In this film I talk about how to organise and plan for a Lithium blood test. Take your meds 12 hours before your appointment. If you don't then your readings will not be accurate. Going away for the weekend and on holidays- tips for making life easier when you are moving around. Keeping stocked up so you never run out of Lithium. I became manic after only 4 days of being out of the theraputic range. Coming off Lithium should be done in a controlled fashion with the help of medical specialists.

I was officially diagnosed with Bipolar 2 in 2013.In this video I had been increased from a dosage of 450mg of lithium (eskalith) ER to two doses of 300mg in the morning and at night (600mg per day). UPDATE 11/12/14 At the time of this video I weighed 93lbs at 5'2. I gained 9 pounds on Lithium. After being taken off Lithium I have dropped back down to 84lbs, my original weight. I have been told by my psychiatrist that this level of muscle spasm and ataxia will take least 6 months to subside before I may see any improvement. My personal opinion (not confirmed by my psychiatrist) is that I stayed far too long on this medication without responding to the warning signs such as diarrhea with

Quetiapine is a second-generation (atypical) antipsychotic. It’s approved by the FDA for schizophrenia, acute bipolar mania/mixed and depression, and bipolar maintenance (as an add-on to lithium or divalproex). The extended-release form is also approved as an add-on for depression. The term unipolar refers to the presence of one pole, or one extreme of mood- depressed mood. This may be compared with bipolar depression which has the two poles of depressed mood and mania (i.e., euphoria, heightened emotion and activity). Bipolar disorder (also known as manic depression) causes serious shifts in mood, energy, thinking, and behavior—from the highs of mania on one extreme, to the lows of depression on the other These are of 2 types • Bipolar I disorder involves periods of severe mood episodes from mania to depression. The highs may even require hospitalization of the person. • Bipolar II disorder is a milder form of mood elevation, involving milder episodes of hypomania that alternate with periods of severe depression The depression that people with bipolar disorder experience is generally of a melancholic or psychotic type and therefore more biological in its nature. The depression found in these bipolar patients can be difficult to treat than unipolar depression as the antidepressants can cause them to have greater mood swings. These patients need to be on mood-stabilizing drugs. The antidepressants alone can actually increase the manic episodes and worsen the disorder. Antidepressants: divided into three main classes: • Selective serotonin reuptake inhibitors (SSRIs) - were developed in the 1980's and are the most common prescribed today. They are sold under brand names such as Prozac, Paxil, Prozac, Luvox, Zoloft, Celexa • Monoamine oxidase inhibitors (MAOIs) There are three types of MAOIs, phenelzine,(Nardil) isocarboxazid and tranylcypromine, ( Parnate) and moclobemide.) • Tricyclic drugs (TCAs). (sold as Amitriptyline, Imipramine) For effective control and to limit the side effects a combination can be used such as Wellbutrin (or bupropion). Mechanisms of action are different hence lower doses of both help control side effects. The combination of Zoloft and wellbutruim earned the popular name Welloft. The MAO inhibitors are more effective than the SSRI drugs. But can cause severe weight gain and therefore patients need to work on diet control, else the spike in blood pressure can be dangerous. They are contraindicated with drugs that work on serotonin. Lamictal (or Lamotrigine) was FDA approved for treating depression in 2003.The main advantage being, no sexual dysfunction, no sexual side effects and no weight gain. There is no sedative effect and dosage is generally much less than used for seizures. If any of the above mentioned combinations are ineffective in controlling depression, the thyroid hormone replacement drug Cytopan or Cytomel or Synthroid can be added to any of the above drug. Atypical anti-psychotics such as low doses of Zyprexa, or Abilify, or Seroquel, and the latest Quetiapine can also be prescribed. 150 mg of Lithium, normally prescribe for Bipolar at much higher doses, can be used safely without having to get regular blood checks. A little over 20% of these are from heroin use and another 40% are from prescription painkillers, many of which are opiate-derived. Every year, as reported by the Centers fro Disease Control and Prevention (CDC), 47,055 deaths are reported as “drug-induced”. Hundreds of thousands more are treated by paramedics or are hospitalized every year from “non-fatal” drug overdoses, many of which result in permanent brain damage or partial paralysis. The cost of substance abuse is so steep, from death and extreme physical harm to heartbroken families and damaged relationships, not to mention the expense of medical care. Yet, addiction still has control over the lives of millions of Americans today. During an overdose, the body begins to shut down due to a lack of oxygen, which is caused by decreased respiration – this can be a result of the sedative effects of the drug or from a blocked airway. “As the oxygenation level drops in their body, their heart will start to slow down, and all their systems will start to shut down one by one,” explains Gina, a member of the nursing staff at Best Drug Rehabilitation. “You’re going to have brain damage, then the cardiac starts, and that’s when a person can die.” Overdoses don’t always result in death, but it is a very urgent matter of getting medical attention as soon as possible, and even then there might not be anything that can be done. “They can die with overdose, let’s not confuse that fact,” Gina continues. “But they also can be brought back. There are substances like Narcan, it can be reversed, but it is also time sensitive. Before coming to our recovery center, Ben had travelled a very hard road. He had become homeless due to his heroin addiction. His thought processes had become so warped by his addiction that it didn’t bother him that he had come so close to dying so many times, including wrecking a car into a tree at 120 mph. “They told me right there at the hospital ‘You know, since you’re homeless, this is the fastest recovery time that you can get.’” Ben recalls. “‘If you just get it amputated, you won’t have to deal with the infection or anything like that.’ And so that’s what I went with. Cause all I really cared about at the time was getting high. I didn’t even care that I lost my leg.” When I woke up, I had no clue what had happened - no recollection whatsoever,” admits Kathryn M., another patient at Best Drug Rehabilitation. “I thought I had gotten into a car accident or something. You never think it’s going to happen to you, but it happened to me.” She was lucky enough to be found after her overdose with enough time to save her life, but it was very close. “I vomited and it went into my lungs. My dad found me about five hours later. They immediately rushed me to the emergency room, when I got there, they put me on a drip of Narcan. As much as my meds help me, it’s a battle to remember to do all the things I need to do to be able to take them: sign up for health insurance, pay my bill, make a doctor appointment, GO to that appointment, get my prescription, find a pharmacy that can fill it, remember to pick it up before it closes, remember to bring my ID so they’ll let me... So when I miss a step and find myself without my meds, it’s easy to wonder...are they really necessary? This is what happened when I forgot to pay my health insurance, stopped taking my medication for two weeks and tried to learn how to live without it. Please share -- because the stigma is real, and no one should have to feel ashamed for getting the treatment they need just because their disorder is mental rather than physical, invisible rather than obvious. Your medication Divalproex Divalproex sodium is also known by the brand name: Depakote, Depakote ER and Depakote Sprinkle capsules. Divalproex sodium comes in 125, 250, and 500 mg tablets, 250 and 500mg extended release tablets, and 125mg sprinkle capsule. Divalproex sodium is most commonly used to treat seizures, bipolar problems, and prevent migraine headaches. No matter what your doctor has you taking it for, divalproex sodium is taken by mouth with or without food 1 to 4 times a day. If upset stomach occurs, take divalproex sodium with food. If you miss a dose, take the missed dose as soon as you think about it. If it is close to time for your next dose, skip the missed dose and resume your normal dosing schedule. Do not take 2 doses at the same time or extra doses. For best results take each dose at the same time each day. Extended-release tablets must be swallowed whole, do not split or break tablets. Divalproex Sprinkle capsules may be swallowed whole or the capsule may be opened and sprinkled on small amount (1 teaspoonful) of soft food like pudding, or applesauce. This is to be used immediately (do not store or chew). Some common side effects with divalproex sodium include headache, nausea, vomiting, diarrhea, tiredness, weakness, tremor, dizziness, blurry eye sight, and loss of hunger. Some less common side effects include swelling of the arms or legs, high or low blood pressure, weight gain, and constipation. Do not take divalproex sodium if: you are pregnant, have liver problems or under the age of 2 years. This medication should not be abruptly discontinued because of the possibility of increasing seizure frequency. While taking divalproex sodium, remember it is important to tell your doctor or pharmacist if: • You have signs of an allergic reaction (rash, itching, swelling, tightness of the chest, etc.) • You have dark urine, are feeling tired, are not hungry, have an upset stomach, are throwing up, or have yellowing of the skin or eyes. • Are pregnant as this drug may cause very bad birth defects. If you are or become pregnant and take this drug for seizures or bipolar disorder, talk to your doctor to see if you need to keep taking this drug. All forms of divalproex sodium including the liquid needs to be stored at room temperature in a dry place away from children or pets.

