Community Acquired Pneumonia Pneumonia is defined as infection of Lung Parenchyma. CLASSIFICATION OF PNEUMONIA Community Acquired Pneumonia (CAP) Hospital Acquired Pneumonia - During or after hospital stay or associated with ventilator. These types of pneumonia tend to have organisms that are much more resistant. Healthcare Associated Pneumonia are patient in nursing home, long term facility as well as patient in acute care hospital for more than 2 days in the past 90 days, IV therapy, woundcare and IV chemo wihtin the last 30 dyas and hemodialysis within the last 30 days. PATHOGENESIS OF COMMUNITY ACQUIRED PNEUMONIA Most common cause is microaspiration which overhwelm host defense, hematogenous, direct spread, and macroaspiration (Decrease gag, cough reflex) may all lead to pneumonia. MICROBIOLOGY OF PNEUMONIA TYPICAL Pneumonia tends to be more severe type of pneumonia. There tends to be more consolidation and mucupurulent sputum. Classic bacteria that causes pneumonia is Streptococcus pneumonia, Staphylococcus aureus, Klebsiella Pneumonia, Psueodmonas Aeroginosa, Hemophilus Influenza and Moraxella Catarrhalis. ATYPICAL Pneumonia is less severe and also referred to as walking pneumonia and invades the epithelial cells and does not affect the alveoli wall. There is patchy diffuse and decrease sputum. Extrapulmonary symptoms is also common such as myalgia bradycardia splenomegaly, rash and CNS Symptoms. Non zoonotic causes include Mycoplasma, chalmydia pneumonia and Legionella which are all intracelular. Zoonotic is psitococas, Q Fever, and Tularemia. MOre commonly viruses such as influenza, parainfluenza and RSV and adenovirus. RISK FACTORS FOR PNEUMONIA Alcoholism - S. Pneumonia, oral anerobes, Klebsiella pneumonia and TB COPD/Smoking - S. Pneumonia, H. Influenza, P. Aeroginosa, Moraxella, Legionalla Lung Abscess - CA-MRSA, Oral Anaerobes, TB Fungal Structural Disease - Psuedomonas, S. Aureus, Burklholderia Influenza - S. Pnuemonia, S.A ureus IV Drug Abusers - Psuedomonas, Anaerobes, S. PNeumonia TB Farm Animals - Coxxiel Burnetti Birds - Chlamydia Rabbits - Tularemia Bat-Bird - Histoplasma Capsilatuma HIV Early - S. pneumonia, H. Influeanza, TB Southwestern US - coccidioides Hanta virus Hotel/Cruise Ships - Legionella Bioterrorism - B. Anthracis, Yersinia Pestis, Tularemia Aspiration - Gram neg, Oral anaerobes Predisposing - hypoxemia, acidosis, toxic inhalation, pumonary embolism, uremia, malnutrition, lung cancer, asthma, AIDS, smoking Age greater than 70 Fungal - Aspergillus, Cryptococcus, Histoplasma, Coccidiomycosis EVALUATION Symptoms - mucupurulent cough or scanty watery cough, Fever, pleuritic chest pain, Nausea and vomiting, Mental status changes, myalgias rash splenomegaly. Signs - tachycardia, RR more than 24 Auscultation audible crackles. Consolidation - dullness to percussion, Bronchial breathing increase vocal fremitus, Egophany Labs - leukocytosis leftward shift, Anemia X-Ray shows consolidation in lobar pneumonia and interstitial pneumonia shows a much more diffuse reticular picture. CT-Scan - no evidence that CT Scan improves outcome DETERMINE PATHOGEN OF PNEUMONIA Outpatient not necessary unless critical agent such as legionella, influenza, MERS-COV, CA-MRSA, Bioterrorism. Severe Pneumonia or ICU patient perform sputum, blood culture, urine antigen test, PCR, Serology, ELISA, Immunofluorescence. Biomarkers procalcitonin helps determine for use of anti-biotics. CRP greater than 40mg/L suggest bacterial. Blood cultures are difficult to get true positive result. S. Pneumonia and H. Influenza have a higher false neg becuase grow easier, while S. aurues and GNB has a high false positive. Shoudl have greater than 25 neutrophils and less that 10 squamous epithelium. MANAGEMENT OF PNEUMONIA PNEUMONIA SEVERITY INDEX - rarely used CURB-65- Confusion, Uremia, Respiratory Rate, BP less than 90 and age over 65. ICU if septic shock, mechanical ventilation requirements. ANTI-BIOTICS Outpatient - Macrolide or doxycycline Comorbidities - add flouroquinolone or Betalactam with macrolide After pathogen identification than base on sensitivity. Admission - Flouroquinolon with B lactam and macrolide ICU Admissions - Anti-pneumooccal B-lactam with Azithromycin and Flouroquinolone Psuedmonas - Pipericillin tazobactam, cefipem, imepinem, meropenem and Aminoglycosdie, azithromycin or AMinoglycoside Floroquinolone. CA-MRSA - Add vancomycin or Lenezolid ADJUVANT THERAPY - Glucocorticoid, Decrease inflammatory response to pneumonia

Chronic Prostatitis And Candida

