Every four minutes someone dies from a stroke, but 80 percent of strokes are preventable. Stroke is the fifth leading cause of death and the leading cause of disability in adults in the United States. During a stroke, the blood supply to a part of the brain is disrupted, and the cells are unable to receive oxygen. Without an immediate restoration of blood supply, brain cells are permanently damaged, resulting in significant physical or mental disability. Recognizing symptoms and risk factors are essential for prompt medical intervention when a stroke occurs. Warning signs of stroke 1 Sudden numbness or weakness in the arms face, or legs If a person complains of sudden numbness or is experiencing weakness in their arms, legs or face, do not ignore it. Perform a quick test by stretching the person's arms (palms up) for 10 seconds. If an arm goes down, it can be a sign of stroke. Look for weak muscles or facial limbs The person may be unable to hold objects or suddenly lose balance while standing. Look for signs that only one side of the person's face or body has weakened. One side of the person's mouth may fall into a smile or may not be able to hold both arms above his or her head. 3 Look for confusion or trouble speaking or understanding speech When certain areas of the brain are affected, the individual may have difficulty speaking or understanding what is being said. Your loved one may seem confused by what you are saying, respond in a way that indicates you do not understand what was said, slur your words, or speak in confused noises that do not look like speech. This can be very frightening. Do your best to calm it down after you call your local emergency number for emergency medical treatment. 4 Ask if the person has vision problems in one or both eyes During a stroke, the vision may suddenly be affected. People say they lose sight in one or both eyes or see double. As the person can not see or is seeing double (if you are having trouble talking, ask him to nod with yes or no if possible). Symptoms of a stroke 5 Watch for the loss of coordination or balance When the person loses strength in their arms or legs, they may notice that the person has difficulty with balance and coordination. He may not be able to pick up a pen or can not coordinate the ride because a leg can not function properly. 6 Sudden and severe headache A stroke is also called a "stroke" and can result in a sudden headache that is described as the worst headache someone has experienced. A headache may be associated with nausea and vomiting because of increased pressure in the brain. 7 dizziness Dizziness is the most common symptom of a stroke. People may misinterpret this symptom by thinking that the person is intoxicated, but he or she may have a stroke. 8 Shortness of breath A person suffering from a stroke will also experience shortness of breath and difficulty breathing. This usually happens when the part of the brain responsible for the respiratory function is affected due to a reduced blood flow.

A stroke occurs when the blood supply to a certain part of the brain is reduced or interrupted. Without oxygen and nutrients from the blood, brain cells cannot function properly and eventually die. There are 2 major types of strokes: ISCHEMIC stroke caused by a BLOCKED artery, and HEMORRHAGIC stroke caused by a RUPTURED artery. Ischemic stroke happens when a blood clot OBSTRUCTS an artery. In some patients, the clot forms locally, inside the blood vessels that supply the brain. This occurs when fatty deposits in an artery, or cholesterol plaques, rupture and trigger blood clotting. In other cases, a clot may travel to the brain from elsewhere in the body. Most commonly, this happens in patients with atrial fibrillation, a heart condition in which the heart does not pump properly, blood stagnates in its chambers and this facilitates blood clotting. The clots may then pass into the bloodstream, get stuck in smaller arteries of the brain and block them. Hemorrhagic stroke, on the other hand, occurs when an artery leaks or ruptures. This can result from high blood pressure, overuse of blood-thinners/anticoagulant drugs, or abnormal formations of blood vessels such as aneurysms and AVMs. As a hemorrhage takes place, brain tissues located BEYOND the site of bleeding are deprived of blood supply. Bleeding also induces contraction of blood vessels, narrowing them and thus further limiting blood flow. Stroke symptoms may include one or more of the following: - Paralysis of muscles of the face, arms or legs: inability to smile, raise an arm, or difficulty walking. - Slurred speech or inability to understand simple speech. - Sudden