Local Anesthetic Infiltration - everything you need to know

10:03
Lately, I have had many questions about the local infiltration of lidocaine... when to use epinephrine? what is buffering and...does it help? how much volume is toxic? what are the symptoms of toxicity? and other similar questions...that made me realize I could use a refresher on this information, so I went to study and make this video review on the subject. We will start from the very basic to very advanced concepts, I hope you enjoy it and learn a lot from it. INFILTRATION ANESTHESIA What is it? It involves the injection of anesthetic agent directly into the tissue prior to surgical manipulation. A field block involves a subcutaneous (SQ) injection of an agent in such a manner as to create a field of anesthesia around the operative site. Indications and Contraindications It can be used for the majority of minor surgical procedures such as excision of skin lesions, incision of abscesses, and suturing of wounds. An advantage of this over nerve block and general anesthesia is that it is considered both quicker and safer. Local infiltration can also help with hemostasis (bleeding control), both because it directly distends the tissue and because one can use epinephrine. A major disadvantage of local infiltration, at least when compared to nerve blocks, is that a relatively large dose of the drug is needed to anesthetize a relatively small area. For large wounds, the amount of anesthetic required may risk systemic toxicity. The maximum allowed volume can be increased by adding epinephrine, using a lower concentration of the anesthetic, or both. Another thing to consider is that local infiltration also distorts the tissue that will be incised or repaired and may to be desirable in areas requiring precise alignment (like a cosmetic repair in the face or lip). Choosing the anesthetic agent Some of the more common local anesthetics are lidocaine, bupivacaine, and procaine. Lidocaine is by far the most commonly used because of its excellent activity profile, low allergenicity and toxicity, user familiarity, and ready availability. PROCAINE - is useful for patients who are allergic to amide anesthetics. Because of its prolonged duration of action, bupivacaine is considered by some physicians to be the preferred anesthetic, especially when postoperative analgesia is desired, for prolonged procedures, or short procedures in the ER that may be interrupted. Lidocaine generally lasts for about 1 hour, while bupivacaine can last up to 6 hours. So...if bupivacaine lasts 6X longer, then why not use it always? Well, there is the potential for injury if the patient cannot feel that part of the body, also having a numb area is also very annoying and can cause patients to 'play around' with it, therefore damaging the tissue. In addition, the pain of administration of bupivacaine is more, and the chances for cardiotoxic and CNS side effects is more. How can you increase the anesthesia time of duration? You can add epinephrine and/or bicarbonate to the lidocaine. By adding epinephrine you also decrease the peak blood level absorption of the medication, therefore decreasing the chances of a toxic reaction, and allows a greater volume to be injected in larger lacerations. However adding epinephrine to lidocaine lowers its pH (makes it more acidic) therefore increasing the pain of local injection. How much volume can be injected safely? For Lidocaine - it is 300 mg in an adult. That is 60 mL of the 0.5% concentration. 30mL of the 1% concentration, and 15mL of the 2% concentration. For Bupivacaine 0.25% it is 175 mg which comes out to 70 mL. If you add epinephrine you can add 50% to the total volume of injection. How long does the different local anesthetics take to numb? They all have the same onset to action. 2 to 5 minutes. COMPLICATIONS of local anesthetics WOUND HEALING - local anesthetics produce cytotoxic effects on cell structure and function in a dose- and time-related manner. Collagen synthesis, for example, is inhibited. At higher concentrations of the anesthetic, the tensile strength of the wound is progressively reduced. Adding epinephrine can cause a further decrease in this tensile strength. Conclusion - if tensile strength is important, don't use epinephrine. If the patient has a poorly healing wound or history of the same, then avoid local anesthetic altogether. Wound Infection - it has long been known that local anesthetics possess antimicrobial activity, at least in vitro.

Drawing lidocaine

02:55
i was at work when i made this video, so there is no commentary...read the description for a general step by step of what you're seeing. my mom asked me if i was doing this safely...so i took my ipod to work and took a video of myself drawing two 1.5ml syringes of lidocaine. i work in a dermatology clinic and use this stuff every single day. lidocaine is a local anesthetic used to numb patients before procedures...in my case they are biopsies, both punch and shave. At first i am fumbling around getting out equipment that wasn't on the table when i started and what not, but the procedure is pretty straight forward. the red needle tip is called a blunt fill, it's significantly less sharp than a regular needle that is used only to draw up medications. The yellowish-looking tip that i screwed on after is the actual needle, an 18g (very small) needle about a half inch long used to actually inject the medication into the patient. the yellow plastic part covers the needle at all times when it is not being directed into the patient's skin. When i started the procedure you'll notice that i uncapped the lidocaine and then wiped it down with an alcohol swab...you don't have to do so with a brand new container but i make it a habit to do it always, and thus not forget to do it when you're re-using a vial of the drug. When drawing, it's best to inject the vial of medication with air, equal to the amount of fluid you would like to withdraw first, which is what you didn't see me doing when i was taking my time on the first syringe. the second one i did not do it...it's really not entirely necessary, just helps avoid air bubbles. When you actually draw the drug out of the vial, pull out an extra half a ml or so, and then inject it back into the vial. this takes out any air bubbles that may be in the syringe. if there is an air bubble stuck on the plunger, flick it a few times as i did on the first one before injecting the extra medication back into the vial. After that you're ready to go...throw out all sharps in a sharps bin, don't uncap a needle unless you're ready to use it...and for god's sake put on gloves and swab the patient's injection site with alcohol before you administer anything with a syringe. Hope this helps...eh...someone? leave a comment if you have any questions :)

