There’s car insurance, home insurance, life insurance, even pet insurance . . . Most of these insurance policies work well and are fairly priced. But there is one glaring exception: health insurance. Only health insurance becomes more complicated and more expensive at the same time. So, the obvious question is: why? To answer this question, we have to start at the beginning. What is insurance? It’s pretty straight-forward: You pay a monthly fee which provides financial protection against unforeseen, sometimes catastrophic, events. People buy homeowners insurance, for example, to protect themselves from the financial loss incurred in the event of a fire, a flood or theft. Because millions of people are paying into the insurance pool, the pool has enough money to cover the unlucky person whose house does burn down. And since insurance is meant to share risk, it only stands to reason that higher-risk individuals have to pay more to be insured. Someone who has had two accidents is going to pay more for car insurance than someone who has never had an accident. Why? Because their track record indicates they are more likely to have another accident. But while insurance provides a bulwark against unforeseen loss, it does not protect against routine expenses. Car insurance protects you in the event that you wind up in a car wreck or your vehicle is stolen, but it doesn’t cover routine maintenance like oil changes, replacing brake pads or tire erosion. Why? Because everyone needs routine oil changes, new brake pads, and new tires. So, there is no risk to protect against. Health insurance in America works very differently. Many of us have health insurance plans that aren’t insurance at all. They’re really pre-paid health care plans. They cover routine check-ups, less serious illnesses, and recurring expenses like prescription medications in addition to protecting you from a health disaster. All of this has made healthcare much more expensive and complex than any other form of insurance. That is true whether you get your insurance through your employer, through the government, or if you pay for your own plan. The Affordable Care Act, known as Obamacare, was passed on the promise that it would fix these issues and bring down healthcare costs. But it has actually made the problem much worse. First, it limited the variety of health insurance plans private companies could offer. It did this by mandating that every plan had to cover the same set of ten health benefits, including preventive care, maternity care, mental health care, and contraception. Second, Obamacare prevented insurers from charging premiums based on the risk they were assuming. A person with a much higher risk of getting sick couldn’t be charged more than a person with a much lower chance. These two aspects of Obamacare – requiring all policies to have certain coverages and not allowing insurance companies to charge more for riskier clients – caused the price of insurance to rise dramatically. Employers provide a variety of health care and medical benefits, usually through insurance coverage. Employees often consider health plans to be one of the most important benefits that companies offer. For several decades, the costs associated with health care coverage have increased considerably in the United States. This raises a number of concerns for both employers and employees because they have to pay for this coverage. Landmark legislation enacted in 2010 changed health care in the United States, making insurance available to an additional 32 million people. Provisions of the Patient Protection and Affordable Care Act (PPACA) were phased in over several years, culminating in universal coverage in 2014. The PPACA includes many important provisions intended to provide affordable health care for all citizens. To achieve this goal, enrollment in health coverage is now mandated for every citizen. Employers offering health care benefits are taking a number of approaches to control and reduce their costs. The most frequently used strategies include the following - Increasing deductibles and copayments, Instituting high-deductible plans, Increasing employee contribution, Limiting family coverage, Switching to consumer-driven health plans and Increasing health preventive and wellness efforts. Some companies have started to offer a variety of innovative health care programs that provide better services to employees. Some insurance companies do, in fact, include fertility treatment and fertility medications and the full diagnostic evaluation in their coverage plan. Some do not. And sometimes, it has nothing to do with the insurance company, but specifically, what the person's employer has arranged with the insurance company. It is very important that you get this information before you see a fertility specialist. There are specialists who take no insurance or take only a limited number of plans. There are also specialists who take multiple plans. The quality of these specialists is generally the same. Some insurance plans will not cover fertility for women who have undergone a tubal ligation and want to have that tubal ligation reversed or have the in vitro fertilization. Some insurance plans have certain limitations, specifically age limitations, on the types of treatments that are available. Other insurance plans will give a certain lifetime dollar amount and don't ask specifically what type of treatments are being done but will not cover above a certain dollar amount. It's very important to speak with your Human Resources department and with the fertility center and particularly with the insurance company to determine what kind of coverage you have. One of the most important things to cover are fertility medications. Fertility medications are inordinately expensive. They can range from $75 a vial to even higher, at times. Some women require five or six thousand dollars worth of medication in one cycle of in vitro fertilization in order to stimulate the ovaries. Those are the women who require a lot of vials due to age factors. Young women require less medications in general, so it could be a lot easier to handle financially in these cases. Again, you must speak with your insurance provider about the coverage that you have for fertility medications. Many centers also have fertility medications that they can give you free of charge. They get these medications from patients who have conceived successfully and have leftover medications that are in closed vials that could be given to patients. In cases in which there is no insurance coverage for in vitro fertilization, it is important to speak to your center about the cost involved for the whole procedure. In general, in vitro fertilization costs vary by geographic location. In some centers, also, special rates or special packages are given to try to control the costs of fertility care. In general, in vitro fertilization will typically cost anywhere from six to twelve thousand dollars depending on the case and procedures that are required. Specifically, in some cases, sperm needs to be injected microscopically into the eggs, and that requires a certain fee. In other cases, embryos are manipulated, and that engenders a certain fee. Again, all the fees have to be determined ahead of time, and fertility centers should give you all the fees in writing prior to the start of the procedure.