Thanks for joining my video. I've had quite a few YouTube requests for some more videos on Balanitis and Prostatitis. Prostatitis is something I've seen now for a long time in a lot of my male patients. It's actually quite a serious problem, and I think a lot of doctors and urologists underplay prostatitis in terms of the connection with a yeast infection. Many of them, in fact, don't even believe that males can have a yeast infection in the prostate, which I find quite hard to believe. If you've had recurrent antibiotics like many different rounds of antibiotics. If you've had many visits to the urologist and basically nothing comes up or the urologist is sick and tired of seeing you or keeps giving you different drugs and you're not getting your resolution. Or if you've got some of these signs and symptoms that I'm about to read out to you, there's a big chance that you could well have a serious prostatic problem in terms of Candida. Some of these problems here, I'm just going to read them out to you. Pain on or difficulty with urination, blood in the urine, joint pain often moving from joint to joint, unexplained skin rashes, increased food intolerances or allergies, poor sleeping, dry irritated eyes, reduced amount of urine, problems with ejaculation, painful ejaculation, ejaculating where there's blood in the semen, having no interest in sex or pleasure in sex anymore. This is common also in prostatitis. Pain down the sides of the legs. There will be pain down here or around the ball sac, pardon the expression, around the testicle area. There could be pain around there or the perineal area. Often a low-grade chronic pain or a dull pain can be signs of prostatitis. Always catching colds or flus. Blurry vision. These are typical signs of Candida aren't they. Binge eating on sweet foods. Liking beer and pizza. Lots of guys like beer, for example. Beer is the worst kind of stuff for you to drink when you've got prostatitis. I've got a good friend of mine who loves drinking beer. He's an electrician friend of mine. He loves beer and he's got a prostate problem, which he's had for many years. And I know that if he doesn't drink beer, if he switches over to vodka or gin, he has less problem with his prostate than if he drinks plenty of beer. But any alcohol consumption is not a good idea with a yeast infection. Each hour in Australia, more than five men die from conditions that are potentially preventable. Australian men experience higher rates of morbidity and mortality than women across many significant health outcome indicators including mental health, cardiovascular disease, cancer and accident and injury. Despite this fact, men tend to have fewer visits to doctors and seek medical assistance at much later stages in their illnesses. The result is that for men the health outcomes are much worse and interventions are more costly and intensive. Even though there is a wide variation amongst the Australian male population, gender is a significant risk factor for premature death across all socioeconomic groups of males but more so in the most disadvantaged. The risk factors for chronic disease such as smoking, poor diet, physical inactivity, overweight and obesity and alcohol and other drug use remain high among men. The situation for Indigenous men is far worse than for the rest of the Australian male population with life expectancy for Indigenous males is estimated to be approximately 11.5 years less than the average for all Australian males. In rural and remote areas men often have limited access to health services as well as recreational and support facilities. Work for rural men is often physically demanding, hazardous and isolating. Suicide rates for males in the farming community are twice the national average. Cultural factors and beliefs are also part of that equation. This program looks at preventative approaches in improving men's health and the need to make health services more attractive to men. It considers the barriers to engaging men in their own health care and the need for health professionals to develop more appropriate communication processes and interventions. As the incidence of pertussis increases amongst infants nationally, statewide and locally, the Virginia Department of Health would like to advise residents on ways they can protect this vulnerable population. Some 80% to 85% of the pertussis sources are family members and caretakers. The rate of pertussis infection in infants too young to be fully immunized (6 months of age and younger) has increased by as much as 20 times. There were 27,550 cases of pertussis in the United States in 2010. In Virginia alone, there were 344 cases. These numbers are expected to increase in 2011. To help combat this growing issue, Hampton Roads health districts are implementing a targeted immunization strategy called "cocooning." A cocoon is a nurturing environment that envelopes and protects new life. Similarly, members of local communities are encouraged to create a "cocoon" of protection, by ensuring that a booster dose of the tetanus, diphtheria, acellular pertussis (Tdap) vaccine is given to postpartum women, contacts/caregivers of infants younger than 1 year, and health care providers, especially those who treat infants less than 1 year of age. Why is cocooning necessary?  