Otitis media is an infection of the middle ear space, which is the ear containing space behind the middle ear drum. Otitis media is one of the most common reasons for children to visit a pediatrician, and one of the most common reasons for children to undergo surgery. As parents, you are familiar with the sleepless nights, and pain, and fussiness. You've probably had many visits to the pediatrician, and you've been frustrated with the difficulty administering antibiotics. If this describes your household, myringotomy with tubes may be an option for your child. Tubes are often recommended in children who have had four infections in the past six months, or who have had persistent fluid for 3 months or more. Occasionally, tubes are placed urgently in children with severe acute infections that are not responding at all to antibiotics. This procedure immediately improves hearing and relieves pain. In most children, tubes prevent further infection and lessen the need for prolonged antibiotics. An adenoidectomy is commonly offered at the time of myringotomy, the tonsils and adenoids are lymphatic tissue. They serve the purpose of recognizing bacteria in your nose and throat, and producing antibodies to help you fight infection. When you've had chronic infections, or chronic allergies, the adenoids become enlarged and they can become infected with the very bacteria they are trying to capture. On your initial office visit, your child will be evaluated by an ear, nose and throat specialist. We will carefully examine the ear with an otoscope, and remove any wax that may be obstructing the canal. The night before surgery, it's very important that you don't have anything to eat or drink. Small children can have breast milk up to four hours before the surgery, or clear liquids like apple juice, water, or black coffee up to two hours before surgery. On the morning of surgery, your child will be registered by the staff at the surgery center. The surgeon will greet you, verify identification, and confirm the planned surgery. Parent's may escort their children to the threshold of the operating suite, and then return to wait in the pediatric perioperative area. Once your child is comfortably asleep and breathing easily, the surgeon will examine the ears under a microscope. All wax and debris will be removed, and then a small cut is made in the front of the ear drum, well away from the delicate bones. Any fluid is evacuated, and a tube is gently inserted on each side. Risks of myringotomy with tubes are perforation, drainage, and hearing loss. About one percent of patients have to have a second operation to have the small hole repaired. A lot of patients are afraid of anesthesia, but in fact, anesthesia today is extremely safe. I've been told that anesthesia is as safe as driving a car. After a brief stay in the recovery room, your child will be returned to you. You probably won't even have time to read the paper. We will give you ear drops immediately after surgery. You may use Ciprodex or Floxin; these are the only two brands that are FDA approved to put in a perforated ear. Swimming is permitted after myringotomy with tubes; I would recommend that you avoid intentionally dunking you child's head in dirty bath tube water, lakes, or rivers. After your tubes are placed, we want to see you back in about two weeks to check the position of the tubes and make sure that the ears are healthy, and we'll follow you every six months thereafter. Hopefully, putting tubes in your ears will restore your hearing and prevent those painful infections.

Radical Mastectomy is the removal of the breast and surrounding tissue. In most cases, mastectomy is required in order to remove cancerous tissue from the body. The extent of tissue removed is determined by the amount of cancer present in your body. A Radical Mastectomy is the most extensive form of breast cancer surgery. It calls for the complete removal not only of the breast, but also of the lymph nodes, as well as part or possibly all of the chest muscle that lies underneath the breast. Lymph nodes are small junctions that join the vessels that make up the lymphatic system. The lymphatic system circulates a bodily fluid called lymph in the same way that the circulatory system carries blood. Your doctor has recommended that you undergo a radical mastectomy because the cancer in your breast may have begun to move into the lymph nodes under your arm as well as into your chest muscle. This procedure may result in the loss of some muscle strength in the arm on the effected side of the body and will permanently change the outward shape and appearance of your chest. Before we introduce you to Sherri via video I want to take a moment and share why we chose her after doing a national search for the right person. First, let me start off by saying that Sherri does NOT have cancer. In her words, she would tell you that she does not have cancer YET. Also, Sherri does not have the BRAC 1 or 2 mutation; however, four maternal relatives were all diagnosed with cancer, most pre-menopausal with the exception being Sherri's mother who was diagnosed in her early 50's. When Sherri met with genetic counselors and received the results that she was not BRAC 1 or 2, the counselors shared with her that there is sure to be something going on in her family's DNA, it simply just has not been identified yet as a marker. The statistics that the counselors gave Sherri on the likelihood that she would develop breast or ovarian cancer at some point in her life were startling. Sherri had been considering a prophylactic mastectomy for some time now, but she will share with you in her own words why 2011 was going to be the year she took control of her own destiny. We are so proud to welcome Sherri to the M:TA Team and we are excited to tell you that we will be documenting through photos and videos the process of her double prophylactic mastectomies and immediate reconstruction. We have two main goals we want to accomplish by sharing Sherri's story. Our first goal is that we want our audience and those who have looked to M:TA for real process pictures of reconstruction to see how tissue expander reconstruction looks today. The process has a much different look than when we photographed my reconstruction six years ago. We will bring to our readers a new perspective on reconstruction from doctors and a patient's true, firsthand account of the journey, from start to finish. For me personally, it was a huge decision to go with Sherri as opposed to a woman who is facing reconstruction along with a cancer diagnosis. But our hope is that as our public charity grows, we will continue to get more and more sponsors who allow us to show many different stories of reconstruction. I am a part of the breast cancer sisterhood, whether I like it or not, and Sherri and I have both come to terms with the fact that while we shared similar surgeries, we have very different stories and outcomes. I am humbled that Sherri is willing to share her story through photographs and video for those women who, like her, don't yet have the disease, but live daily with the anxiety of getting it.
