There is so much research that's been done over the years. And certainly research that's being done right now. So if you're curious about reflexology and cancer, I so encourage you to look into the studies. There's a major one being done right now with the National Cancer Institute in this country, the United States, with the NIH. The National Institutes of Health. It's Michigan State University and they are doing incredible work on breast cancer patients and reflexology. This is their second grant and it's really astonishing work. So just research it if you like. Branch Reflexology. Barbara Bower. There's some wonderful people doing that. So it's out there. With cancer, it really depends on the person you're with. If they have cancer, it's really important that you're communicating with them the whole time. If they're undergoing some kind of chemotherapy, they might be really, really sensitive in their feet. So you might want to focus more on relaxation techniques. So we'll start with those. Some of the ones you can use might be the lung press for example. With the lung press you're going to go ahead and make a fist and you're going to with your thumb go ahead and insert it there. And then this hand will support the foot. So this part of the hand will be on the ball of their foot. So we bring it right there and we're just going to support with these fingers. Then to do the lung press we go ahead and press forward and then we kind of wave their foot towards us back. And we go front and back. We're going to do this bilaterally. So it means we're going to be doing it on both feet. We could do it the other direction too. You'll do the other foot as well. Some of the other things you could do could be the metatarsal mover. Just with your thumbs moving the metatarsal heads ever so gently. Again it's really important sometimes the just gentlest of touch for someone who's going through cancer is really, really special. So I encourage you to look into how to be present with your friend or family member and even just holding the feet can be really valuable. You can also work on the foot, in general, as a whole. So you could do all the reflexology techniques. You could do the thumb walking, the finger walking, in particular for the entire body. So we can for instance to the whole arch or belly of the foot. You could do the thumb walking up. You're passing through all of these wonderful vital organ reflexes. Certainly get your maps and look at what you're passing through. You might want to give a little more or less attention to something depending on the kind of cancer your person has. In addition you also can do thumb walking on the heels. You could do it on the ball of the foot. The whole lymphatic system falls through here and across here. So certainly you could do everything, but the relaxation techniques tend to be very popular amongst cancer patients.

Most patients with advanced lung cancer will have cancer that spread beyond the chest cavity and affects other organs. For those patients, the most common treatment approach is chemotherapy, which is generally administered intravenously by vein or less commonly a pill by mouth. That chemotherapy is generally administered on a regular schedule. Oftentimes every third week with periodic examinations to be sure that the patient is tolerating the side effects of that treatment appropriately. Generally, the treating physician will have a visit with the patient to decide the exact course of treatment and the number of treatment courses to expect, prior to making measurements to determine how well that treatment is working for that individual patient. Most patients will be treated over the course of four, possibly six months with the goal to contain and control the cancer and lessen the symptoms. During the course of treatment for advanced lung cancer, there can be complications that occur that need to be managed by the physician. You will need to have a meeting with your Doctor to understand the kinds of things that might occur as a consequence of the effects of your treatment and how those might be managed. Some of those complications can be quite serious and may require urgent, even emergent evaluation. It is vitally important that you understand the nature of your treatment, what to expect in terms of side effects and how to manage them so that you can gain the most benefit from your treatment for your lung cancer. When Wilmington resident Joan O'Beirne, 68, started experiencing shortness of breath during her daily activities, she had no idea what could be causing it or that she was about to become part of a pioneering research study. A widow with three grown children, O'Beirne has always been active and healthy, never smoking, playing tennis, and more recently watching her grandchildren every afternoon. When she noticed her shortness of breath, she consulted her primary care doctor who sent her to a lung specialist. Her early tests were inconclusive; she showed no sign of chronic obstructive pulmonary disease. Seeking an answer, O'Beirne's doctors ordered additional testing for cardiac disease and blood clots as well as a CT scan of her lungs. Finally, the CT showed scar tissue in the upper left lobe of her lung that a radiologist suspected might be a sign of cancer. A PET scan confirmed active cancer cells and she was sent to a lung surgeon for a biopsy to determine the extent of her cancer. After a surgeon advised that her tumor would require a surgical biopsy—a procedure that increased the possibility of spreading the cancer—O'Beirne sought a second opinion. She met with Massachusetts General Hospital and North Shore Medical Center thoracic surgeon and lung specialist, Dean Donahue, M.D. With O'Beirne as an active member of her own care team, she decided to first have Dr. Donahue and the interventional radiologist perform a needle biopsy and send the sample to pathology for staging. "With reassurance from Dr. Donahue, I felt completely comfortable with this approach," said O'Beirne. O'Beirne's biopsy confirmed Stage 2A cancer and Dr. Donahue then performed surgery