There are mainly six types of diabetic foot complications like skin changes, neuropathy, calluses, foot ulcers, poor circulation, and amputation. 1. Pat Schulkins is one of the millions of Americans with type 2 diabetes. "Oh I probably have had it around 15 years," says Pat Schulkins. She monitors her glucose, but when it came to the state of her feet, Pat was unfeeling. About half of diabetics have this side effect called neuropathy. "These patients, especially when they get really bad, lose the ability to feel their feet. So they don't know when there's a problem," says Dr. Andrew Belis, foot and ankle surgeon on staff with Lee Memorial Health System. Diabetic neuropathy is nerve damage brought on by uncontrolled blood sugar. The feet and toes are most commonly affected. The lack of sensation increases the risk of serious problems when injuries and sores go unnoticed. That's exactly what happened to Schulkins. "It was like a big blister kind of, inward though. It wasn't sticking out like a blister does," says Schulkins. By the time she noticed the ulcer on her foot, it was almost too late to save. "The ulcer was there and it was chronic and resistant to care. And we got to change courses and address the underlining bone structure, so we basically reconstructed her foot so there's not these high pressure spots where the ulcer or the bone would break through the skin," says Dr. Belis. Experts stress the importance of knowing the symptoms, which may include, tingling, burning, pain or numbness in the feet; getting annual foot exams and wearing approved diabetic shoes if needed. "Even simple things like an ingrown nail can get infected; it can get down to the bone and can risk losing the toe. You or I notice a sore or even a small cut on your foot people with diabetes have less feeling they may not notice it until two or three weeks later. By that point it could have erupted into a full blown infection," says Dr. Belis. Although she managed to side step an amputation, Schulkins will never take her feet for granted. "And I know, I never thought it would happen," says Schulkins. Skin changes: Velvety dark skin, known as achantosis nigricans, can appear on the neck, groin and armpit. 9 Diabetes Pain Symptoms, You Should Not Ignore: 1. Increased urination, excessive thirst If you need to urinate frequently—particularly if you often have to get up at night to use the bathroom—it could be a symptom of diabetes. The kidneys kick into high gear to get rid of all that extra glucose in the blood, hence the urge to relieve yourself, sometimes several times during the night. The excessive thirst means your body is trying to replenish those lost fluids. 2. weight loss Overly high blood sugar levels can also cause rapid weight loss, say 10 to 20 pounds over two or three months—but this is not a healthy weight loss. Because the insulin hormone isn't getting glucose into the cells, where it can be used as energy, the body thinks it's starving and starts breaking down protein from the muscles as an alternate source of fuel. 3. hunger Excessive pangs of hunger, another sign of diabetes, can come from sharp peaks and lows in blood sugar levels. When blood sugar levels plummet, the body thinks it hasn't been fed and craves more of the glucose that cells need to function. 4. skin itch Itchy skin, perhaps the result of dry skin or poor circulation, can often be a warning sign of diabetes, as are other skin conditions, such as acanthosis nigricans. "This is a darkening of the skin around the neck or armpit area," Dr. Collazo-Clavell says. 5. Slow healing Infections, cuts, and bruises that don't heal quickly are another classic sign of diabetes. This usually happens because the blood vessels are being damaged by the excessive amounts of glucose traveling the veins and arteries. This makes it hard for blood—needed to facilitate healing—to reach different areas of the body. 6. Yeast infections "Diabetes is considered an immunosuppressed state," Dr. Collazo-Clavell explains. That means heightened susceptibility to a variety of infections, although the most common are yeast (candida) and other fungal infections, she says. Fungi and bacteria both thrive in sugar-rich environments. 7. irritability "When people have high blood sugar levels, depending on how long it's been, they can get used to chronically not feeling well," says Dr. Collazo-Clavell. "Sometimes that's what brings them into the office." Getting up to go to the bathroom several times during the night will make anyone tired, as will the extra effort your body is expending to compensate for its glucose deficiency. And being tired will make you irritable. 