After many trials and errors, this is how I figured out how to dispense baby medicine without the baby spitting it out. I have to make sure she gets her entire dose, so I am extra careful no to spill. Just go slow and steady and about half way back on the tongue. ACID REFLUX IN INFANTS AND CHILDREN Symptoms, Causes & Treatments OVERVIEW Infant reflux occurs when food backs up (refluxes) from a baby's stomach, causing the baby to spit up. Sometimes called gastroesophageal reflux (GER), the condition is rarely serious and becomes less common as a baby gets older. It's unusual for infant reflux to continue after age 18 months. Reflux occurs in healthy infants multiple times a day. As long as your baby is healthy, content and growing well, the reflux is not a cause for concern. Rarely, infant reflux can be a sign of a medical problem, such as an allergy, a blockage in the digestive system or gastroesophageal reflux disease (GERD). SYMPTOMS Infant reflux generally isn't a cause for concern. It's very unusual for the stomach contents to have enough acid to irritate the throat or esophagus and to cause signs and symptoms. • Isn't gaining weight • Consistently spits up forcefully, causing stomach contents to shoot out of his or her mouth (projectile vomiting) • Spits up green or yellow fluid • Spits up blood or a material that looks like coffee grounds • Refuses food • Has blood in his or her stool • Has difficulty breathing or a chronic cough • Begins spitting up at age 6 months or older • Is unusually irritable after eating CAUSES In infants, the ring of muscle between the esophagus and the stomach — the lower esophageal sphincter (LES) — is not yet fully mature. That allows stomach contents to flow backward. Eventually, the LES will open only when your baby swallows and will remain tightly closed at other times, keeping stomach contents where they belong. The factors that contribute to infant reflux are common in babies and often can't be avoided. These factors include: • Babies lying flat most of the time • An almost completely liquid diet • Babies being born prematurely TREATMENTS Infant reflux usually clears up by itself. In the meantime, your doctor might recommend: • Giving your baby smaller, more-frequent feedings. • Interrupting feedings to burp your baby. • Holding your baby upright for 20 to 30 minutes after feedings. • Eliminating dairy products, beef or eggs from your diet if you're breast-feeding, to test if your baby has an allergy. • Switching the type of formula you feed your baby. • Using a different size of nipple on baby bottles. A nipple that is too large or too small can cause your baby to swallow air. • Thickening formula or expressed breast milk slightly and in gradual increments with rice cereal. Although recognized as a reasonable strategy, thickening adds potentially unnecessary calories to your baby's diet. Medication Reflux medications aren't recommended for children with uncomplicated reflux. These medications can prevent absorption of calcium and iron, and increase the risk of certain intestinal and respiratory infections. However, a short-term trial of an acid-blocking medication — such as ranitidine for infants age 1 month to 1 year or omeprazole (Prilosec) for children age 1 year or older — might be recommended if your baby: • Has poor weight gain and more-conservative treatments haven't worked • Refuses to feed • Has evidence of an inflamed esophagus • Has chronic asthma and reflux Surgery Rarely, the lower esophageal sphincter is surgically tightened to prevent acid from flowing back into the esophagus. This procedure (fundoplication) is usually done only when reflux is severe enough to prevent growth or to interfere with your baby's breathing.

To better understand GERD, it's important to know what happens in the normal digestive process. As you chew your food and swallow, food particles travel from your mouth to the esophagus. The esophagus is a muscular tube that contracts and relaxes in a wave-like motion to help move food and liquids down toward your stomach. This is called "peristalsis." Just before the esophagus gets to the stomach, it travels through your diaphragm. The diaphragm is a broad muscle that separates your stomach from your chest cavity. The opening in the diaphragm where the esophagus travels through, is called the "hiatus." You'll hear more about the hiatus, and how it can be associated with GERD, a little later. The esophagus then joins the stomach at the "lower esophageal sphincter," also called the "LES." This sphincter acts as a doorway between the esophagus and the stomach. After you swallow, it opens to let food into the stomach. Then, the sphincter closes to keep food and stomach juices from going back up into the esophagus. The hiatus also helps close off the entryway to the stomach. Inside your stomach, strong acids and enzymes make up the stomach juices that break down your food. Your stomach has special mechanisms that help protect it from these strong juices. But your esophagus doesn't have these same defenses, so it's important for the LES to close off the stomach opening to keep the juices in the stomach. Sometimes, the acidic contents of the stomach do go back up, or reflux, into the esophagus, and some reflux is normal. Much of the time, this never causes any problems because the esophagus also has some ways to protect itself. For example, the saliva in your esophagus can help neutralize stomach acid, and gravity and peristalsis help to wash the saliva and stomach juices back down into the stomach. But other times, reflux can cause the burning, pressure, or pain in your chest or throat that most people call heartburn or acid indigestion.

You can see at 2:20min what her dry spells are like as she is having a small one. She had a little bit of coucous with sauce (which she had the day before and did fine) but she has the same dry spell after milk or pureed food. Im still not sure what it is. Me moved her prevacid solutabs from 7.5mg to 15mg and it helps, as she was having dry spell with pretty much every meal aside from her 2am milk that she takes while sleeping with no issues whatsoever. She does aspirates so we thicken all of her liquids to a honey consistency. She also has tracheomalacia (floppy airways) If anyone has a comment about what else it could be or has a similar stuation, please contact me! ____________________________________ UPDATE At 1.5 she learn to eat without coughing as much but her swallowing system really adapted to some of her issues at 2.5 yr old. At that point, we still thought it was EA/TEF related, basically low esophageal motility from the surgery and her aspirations. After much medical research I suspected a laryngeal cleft, I talked to several specialist about it, they said it was too rare to be that especially since she had never had an aspiration pneumonia, so not worth an endoscopy. At 4.5, she was due to do an GI endoscopy to check for Barett's (precancerous cells due to GERD), I requested for ENT to be there and check for a cleft. Sure enough, she had one. We decided to take the trip to Boston Hospital to see doctor Reza Rahbar who is an expert in this rare birth defect. She is now 6 and still having some aspiration and light coughing but she is eating by mouth and doing fairly well. I truly hope this can help some parents out there!
