New Delhi, Nov 28 (ANI): Lung cancer is one of the most common cancers in the world. There are two major types of lung cancers, Non Small Cell Lung Cancer (NSCLC) and Small Cell Lung Cancer (SCLC). Knowing which type you have is important because it affects your treatment options and your outlook. Surgery, radiation, chemotherapy, targeted treatments and immunotherapy alone or in combination are used to treat lung cancer patients. Use of individual treatment options would be based on the stage of cancer and symptoms. Surgery: Most stage 1 and stage 2 lung cancer patients are treated with surgery to remove the tumor if the patient is very fit. As most of the lung cancer patients are not very fit and suffer from poor lung and heart condition, up to 10% of patients only can undergo curative lung surgery. Radiation: Patients with early lung cancer, but not suitable for surgery (stage 1& 2), or advanced lung cancer (stage 3) are treated with radiation with the intention to cure. It's being hailed as a breakthrough medicine; a twice-daily pill that targets the growth of a specific type of lung cancer. The FDA fast-tracked its approval. "I think it was very exciting because it's sort of a proof that these targeted therapies for cancers can be affective," says Dr. Lowell Hart, a hematologist/oncologist on the medical staff of Lee Memorial Health System. Dr. Lowell Hart has three patients on the drug, now named Xalkori. He saw the drug make a dramatic impact. "It was very impressive to me that these were sick patients who had had their cancer progress through chemotherapy no longer working on chemo, so we would expect them to not do very well and perhaps not live very long but have had very dramatic responses to the drug," says Dr. Hart. The drug only works on the 5-10% of lung cancers which have a specific gene knows as ALK. Many people with it are non-smokers who developed 'small lung cancer'. "All three of my patients that have this mutation are women and non-smokers," says Dr. Hart. The new drug blocks protein produced by the ALK gene. It's a targeted therapy, which researchers believe is the future of controlling or curing cancer. The FDA also approved a screening test. "I am personally checking everybody and I think you know my partners are in general checking everybody and the tests are now commercially available," says Dr. Hart. Xalkori comes with a high price tag, upwards of $100,000 a year, on par with traditional chemo. Drug maker Pfizer is expected to work with insurance companies to limit out of pocket expenses, important in making the drug available to those who need it. "I've seen that very few times in my career of over 25 years. I said this drug is the real thing," says Dr. Hart. Either in people who are current smokers, or have smoked in the past. The most common one is cough that doesn’t go away. Almost all of us get coughs, once in a while, that we get for a few days, or a week or two, but they typically go away. So, if it persists for longer than several weeks…The other one is shortness of breath. One of the things that lung cancer can do is it can block off airways, it can put fluid in your lungs, and it can make it more difficult for people to breathe. You can also get chest pain. That usually comes from lung cancers that grow up against your chest wall that cause the pain. The other one, and the one that you should seek medical attention for right away is coughing up blood. The most effective treatment for lung cancer, as nearly every other cancer, is being able to catch it early enough where you can do a surgical resection, or removing it with an operation. If it goes into the lymph nodes in the middle of the chest, then you use a different approach, and that is, you typically give chemotherapy drugs, either through the vein, or you take orally, with radiation therapy. Within the last decade, we’ve begun to find the specific genetic changes that cause the lung cancers, and also develop drugs directly aimed at those targets. One of the first ones that was discovered was, in part, discovered by investigators here at the Dana-Farber Cancer Institute, and that’s called a mutation in the epidermal growth factor receptor. Instead of giving them chemotherapy, we can give them what we call a targeted agent (currently, are in the United States) and instead of being able to treat people for the three or four months that I talked about before, the people can go for a year or longer on the therapy. We actually have four different other changes, for which we’re doing investigational therapies to find out if we can match these up. And it’s our goal to expand this to make up about half of lung cancers in the next five to ten years. The most important thing you can do to avoid lung cancer is to never start smoking. And that’s a message we can’t repeat too many times.