and severe headache, vomiting, dizziness or reduced consciousness. Cerebral stroke is a medical emergency and requires immediate attention. It is essential to determine if a stroke is ischemic or hemorrhagic before attempting treatment. This is because certain drugs used for treatment of ischemic strokes, such as blood thinners, may CRITICALLY aggravate bleeding in hemorrhagic strokes. - For ischemic strokes, emergency treatment aims to restore blood flow by removing blood clots. Medication, such as aspirin and tissue plasminogen activator, TPA, are usually the first options. TPA may be given intravenously, or, in the case the symptoms have advanced, delivered directly to the brain via a catheter inserted through an artery at the groin. Blood clots may also be removed mechanically by a special device delivered through a catheter. - Emergency treatment for hemorrhagic strokes, on the other hand, aims to stop bleeding, reduce blood pressure, and prevent vasospasm and seizures. These goals are usually achieved by a variety of drugs. If the bleeding is significant, surgery may be required to drain the blood and reduce intracranial pressure. Preventive treatments for strokes include: - Removal of cholesterol plaques in carotid arteries that supply the brain - Widening of narrowed carotid arteries with a balloon, and sometimes, a stent. This is usually done with a catheter inserted at the groin. - Various procedures to prevent rupturing of brain aneurysms, such as clipping and embolization. - Removal or embolization of vascular malformations - Bypassing the problematic artery

Dr. Ebraheim’s educational animated video describes spinal cord injury and possible death from a broken neck. The bones in the neck are called cervical vertebrae. There are seven cervical vertebrae. These vertebrae are the bony structures that protect the spinal cord. The nerve roots arise from the spinal cord and give branches that supplies sensation, motor function and movement to a specific muscle or a specific group of muscles. A broken neck does not always mean death will occur, however injury to the spinal cord may cause death. The actor Christopher Reeve suffered a broken neck and he became a quadriplegic. He survived his injury because the problem was recognized and treated quickly and appropriately. There are three types of injury that may occur with a broken neck: 1-Broken vertebra, spinal cord is OK. The vertebral fracture is treated with or without surgery. Dislocations are usually treated with surgery. The patient has to be handled carefully to avoid any further spinal injury. 2-Broken vertebra- spinal cord injury is incomplete (partial injury). There are four syndromes that can occur: a-Central cord syndrome: it is caused by hyperextension injury. It has a favorable prognosis but poor recovery of the hand function. The lesion occurs in the central part of the spinal cord and the grey matter. The injury can be caused by minimal trauma in the elderly, usually caused by osteophytes. Motor weakness is more severe in the upper extremities compared to the lower extremities. b-Anterior cord syndrome: there will be complete motor paralysis and impairment of the sensory functions. Anterior cord syndrome has the worst prognosis. c-Brown-Sequard syndrome has the best prognosis with 90% recovery. It is caused by hemisection of the spinal cord usually due to penetrating trauma. There will be ipsilateral deficit of the motor function, proprioception, vibration, and deep touch. There will be contralateral loss of pain and temperature, because the spinothalamic tract crosses at the spinal cord. This syndrome is very rare and is associated with loss of proprioception, deep touch and vibration. 3-Broken vertebra- complete spinal cord injury: if the patient has no movement below the level of the injury immediately after a suspected neck injury, it does not mean that the patient’s lesion is complete (totally paralyzed). The physician may have to wait 24-72 hours until the bulbocavernosus reflex returns which means the end of the spinal shock. Then the neurological status of the patient will be precisely assessed. If the spinal cord is severely damaged, quadriplegia will occur. There will be no movement or sensation below the lesion after the return of the bulbocavernosus reflex, however if the injury is at or above the level of C4, then the diaphragm will be affected and breathing will be affected. If injury to the cervical spine occurs below C4, paralysis alone may occur and breathing may not be badly affected. How a person dies from a neck injury that severs the spinal cord? 1-The patient may also die from other associated injuries such as head injury, liver, spleen, lung, or pelvis. Checking the ABC’s (airway, breathing and circulation) is important to identify other significant injuries. Increased emergency medical training and first aid is important. 