91% of the deaths from pertussis in the last decade are in infants, 6 months of age or less.  More than 50% of infants, less than 1 year of age, who get pertussis must be hospitalized.  Coughing fits due to pertussis infection can last for up to 10 weeks or more, with the disease being communicable in the first 3 weeks.  Less than 10% of adults have protection from the disease and 4 out of 5 adults with the disease are capable of spreading the germ and have no symptoms. Most adults don't seek medical treatment and/or not diagnosed.  A 2007 study published in the Pediatric Infectious Disease Journal, reports that 44% of the source of pertussis infection in infants was a parent, sibling or grandparent. Other caregivers (i.e., daycare providers etc.) are included in the remaining 56%. Page 1 of 2 The Tdap vaccine is very safe, with minimal side effects, e.g., redness and soreness at site of injection. It can be given even if a recent tetanus and diphtheria (Td) shot has been received. It is recommended and approved for use in:  Pregnant women in the last weeks of the 2nd trimester or during the 3rd trimester,  Women immediately after giving birth,  Any woman who may become pregnant,  Routine tetanus and diphtheria booster or wound management,  All adults who anticipate having close contact with an infant less than 12 months of age,  Healthcare workers,  Booster before 6th grade,  College enrollees. This sexually transmitted disease (STD) is often asymptomatic, but if left untreated, it can lead to long-term health consequences, including chronic pelvic pain, ectopic pregnancy, and infertility. Gonorrhea can also increase the risk of contracting and transmitting HIV. Cephalosporins are currently recommended to treat gonorrhea in the United States. For the past few years, providers have used combination therapy with either cefixime, an oral cephalosporin, or ceftriaxone, an injectable cephalosporin, plus a second antibiotic, to treat this common STD. Recent laboratory data suggest that the effectiveness of cefixime for treating gonorrhea may be declining. For this reason, CDC has updated its gonorrhea treatment guidelines and no longer recommends the routine use of cefixime. Instead, CDC now recommends using ceftriaxone along with a second antibiotic to treat gonorrheaFor patients with uncomplicated genital, rectal, and pharyngeal gonorrhea, CDC now recommends combination therapy with ceftriaxone 250 mg as a single intramuscular dose, plus either azithromycin 1 g orally in a single dose or doxycycline 100 mg orally twice daily for 7 days. There are times, however, when it may be necessary to use an alternative antibiotic regimen that does not include ceftriaxone. In instances where ceftriaxone is not available, CDC recommends cefixime 400 mg orally, plus either azithromycin 1 g orally or doxycycline 100 mg orally twice daily for 7 days. For patients with a severe allergy to cephalosporins, CDC recommends a single 2-g dose of azithromycin orally. In both of these circumstances, CDC recommends a test of cure for these patients 1 week after treatment, and this is an important change in CDC's treatment guidelines. In addition to these treatment changes, we encourage physicians to take 2 additional steps to ensure successful treatment outcomes for their patients. First, monitor your patient for treatment failure. Patients who have persistent symptoms after treatment should be retested by culture. If these cultures are positive for the gonococcus, isolates should be submitted for resistance testing. A test of cure should be conducted 1 week after retreatment. Providers should also ensure that the patient's sex partners from the preceding 60 days are promptly evaluated and treated. Second, report suspected treatment failures. Any suspected treatment failure should be reported to CDC through local or state public health officials within 24 hours. It is important to know that a single 250-mg injection of ceftriaxone is effective in treating gonorrhea at all anatomic sites. There are no clinical data to support the use of higher doses of ceftriaxone. Also, the use of azithromycin as the second antibiotic used in combination therapy may be preferable to the use of doxycycline, for 2 reasons. The changes in the gonorrhea treatment guidelines were prompted by laboratory data from CDC's Gonococcal Isolate Surveillance Project (GISP). These data demonstrated recent significant increases in the percentage of Neisseria gonorrhoeae isolates with elevated cefixime minimum inhibitory concentrations (MICs). The percentage of isolates with elevated cefixime MICs increased from 0.1% in 2006 to 1.7% in 2011, a 17-fold increase.
