Divalproex Medication Information (dosing, side effects, patient counseling)
Your medication Divalproex Divalproex sodium is also known by the brand name: Depakote, Depakote ER and Depakote Sprinkle capsules. Divalproex sodium comes in 125, 250, and 500 mg tablets, 250 and 500mg extended release tablets, and 125mg sprinkle capsule. Divalproex sodium is most commonly used to treat seizures, bipolar problems, and prevent migraine headaches. No matter what your doctor has you taking it for, divalproex sodium is taken by mouth with or without food 1 to 4 times a day. If upset stomach occurs, take divalproex sodium with food. If you miss a dose, take the missed dose as soon as you think about it. If it is close to time for your next dose, skip the missed dose and resume your normal dosing schedule. Do not take 2 doses at the same time or extra doses. For best results take each dose at the same time each day. Extended-release tablets must be swallowed whole, do not split or break tablets. Divalproex Sprinkle capsules may be swallowed whole or the capsule may be opened and sprinkled on small amount (1 teaspoonful) of soft food like pudding, or applesauce. This is to be used immediately (do not store or chew). Some common side effects with divalproex sodium include headache, nausea, vomiting, diarrhea, tiredness, weakness, tremor, dizziness, blurry eye sight, and loss of hunger. Some less common side effects include swelling of the arms or legs, high or low blood pressure, weight gain, and constipation. Do not take divalproex sodium if: you are pregnant, have liver problems or under the age of 2 years. This medication should not be abruptly discontinued because of the possibility of increasing seizure frequency. While taking divalproex sodium, remember it is important to tell your doctor or pharmacist if: • You have signs of an allergic reaction (rash, itching, swelling, tightness of the chest, etc.) • You have dark urine, are feeling tired, are not hungry, have an upset stomach, are throwing up, or have yellowing of the skin or eyes. • Are pregnant as this drug may cause very bad birth defects. If you are or become pregnant and take this drug for seizures or bipolar disorder, talk to your doctor to see if you need to keep taking this drug. All forms of divalproex sodium including the liquid needs to be stored at room temperature in a dry place away from children or pets.

The risk of medication misadventure is often higher with psychotropic drugs. Health professionals need to remain vigilant with all patients taking psychotropics and be aware of the signs and symptoms requiring investigation. This program looks at the safe use of commonly prescribed anti-depressants, mood stabilisers, anxiolytics, hypnotics and antipsychotic drugs. It examines issues related to their toxicity, interactions, overdose and adverse effects. It deals with questions of compliance and consent, failure to respond to treatment, monitoring, management and contra-indication. The program discusses the identification and reporting of adverse drug reactions as well as their management and prevention. Patients are vulnerable to major adverse outcomes after hospital stay. Readmissions are common: nearly one in five adults aged more than 65 is readmitted to hospital within 30 days of discharge. Death is also common in this first month, during which rates of post-discharge mortality may exceed initial inpatient mortality. The range of illnesses to which patients are susceptible is extremely broad. This period of heightened and generalized vulnerability to a broad spectrum of conditions has been called the post-hospital syndrome. In this research Dharmarajan et al set out to characterize the absolute risks for older patients of readmission to hospital and death in the year after hospitalization for heart failure, acute myocardial infarction, or pneumonia. They concluded that risk declines slowly for older patients after hospitalization for heart failure, acute myocardial infarction, or pneumonia and is increased for months. Specific risk trajectories vary by discharge diagnosis and outcome. Patients should remain vigilant for deterioration in health for an extended time after discharge. Health providers can use knowledge of absolute risks and their changes over time to better align interventions designed to reduce adverse outcomes after discharge with the highest risk periods for patients. CT-Scan - no evidence that CT Scan improves outcome DETERMINE PATHOGEN OF PNEUMONIA Outpatient not necessary unless critical agent such as legionella, influenza, MERS-COV, CA-MRSA, Bioterrorism. Severe Pneumonia or ICU patient perform sputum, blood culture, urine antigen test, PCR, Serology, ELISA, Immunofluorescence. Biomarkers procalcitonin helps determine for use of anti-biotics. CRP greater