Do not take 2 doses at the same time or extra doses. For best results take each dose at the same time each day. Extended-release tablets must be swallowed whole, do not split or break tablets. Use dry hands to take the dissolvable tablet out of the foil and place on top of your tongue. Allow tablet to melt, then swallow. Some common side effects with lamotrigine include nausea, vomiting, dizziness, drowsiness, tiredness, weakness, and constipation. Some less common side effects include a very bad skin reaction, headache, abnormal thinking, swelling of the arms or legs, and weight gain or loss. Do not take lamotrigine if: you have an allergic reaction like a rash, water retention, hives, extensive itching, or mouth ulcers to lamotrigine or any component of the formulation. Please be careful. I'm no alarmist about drugs, but lithium can easily become poisonous to the body. It's not hard for your body to have a real negative reaction. Especially if you dose up too fast. It can dehydrate you to the point of delirum, too, so drink a lot of water at least. And I know they say it about all mood drugs, so i don't wanna sound annoying, but you TRULY don't wanna drink alcohol with this one. I've seen two people go to the ER who had been on lots of other meds, but they drank on lithium & it was different. One person got severely disoriented and dehydrated enough to end up in hospital two days. The other ended up with some kidney disease or short term failure because he took one extra pill (250mg each totalling 500mg, I think?) and drank that night, and going to gym in morn. The stuff is released faster when you drink and exercise, and the combo can catch up faster than you'd expect.