8. blurry vision Having distorted vision and seeing floaters or occasional flashes of light are a direct result of high blood sugar levels. The good news is that this symptom is reversible once blood sugar levels are returned to normal or near normal. But let your blood sugar go unchecked for long periods and the glucose will cause permanent damage, possibly even blindness. And that's not reversible. 9. Tingle Tingling and numbness in the hands and feet, along with burning pain or swelling, are signs that nerves are being damaged by diabetes. Still, as with vision, if blood sugar levels are allowed to run rampant for too long, neuropathy (nerve damage) will be permanent. "That's why we try to control blood sugar as quickly and as well as possible," she says. 10. blood test Several tests are used to check for diabetes, but a single test result is never enough on its own to diagnose diabetes (the test has to be repeated). One is the fasting plasma glucose test, which checks your blood sugar after a night (or eight hours) of not eating. Blood glucose above 126 milligrams per deciliter (mg/dL) on two occasions means you have diabetes. Having some of the signs of diabetes doesn’t mean you definitely have the condition, but you should always contact your GP, just to make sure. Type 2 diabetes can cause serious long-term health problems. It can be easy to miss as it develops slowly, especially in the early stages when it can be harder to spot the symptoms. If left untreated diabetes affects many major organs, including your heart, blood vessels, nerves, eyes and kidneys. Being diagnosed early and controlling your blood sugar levels can help prevent these complications. COMMON SYMPTOMS OF DIABETES: • Urinating more often than usual, particularly at night • Feeling very thirsty • Feeling very tired • Unexplained weight loss • Itching around the penis or vagina, or frequent episodes of thrush • Cuts or wounds that heal slowly • Blurred vision – caused by the lens of the eye becoming dry WHY DO WE GET THESE SYMPTOMS IN DIABETES: These symptoms occur because some or all of the glucose stays in the blood, and isn’t being used as fuel for energy. Pressure. Wearing tight shoes or sitting on your foot can give you a numb foot or leg or cause pins and needles. This kind of numbness has an obvious cause, gets better when the pressure is removed and doesn't cause any further problems. 2. Trapped nerve. Numbness or pins and needles can also be due to a trapped nerve. A slipped disc or back problem can put pressure on a nerve that travels from your back, down your leg and into your toes. A trapped nerve in the neck can also cause numbness or pins and needles anywhere from your neck, down your arms and into your fingers. Carpal tunnel syndrome is a trapped nerve at the wrist, giving you pins and needles and pain in the hand and loss of grip. 3. Diabetes. Diabetes can damage small blood vessels that supply nerves in fingers and toes. This can cause pins and needles, pain or numbness in the hands and feet (peripheral neuropathy). Being unable to feel anything in your hands and feet can be dangerous, as you may stumble, drop things or not realise when you are touching something hot. 4. Injury. Damage to nerve endings in fingers or toes can be the result of an injury. People who use vibrating tools a lot may also develop nerve damage and may experience pins and needles. 5. Medicines. Some medicines can cause nerve damage. It is usually reversible when the medicine is stopped. The medicines include some chemotherapy medicines used to treat breast cancer and lymphoma, antiretrovirals used to treat HIV/AIDS, and the antibiotic metronidazole. 6. Diseases that damage nerves. Many conditions can damage the nervous system and cause areas of numbness or pins and needles. These include stroke, multiple sclerosis and brain tumours. These conditions are serious but relatively rare and will all cause other symptoms in addition to the numbness/pins and needles. 