April 28, 2013- Begins around 0:26. GI doc thinks this IS sandifer's, though he is not 100% (mostly because my son doesn't appear uncomfortable in the clip). Currently on 10mg of Prilosec once a day and may need to add Zantac at night. February 18, 2014- at 18 months we are still struggling to figure this out. Eating is a chore everyday. He still arches and hates being on his back. Fights me at every meal. He doesn't even care for cookies. Who doesn't like cookies?! He remains in the 3rd percentile for weight. An upper GI in November revealed an H Pylori infection and showed evidence of mild reflux and esophagitis (throat irritation). But what doesn't make sense is that he had been on Prilosec for the previous 6 months so why would it still be irritated if the Prilosec was working?!!! I had hoped H pylori was the culprit but of course it wasn't. He received antibiotics for two weeks and a recent stool sample confirmed it's gone, but his discomfort continued. For awhile I was giving him Zantac twice daily but after complaining to the GI doc once again we are trying Nexium. He's been on it 4 days now and no noticeable improvement. Will give it two weeks before stopping.

Pylori. وهو ما يُعرف اليوم بالعلاج المثلث المكون من المضادات الحيوية الثلاثة: ميترونيدازول (metronidazole)، اموكسيتسلين (amoxicillin)، كلاريتروميتسين (clarithromycin)، لمدة اسبوعين. العلاج بواسطة الأدوية المضادة لافراز أحماض المعدة (Antisecretory) والتي تساعد على التئام القرحة بشكل أفضل. من الجدير بالذكر ان تناول هذا النوع من الأدوية لهذه الفئة من المرضى (قرحة ناجمة عن تلوث بالجرثومة الملوية البوابية (H. Pylori) يتعلّق اذا كانت هذه القرحة كبيرة ومعقّدة من حيث معطياتها أو لا. لدى الأشخاص الذين يتناولون الأدوية من نوع (NSAID’s) كالاسبيرين وغيرها من مضادات الالتهاب اللاستيروئيدية على الطبيب المعالج أن يقرر ما اذا كانت هنالك حاجة لايقاف استعمال هذه الأدوية وذلك يتعلق بعدة عوامل كالزمن الذي مضى على استعمالها من قبل المريض، كبر الجرح وتعقيده، والخطورة مقابل الفائدة جراء توقف المريض عن استخدام هذا النوع من الدواء. 2- عدم وجود تلوث مثبت بالجرثونة الملوية البوابية: قبل كل شيء على الطبيب التأكد من عدم وجود التلوث بهذه الجرثومة بأكثر من فحص واحد. ففحصًا سلبيًا واحدًا لعدم وجود هذه الجرثومة غير كافٍ للتأكّد الفعلي. العلاج بواسطة الأدوية المضادة لافراز أحماض المعدة (Antisecretory) والتي تساعد على التئام القرحة بشكل أفضل. هنالك ثلاثة فئات رئيسية لهذه الأدوية وهي: مثبطات مضخة البروتون (proton pumb inhibitor): الأدوية المتوفرة في السوق لهذا النوع هي (omeprazole, esomeprazole, lansoprazole, dexlansoprazole, pantoprazole, rabeprazole,). محصرات مستقبلات الهيستامين من نوع (H2): الأدوية المتوفرة في السوق لهذا النوع هي (cimetidine, ranitidine, famotidine, and nizatidine). الادوية التي تحتوي على بروستاغلاندينات اصطناعية مثل: ميزوبروستول. (Misoprosol) القرحة العملاقةوفي هذه الحالة وبالاضافة الى جميع امكانيات العلاج التي طرحت سابقًا. يوصى في هذا النوع بالعلاج بواسطة مثبطات مضخة البروتون (proton pumb inhibitor) لفترة زمنية طويلة على مدار 12 اسبوع.بالإضافة إلى ضرورة اجراء تنظير داخلي لمراجعة مكان القرحة وذلك للخطورة الزائدة في هذه الانواع من القرحات لان تكون ورم خبيث. كذلك، يوصى باللجوء الى العلاج الجراحي لقرحة المعدة في حال التاكد من وجود اورام خبيثة او عند الاشتباه بوجود اورام خبيثة، بعد فشل العلاج المناسب والمتواصل في شفاء القرحة. متابعة المرضى بعد عملية العلاج: المرضى مع قرحة في الاثني عشر: عادة لا توجد حاجة لاجراء متابعة وفحص تنظير داخلي للاطمئنان على عملية شفاء القرحة والتئامها. الا في حالة عودة الأعراض الأولية التي كان يعاني منها المريض قبل تناول العلاج. المرضى مع قرحة في المعدة: هنالك عدة آراء طبية والتي تتناول متابعة المرضى من هذه الفئة وذلك لخطورة وجود مرض خبيث في منطقة القرحة ولهذا ينصح بعض الأطباء باجراء تنظير داخلي بعد فترة 6 اسابيع من تناول العلاج. عادة المرضى المطالبين بإعادةالفحص هم من تتوفر لديهم صفات القرحة التالية: قرحة مِعَدية دون استعمال دواء من مضادات الالتهاب اللاستيروئيدية مؤخرًا، وجود جرثومة الملوية البوابية H.
