Stomach cancers tend to develop slowly over many years. Before a true cancer develops, pre-cancerous changes often occur in the lining of the stomach. Causes Gastric cancer may often be multifactorial, involving both inherited predisposition and environmental factors. Environmental factors implicated in the development of gastric cancer include diet, Helicobacter pylori infection, previous gastric surgery, pernicious anemia, adenomatous polyps, chronic atrophic gastritis, and radiation exposure. Diet A diet rich in pickled vegetables, salted fish, salt, and smoked meats correlates with an increased incidence of gastric cancer. Atrophic gastritis is a chronic disorder occurring in up to 8% of the general population. The positivity of autoantibodies against parietal cells and/or intrinsic factor, the co-existence of autoimmune diseases such as thyroid autoimmune disease or type 1 diabetes are frequently observed. The 5-year survival rate for people diagnosed with leukemia is about 57 percent, which may provide a measure of hope for those who are diagnosed. The following are signs and symptoms that could indicate leukemia. 1. Tiny Spots or Dots on the Skin These tiny spots are technically known as petechiae and may show up as patches of very small purple or red dots that are often observed on the arms, back, chest and face. Since leukemia can cause a decrease in the number of clot-forming platelets in the blood, smaller blood vessels are more likely to break causing petechiae, which can easily be mistaken for some type of rash. 2. bonee and Joint Pain This is one of the most common symptoms of people who have developed leukemia and is characterized by pain that feels like it originates deep in the bonees. This can manifest as small lumps or bumps that show up in places such as the neck, stomach, under the arms or in the groin area that may take on a color that has hints of blue or purple. Normally, these kinds of abnormalities form as a result of less serious infections and do not generally persist for longer than a week, or so. 5. Fatigue The blood is absolutely essential for delivering a constant supply of nutrients and oxygen to the cells of the body and when leukemia interferes with that process, feeling tired or worn out all the time is often the result. It will often become very difficult to complete routine physical tasks when suffering from these symptoms, which can also take an emotional toll on victims of this disease. 6. Bruising and Bleeding As mentioned previously, the blood is often unable to do its job properly when leukemia is a factor, and this often makes it much easier to suffer bruising just about anywhere on the body. It’s especially telling when bruises begin to show up in areas where bruising has never been a problem previously. Bruises may also appear darker in color and look worse than they have in the past. Whereas, the diffuse variant of stomach cancer is not associated with H. pylori infection. Due to successful anti H. pylori therapies available, the incidence of intestinal variant of stomach cancer is coming down. On the other hand, the incidence of diffuse variant is increasing. Dietary factors such as high salt or nitrate containing foods increase the incidence of intestinal variant of stomach cancer. Also, increased intake of smoked or packaged or processed food increases the incidence of intestinal subtype. Smoking is also a major risk factor for the intestinal subtype of stomach cancer, and the incidence increases with the intensity and duration of smoking. Any long standing benign ulcers of the stomach may also increase the risk of developing intestinal subtype of stomach cancer. On the other hand, the most important risk factor for diffuse variant of stomach cancer is genetical or familial, that is, it may be inherited from other family members. Stomach cancer may also be classified based on their location, that is, whether it is proximally located or distally located in the stomach. Proximally located means either in the gastroesophageal junction or cardia, whereas, distally located means either in the body or pylorus of stomach. This figure shows the location of gastroesophageal junction tumors, that is, the tumors located in the lower part of the esophagus, gastroesophageal junction or the upper part of stomach. The risk factors for all these three is almost similar. Whereas, this figure shows the location of the tumors present in the body and pylorus. The risk factors for these are different. The intestinal subtype of stomach cancer is less commonly seen in the GE junction and proximal part of stomach, and more commonly seen in the body and pylorus. And the diffuse variant is seen equally in the cardia, body and the pylorus of stomach. There has been a gradual increase in the incident of GE junction and proximal stomach tumors and a gradual decrease in the tumors in the body and pylorus, Let's understand why. H pylori infection is very commonly seen all over the world. Long standing infection with H. pylori leads to damage to normal epithelium of stomach, which is called as atrophic gastritis, due to which there is a decrease in gastric acid production. Due to chronic gastritis caused by H. pylori infection, there is a increased chance of cancers in the body and pylorus of the stomach. On the other hand, atrophic gastritis caused by chronic H. pylori infection decreases the acid production by the stomach. It reduces the chances of GERD, thereby reducing the chances of cancer in the GE junction and the cardia region of the stomach. But nowadays, due to successful anti H. pylori therapy available, the cancers in the body and the pylorus have decreased, whereas, those in the GE junction and cardia have increased. Pancreatic Cancer Field Neoplastic progression of exocrine glands evolves along the same trajectory. Yet the nomenclature of this trajectory differs for each gland. I shall now show an analogy between mammary gland and exocrine pancreas. Both glands constitute one field which evolves along the following stages: 1. Acute inflammation 2. Chronic inflammation 3. Dysplasia with multiple foci of ductal and lobular carcinoma in situ (DCIS and LCIS) 4. Dysplasia with a carcinoma 5. Metastasis Like all tissues of the organism, excretory glands are composed of tissue proliferative units (TPUs). Two TPU types operate in exocrine pancreas: 1. Columnar, located in excretory duct. 2. Glandular, called terminal duct lobular unit (TDLU) or acinus. Excretory duct TPU consists of a two compartment cell renewal system directed from the basement membrane into the duct lumen. As cells divide they stream inward and are sloughed off. Reading the literature one gets the impression as if pancreatic neoplasia starts only in the excretory duct. PanIN -1 PanIN-2 PanIN-3 , are excretory duct lesions. In reality most of the pancreatic lesions are LCIS analogs. With their own precursors. Maturation arrest is the hallmark of neoplastic progression. Cells die young, immature and do not differentiate. Q-compartment shrinks, and P-compartment widens.