An HSA- is a health savings account. An HSA is a special kind of tax-advantaged account that’s designed to help you pay for health care expenses. Anyone can open an HSA, as long as they: • Have health coverage through a qualified high-deductible health plan. If you don’t have this type of health plan, you CANNOT open an HSA. • Are NOT covered by any other health plan • Are NOT enrolled in Medicare, and • CANNOT be claimed as a dependent on someone else’s tax return. As previously mentioned, you can withdraw money from your HSA without paying taxes or a penalty as long as those dollars are used to pay for qualified medical expenses. Qualified medical expenses include things that are associated with your health plan, such as: • Deductibles and coinsurance, • Vision care, • Dental expenses, • Over-the-counter supplies, and • Some insurance premiums. The account is owned by you, so no matter where you work, the money that’s in the account, always belongs to you—even if it was contributed by your employer. You can use the money in the account to pay for your current medical expenses, or save it for future medical expenses – including those in retirement. There are many special advantages to opening an HSA. You’ve probably heard of the Flexible Spending Account…well, unlike the Flexible Spending Account there’s no “use it or lose it” with an HSA. Any dollars left in your account at the end of the year roll over to the next year. That means you decide whether to spend or save your HSA dollars, which makes it easier to save for the future. Another advantage of the HSA is what we call triple tax savings. Here’s how that works: • Not only does the money that’s deposited into the account reduce your taxable income, • But, the balance in your account earns interest tax free, and • Any withdrawals from the account are tax free as long as the moneys is used for a qualified medical expense. That gives you a triple tax savings. And finally, one of the greatest advantages of the HSA is that it’s portable. Unlike other medical spending accounts that are owned by your employer, the HSA belongs to you, even if you change jobs or retire. Using your HSA is easy. Let’s start by talking about contributions. Both you and your employer can contribute money to your HSA. Remember, employer contributions are not taxable to you as employee, and once the money is deposited into your account, it belongs to you. The best way to contribute to your account is through automatic payroll deductions, if your employer allows this. Not only is this convenient, but payroll deductions help you make consistent contributions, and you get the most tax savings this way. Plus, you have the flexibility to change your payroll deduction at any time throughout the year. You have a lot of flexibility when contributing to your HSA. You can put money into your account at any time during the year, and you can even contribute up until the tax filing deadline, which gives you more time to fully fund your account. If you fund your account on a post-tax basis, you’ll realize the tax savings when you file your taxes for the year. This type of contribution becomes an “above-the-line” deduction. Let’s talk about how much you can contribute to your HSA. Every year, the IRS sets a maximum amount that you can contribute to your HSA. This maximum is based on your health plan contract type – single or family. • If you have single coverage, you can contribute a maximum of $3,450 in 2018. • If you have family coverage, you can contribute a maximum of $6,900 in 2018. These maximums include contributions made by you and your employer. Together, your contributions cannot exceed these amounts. Every year the amounts change. Of course, there are exceptions to every rule, and with HSA contributions the exception is that account holders who are 55 or older at any time during that year. These individuals can contribute an additional $1,000 every year to an HSA. This is called a catch-up contribution. There are just a few things to keep in mind when using your HSA. • Withdrawals from your account are tax free as long as the money is used for a qualified expense. • Expenses must be incurred on, or after, the date that your HSA

When I consider purchasing an individual health insurance plan for myself or my family, do I have any financial obligations beyond the monthly premium and annual deductible? Answers: It depends on the plan, but some plans have the following cost-sharing elements that you should be aware of. Co-Payments: Some plans include a co-payment, which is typically a specific flat fee you pay for each medical service, such as $30 for an office visit. After the co-payment is made, the insurance company typically pays the remainder of the covered medical charges. Deductibles: Some plans include a deductible, which typically refers to the amount of money you must pay each year before your health insurance plan starts to pay for covered medical expenses. Coinsurance: Some plans include coinsurance. Coinsurance is a cost sharing requirement that makes you responsible for paying a certain percentage of any costs. The insurance company pays the remaining percentage of the covered medical expenses after your insurance deductible is met. Out-of-pocket limit: Some plans include an out-of-pocket limit. Typically, the out-of-pocket limit is the maximum amount you will pay out of your own pocket for covered medical expenses in a given year. The out-of-pocket limit typically includes deductibles and coinsurance. But, out-of-pocket limits don't typically apply to co-payments. Lifetime maximum: Most plans include a lifetime maximum. Typically the lifetime maximum is the amount your insurance