to remove the upper lobe of her left lung. "After the surgery, I thought I would be fine," said O'Beirne. "I was surprised to learn that chemotherapy decreases the chance that my cancer will return from 50 percent to about 35 to 45 percent. So I opted for the treatment." She had chemotherapy at the Mass General/ North Shore Cancer Center, under the care of medical oncologist Rachel Rosovsky,M.D. In recent years, researchers have been able to pinpoint, with greater accuracy than ever before, the exact genetic abnormalities in certain cancers that turn healthy cells into cancerous ones. In 2004, physician researchers from the Massachusetts General Hospital Cancer Center were the first to identify that a mutation in the epidermal growth-factor receptor (EGFR) was responsible for 10 percent of lung cancers —and 50 percent of those in non-smokers. Today, patients at the Mass General/ North Shore Cancer Center have their tumors screened for many types of cancer to determine their genotype, or genetic "fingerprint." This genetic intelligence is then used to match the appropriate "smart drug" that is specifically crafted to interrupt the cancer-causing mechanism, with minimal side effects, to the specific genetic mutation. O'Beirne is one of many patients who is benefitting from the comprehensive care and world-class research provided at the Mass General/North Shore Cancer Center. Genotyping revealed that her cancer was caused by the EGFR mutation. She is now enrolled in a clinical research trial evaluating the effectiveness of an existing EGFR-mutation "smart drug" that had previously proven effective in shrinking tumors in patients with advanced metastatic cancer. In O'Beirne's trial, researchers are evaluating whether the drug—Tarceva— can prevent the recurrence of lung cancer in patients at an earlier stage of the disease. "I feel so fortunate to be part of this research. I only have to take one pill every day for two years," said O'Beirne. "I can't believe this little pill might be able to prevent my cancer from coming back." "I'm so grateful for the wonderful care I've received at the Mass General/ North Shore Cancer Center," said O'Beirne. "We're so lucky to have access to such amazing surgeons and cutting-edge treatments and research so close to home."

Photo dynamic therapy (PDT) is a new, two-step cancer treatment procedure that combines laser technology, fiber optics and a special light-activated drug called Photofrin. With PDT, cancer cells can be selectively destroyed while normal cells are spared. The Federal Drug Administration recently granted approval for the procedure, making PDT available for some lung cancer patients. Lung cancer kills more people than any other cancer. But now, patients suffering from advanced lung cancer have an alternative to risky surgery. The Federal Drug Administration has just approved a tumour blasting procedure that combines special light-activated drugs with laser technology. Medical physicists and doctors are calling this one of the most promising new weapons in the war against cancer. SOUNDBITE: (English) "The drug is not active initially after injection. It is activated by light, a 630 wavelength light which is actually present in sunlight." SUPER CAPTION: Dr. Jim Luketich, University of Pittsburgh Medical Center The procedure works because a special photo reactive drug accumulates in the cancer cells. Then, when a red laser light shines on the area, the drug switches on creating a toxic form of oxygen that destroys the tumour. In a matter of a few days the tumour shrivels up and dies. SOUNDBITE: (English) "If you look at the tumour cells for instance in the lungs or the esophagus two days later, what you see is the tumor has all turned white and is just crumbling away." This class of drug seems to have even less side effects. Immune check point inhibitors (PD-L1) targeted drugs have already shown a promising role, and are likely to emerge as main therapy for lung cancers in future. Either in people who are current smokers, or have smoked in the past. The most common one is cough that doesn’t go away. Almost all of us get coughs, once in a while, that we get for a few days, or a week or two, but they typically go away. So, if it persists for longer than several weeks…The other one is shortness of breath. One of the things that lung cancer can do is it can block off airways, it can put fluid in your lungs, and it can make it more difficult for people to breathe. You can also get chest pain. That usually comes from lung cancers that grow up against your chest wall that cause the pain. The other one, and the one that you should seek medical attention for right away is coughing up blood. The most effective treatment for lung cancer, as nearly every other cancer, is being able to catch it early enough where you can do a surgical resection, or removing it with an operation. If it goes into the lymph nodes in the middle of the chest, then you use a different approach, and that is, you typically give chemotherapy drugs, either through the vein, or you take orally, with radiation therapy. Within the last decade, we’ve begun to find the specific genetic changes that cause the lung cancers, and also develop drugs directly aimed at those targets. One of the first ones that was discovered was, in part, discovered by investigators here at the Dana-Farber Cancer Institute, and that’s called a mutation in the epidermal growth factor receptor. Instead of giving them chemotherapy, we can give them what we call a targeted agent (currently, are in the United States) and instead of being able to treat people for the three or four months that I talked about before, the people can go for a year or longer on the therapy. We actually have four different other changes, for which we’re doing investigational therapies to find out if we can match these up. And it’s our goal to expand this to make up about half of lung cancers in the next five to ten years. The most important thing you can do to avoid lung cancer is to never start smoking. And that’s a message we can’t repeat too many times.

