Community Acquired Pneumonia Pneumonia is defined as infection of Lung Parenchyma. CLASSIFICATION OF PNEUMONIA Community Acquired Pneumonia (CAP) Hospital Acquired Pneumonia - During or after hospital stay or associated with ventilator. These types of pneumonia tend to have organisms that are much more resistant. Healthcare Associated Pneumonia are patient in nursing home, long term facility as well as patient in acute care hospital for more than 2 days in the past 90 days, IV therapy, woundcare and IV chemo wihtin the last 30 dyas and hemodialysis within the last 30 days. PATHOGENESIS OF COMMUNITY ACQUIRED PNEUMONIA Most common cause is microaspiration which overhwelm host defense, hematogenous, direct spread, and macroaspiration (Decrease gag, cough reflex) may all lead to pneumonia. MICROBIOLOGY OF PNEUMONIA TYPICAL Pneumonia tends to be more severe type of pneumonia. There tends to be more consolidation and mucupurulent sputum. Classic bacteria that causes pneumonia is Streptococcus pneumonia, Staphylococcus aureus, Klebsiella Pneumonia, Psueodmonas Aeroginosa, Hemophilus Influenza and Moraxella Catarrhalis. ATYPICAL Pneumonia is less severe and also referred to as walking pneumonia and invades the epithelial cells and does not affect the alveoli wall. There is patchy diffuse and decrease sputum. Extrapulmonary symptoms is also common such as myalgia bradycardia splenomegaly, rash and CNS Symptoms. Non zoonotic causes include Mycoplasma, chalmydia pneumonia and Legionella which are all intracelular. Zoonotic is psitococas, Q Fever, and Tularemia. MOre commonly viruses such as influenza, parainfluenza and RSV and adenovirus. RISK FACTORS FOR PNEUMONIA Alcoholism - S. Pneumonia, oral anerobes, Klebsiella pneumonia and TB COPD/Smoking - S. Pneumonia, H. Influenza, P. Aeroginosa, Moraxella, Legionalla Lung Abscess - CA-MRSA, Oral Anaerobes, TB Fungal Structural Disease - Psuedomonas, S. Aureus, Burklholderia Influenza - S. Pnuemonia, S.A ureus IV Drug Abusers - Psuedomonas, Anaerobes, S. PNeumonia TB Farm Animals - Coxxiel Burnetti Birds - Chlamydia Rabbits - Tularemia Bat-Bird - Histoplasma Capsilatuma HIV Early - S. pneumonia, H. Influeanza, TB Southwestern US - coccidioides Hanta virus Hotel/Cruise Ships - Legionella Bioterrorism - B. Anthracis, Yersinia Pestis, Tularemia Aspiration - Gram neg, Oral anaerobes Predisposing - hypoxemia, acidosis, toxic inhalation, pumonary embolism, uremia, malnutrition, lung cancer, asthma, AIDS, smoking Age greater than 70 Fungal - Aspergillus, Cryptococcus, Histoplasma, Coccidiomycosis EVALUATION Symptoms - mucupurulent cough or scanty watery cough, Fever, pleuritic chest pain, Nausea and vomiting, Mental status changes, myalgias rash splenomegaly. Signs - tachycardia, RR more than 24 Auscultation audible crackles. Consolidation - dullness to percussion, Bronchial breathing increase vocal fremitus, Egophany Labs - leukocytosis leftward shift, Anemia X-Ray shows consolidation in lobar pneumonia and interstitial pneumonia shows a much more diffuse reticular picture. CT-Scan - no evidence that CT Scan improves outcome DETERMINE PATHOGEN OF PNEUMONIA Outpatient not necessary unless critical agent such as legionella, influenza, MERS-COV, CA-MRSA, Bioterrorism. Severe Pneumonia or ICU patient perform sputum, blood culture, urine antigen test, PCR, Serology, ELISA, Immunofluorescence. Biomarkers procalcitonin helps determine for use of anti-biotics. CRP greater than 40mg/L suggest bacterial. Blood cultures are difficult to get true positive result. S. Pneumonia and H. Influenza have a higher false neg becuase grow easier, while S. aurues and GNB has a high false positive. Shoudl have greater than 25 neutrophils and less that 10 squamous epithelium. MANAGEMENT OF PNEUMONIA PNEUMONIA SEVERITY INDEX - rarely used CURB-65- Confusion, Uremia, Respiratory Rate, BP less than 90 and age over 65. ICU if septic shock, mechanical ventilation requirements. ANTI-BIOTICS Outpatient - Macrolide or doxycycline Comorbidities - add flouroquinolone or Betalactam with macrolide After pathogen identification than base on sensitivity. Admission - Flouroquinolon with B lactam and macrolide ICU Admissions - Anti-pneumooccal B-lactam with Azithromycin and Flouroquinolone Psuedmonas - Pipericillin tazobactam, cefipem, imepinem, meropenem and Aminoglycosdie, azithromycin or AMinoglycoside Floroquinolone. CA-MRSA - Add vancomycin or Lenezolid ADJUVANT THERAPY - Glucocorticoid, Decrease inflammatory response to pneumonia