than 40mg/L suggest bacterial. Blood cultures are difficult to get true positive result. S. Pneumonia and H. Influenza have a higher false neg becuase grow easier, while S. aurues and GNB has a high false positive. Shoudl have greater than 25 neutrophils and less that 10 squamous epithelium. MANAGEMENT OF PNEUMONIA PNEUMONIA SEVERITY INDEX - rarely used CURB-65- Confusion, Uremia, Respiratory Rate, BP less than 90 and age over 65. ICU if septic shock, mechanical ventilation requirements. ANTI-BIOTICS Outpatient - Macrolide or doxycycline Comorbidities - add flouroquinolone or Betalactam with macrolide After pathogen identification than base on sensitivity. Admission - Flouroquinolon with B lactam and macrolide ICU Admissions - Anti-pneumooccal B-lactam with Azithromycin and Flouroquinolone Psuedmonas - Pipericillin tazobactam, cefipem, imepinem, meropenem and Aminoglycosdie, azithromycin or AMinoglycoside Floroquinolone. Chronic Prostatitis And Candida
Thanks for joining my video. I've had quite a few YouTube requests for some more videos on Balanitis and Prostatitis. Prostatitis is something I've seen now for a long time in a lot of my male patients. It's actually quite a serious problem, and I think a lot of doctors and urologists underplay prostatitis in terms of the connection with a yeast infection. Many of them, in fact, don't even believe that males can have a yeast infection in the prostate, which I find quite hard to believe. If you've had recurrent antibiotics like many different rounds of antibiotics. If you've had many visits to the urologist and basically nothing comes up or the urologist is sick and tired of seeing you or keeps giving you different drugs and you're not getting your resolution. Or if you've got some of these signs and symptoms that I'm about to read out to you, there's a big chance that you could well have a serious prostatic problem in terms of Candida. Some of these problems here, I'm just going to read them out to you. Pain on or difficulty with urination, blood in the urine, joint pain often moving from joint to joint, unexplained skin rashes, increased food intolerances or allergies, poor sleeping, dry irritated eyes, reduced amount of urine, problems with ejaculation, painful ejaculation, ejaculating where there's blood in the semen, having no interest in sex or pleasure in sex anymore. This is common also in prostatitis. Pain down the sides of the legs. There will be pain down here or around the ball sac, pardon the expression, around the testicle area. And I know that if he doesn't drink beer, if he switches over to vodka or gin, he has less problem with his prostate than if he drinks plenty of beer. But any alcohol consumption is not a good idea with a yeast infection. As I mentioned, reduced sexual desire, bronchial cough, asthma, problems with sinus, nosebleeds, palpitations, irritable bowel, cold hands, cold feet, sugar cravings, panic attacks, headaches, the list goes on and on and on. There are so many different signs and symptoms of chronic prostatitis. What do you do if you've got chronic prostatitis? Well, if your doctor is giving you no joy at all, and the urologist is looking at you like you're talking some kind of a weird language; it's time for you to have a good think about your case history yourself. Do you have a past history of taking antibiotics? Have you been doctor hopping from one doctor or naturopath to another without any result? Do you have all these concomitant symptoms? By concomitant, I mean do you have all these extra symptoms apart from that pain, burping, bloating, farting, sugar cravings, sleeping problems, sweating. Each hour in Australia, more than five men die from conditions that are potentially preventable. Australian men experience higher rates of morbidity and mortality than women across many significant health outcome indicators including mental health, cardiovascular disease, cancer and accident and injury. Despite this fact, men tend to have fewer visits to doctors and seek medical assistance at much later stages in their illnesses. The result is that for men the health outcomes are much worse and interventions are more costly and intensive. Even though there is a wide variation amongst the Australian male population, gender is a significant risk factor for premature death across all socioeconomic groups of males but more so in the most disadvantaged. The risk factors for chronic disease such as smoking, poor diet, physical inactivity, overweight and obesity and alcohol and other drug use remain high among men. The situation for Indigenous men is far worse than for the rest of the Australian male population with life expectancy for Indigenous males is estimated to be approximately 11.5 years less than the average for all Australian males. In rural and remote