John T. Hamm, M.D., Medical Director of the Norton Cancer Institute Research Program, answers the question, what is the difference between non-small cell lung cancer and small cell lung cancer? One of the important differences in lung cancers, to break it down into two categories: there's what we call non-small cell lung cancer and there's small cell lung cancer and for thirty or forty years we've divided them that way and the reason is because small cell lung cancer grows very fast, divides very fast and surgery even for early disease, is generally not an option. It's already spread by the time the surgeon takes it out or about the time the patient recovers. So, they stopped doing surgery on small cell lung cancer a long time ago. On the other hand, small cell lung cancer responds very well to chemotherapy. It shrinks very quickly and we get dramatic responses. If someone just has a limited a small cell lung cancer we can cure them with the combination chemotherapy and radiation. If they have extensive, meaning it's spread somewhere else (small cell lung cancer) we can't cure it but we can significantly prolong their survival by treatment as well. Non-small cell lung cancer, which is the more common one with eighty percent of lung cancers, is approached for surgically and then with either radiation or chemotherapy or chemotherapy alone. The other thing the past two or three years about non-small cell as we've been able to break it down even smaller groups. There's the adenocarcinomas and the squamous cell carcinomas and the others and some of those have genetic mutations and there are drugs for specific mutations For some of these non-small cell lung cancers, oral drugs you take by mouth at home, very few side effects and work very well to control the disease so it's important if you have a non-small-cell for your tumor to be tested for genetic mutations to see if any of those drugs are applicable for you. James doctors are using lasers to improve the quality of life for some patients whose lung cancer has affected their windpipes. In some late-stage lung cancers, tumors can go through patients’ windpipes or press against them, blocking the airways. James pulmonology expert Dr. Shaheen Islam explains how an advanced laser surgery, directed through a patient’s mouth and into the windpipe, can vaporize the tumor, open the airway, and if needed, insert a stent to further open the airway, restoring better breathing. The procedure, performed under local anesthesia, improves patients’ quality of life, and often enables them to proceed to additional treatment, such as chemotherapy or further surgery.
It's being hailed as a breakthrough medicine; a twice-daily pill that targets the growth of a specific type of lung cancer. The FDA fast-tracked its approval. "I think it was very exciting because it's sort of a proof that these targeted therapies for cancers can be affective," says Dr. Lowell Hart, a hematologist/oncologist on the medical staff of Lee Memorial Health System. Dr. Lowell Hart has three patients on the drug, now named Xalkori. He saw the drug make a dramatic impact. "It was very impressive to me that these were sick patients who had had their cancer progress through chemotherapy no longer working on chemo, so we would expect them to not do very well and perhaps not live very long but have had very dramatic responses to the drug," says Dr. Hart. The drug only works on the 5-10% of lung cancers which have a specific gene knows as ALK. Many people with it are non-smokers who developed 'small lung cancer'. "All three of my patients that have this mutation are women and non-smokers," says Dr. Hart. The new drug blocks protein produced by the ALK gene. It's a targeted therapy, which researchers believe is the future of controlling or curing cancer. The FDA also approved a screening test. "I am personally checking everybody and I think you know my partners are in general checking everybody and the tests are now commercially available," says Dr. Hart. Xalkori comes with a high price tag, upwards of $100,000 a year, on par with traditional chemo. Drug maker Pfizer is expected to work with insurance companies to limit out of pocket expenses, important in making the drug available to those who need it. "I've seen that very few times in my career of over 25 years. I said this drug is the real thing," says Dr. Hart.