7. Vitamin deficiency. Vitamin B12 deficiency is common among very elderly people, vegans and people with a condition called pernicious anaemia. The lack of vitamin B12 causes anaemia and nerve damage 8. Alcoholism. Alcoholics are more likely to have a thiamine or other important vitamin deficiencies because of poor dietary habits, a common cause of peripheral neuropathy. It's also possible that alcoholism itself can cause nerve damage, a condition that some researchers call alcoholic neuropathy. Treatments for Tingling Hands and Feet. Successful treatment depends on an accurate diagnosis and treatment of the underlying cause of the tingling. As long as the peripheral nerve cells have not been killed, they have the ability to regenerate. Although no treatments are available for inherited types of peripheral neuropathy, many of the acquired types can be improved with treatment. For example, good blood sugar control in diabetes can slow the progression of diabetic neuropathy; vitamin supplementation can correct peripheral neuropathy in people with vitamin deficiencies. General lifestyle recommendations include maintaining an optimal weight, avoiding exposure to toxins, following a doctor-supervised exercise program, eating a balanced diet, and avoiding or limiting alcohol consumption. Recommendations also include quitting smoking, which constricts blood supply to blood vessels supplying nutrients to peripheral nerves. In some cases, tingling and other symptoms of peripheral neuropathy may be reduced with prescriptions originally developed for treating seizures and depression. Like and share this video. Well, those of us with diabetes probably do know that there are 2 or maybe 3 depending how you want to classify it, types of diabetes. Some will call prediabetes a form of diabetes, and I guess that is true, kind of like pre-school is kind of like school. Then there are Type 1 and Type 2 diabetes. And don’t forget about gestational diabetes. We will talk briefly about that one at the end. In this video, I will attempt to give you the similarities and differences in each as well some possible causes. Diabetes affects approximately 30 million Americans today. That is nearly 10% of the population. Diabetes is a disorder of the endocrine system. With diabetes, blood sugar levels stay high because either the pancreas can no longer produce enough insulin or the cells of the body are resistant to insulin and the pancreas can’t keep up. Either way, the glucose or sugar level in the blood stream becomes too high and the body cannot function properly. Pre-Diabetes In general, people who have a fasting plasma blood glucose in the 100-125 mg/dl range are defined as having impaired fasting glucose or pre-diabetes. Like many diseases, early detection can be very good. Talk to your doctor about testing for pre-diabetes. You may be able to prevent type 2 diabetes Type 1 Diabetes This used to be called juvenile diabetes or insulin-dependent diabetes. Type 1 diabetes occurs because the insulin-producing cells of the pancreas are actually destroyed by the immune system. Type 1 diabetes is actually considered an autoimmune disease. Its onset has nothing to do with diet or lifestyle. There is nothing you can do to prevent Type 1, and—at present—nothing you can do to get rid of it. Type 1 diabetes most commonly starts in people under the age of 20, but may occur at any age. According to the Mayo Clinic, the most common symptoms of Type 1 are: Increased thirst Frequent urination Bedwetting in children who previously didn't wet the bed during the night Extreme hunger Unintended weight loss Irritability and other mood changes Fatigue and weakness Blurred vision In females, a vaginal yeast infection There is no cure for Type 1 and it cannot be reversed. To determine diabetes, a blood sample will be taken after an overnight fast. A fasting blood sugar level less than 100 is normal. A fasting blood sugar level from 100 to 125 is considered prediabetes. If it's 126 or higher on two separate tests, you have diabetes. If you're diagnosed with diabetes, the doctor may do other tests to distinguish between type 1 and type 2 diabetes — since the two conditions often require different treatments. Type 2 Diabetes According to Web MD, with type 2 diabetes, the body continues to produce insulin, although insulin production by the body may significantly decrease over time. The pancreas produces either not enough insulin, or the body is unable to recognize insulin and use it properly. When there isn't enough insulin or the insulin is not used as it should be, glucose can't get into the body's cells to be used as energy. This glucose then builds up in the blood. This condition was once known adult-onset or noninsulin-dependent diabetes. More common in adults, type 2 diabetes increasingly affects children as childhood obesity increases. There's no cure for type 2 diabetes, but you may be