TRANSCRIPTION: Hi my name is Karyn. I am 41 and about five years ago I had a complete hysterectomy. Since then I had a bad tumble. About six months ago I came into contact with Dr. Shel and it was the best decision that I could have made. Whenever I saw Dr. Shel for the first time I was physically and mentally exhausted. I wasn't sleeping at night. I couldn't handle the stresses of family and work. I would lose my temper very easily with my family, with my co-workers, and it just wasn't me. So, I had gone to my gynacologist to get reevaluated to see if an adjustment of my hormones needed to be to be made. I went there, I got some sort of adjustment and it just wasn't working. If it is done at a younger age, the hysterectomy along with the ovaries they will have severe menopausal symptoms like osteoporosis or hot flushes, and maybe myocardial infarction. So it is best to start the hormonal replacement therapy immediately after the surgery. But if the ovaries are retained they will have some hormones left. So they will not require hormone replacement therapy but at a later stage if they have symptoms. At a later stage if the hysterectomy is done and the ovaries are removed, depending upon the symptoms we can start and empirically also people do keep the patients on HRT to prevent the future myocardial infarction.

http://PreOp.com - Patient Education Company Your doctor has recommended that you have a hysterectomy with the removal of the ovaries and fallopian tubes. But what does that actually mean? Hysterectomy is the removal of the uterus - the organ that holds and protects the fetus during pregnancy. Hysterectomy often also involves the removal of other parts of the reproductive system, including the ovaries - where eggs are produced - the fallopian tubes which carry the eggs to the uterus and the cervix - or neck of the uterus. There are many different reasons why a doctor may recommend this kind of surgery. Patient Education In many cases, disease or the growth of abnormal tissue will lead a doctor to recommend the removal of the uterus, the ovaries and fallopian tubes. In some cases, unusually heavy menstrual flow and the accompanying discomfort may make hysterectomy an important treatment option for patient and physician to consider. But no matter what the reason behind it, you should be aware that the removal of the uterus and other reproductive organs is a serious step and it can mean significant changes in your life. Patient Education After having a hysterectomy, you will not be able to have children and if because your ovaries are removed as part of the procedure, you may even need to take medication to replace hormones that your body once produced on its own. So make sure that you ask your doctor to carefully explain the reasons behind this recommendation. Your Procedure: On the day of your operation, you will be asked to put on a surgical gown. You may receive a sedative by mouth and an intravenous line may be put in. You will then be transferred to the operating table. In the operating room, a nurse will begin preparation by clipping or shaving the abdomen. The anesthesiologist will begin to administer anesthesia - most probably general anesthesia. The surgeon will then apply an antiseptic solution to the skin place a sterile drape around the operative site... After allowing a few minutes for the anesthetic to take affect your doctor will decide whether to make a vertical or horizontal incision. An incision is made cutting through the skin and muscle of the abdomen. Next, the surgeon will inspect the general condition of the abdominal organs. Once the ovaries are exposed the uterus can then be separated from the bladder. All arteries, veins and ligaments connected to the uterus, ovaries and fallopian tubes are tied off and cut. Now the uterus can be pulled upward. This stretches the vagina - allowing the surgeon to cut the uterus free at the cervix. The surgeon closes the top of the vagina with stitches, and provides added support by attaching the ligaments that once held the uterus, ovaries and fallopian tubes in place. The incision is then closed and a drainage tube may be left inserted at the site. Finally, a sterile bandage is applied. Patient Education Company

Pelvic floor exercises can be performed daily and kept within the range of personal comfort - avoid overdoing your exercises especially when starting out. If you experience any discomfort during or after your pelvic floor exercises after hysterectomy stop and let your body heal before recommencing. If any pelvic floor discomfort persists have this assessed by your doctor. Sitting Upright Pelvic Floor Exercises Doing upright seated pelvic floor exercises after hysterectomy helps to encourage the pelvic floor muscles to lift upwards against the downward force of gravity. Upright exercises can be used as a progression position from lying down pelvic floor exercises. * Sit upright on a comfortable chair ideally a dining chair rather than a soft lounge chair. * Check you have the correct posture for pelvic floor exercises: your spine should be lengthened and you should have the inward curve in your lower back. * Feel your pelvic floor openings against the surface of the chair. * Squeeze and lifting in and around your 3 pelvic openings for a few seconds. * Relax your pelvic floor back to resting position and breathe into your belly. * Repeat 1-3 exercises in a row within your range of comfort. * Try to avoid the mistakes listed above when doing seated pelvic floor exercises. Progressing Your Pelvic Floor Exercises After Hysterectomy Pelvic floor muscles become stronger when challenged to work harder. This challenge needs to be applied gradually over time as your body heals after surgery. Techniques to progress pelvic floor strengthening: * Doing more exercises up to 8-12 consecutive exercises (up to 3 times daily) * Holding for longer during each exercise up to 10 seconds at a time * Contracting your pelvic floor muscles strongly * Doing your exercises standing upright * Decreasing the rest between exercises * Contracting your pelvic floor to hold up against the downward force of a cough or sneeze. It change where fat is stored on your body (i.e. more gets deposited in your midsection and less in your lower body). 2. It changes where estrogen is produced. The ovaries no longer make estrogen, but your fat cells do, so higher body fat% is not frowned upon by your post-menopausal body. Cortisol is another hormone that is not automatically thought about with menopause, but it contributes to belly fat. Think of cortisol as the stress hormone, physical or emotional stress, even a lack of sleep causes an increased release of cortisol. Bottom Line: you have to work with your body during and after menopause to get it to do what you want. The trick is to starve your fat cells without starving yourself.