One of the important differences in lung cancers, to break it down into two categories: there's what we call non-small cell lung cancer and there's small cell lung cancer and for thirty or forty years we've divided them that way and the reason is because small cell lung cancer grows very fast, divides very fast and surgery even for early disease, is generally not an option. It's already spread by the time the surgeon takes it out or about the time the patient recovers. So, they stopped doing surgery on small cell lung cancer a long time ago. On the other hand, small cell lung cancer responds very well to chemotherapy. It shrinks very quickly and we get dramatic responses. If someone just has a limited a small cell lung cancer we can cure them with the combination chemotherapy and radiation. If they have extensive, meaning it's spread somewhere else (small cell lung cancer) we can't cure it but we can significantly prolong their survival by treatment as well. Non-small cell lung cancer, which is the more common one with eighty percent of lung cancers, is approached for surgically and then with either radiation or chemotherapy or chemotherapy alone. The other thing the past two or three years about non-small cell as we've been able to break it down even smaller groups. There's the adenocarcinomas and the squamous cell carcinomas and the others and some of those have genetic mutations and there are drugs for specific mutations For some of these non-small cell lung cancers, oral drugs you take by mouth at home, very few side effects and work very well to control the disease so it's important if you have a non-small-cell for your tumor to be tested for genetic mutations to see if any of those drugs are applicable for you. When Wilmington resident Joan O'Beirne, 68, started experiencing shortness of breath during her daily activities, she had no idea what could be causing it or that she was about to become part of a pioneering research study. A widow with three grown children, O'Beirne has always been active and healthy, never smoking, playing tennis, and more recently watching her grandchildren every afternoon. When she noticed her shortness of breath, she consulted her primary care doctor who sent her to a lung specialist. Her early tests were inconclusive; she showed no sign of chronic obstructive pulmonary disease. Seeking an answer, O'Beirne's doctors ordered additional testing for cardiac disease and blood clots as well as a CT scan of her lungs. Finally, the CT showed scar tissue in the upper left lobe of her lung that a radiologist suspected might be a sign of cancer. A PET scan confirmed active cancer cells and she was sent to a lung surgeon for a biopsy to determine the extent of her cancer. After a surgeon advised that her tumor would require a surgical biopsy—a procedure that increased the possibility of spreading the cancer—O'Beirne sought a second opinion. She met with Massachusetts General Hospital and North Shore Medical Center thoracic surgeon and lung specialist, Dean Donahue, M.D. With O'Beirne as an active member of her own care team, she decided to first have Dr. Donahue and the interventional radiologist perform a needle biopsy and send the sample to pathology for staging. "With reassurance from Dr. Donahue, I felt completely comfortable with this approach," said O'Beirne. O'Beirne's biopsy confirmed Stage 2A cancer and Dr. Donahue then performed surgery to remove the upper lobe of her left lung. "After the surgery, I thought I would be fine," said O'Beirne. "I was surprised to learn that chemotherapy decreases the chance that my cancer will return from 50 percent to about 35 to 45 percent. So I opted for the treatment." Today, patients at the Mass General/ North Shore Cancer Center have their tumors screened for many types of cancer to determine their genotype, or genetic "fingerprint." This genetic intelligence is then used to match the appropriate "smart drug" that is specifically crafted to interrupt the cancer-causing mechanism, with minimal side effects, to the specific genetic mutation. O'Beirne is one of many patients who is benefitting from the comprehensive care and world-class research provided at the Mass General/North Shore Cancer Center. Genotyping revealed that her cancer was caused by the EGFR mutation. She is now enrolled in a clinical research trial evaluating the effectiveness of an existing EGFR-mutation "smart drug" that had previously proven effective in shrinking tumors in patients with advanced metastatic cancer. In O'Beirne's trial, researchers are evaluating whether the drug—Tarceva— can prevent the recurrence of lung cancer in patients at an earlier stage of the disease. "I feel so fortunate to be part of this research. I only have to take one pill every day for two years," said O'Beirne. "I can't believe this little pill might be able to prevent my cancer from coming back." "I'm so grateful for the wonderful care I've received at the Mass General/ North Shore Cancer Center," said O'Beirne. "We're so lucky to have access to such amazing surgeons and cutting-edge treatments and research so close to home."

Lawmakers stressed the decision to loosen regulations was regrettable... and that it prioritizes the financial burdens of pharmaceutical companies over the people's health and safety. But experts say... Olita does patients more good than harm,... being a drug especially for lung cancer patients who have a tolerance to other treatments. "These patients have no alternative to these drugs for an effective cure or to prolong their lives. The drugs are completely different from those that general cancer patients would take."