A mum has recalled the terrifying moment when her daughter started thrashing about in Asda, like a child 'possessed,' leading to a rare diagnosis of 'exorcist syndrome'- triggered by severe tonsillitis.One minute Nikki Ashcroft's daughter, Amelia, now nine, was a normal, happy little girl, the next she started continually smacking her lips - culminating in her outburst in the food aisles, reminiscent of the behaviour displayed by the girl in the cult horror film.Single mum, Nikki, 33, of Wigan, Greater Manchester, who also has an older son, Brooke, 11, explained: 'The day before Bonfire night in 2016, we were in Asda and Amelia just started randomly screaming and shouting.'I had to restrain her and hold her arms at the side of her. I was mortified and just thought, "What has got into her?"'Constantly struck down by childhood illnesses, Amelia suffered with recurring tonsillitis.Nikki added: 'She was always at the doctor's and, in November 2016, just before her outburst, she was really, really bad with tonsillitis.'Her temperature soared to 40C and she was hallucinating. We got her on antibiotics, but there was one day I noticed she was continually smacking her lips. It went on for a few days.'After the incident in Asda, her character completely changed, prompting Nikki to take drastic action.She continued: 'Amelia was never easy. She had lots of attitude and liked to have her own way, but these outbursts were out of character and I had no idea what had happened. I was so shocked she was behaving like this.'Desperate for help, I videoed her during an outburst and posted it on Facebook, in a disability forum.'A lady messaged me some information about Pediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococcal Infection (PANDAs). I had never heard of it, but started googling and realised Amelia had a lot of the symptoms.'PANDAs - also known as 'exorcist syndrome' - is a rare condition, caused when a streptococcal infection, like tonsillitis, triggers a misdirected immune response, resulting in inflammation on a child's brain.This causes life changing symptoms like anxiety, tics, alterations in personality and obsessive compulsive disorder.Spurred on by the Facebook reaction, Nikki took the video footage of Amelia to their GP, who agreed to look into PANDAS, despite not having heard of it and referred them to an ear, nose and throat (ENT) specialist.Over the next 10 weeks, Amelia's tics and mood changes continued, but in January 2017, her behaviour started to improve.'Her behaviour can be really uncontrolled'Full-time mum Nikki added: 'We had a couple of weeks where it was ok. Then she got mild tonsillitis again and her tics came back again.'That lasted again for about 10 weeks. Patients are vulnerable to major adverse outcomes after hospital stay. Readmissions are common: nearly one in five adults aged more than 65 is readmitted to hospital within 30 days of discharge. Death is also common in this first month, during which rates of post-discharge mortality may exceed initial inpatient mortality. The range of illnesses to which patients are susceptible is extremely broad. This period of heightened and generalized vulnerability to a broad spectrum of conditions has been called the post-hospital syndrome. In this research Dharmarajan et al set out to characterize the absolute risks for older patients of readmission to hospital and death in the year after hospitalization for heart failure, acute myocardial infarction, or pneumonia. They concluded that risk declines slowly for older patients after hospitalization for heart failure, acute myocardial infarction, or pneumonia and is increased for months. Specific risk trajectories vary by discharge diagnosis and outcome. Patients should remain vigilant for deterioration in health for an extended time after discharge. Health providers can use knowledge of absolute risks and their changes over time to better align interventions designed to reduce adverse outcomes after discharge with the highest risk periods for