2-Diaphragmatic paralysis at or above the level of C4 can cause asphyxiation. Patient will probably require urgent intubation. 3-Look for other injuries! The cervical spine is known to have more fractures in different levels. Transport the patient carefully no matter if you are in the field or at the scene. The treatment begins at the scene with proper spine immobilization. Immobilization is usually done with a cervical collar. 4-Neurogenic shock: the nerve supply to the heart and blood vessels will be affected. Bradycardia (slow heart beat), hypotension (drop in blood pressure) will occur. Circulatory collapse due to loss of the sympathetic tone. 5-Manipulation of the neck may injure the vertebral arteries with acute brain infarction and stroke. 6-Autonomic dysreflexia : potentially fatal and presents with headache, agitation, hypertension. The spine is made flexible by discs located between each vertebra and ligaments made of tough elastic fibers which hold the vertebrae together. The spine gives the body stability and protects the spinal cord which is located in a narrow canal that runs through the center of each vertebra. Together the brain and spinal cord make up the central nervous system. The function of the spinal cord is to relay messages from the brain to the body and from the body back to the brain. All movements of the body and limbs and all sensation are relayed through the spinal cord and the spinal cord also plays a part in the transmission of messages from the autonomic nervous system which regulates many body functions such as heartbeat, blood pressure and temperature regualation. Damage to the spinal cord can disturb any or all of these functions. We advise that you should not punch to your attacker’s jaw in a street fight or self defence situation. You may knock them out with one punch, but you are also likely to cripple your hands or worst if you punch the person in the mouth. This week we look at how to fight with knockout strikes that are less risky and more flexible for self defence. Most fighters will tell to punch as hard as you can at someone’s jaw, but what happens if the person drops their head when they punch? Most people tend to drop their heads in instinct or simple self defence survival when they throw a punch in a fight. This leads to a greater risk of hitting the top of their skull and crippling your hands. So we advise that you use open hands to strike in a street fight. Our belief is that you should limit the use of your fist, unless you have well conditioned hands, and aim for soft targets such as the side of the neck. In part one of our video we look at where you actually strike for a knockout punch. The second, second describes why hitting the side of the neck works so well, what we describe as the science behind strike. Dr. Ebraheim’s educational animated video describes the condition known as foot.drop , which occurs due to Peroneal nerve injury . Causes and treatment of Peroneal nerve injury is outlined . What is foot drop? The inability to raise the front part of the foot due to weakness or paralysis of the tibialis anterior muscle that lifts the foot. A common symptom of foot drop is high steppage gait often characterized by raising the thigh up in an exaggerated fashion while walking. When the person with foot drop walks, the foot slaps down onto the floor. Foot drop usually results from injury to the peroneal nerve which is susceptible to injury at any point of its course. The peroneal nerve begins from the L4,L5, S1 and S2 nerve roots and joins the tibial nerve to form the sciatic nerve. The sciatic nerve starts n the lower back and runs through the buttock and lower limb. In the lower thigh, just above the back of the knee, the sciatic nerve divides into two nerve, the tibial and peroneal nerves, which innervate different parts of the lower leg. The common peroneal nerve then ravels anterior, around the fibular neck, dividing into superficial and deep peroneal nerves. The deep peroneal nerve gives innervation to the tibialis anterior muscle of the lower leg which is responsible for dorsiflexion of the ankle. Conditions causing foot drop: 1-L4-L5 disc herniation A herniated disc compressing the L5 nerve root may cause foot drop. 2-Lumbosacral plexus injury due to pelvic fracture 3-Sciatic nerve injury: hip dislocation The common peroneal division of the sciatic nerve is commonly injured during fracture dislocation injuries involving the hip. 4-Injury to the knee: kee dislocation In the event of a knee dislocation, it is important to check for common peroneal nerve and popliteal artery injury. 