A new dietary supplement for the regrowth of hair in women - Viviscal - has been gaining in popularity and use. In this episode of DermTV, Dr. Schultz discusses the new Viviscal hair regrowth study. Since efficacy is marginal at best, many women seek alternative treatments to help regrow hair. Today I’ll tell you about “Viviscal”, a new dietary supplement containing a proprietary blend that includes marine extracts and has been shown to have encouraging results. Over time, diffuse hair loss in women can progress to cosmetically obvious thinning. While finasteride – also known as Propecia – has been shown to be very effective in regrowing hair in men, its use in post menopausal women has not been effective and it shouldn’t be used before menopause. On the other hand, Viviscal, a dietary supplement containing a proprietary combination of marine extracts was shown to be moderately helpful in a placebo, controlled study involving 15 women with self-perceived hair thinning. They took the pill for 180 days and the number of hairs in a designated area of the scalp were counted before and after the study. The mean number of mature hairs in the target region increased from 271 at the beginning of the study to 571 at 90 days and 610 at 180 days. These results were significantly higher than the placebo treated group. For the stat geeks watching, the p value was less than 0.001… a very compelling result. Additionally, in response to a self-assessment questionnaire, more subjects in the treated group reported increased hair volume than in the placebo group. And there were no side effects in the treated group. The bottom line is that this study supports the efficacy and tolerability of the marine extract supplement for women. In my opinion, there’s very little known downside at this time and the upside is promising. Viviscal is available on line and in some doctor’s offices. A decrease in these hormones also triggers an increase in the production of androgens, or a group of male hormones. Androgens shrink hair follicles, resulting in hair loss on the head. It occurs after 18 years of age mostly. In severe cases, it may be noticeable as early as 12 years. However, most of the patients who approach us for treatment are in the age group of 25-40 years. There is a second peak incidence at menopause, between 50 and 60 years of age What is the cause? The reason for this hair loss is thought to be due to multiple factors. It may be influenced by genetics, hormonal imbalance or even environmental factors. The age of onset, how it will progress, the severity that is whether it will be restricted to one area or involve the entire scalp and the pattern of hair loss, are all determined by genetics. multiple marriages, lack of photo protection, higher income and little physical activity. However, more studies are required to prove an association with these factors. Hormones have also been implicated in the causation. Though it has not been unequivocally proven. Especially androgens. It is thought that testosterones and DHT play an important role in hair loss. Also, the lack of estrogen with age is responsible. The levels of estrogen drop after menopause which is why we see this condition more after menopause. Women with FPHL usually first notice a gradual thinning of their hair, mostly on the top of their heads, and their scalp becomes more visible. Over time, the hair on the sides may also become thinner. The patient may notice that her “ponytail” is much smaller or that her longer hair looks “skimpy” at the ends. The thinning of the hair can vary in extent but it is extremely rare for a woman to become bare on top. lower systemic estrogen levels have been implicated in the increase of FPHL after menopause How does it appear? Female pattern hair loss (FPHL) has also been called androgenetic alopecia, male pattern baldness in women, and diffuse alopecia. FPHL is a common condition, affecting up to 50% of women. It presents as hair thinning, mainly on the crown, but sometimes also affecting the parietal, posterior vertex and bitemporal areas. There is relative preservation of hair density on the occipital scalp. The frontal hairline is usually preserved. There is often accentuation of the thinning in the midline, just posterior to the frontal hairline (Christmas tree pattern). Rarely, there is deep bitemporal notching and frontal recession similar to male balding. This pattern of thinning in a woman suggests possible hyperandrogenism. FPHL can occur anytime after puberty, however there are two peaks of onset, in a woman’s 20′s and her mid to late 40′s. FPHL can also occur in association with hormonal changes such as post-partum and perimenopausally. In my experience, the most common time of onset is mid to late 40′s, often in association with other signs of impending menopause. A patient with FPHL often complains of being able to see her scalp, experiencing sunburn on the scalp for the first time or a decreased hair density, particularly affecting the crown. Most patients perceive that their hair loss is occurring in a pattern with sparing of the occipital region although some patients feel that the hair loss is diffuse.

A lot of patients come and ask me, is there some natural remedy for preventing or reversing hair loss. But unfortunately hair loss is a not very reversible problem. Some people do have very acute hair loss. All this acute telogen effluvium, which happens after an incident, maybe a viral illness like dengue or a typhoid illness and then even a pregnancy. Post that event, about 3 – 6 months later, there is a lot of shedding of hair. We call this acute telogen effluvium. In this treatment always the hair grows back adequately, but when an event has happened quite some time in the past like 3 years ago, and then there is no regrowth of the hair, then we call this chronic telogen effluvium, and in this chronic telogen effluvium, the concern is that there are repeated minute insults which keep happening to the hair, either in the form of crash dieting or thyroid problem or micronutrient deficiencies like iron deficiency, vitamin B12 deficiency, Vitamin D3 deficiency and all this chronically lead to the hair loss.