able to manage the condition by eating well, exercising and maintaining a healthy weight. If diet and exercise aren't enough to manage your blood sugar well, you also may need diabetes medications or insulin therapy. The symptoms for Type 2 are basically the same as for Type 1 with a few additions: Slow-healing sores or cuts Itching of the skin (usually in the vaginal or groin area) Recent weight gain Numbness or tingling of the hands and feet Impotence or erectile dysfunction A quick word about gestational diabetes, Hormone changes during pregnancy can affect insulin's ability to work properly. The condition, called gestational diabetes, occurs in about 4% of all pregnancies. GB is tested for during the pregnancy and is treated if present. Dr. Ebraheim’s educational animated video describes Meralgia Paresthetica. Meralgia paresthetica is a pain or burning sensation felt over the anterolateral aspect of the thigh usually caused by compression of the lateral femoral cutaneous nerve (LFCN). Wearing super-tight jeans may cause this compression of the LFCN. Usually the patient complains of tingling, numbness, and pain as well as sensitivity to the touch in the area of the skin innervated by that nerve (the outer part of the thigh). There will be hypersensitivity over the anterolateral aspect of the thigh and the patient will sometimes notice this sensitivity while placing their hands of car keys inside the pocket. Causes of Meralgia Paresthetica are tight pants, belt, corset braces, obesity and weight gain, pregnancy, local trauma and diabetes. Injury is usually caused by compression as the nerve passes under the inguinal ligament. Anatomy of the LFCN The lateral femoral cutaneous nerve of the thigh arises from the branches of L2 and L3. The variations in the course of the LFCN in the area of the ASIS can cause entrapment of the nerve near the iliac crest causing Meralgia Paresthetica. The LFCN can become injured during harvesting anterior iliac crest bone graft, illioinguinal approach for acetabulum fixation, application of external fixator of the pelvis, total hip replacement by anterior approach or Smith Peterson approach. The LFCN usually passes under the inguinal ligament approximately 2 cm medial to the ASIS. Once outside the pelvis, the nerve splits and pierces the fascia, running over the lateral aspect of the thigh in the subcutaneous region and lies superficial to the Sartorius muscle. The course of the LFCM in the area of the ASIS is variable: under ligament, over ligament, over & under ligament and over iliac crest. These variations can cause entrapments of the nerve near the iliac crest causinf meralgia paresthetica. Recognizing the variability and the relationship of this nerve and its branches is important to avoid injury to the nerve. It is difficult to establish a safe zone for the nerve during surgical approaches to the acetabulum or the proximal femur. Disc herniation that affects the L2-L3 nerve roots can also cause Meralgia Paresthetica. Consider spine pathology when dealing with Meralgia Paresthetica. Signs of Meralgia Paresthetica include burning, tingling sensation, hypersensitivity to heat where warm water feels like it is burning the skin. The condition is usually diagnosed by the patient describing the symptoms in the area of the thigh. Some people say it is an elusive or obscure diagnosis for many physicians and can be missed or overlooked. It can lead to a significant disability if missed. The condition could be mistaken for other disorders. Physicians may think it is a hip, groin, abdominal or iliotibial band problem. This can cause misdiagnosis and may lead to other unnecessary treatment given to the patient. Treatment •physical therapy •anti-inflammatory medications •weight loss •strengthening the abdominal muscles •avoid wearing tight clothing and tight bands around the pelvis. •Steroid injection: Ultrasound guidance for steroid injection is better than blind injection. Injection can be diagnostic and therapeutic aiding in the diagnosis. •Surgery is done only in severe cases. Surgery is a last resort in order to decompress the nerve. So, kidneys work overtime to filter and absorb excess sugar. • Excess sugar is excreted through urine and this triggers to frequent urination. • Diabetic patient experience disturb sleep due to polyuria. 2) Excessive thrust : • It is also called polydipsia. • Excessive thrust accompanied by excessive dryness of mouth. • In diabetes, kidney loses the ability to pull out glucose from the water.