5-Established compartment syndrome •Foot drop is a late finding •Irreversible muscle and nerve ischemia occur in patients if fasciotomy is not performed. •Fasciotomy should be done early. •4 hours of ischemia may be tolerated, but by 8 hours, the damage is often irreversible. Treatment of peroneal nerve injury/foot drop: •If a disc herniation in the low back is impinging on the nerve and causing symptoms of foot drop, then the herniated disc should be treated or removed. •Obtain EMG and nerve studies of the patient. •Recovery may take 1 year to 18 months. •The big toe is the last to recover. •If no recovery is achieved, explore the nerve for repair, graft or tendon transfer. Dr. Ebraheim’s educational animated video describing the condition known as radial nerve palsy. The radial nerve originates from the posterior cord of the brachial plexus. Radial nerve injury results from acute trauma or compression of the radial nerve. Condition is often referred to as •Saturday night palsy: alcohol is sometimes a factor as person falls asleep with the back of their arm compressed by a chair back, bar edge, etc. •Honeymoon palsy: from another individual sleeping on one’s arm overnight, compressing the nerve. •Crutch palsy: compression on nerve from walking with crutches. The radial nerve supplies the extensor muscles allowing for extension of the wrist and fingers in addition to supplying the triceps that extends the elbow. If the muscles are not working properly, the patient will experience the condition known as “wrist drop”. A radial nerve injury classically occurs by fractures involving the distal third of the humeral shaft. The patient will be unable to perform the “hitch hike” sign. If the nerve is compressed at the axilla, the triceps muscle will be affected. Patient will not be able to extend the elbow in addition to wrist drop. Treatment of radial nerve palsy •Brace, cockup splint •Physical therapy to help maintain the muscle strength and avoid contracture. •EMG and nerve studies: Brachioradialis is the first muscle to recover. Can be done as early as three weeks and follow the progress. •Nerve exploration, repair or graft: nerve exploration: Nerve exploration is indicated in open humeral fracture with radial nerve palsy or in conditions that have had sufficient time without nerve recovery. •Tendon transfer if nerve cannot be repaired or the function cannot be restored.

You might not know much about your kidneys and what they do right now...so I'm going to tell you a few basic facts before I list some of the signs of kidney problems... As you can see from this video, your kidneys are two bean shaped organs that are located just a bit above your belt line on either side of your body... Each kidney has thousands of tiny tissues called nephrons that filter waste materials, toxins and excess fluids from your bloodstream. These are flushed out of your body in the form of urine. urine. Here's a list of the most typical signs of kidney problems that occur from a variety of possible disorders, which may include kidney stones, kidney infections, kidney cancer or kidney damage from some type of physical trauma, like a severe blow to the kidney area. These symptoms are also likely to show up if you have certain conditions like diabetes or high blood pressure. Lower back pain is very often one of the first symptoms of kidney trouble. This back pain may be a mild, nagging ache, or it can be so severe it's actually debilitating. Another common indicator of a kidney disorder is discomfort during urination and urine that is discolored and cloudy or even tinged with blood. Keep in mind, however, these symptoms are also typical of a bladder infection, so it's important to see your doctor for a diagnosis. Most people with kidney trouble also find themselves heading for the bathroom more often than usual, even if little or no urine actually flows out. But this too can also just be a symptom of a bladder infection. Signs of more serious kidney problems like impending kidney failure, a kidney infection or kidney cancer can include: changes in blood pressure, muscle weakness, anemia, lethargy and sleepiness, loss of appetite, cognitive difficulties, stomach discomfort, general swelling, fever and chills, nausea and vomiting, breathlessness, night sweats, increased heart rate and unexplained weight loss. Signs of kidney problems shouldn't be ignored. When you experience one or more of the symptoms I've listed, consult your doctor as soon as you can.

