More and more Americans are putting out their cigarettes for good. In 2015, just 15.1 percent of American adults were smokers, a decline of almost 2 percentage points compared to 2014, according to a report released by the Centers for Disease Control and Prevention. The last decline that significant was from 1992 to 1993. The smoking rate has declined almost every year since 1997, when almost 25 percent of U.S. adults said they smoked. The decline comes with an increase in public awareness campaigns about the health hazards of smoking as well as the growing popularity of e-cigarettes, which are often marketed as a safer alternative to typical cigarettes — although some studies suggest they don't actually help people quit, and there isn't a consensus on whether they're harmful to a person's health. The U.S. Food and Drug Administration announced earlier this year that e-cigarettes will be subject to federal regulation. Smoking is the leading cause of preventable death in the U.S., with more than 480,000 deaths each year. A West Bank business is sending staff off on a free holiday - if they permanently stop smoking. It's part of a campaign to get more people to kick the habit. The Palestinian Ministry of Health says it spends 80 percent of its budget on treating conditions associated with tobacco use. A busy street in Ramallah, in the West Bank. Just like most days, the smell of cigarette smoke hangs in the air. According to the Palestinian Central Bureau of Statistics, 30 percent of the population here are smokers. They spend 1.2 million dollars every day on cigarettes - nearly 400 million dollars a year. And the health implications of the addiction are well known. Globally, tobacco is said to kill around six million people each year. And Palestinians don't escape that fate. Dr. Asaad Ramlawi, from the Palestinian Ministry of Health, says smokers are at risk of developing serious health conditions. "Heart and arteries disease, lung disease, different types of cancer and diabetes - all these diseases exist in the Palestinian society in a very large number," he explains. "We spend 80 percent of the Palestinian Ministry of Health budget for the treatment of these diseases. Seventy percent of the death of Palestinians is from these diseases yearly and of course smoking is a major cause of these diseases. Anti-smoking (measures) in the Palestinian society and especially for the young generation during the coming years will lead to economic recovery and improve the health level of the society." And there are efforts to curb the habit. Cigarette advertisements have been banned and machines selling the tobacco goods are not allowed in public areas. Now big business is joining the battle. The Bank of Palestine has launched a campaign to encourage its employees to quit smoking. The bank had already introduced restrictions on when and where its workers can take cigarette breaks. But now it is rewarding those who kick the habit too. "At the beginning we instructed our employees not to smoke inside the bank building or even at the entrance of the building and there is a limit to the number of times the employee can smoke," says Thaer Hamayel, a spokesperson for the bank. "But after that we decided to launch a campaign to encourage our employees to quit smoking by sending those who quit smoking with their family to another country like Turkey for a tour covered by the bank in order to encourage them to quit this harmful habit." Eight employees have quit smoking and been rewarded with a five day trip to Istanbul for their success. One of those is Marwan Mafarjeh, a branch manager. He picked up the habit as a university student. But after 22 years as a smoker, there were lots of reasons to give up. "I decided to quit smoking because I thought about my health and my expenses and also the work environment. I am working as a manager at the bank and I can't go out of the building each time I needed to smoke. At the same time I was wasting my time doing this and I like to use the time of smoking to do something useful as well as the positive effect on me economically and health wise," he says. Mafarjeh gave up smoking a year ago and says other people should consider doing the same. "I advise everyone to quit smoking no matter what, it doesn't matter how and with what method, and instead use the money he spends for smoking for sports, special needs and other things that he can benefit from." For Mafarjeh, that saving is 300 US dollars a month. And quitting means he now leads a much healthier lifestyle. Our latest Freakonomics Radio episode is called “How to Make People Quit Smoking.” The gist: the war on cigarettes has been fairly successful in some places. But 1 billion humans still smoke — so what comes next? In the U.S., roughly 70 percent of smokers say they want to quit. But when they try, some 90 percent of them fail. So what does get people to smoke less? Something must be working: the smoking rate in the U.S. has fallen by more than half. Kenneth Warner, an economist at the University of Michigan School of Public Health, has been doing tobacco-policy research since the 1970’s. One of the most powerful smoking deterrents, he says, is making cigarettes more