areas men often have limited access to health services as well as recreational and support facilities. Work for rural men is often physically demanding, hazardous and isolating. Suicide rates for males in the farming community are twice the national average. Cultural factors and beliefs are also part of that equation. This program looks at preventative approaches in improving men's health and the need to make health services more attractive to men. It considers the barriers to engaging men in their own health care and the need for health professionals to develop more appropriate communication processes and interventions. This sexually transmitted disease (STD) is often asymptomatic, but if left untreated, it can lead to long-term health consequences, including chronic pelvic pain, ectopic pregnancy, and infertility. Gonorrhea can also increase the risk of contracting and transmitting HIV. Cephalosporins are currently recommended to treat gonorrhea in the United States. For the past few years, providers have used combination therapy with either cefixime, an oral cephalosporin, or ceftriaxone, an injectable cephalosporin, plus a second antibiotic, to treat this common STD. Recent laboratory data suggest that the effectiveness of cefixime for treating gonorrhea may be declining. For this reason, CDC has updated its gonorrhea treatment guidelines and no longer recommends the routine use of cefixime. Instead, CDC now recommends using ceftriaxone along with a second antibiotic to treat gonorrheaFor patients with uncomplicated genital, rectal, and pharyngeal gonorrhea, CDC now recommends combination therapy with ceftriaxone 250 mg as a single intramuscular dose, plus either azithromycin 1 g orally in a single dose or doxycycline 100 mg orally twice daily for 7 days. There are times, however, when it may be necessary to use an alternative antibiotic regimen that does not include ceftriaxone. In instances where ceftriaxone is not available, CDC recommends cefixime 400 mg orally, plus either azithromycin 1 g orally or doxycycline 100 mg orally twice daily for 7 days. For patients with a severe allergy to cephalosporins, CDC recommends a single 2-g dose of azithromycin orally. In both of these circumstances, CDC recommends a test of cure for these patients 1 week after treatment, and this is an important change in CDC's treatment guidelines. In addition to these treatment changes, we encourage physicians to take 2 additional steps to ensure successful treatment outcomes for their patients. First, monitor your patient for treatment failure. Patients who have persistent symptoms after treatment should be retested by culture. If these cultures are positive for the gonococcus, isolates should be submitted for resistance testing. A test of cure should be conducted 1 week after retreatment. Providers should also ensure that the patient's sex partners from the preceding 60 days are promptly evaluated and treated. Second, report suspected treatment failures. Any suspected treatment failure should be reported to CDC through local or state public health officials within 24 hours. It is important to know that a single 250-mg injection of ceftriaxone is effective in treating gonorrhea at all anatomic sites. There are no clinical data to support the use of higher doses of ceftriaxone. Also, the use of azithromycin as the second antibiotic used in combination therapy may be preferable to the use of doxycycline, for 2 reasons. First, azithromycin is taken as a single pill, which is easier and more convenient for a patient. And secondly, there was a substantially higher prevalence of gonococcal resistance to tetracycline than to azithromycin among the isolates studied for this MMWR. The changes in the gonorrhea treatment guidelines were prompted by laboratory data from CDC's Gonococcal Isolate Surveillance Project (GISP). These data demonstrated recent significant increases in the percentage of Neisseria gonorrhoeae isolates with elevated cefixime minimum inhibitory concentrations (MICs). The percentage of isolates with elevated cefixime MICs increased from 0.1% in 2006 to 1.7% in 2011, a 17-fold increase. Gonorrhoea is the second most common Sexual Transmitted Disease in UK for both heterosexual and homosexual Gonorrhoea Sexual Transmitted Disease always occurs by Sex (Oral, Vaginal, and Anal Sex) Gonorrhoea can be a cause of Infertility for both Men and Women therefore should be treated properly as soon as possible . Gonorrhoea should be treated by a Gynaecologist with interest of Infertility and sexual health and sexual transmitted diseases, should the treatment to be carried out by GUM Centres,, patient must be followed up by Infertility Gynaecologist to check if any damages has occurred. Gonorrhoea should not be treated by Pharmacist, GP, or on line