphenol-A (BPA), a man-made chemical, perturbed the estrogen receptor pathway. Under basal condition, estrogen enters the cell and binds to estrogen receptors in the cytosol. The complex dimerizes then enters the nucleus and becomes a transcription factor, influencing gene expression. One such gene that it activates is c-Myc, a proto-oncogene involved in many cell processes, including cell cycle progression and apoptosis. When over-expressed or mutated, c-Myc can become an oncogene and signal tumor growth. BPA over-activates this pathway because, as an estrogen-mimic, it is recognized by and binds to the estrogen receptor. BPA then follows the same pathway discussed above. It can cause the gene c-Myc to be overexpressed, possibly enhancing cell proliferation. The increase in DNA replication and cellular divisions heightens the likelihood of multiple mutations, which can result in cancer.

Abstract: Ospemifene is a selective estrogen receptor modulator (SERM), or estrogen receptor agonist/antagonist, that was recently approved by the US Food and Drug Administration for the treatment of dyspareunia associated with vulvar and vaginal atrophy, a chronic condition that affects up to 60% of postmenopausal women. Ospemifene is the first and only nonestrogen compound approved for this indication. Compared with other approved SERMs, such as tamoxifen, toremifene, bazedoxifene, and raloxifene, the estrogen-like effects of ospemifene in the vaginal epithelium are unique. This review first discusses the rationale for developing ospemifene, including its mechanism of action, and then focuses on the clinical development of ospemifene for the treatment of dyspareunia associated with vulvar and vaginal atrophy. Included are discussions of the effects of ospemifene on the endometrium, serum lipids, coagulation markers, bone, and breast cancer. In conclusion, ospemifene is a SERM with a unique estrogen agonist/antagonist tissue profile that was recently approved in the US for the treatment of dyspareunia associated with vulvar and vaginal atrophy in postmenopausal women. Ospemifene warrants further clinical investigation for the treatment and prevention of osteoporosis and breast cancer. Estrogen signaling has been proposed in parathyroid tumor genesis. In a recent publication, we found estrogen receptors present in parathyroid tissue and tumors. In addition to detecting the receptor, we have utilized scanner and image analysis technology to measure nuclear staining intensity of Estrogen Receptor beta within immunohistochemical slides. These expression data was correlated to clinical features of the patients. Tamoxifen and raloxifene are effective at preventing breast cancer in women at high risk for the disease. But they don't work for everyone, and can have dangerous side effects. A new study led by physicians at Allegheny General Hospital found that a genetic test can predict which women will be helped by tamoxifen or raloxifene.