To celebrate, we thought we would put together a little red, white, and blue for YOU! As many of you know by now, cysts come in all shapes, sizes, and colors! I thought on this holiday weekend, I would explain to you why the following cysts are Red, White, & Blue! Red: This is a cyst excision on the left cheek, and I think it's pretty obvious why there is a little red here. After all, this is a surgical excision! White: Local anesthetic used contains lidocaine, sodium bicarbonate, and epinephrine. The NaHCO3 (sodium bicarb) is used to buffer the lidocaine, raise the pH so that it burns less when it is injected. Epinephrine is added because this constricts local blood vessels which helps to minimize bleeding, and also increases the time of anesthesia, keeping lidocaine in the area for longer. Cysts are most commonly white in color, containing macerated keratin (wet skin cells). Blue: This cyst I was pretty pleased to remove intact. In general, this is what I try to do.. it keeps the area cleaner and neater, and also assures me more that we have removed the entire cyst and it's wall completely. This cyst had a white fibrous membrane around it and the cyst was more grey in color, so it had a slight bluish hue as it was removed. Happy Red White & Blue Day! Have a great holiday weekend! Extracting Lidocaine from Anal Lubricant

10:53
Apparently I can't monetize this video because Youtube doesn't like the word "lidocaine." So, if you liked it, please donate because I had to buy this lube twice at the good Canadian price of $40 each time. Yes, I spend $80 on anal lube... (not including the $20-$30 i spent on trying to extract from other products). Oh my god. That moment when you realize your video cost yoy $100+ (CAD, aka monopoly money). In this lovely video today, we will be extracting lidocaine, a local anesthetic, from anal lubricant! I wonder if anal lube sales will go up after this... She has had this lipoma removed in the past but it grew back. As you can see it's pretty big in size. Perhaps the largest lipoma I have ever removed. If an area of the skin this size needs to be locally anesthetized it's common to use the "tumescent technique" which is the same way I numb people when I perform body liposuction. Essentially, this is when we dilute the lidocaine, in saline (with epinephrine and bicarb as usual), so we can spread it out to a wider area. Lidocaine in large volumes in our body is actually life threatening, so we always monitor how much lidocaine we are administering if we know we need to anesthetize a large area. I do believe there was "normal" fat above this lipoma that I had to remove to get to this actual lipoma, so there may be a slight depression in the area when it fully heals, but my fingers are crossed because it looks pretty good so far! I DON'T use liposuction to remove lipomas because this "breaks up" the lipoma and I do believe there is high chance that a lipoma reoccurs this way. I'll post her follow up pictures in an update video in the near future! Thank you for watching!! A lipoma is slow-growing, benign growth of fat cells. It is contained in a thin, fibrous capsule and found right under the skin. A lipoma is typically not tender and moves around easily with slight pressure. A lipoma is not cancerous and treatment generally is not necessary. There is also a condition called familial lipomatosus, where people develop multiple lipomas, especially on the arms and legs, and other family members have these growths as well. If the lipoma is on a pressure-bearing area, it may create discomfort and this is when people seek removal. People also request removal because they don’t like the appearance of these bumps. Often a small incision can be made over the lipoma and they can be “popped” out easily. This is a simple in-office surgical procedure under local anesthesia

For more information on Organic Sulfur go to www.happybodystore.com MSM is a great mineral supplement used to reduce pain and inflammation. You can increase the healing benefits of this mineral by adding it to some aloe gel and applying it to the skin as a topical pain reliever. Aloe gel is a fantastic base to use, as it is water based, allowing the water soluble Organic Sulfur to be dissolved in with no effort at all. Aloe also naturally contains sulfur, increasing the efficacy. Aloe gel can be purchased at your local health food store, or can be found at various supplies on Amazon, and elsewhere online. Ensure it is good quality aloe gel, as everything we absorb into our skin is directly absorbed into our bloodstream. I sourced my aloe jelly here: http://www.essentialwholesale.com/product/1698/aloe-jelly The essential Oils I used in this gel were from Young Living. Always ensure the highest quality therapeutic oils are used. Lavender: Excellent for skin repair and provides a calming scent Copaiba: Powerful anti inflammatory, very mild scent Peppermint: Provides a cooling sensation and helps drive the MSM and other oils deeper into the tissues Remember that a Health Body is a Happy Body!

Migraine Cure Nerve Block | Auburn Medical Group

16:22
Most migraine attacks can be resolved with injection of local anesthetics to accomplish nerve blocks. Here we inject the Greater Occipital Nerve, the Lesser Occipital Nerve, and the rare Dorsal Occipital Nerve. Other nerves sometimes blocked for migraine relief include the Supraorbital Nerve, the Supratrochlear Nerve, and the Auriculotemporal Nerve. These injections differ from the use of triptan medications because they can be given at any time during the migraine to provide relief. For this injection, each site was administered 2 ml 1% Lidocaine with Epinephrine with 0.2 ml of sodium bicarbonate. Only 1.5 ml was used on each of the left Greater and Lesser Occipital Nerves.