patients. Dr Ahmed Ismail BBC Interview Sexual Transmitted Disease Infertility Concern BBC Interview for Dr Ahmed Ismail Consultant Gynaecologist and Obstetrician, Infertility Expert Dr Ahmed Ismail has been interviewed on the 27th December by BBC News following the media scare from chief medical officer Sally Davies of Great Britain warning the casual treatment of Gonorrhoea leading to untreatable Gonorrhoea. Gonorrhoea is the second most common Sexual Transmitted Disease in UK for both heterosexual and homosexual Gonorrhoea Sexual Transmitted Disease always occurs by Sex (Oral, Vaginal, and Anal Sex) Gonorrhoea can be a cause of Infertility for both Men and Women therefore should be treated properly as soon as possible . Gonorrhoea should be treated by a Gynaecologist with interest of Infertility and sexual health and sexual transmitted diseases, should the treatment to be carried out by GUM Centres,, patient must be followed up by Infertility Gynaecologist to check if any damages has occurred. Gonorrhoea should not be treated by Pharmacist, GP, or on line medications. Gonorrhoea should be treated with a maximum care with careful history to identify the parties involved in sexual activities and all should be investigated and treated at the same time if possible. Women with discharges following sexual activities must be seen and Gonorrhoea must be excluded in almost most of the cases. Women diagnosed with Gonorrhoea must have her partner or partners treated and with men if they were diagnosed with Gonorrhoea same will apply for all partners exposed to sexual activities by him. Casual treatment with antibiotics of urethral discharges and vaginal discharges or anal discharges can lead to what is called Super Gonorrhoea (Untreatable Gonorrhoea) Condom is not Sexual Transmitted Disease Barrier this is the usual advise to all London Gynaecology Patients (Queens Clinic Patients)' 27 December 2015 Chief Medical Office Report Gonorrhoea could become an untreatable disease, England's chief medical officer has warned. Dame Sally Davies has written to all GPs and pharmacies to ensure they are prescribing the correct drugs after the rise of "super-gonorrhoea" in Leeds. Her warning comes after concerns were raised that some patients were not getting both of the antibiotics needed to clear the infection. Sexual health doctors said gonorrhoea was "rapidly" developing resistance. A highly drug-resistant strain of gonorrhoea was detected in the north of England in March. That strain is able to shrug off the antibiotic azithromycin, which is normally used alongside another drug, ceftriaxone. In her letter, the chief medical officer said: "Gonorrhoea is at risk of becoming an untreatable disease due to the continuing emergence of antimicrobial resistance." Soaring cases But while an injection of ceftriaxone and an azithromycin pill are supposed to be used in combination, this may not always be the case for all patients. Earlier this year, the British Association for Sexual Health and HIV (BASHH) warned that some online pharmacies were offering only oral medication. Using just one of the two drugs makes it easier for the bacterium to develop resistance. The letter, which is also signed by chief pharmaceutical officer Dr Keith Ridge, stated: "Gonorrhoea has rapidly acquired resistance to new antibiotics, leaving few alternatives to the current recommendations. "It is therefore extremely important that suboptimal treatment does not occur." What is gonorrhoea? Often the person has no symptoms, however, but can still easily spread the disease to others. Untreated infection can lead to infertility, pelvic inflammatory disease and can be passed on to a child during pregnancy. Gonorrhoea is the second most common sexually transmitted infection in England and cases are soaring. The number of infections increased by 19% from 29,419 in 2013 to 34,958 the following year. Dr Jan Clarke, the president of BASHH, told the BBC News website: "We're really pleased that the chief medical officer has stressed that gonorrhoea needs this approach to treatment due to the rapid development of resistance. "We need to protect what we've got and we need to encourage pharmacists and general practitioners to follow first-line treatment."