expensive. WARNER: The effects of tax and price are more well studied than any other area of tobacco control. We have a lot of data about that. What we know is that if you increase the price by 10% you will decrease total cigarette consumption by 3 to 4%. The Vanderbilt economist Kip Viscusi agrees, calling price a “powerful tool” that doesn’t wear out. And yet one out of five Americans still smokes. Worldwide, there are about 1 billion smokers, with the highest rates in Asia. So besides price and tax controls, what makes people quit smoking? In the podcast, you’ll hear from Jeffrey Harris, a physician and economist at M.I.T. who has studied tobacco control in Uruguay, which he calls “a pioneer” in smoking cessation. You’ll also hear about the upside of smoking — or at least of getting nicotine in your system. Paul Newhouse, an M.D. who runs the Center for Cognitive Medicine at Vanderbilt, is doing research to learn if nicotine (divorced from smoking, ideally) has strong medicinal efficacy: NEWHOUSE: It appears to activate a class of what we call receptors, [which are] important for regulating a whole variety of brain functions. And so we think that nicotinic receptors are important for things like attention, for behavioral strategies, for what we call executive functioning, which is the ability to make decisions and evaluate information, we think it’s important for memory, and so that has led us to thinking about what particular disorders might be helped by stimulating nicotinic receptors with nicotine or with something else. Among the disorders that Newhouse and others in his field think may benefit from nicotine therapy: schizophrenia, Parkinson’s disease, Alzheimer’s disease, anxiety, depression, and many others. If nicotine does indeed work as theorized, that may explain why so many people have such a hard time quitting smoking: they are self-medicating. So when they come together in a group and they find out that everyone shares that same guilt and shame about the addiction it lets go of so much of it. And then it unblocks their belief that they can quit. And you can. Once people quit smoking a lot of feelings come up because all those feelings have been pushed down from smokings. Oh, let me smoke over this or let me smoke over that. Oh this happened, give me a cigarette or that happened give me a cigarette. I have a test. I have a project at work. Let me smoke, smoke, smoke. I mean it comes up over everything so getting counseling when you put down the cigarette because everything all the anxiety, the panic, all the different feelings, the stuff that you've pushed down all these years. When you see a tragic accident a paramedic would tell you, take deep breaths slowly in through your nose and out through your mouth. Well, it works the same way with the nicotine withdrawal. There are many things that you can do to also help you with nicotine withdrawal. Don't let yourself get too lonely. I've already mentioned tired. If you're angry try to deep breathe. Try to talk it out with a friend whatever's going on, because these things can be exacerbated by the withdrawal. You could end up picking up a cigarette instead of dealing with the actual situation. And so, a lot of people have looked at the reasons why that is. So, there's been a lot of different research to look at why there's this relationship between schizophrenia and smoking. One theory has to do with self medication, that people with schizophrenia smoke to reduce stress and to reduce symptoms like depression and agitation. As well, smoking seems to help with some of the cognitive symptoms of schizophrenia, so it helps them to think clearer and decreases some of the confusion that can happen in schizophrenia. But, the biggest of reason that has stood up in the studies may be boredom, that a lot of people with schizophrenia end up being institutionalize for periods of time, being out of work and that leads to a lot of idle time that leads to smoking behavior. Obviously, we know that smoking leads to poor outcomes in terms of heart disease, lung disease, and cancer, and so we know that this is something that needs to address. So, there's a lot of energy being put into smoking cessation programs for people with schizophrenia and other serious mental illnesses. And so, these can be access you know through your treatment providers. Both your psychiatrist and primary care providers have been involve in these efforts. You can call 311 in New York City or you can access your local city government to obtain things. In New York City, we provide nicotine replacement patches and gum to people for free. And in hospitals they've also put in smoking cessation programs. It used to be that in every psychiatric ward across the country, there was a smoking room or you could just actually smoke on the unit. Because, incredibly, they make no substantial distinction between e-cigarettes and real cigarettes -- even though they are completely different products. But unlike normal everyday products, any potential risk posed by e-cigarettes is far outweighed by a real – not potential – good: saving lives by providing the nicotine that smokers enjoy without delivering the deadly toxins that can kill them.