medications. Gonorrhoea should be treated with a maximum care with careful history to identify the parties involved in sexual activities and all should be investigated and treated at the same time if possible. Women with discharges following sexual activities must be seen and Gonorrhoea must be excluded in almost most of the cases. Women diagnosed with Gonorrhoea must have her partner or partners treated and with men if they were diagnosed with Gonorrhoea same will apply for all partners exposed to sexual activities by him. Dame Sally Davies has written to all GPs and pharmacies to ensure they are prescribing the correct drugs after the rise of "super-gonorrhoea" in Leeds. Her warning comes after concerns were raised that some patients were not getting both of the antibiotics needed to clear the infection. Sexual health doctors said gonorrhoea was "rapidly" developing resistance. A highly drug-resistant strain of gonorrhoea was detected in the north of England in March. That strain is able to shrug off the antibiotic azithromycin, which is normally used alongside another drug, ceftriaxone. In her letter, the chief medical officer said: "Gonorrhoea is at risk of becoming an untreatable disease due to the continuing emergence of antimicrobial resistance." Soaring cases But while an injection of ceftriaxone and an azithromycin pill are supposed to be used in combination, this may not always be the case for all patients. Earlier this year, the British Association for Sexual Health and HIV (BASHH) warned that some online pharmacies were offering only oral medication. Using just one of the two drugs makes it easier for the bacterium to develop resistance. The letter, which is also signed by chief pharmaceutical officer Dr Keith Ridge, stated: "Gonorrhoea has rapidly acquired resistance to new antibiotics, leaving few alternatives to the current recommendations. Untreated infection can lead to infertility, pelvic inflammatory disease and can be passed on to a child during pregnancy. Gonorrhoea is the second most common sexually transmitted infection in England and cases are soaring. The number of infections increased by 19% from 29,419 in 2013 to 34,958 the following year. Dr Jan Clarke, the president of BASHH, told the BBC News website: "We're really pleased that the chief medical officer has stressed that gonorrhoea needs this approach to treatment due to the rapid development of resistance. "We need to protect what we've got and we need to encourage pharmacists and general practitioners to follow first-line treatment." Dr Andrew Lee, from Public Health England, added: "Investigations are ongoing into a number of cases of anti-microbial resistant gonorrhoea. Cocooning against Pertussis (Whooping Cough) on the rise - Health Watch
As the incidence of pertussis increases amongst infants nationally, statewide and locally, the Virginia Department of Health would like to advise residents on ways they can protect this vulnerable population. Some 80% to 85% of the pertussis sources are family members and caretakers. The rate of pertussis infection in infants too young to be fully immunized (6 months of age and younger) has increased by as much as 20 times. There were 27,550 cases of pertussis in the United States in 2010. In Virginia alone, there were 344 cases. These numbers are expected to increase in 2011. To help combat this growing issue, Hampton Roads health districts are implementing a targeted immunization strategy called "cocooning." A cocoon is a nurturing environment that envelopes and protects new life. Similarly, members of local communities are encouraged to create a "cocoon" of protection, by ensuring that a booster dose of the tetanus, diphtheria, acellular pertussis (Tdap) vaccine is given to postpartum women, contacts/caregivers of infants younger than 1 year, and health care providers, especially those who treat infants less than 1 year of age. Why is cocooning necessary?  91% of the deaths from pertussis in the last decade are in infants, 6 months of age or less.  More than 50% of infants, less than 1 year of age, who get pertussis must be hospitalized.  Coughing fits due to pertussis infection can last for up to 10 weeks or more, with the disease being communicable in the first 3 weeks.  Less than 10% of adults have protection from the disease and 4 out of 5 adults with the disease are capable of spreading the germ and have no symptoms. Most adults don't seek medical treatment and/or not diagnosed.  A 2007 study published in the Pediatric Infectious Disease Journal, reports that 44% of the source of pertussis infection in infants was a parent, sibling or grandparent. Other caregivers (i.e., daycare providers etc.) are included in the remaining 56%. It can be given even if a recent tetanus and diphtheria (Td) shot has been received.