plan will pay for covered medical expenses in the course of your lifetime. Exclusions & Limitations: Most health insurance carriers disclose exclusions & limitations of their plans. It is always a good idea to know what benefits are limited and which services are excluded on your plan. You will be obligated to pay for 100% of services that are excluded on your policy. Beginning September 23, 2010, the Patient Protection and Affordable Care Act (health care reform) begins to phase out annual dollar limits. Starting on September 23, 2012, annual limits on health insurance plans must be at least $2 million. By 2014 no new health insurance plan will be permitted to have an annual dollar limit on most covered benefits. Some health insurance plans purchased before March 23, 2010 have what is called "grandfathered status." Health Insurance Plans with Grandfathered status are exempt from several changes required by health care reform including this phase out of annual limits on health coverage. If you purchased your health insurance policy after March 23, 2010 and you're due for a routine preventive care screening like a mammogram or colonoscopy, you may be able to receive that preventive care screening without making a co-payment. You can talk to your insurer or your licensed eHealthInsurance agent if you need help determining whether or not you qualify for a screening without a co-payment. There are five important changes that occurred with individual and family health insurance policies on September 23, 2010. Those changes are: 1. Added protection from rate increases: Insurance companies will need to publically disclose any rate increases and provide justification before raising your monthly premiums. 2. Added protection from having insurance canceled: An insurance company cannot cancel your policy except in cases of intentional misrepresentations or fraud. 3. Coverage for preventive care: Certain recommended preventive services, immunizations, and screenings will be covered with no cost sharing requirement. 4. No lifetime maximums on health coverage: No lifetime limits on the dollar value of those health benefits deemed to be essential by the Department of Health and Human Services. 5. No pre-existing condition exclusions for children: If you have children under the age of 19 with pre-existing medical conditions, their application for health insurance cannot be declined due to a pre-existing medical condition. In some states a child may need to wait for the state's open-enrollment period before their application will be approved.

Because there is no right or wrong answer for this question, it is important to appear friendly. 2. "What are your strengths?" This is a popular interview question. They want to know what you think of yourself. Although this is a general question, there is a wrong and right answer. The wrong answer is a generic answer saying you are organized and friendly. Although it will not hurt you during the interview, it will certainly not help you either. Answer this question based on the type of job you are applying for. 3. "What are your weaknesses?" For this answer, you should display a weakness that can be seen as a strength. There are many types of answers that will work. Some answers will be good answers for certain jobs, while the same answer will be a bad answer for a different job. Select an answer that will work for the position you are applying for. 4. "Where do you see yourself in five years?" This question primarily depends on where you are in your career. A person with 5 years of experience will have different goals than a person with no work experience.This question is asked to see how serious a candidate is about his or her career. Some people might not know their goals, and some people might have goals of becoming rich and retiring early. Those are incorrect answers for this question. The type of answer you want to give is an ambitious answer that shows you really love your career. A good interviewer will read between the lines and find out if a person is going to be a hard worker or just a mediocre one. Being descriptive and shooting for a big goal is something interviewers want to hear. 5. "What do you know about our company?" A typical job interview question, asked to find out how much company research you have conducted, is "What do you know about this company?" Prepare in advance, and in a word, research, so you can provide relevant and current information about your prospective employer to the interviewer. 6. "Are you good at dealing with change?" Dealing with change is common in the work place. A simple yes will not be sufficient to impress the interviewer. This is another type of question where everyone will have similar answers. Of course everyone is going to claim being excellent dealing with change. You got to communicate that you are really good at dealing with change. 7. "Do you work well under pressure?" In most cases, the best answer to this question is answering yes. Working well under pressure is a good trait to have. However, I think if you answer that you work the same with pressure and without pressure, the interviewer will be more impressed. However, you will need to explain in words why this is better. 8. "How do you make important decisions?" There are many ways to answer this question, and if you have a reasonable method of making decisions, it will probably be sufficient. One answer I thought of included not being afraid of asking your manager. You can follow up by saying even the best needs mentoring, and you always want to improve. So basically, this could work as an answer, but depending on the job, you might have a better shot with an answer like my example.