Again, this is more prevalent these days because we’re sitting so long, hunched over the computer with poor posture. Finally, another major reason for poor blood flow would be because inflammation... Which requires an anti-inflammatory diet and anti-inflammatory supplements, herbs and vitamins. It’s VERY IMPORTANT to know that poor blood flow and circulation is one of the primary causes of a heart attack or stroke. This is because it can lead to: -Hypertension and high blood pressure -Hardening and narrowing of your arteries -Higher cholesterol and plaque build up So whether you feel it in your hands or your feet, it’s a WARNING SIGN that you MUST improve your blood flow and circulation. SOLUTIONS So let’s continue and I’ll quickly reveal one of the 3 best solutions for improving blood flow and circulation to your hands Hand Squeezing/Gripping This is for strengthening your grip and your hand muscles, while increasing blood flow. Grab anything you can squeeze… this can be something “soft”, such as rolled up socks. Or something a lot firmer like a tennis ball. If you’re weak, start with something softer and build up to a firmer tennis ball as you gain strength. You should squeeze the object, then HOLD for at least 1-2 seconds, then release. That’s ONE repetition. Do at least 10 repetitions and work up to 20-30 reps… or until you feel a good burn in your hands. That’s ONE set. Then back to the original hand. Better to start with your STRONGER hand. You should do 2-3 sets of at least 15 or more repetitions, 3x weekly on alternate days. Such as Mon, Wed and Fri. Remember, you need to squeeze and HOLD to really FEEL the muscles contract and increase blood flow and muscle contractions to the area. After you're done, massage and stretch your hands and forearms. Wrist Curls Now we want to strengthen your wrist and your forearm muscles. To do this, you’ll do wrist curls and reverse wrist curls. To do a wrist curl, you’ll need a light dumbbell and slowly curl the weight from the bottom to the top. You can put your forearms on your leg, on a bench or anything stable. Do 3 sets of 15-25 repetitions, 3x weekly. And then you’ll also want to do reverse wrist curls for the top of your forearms. Same idea, but going the opposite direction. Again, 3 sets of 15-25 repetitions, 3x weekly. Just like the hand squeezing/gripping exercise, do these on alternate days … But this time, let’s do it on OTHER days -- Tues/Thurs/Sat if possible. This way we are exercising your hands and forearms every day. However, if that’s not possible, just do them all on the same days as the gripping exercise -- Mon/Wed/Fri Walking Even though this video is about blood flow to your hands, your BODY as a WHOLE requires better circulation... And I think walking is one of the best exercises you can do and almost anyone can do it, any where, any time of the day and it doesn’t cost you anything. Ideally you would walk outside in fresh air if possible. First thing morning and/or late evening are good times but, whenever you can is a better than nothing. Try your best to walk just a little bit every single day. James was a professional athlete and decided to have spinal surgery after suffering with intermittent pain from a disc herniation, in the hopes of furthering his career upon his return to sport. After waking from the surgery with a paralysed left leg and excruciating pain, then later nearly losing his leg when advised to amputate the leg, James was understandably left devastated. Going into hospital for a routine operation, is normally a straight forward procedure, but unfortunately there are instances when such operations can go wrong. Social worker Stephen Onley, 52, from Bromsgrove in Worcestershire, was left paralysed after doctors failed to monitor him correctly following a routine hip operation in 2010. Stephen requested not to be given an epidural to control the pain after the surgery because he was aware of the risk of spinal injury linked to this type of anaesthetic through his work with disabled people. Despite this, he was given the epidural whilst under general anaesthetic. Following the surgery, Stephen suffered a severe drop in blood pressure that was not detected, even though the doctors caring for him would have been well aware that this was possible. By the following day, Stephen’s blood pressure had fallen dangerously low, resulting in a lack of blood to his spinal cord, which caused irreparable damage. Stephen is now permanently paralysed from the waist down and now relies on a wheelchair. He has had to give up his work and his wife Caroline has to provide the 24-hour care he now needs.
